    The Costs of Missing the Millennium Development Goal on Gender Equity

    At the Millennium Summit, the world community pledged to promote gender equality and chose as a specific target the achievement of gender equity in primary and secondary education by the year 2005 in every country of the world. Based on the findings from a growing empirical literature that suggests that gender equity in education promotes economic growth and reduce fertility, child mortality, and undernutrition, we estimate what the costs in terms of growth, and forgone fertility, mortality and undernutrition reduction, will be for the 45 countries that are, on current projections, unlikely to meet the target. Our estimates suggest that, by 2005, the countries that are off track are likely to suffer 0.1-0.3 percentage points lower per capita growth rates as a result and will have 0.1-0.4 more children per woman, and, by 2015, an average of 14 per 1000 higher rates of under five mortality and 2.4 percentage points higher prevalence of underweight children under five. Sensitivity analyses suggest that the results are quite robust to using different specifications and approaches to estimating these losses

    The normal dimensions of the sella turcica in Jordanians: a study on lateral cephalograms

    Background: The sella turcica is an important anatomic and radiologic entity. The dimensions of the sella turcica vary greatly in normal individuals and are influenced by genetic and local factors. The main objective of this study is to build up a normative database of the dimensions of the sella turcica in the Jordanian population. Materials and methods: For this purpose 509 computed cephalograms of 252 male and 257 female healthy Jordanians aged 10–40 years were collected and divided into two adolescent and adult age-groups for both genders. Viewbox 3 software was used to determine linear dimensions and area of the sella turcica. Results: Our results showed that the overall values for width, length, height, area, and aperture (interclinoid distance) were 8.72, 7.68, 6.25, 40.80, 3.92, and 8.67, 7.42, 6.38, 41.26, 3.68 mm (mm2) for males and females, respectively. Significant differences were found between adult male group on one hand and adolescent female group and adult female group on the other hand. Between age categories and within male and female groups, sella parameters were significantly different between adolescent and adult age-groups. Conclusions: Our data clearly confirm the classical notion of general trend of increase in parameters with age, irrespective of gender, with the area scoring the highest increase, and the height in females and length in males contributing most obviously to this trend. The aperture appears to decrease in females while it increases in males as adulthood is reached. This seems to reflect differential growth of the pituitary gland under neurohormonal effects

    The carrying angle: racial differences and relevance to inter-epicondylar distance of the humerus

    The human carrying angle (CA) is a measure of the lateral deflection of the forearm from the arm. The importance of this angle emerges from its functional and clinical relevance. Previous studies have correlated this angle with different parameters including age, gender, and handedness. However, no reports have focused on race-dependent variations in CA or its relation to various components of the elbow joint. This study aimed to investigate the variations in CA with respect to race and inter-epicondylar distance (IED) of the humerus. The study included 457 Jordanian and 345 Malaysian volunteers with an age range of 18–21 years. All participants were right-hand dominant with no previous medical history in their upper limbs. Both CA and IED were measured by well-trained medical practitioners according to a well-established protocol. Regardless of race, CA was greater on the dominant side and in females. Furthermore, CA was significantly greater in Malaysian males compared to Jordanian males, and significantly smaller in Malaysian females compared to their Jordanian counterparts. Finally, CA significantly decreased with increasing IED in both races. This study supports effects of gender and handedness on the CA independent of race. However, CA also varies with race, and this variation is independent of age, gender, and handedness. The evaluation also revealed an inverse relationship between CA and IED. These findings indicate that multiple factors including race and IED should be considered during the examination and management of elbow fractures and epicondylar diseases

The normal shape of sella turcica has been categorized as circular, oval, and flat, and several variants have been described. The sella turcica increases in size with age, but there is no agreement upon significance of gender-related differences. The main objective of this study is to build up a normative database of the shape of the sella turcica in the Jordanian population with reference to age and gender. For this purpose 509 computed cephalograms: 252 males and 257 females, aged 10 to 40 years old healthy Jordanians were collected and divided into adolescent and adult age groups in both genders. Viewbox 3 software was used to determine width and height, and shape of the sella turcica was accordingly assessed. Variants of shape were also documented. Our results showed that females exhibit higher percentage of overall normal shape but lower relative percentages of oval and flat shapes, as compared with males. In contrast to males, females also show decreasing trend of normal shape as age advances. Females and males change obviously in favor of oval shape. In terms of variants, as individuals advance in age, bridging and irregular dorsum clearly predominate. In terms of width and height, significant differences were evident between adult male group on one hand and adolescent female group and adult female group on the other hand. Between age categories and within male and female groups, sella linear dimensions were significantly different between adolescent and adult age groups. The significant increase of linear parameters of sella turcica across age groups indicates that there is significant degree of growth in sella turcica across the studied age groups. However, this growth does not affect the shape of sella turcica as the shape seems to be preserved across the studied age groups.     A Blind Spot in Girls’ Education: Menarche and its Webs of Exclusion

    Despite notable progress in girls' education over the last decade, gender-based differences continue to shape educational outcomes. One of the most overlooked of these differences is the process of maturation itself, including menstruation. This paper presents the findings of a study that assessed the impact of sanitary care on the school attendance of post-pubertal girls, as well as the implications of menarche for their well-being. The study found that the provision of adequate sanitary care represents a relatively unrecognized but potentially fruitful tool in strategies that aim to improve girls' educational outcomes, one that warrants policy consideration among development planner

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely
