    In this article, Médicos Sin Fronteras (MSF) Spain faces the challenge of selecting, piecing together, and conveying in the clearest possible way, the main lessons learnt over the course of the last seven years in the world of medical care for Chagas disease. More than two thousand children under the age of 14 have been treated; the majority of whom come from rural Latin American areas with difficult access. It is based on these lessons learnt, through mistakes and successes, that MSF advocates that medical care for patients with Chagas disease be a reality, in a manner which is inclusive (not exclusive), integrated (with medical, psychological, social, and educational components), and in which the patient is actively followed. This must be a multi-disease approach with permanent quality controls in place based on primary health care (PHC). Rapid diagnostic tests and new medications should be available, as well as therapeutic plans and patient management (including side effects) with standardised flows for medical care for patients within PHC in relation to secondary and tertiary level, inclusive of epidemiological surveillance systems

    Is There a Significant Excess in Bottom Hadroproduction at the Tevatron?

    We discuss the excess in the hadroproduction of B mesons at the Tevatron. We show that an accurate use of up-to-date information on the B fragmentation function reduces the observed excess to an acceptable level. Possible implications for experimental results reporting bottom quark cross sections, also showing an excess with respect to next-to-leading order theoretical predictions, are discussed.Comment: 5 pages, Latex, 4 figures.     Discussions about Bose-Einstein correlations between decay products of coproduced W-bosons again raise the question about the behaviour of correlations if several strings are produced.     Anomalous prompt photon production in hadronic collisions at low-xTx_T

    We investigate the discrepancy that exists at low-xT=2pT/sx_T=2p_T/\sqrt{s} between the next--to--leading order QCD calculations of prompt photon production and the measured cross section. The central values of the measured cross section are of order 100\% larger than QCD predictions in this region. It has been suggested that the bremsstrahlung contribution may account for this discrepancy. The quark fragmentation function Dγ/q(z)D_{\gamma/q}(z) has not been measured and an exactly known asymptotic form is normally used in calculations. We examine the effect of much larger fragmentation functions on the QCD predictions. After illustrating the effect of the large fragmentation functions in some detail for recent CDF data at s\sqrt{s}=1.8~TeV, we perform a χ2\chi^2 fit to 8 prompt photon data sets ranging in CMS energy from 24~GeV to 1.8~TeV. While a large fragmentation function normalization may prove to play an important role in resolving the discrepancy, the present theoretical and experimental uncertainties prevent any definite normalization value from being determined.Comment: 14 pages, LBL-33122 and UCB-PTH-92/38. 13 figures available by email, specify postscript or topdrawe

    Inclusive B-Meson Production in e^+ e^- and p p-bar Collisions

    We provide nonperturbative fragmentation functions for B mesons, both at leading and next-to-leading order in the MS-bar factorization scheme with five massless quark flavors. They are determined by fitting the fractional energy distribution of B mesons inclusively produced in e^+ e^- annihilation at CERN LEP1. Theoretical predictions for the inclusive production of B mesons with high transverse momenta in p p-bar scattering obtained with these fragmentation functions nicely agree, both in shape and normalization, with data recently taken at the Fermilab Tevatron.Comment: 20 pages (Latex), 6 figures (Postscript

    Transverse Momentum Distributions for Heavy Quark Pairs

    We study the transverse momentum distribution for a pairpair of heavy quarks produced in hadron-hadron interactions. Predictions for the large transverse momentum region are based on exact order αs3\alpha_s^3 QCD perturbation theory. For the small transverse momentum region, we use techniques for all orders resummation of leading logarithmic contributions associated with initial state soft gluon radiation. The combination provides the transverse momentum distribution of heavy quark pairs for all transverse momenta. Explicit results are presented for bbˉb\bar b pair production at the Fermilab Tevatron collider and for ccˉc\bar c pair production at fixed target energies.Comment: LaTeX (27 pages text, 8 figures not included, but available on request

    RHIC-tested predictions for low-pTp_T and high-pTp_T hadron spectra in nearly central Pb+Pb collisions at the LHC

    We study the hadron spectra in nearly central AA+AA collisions at RHIC and LHC in a broad transverse momentum range. We cover the low-pTp_T spectra using longitudinally boost-invariant hydrodynamics with initial energy and net-baryon number densities from the perturbative QCD (pQCD)+saturation model. Build-up of the transverse flow and sensitivity of the spectra to a single decoupling temperature \Tdec are studied. Comparison with RHIC data at \ssNN=130 and 200 GeV suggests a rather high value \Tdec=150 MeV. The high-pTp_T spectra are computed using factorized pQCD cross sections, nuclear parton distributions, fragmentation functions, and describing partonic energy loss in the quark-gluon plasma by quenching weights. Overall normalization is fixed on the basis of p+pˉ\bar{\rm p}(p) data and the strength of energy loss is determined from RHIC Au+Au data. Uncertainties are discussed. With constraints from RHIC data, we predict the pTp_T spectra of hadrons in 5 % most central Pb+Pb collisions at the LHC energy \ssNN=5500 GeV. Due to the closed framework for primary production, we can also predict the net-baryon number at midrapidity, as well as the strength of partonic energy losses at the LHC. Both at the LHC and RHIC, we recognize a rather narrow crossover region in the pTp_T spectra, where the hydrodynamic and pQCD fragmentation components become of equal size. We argue that in this crossover region the two contributions are to a good approximation mutually independent. In particular, our results suggest a wider pTp_T-region of applicability for hydrodynamical models at the LHC than at RHIC.Comment: 45 pages, 16 eps-figure

    The multidimensional comprehension of Chagas disease. Contributions, approaches, challenges and opportunities from and beyond the information, education and communication field

    Chagas is a complex, multidimensional phenomenon in which political, economic, environmental, biomedical, epidemiological, psychological, and sociocultural factors intersect. Nonetheless, the hegemonic conceptualisation has long envisioned Chagas as primarily a biomedical question, while ignoring or downplaying the other dimensions, and this limited view has reinforced the disease's long neglect. Integrating the multiple dimensions of the problem into a coherent approach adapted to field realities and needs represents an immense challenge, but the payoff is more effective and sustainable experiences, with higher social awareness, increased case detection and follow-up, improved adherence to care, and integrated participation of various actors from multiple action levels. Information, Education, and Communication (IEC) initiatives have great potential for impact in the implementation of multidimensional programs of prevention and control successfully customised to the diverse and complex contexts where Chagas disease persists
