Costs associated with febrile neutropenia in solid tumor and lymphoma patients - an observational study in Singapore.
BackgroundThe primary objective was to describe the total direct inpatient costs among solid tumor and lymphoma patients with chemotherapy-induced febrile neutropenia (FN) and the factors that were associated with higher direct cost. The secondary objective was to describe the out-of-pocket patient payments and the factors that were associated with higher out-of-pocket patient payments.MethodsThis was a single-center observational study conducted at the largest cancer center in Singapore. All of the adult cancer patients hospitalized due to FN from 2009 to 2012 were studied. The primary outcomes were the total hospital cost and the out-of-pocket patient payments (adjusted by government subsidy) per FN episode. Univariate analysis and multiple linear regression were conducted to identify the factors associated with higher FN costs.ResultsThree hundred and sixty seven adult cancer patients were documented with FN-related hospitalizations. The mean total hospital cost was US3,779-4,607) and the mean out-of-pocket patient payment was US1,976-2,484), per FN episode. The factors associated with a higher total hospital cost were longer length of stay, severe sepsis, and lymphoma as underlying cancer. The out-of-pocket patient payment was positively associated with longer length of stay, severe sepsis, lymphoma diagnosed as underlying cancer, the therapeutic use of granulocyte colony-stimulating factor (GCSF), the private ward class, and younger patients.ConclusionsThe total hospital cost and out-of-pocket patient payments of FN management in lymphoma cases were substantial compared with other solid tumors. Factors associated with a higher FN management cost may be useful for developing appropriate strategies to reduce the cost of FN for cancer patients
What is a hospital bed day worth? A contingent valuation study of hospital Chief Executive Officers
BACKGROUND: Decreasing hospital length of stay, and so freeing up hospital beds, represents an important cost saving which is often used in economic evaluations. The savings need to be accurately quantified in order to make optimal health care resource allocation decisions. Traditionally the accounting cost of a bed is used. We argue instead that the economic cost of a bed day is the better value for making resource decisions, and we describe our valuation method and estimations for costing this important resource. METHODS: We performed a contingent valuation using 37 Australian Chief Executive Officers’ (CEOs) willingness to pay (WTP) to release bed days in their hospitals, both generally and using specific cases. We provide a succinct thematic analysis from qualitative interviews post survey completion, which provide insight into the decision making process. RESULTS: On average CEOs are willing to pay a marginal rate of 436 for an Intensive Care Unit (ICU) bed day, with estimates of uncertainty being greater for ICU beds. These estimates are significantly lower (four times for ward beds and seven times for ICU beds) than the traditional accounting costs often used. Key themes to emerge from the interviews include the importance of national funding and targets, and their associated incentive structures, as well as the aversion to discuss bed days as an economic resource. CONCLUSIONS: This study highlights the importance for valuing bed days as an economic resource to inform cost effectiveness models and thus improve hospital decision making and resource allocation. Significantly under or over valuing the resource is very likely to result in sub-optimal decision making. We discuss the importance of recognising the opportunity costs of this resource and highlight areas for future research. Measuring measurement
Measurement connects the world of quantum phenomena to the world of classical
events. It plays both a passive role, observing quantum systems, and an active
one, preparing quantum states and controlling them. Surprisingly - in the light
of the central status of measurement in quantum mechanics - there is no general
recipe for designing a detector that measures a given observable. Compounding
this, the characterization of existing detectors is typically based on partial
calibrations or elaborate models. Thus, experimental specification (i.e.
tomography) of a detector is of fundamental and practical importance. Here, we
present the realization of quantum detector tomography: we identify the optimal
positive-operator-valued measure describing the detector, with no ancillary
assumptions. This result completes the triad, state, process, and detector
tomography, required to fully specify an experiment. We characterize an
avalanche photodiode and a photon number resolving detector capable of
detecting up to eight photons. This creates a new set of tools for accurately

UK adults' implicit and explicit attitudes towards obesity: a cross-sectional study.
Background: Anti-fat attitudes may lead to stigmatisation of and lowered self-esteem in obese people. Examining anti-fat attitudes is warranted given that there is an association with anti-fat behaviours. Previous studies, mainly outside the UK, have demonstrated that anti-fat attitudes are increasing over time. Methods: The study was cross-sectional with a sample of 2380 participants (74.2 % female; aged 18–65 years). In an online survey participants reported demographic characteristics and completed a range of implicit and explicit measures of obesity related attitudes. Results: Perceptions of obesity were more negative than reported in previously. Main effects indicated more negative perceptions in males, younger respondents and more frequent exercisers. Attitudes about obesity differed in relation to weight category, and in general were more positive in obese than non-obese respondents. Conclusions: This is the first study to demonstrate anti-fat attitudes across different sections of the UK population. As such, this study provides the first indication of the prevalence of anti-fat attitudes in UK adults. Interventions to modify these attitudes could target specific groups of individuals with more negative perceptions as identified here. Future work would be useful that increases understanding of both implicit and explicit attitudes towards obesity
Scintillation-limited photometry with the 20-cm NGTS telescopes at Paranal Observatory
Ground-based photometry of bright stars is expected to be limited by atmospheric scintillation, although in practice observations are often limited by other sources of systematic noise. We analyse 122 nights of bright star (Gmag ≲ 11.5) photometry using the 20-cm telescopes of the Next-Generation Transit Survey (NGTS) at the Paranal Observatory in Chile. We compare the noise properties to theoretical noise models and we demonstrate that NGTS photometry of bright stars is indeed limited by atmospheric scintillation. We determine a median scintillation coefficient at the Paranal Observatory of CY=1.54, which is in good agreement with previous results derived from turbulence profiling measurements at the observatory. We find that separate NGTS telescopes make consistent measurements of scintillation when simultaneously monitoring the same field. Using contemporaneous meteorological data, we find that higher wind speeds at the tropopause correlate with a decrease in long-exposure (t = 10 s) scintillation. Hence, the winter months between June and August provide the best conditions for high-precision photometry of bright stars at the Paranal Observatory. This work demonstrates that NGTS photometric data, collected for searching for exoplanets, contains within it a record of the scintillation conditions at Paranal
Quantum Process Tomography of a Universal Entangling Gate Implemented with Josephson Phase Qubits
Quantum logic gates must perform properly when operating on their standard
input basis states, as well as when operating on complex superpositions of
these states. Experiments using superconducting qubits have validated the truth
table for particular implementations of e.g. the controlled-NOT gate [1,2], but
have not fully characterized gate operation for arbitrary superpositions of
input states. Here we demonstrate the use of quantum process tomography (QPT)
[3,4] to fully characterize the performance of a universal entangling gate
between two superconducting quantum bits. Process tomography permits complete
gate analysis, but requires precise preparation of arbitrary input states,
control over the subsequent qubit interaction, and simultaneous single-shot
measurement of the output states. We use QPT to measure the fidelity of the
entangling gate and to quantify the decoherence mechanisms affecting the gate
performance. In addition to demonstrating a promising fidelity, our entangling
gate has a on/off ratio of 300, a level of adjustable coupling that will become
a requirement for future high-fidelity devices. This is the first solid-state
demonstration of QPT in a two-qubit system, as solid-state process tomography
has previously only been demonstrated with single qubits [5,6]
Challenges to adaptation: a fundamental concept for the shared socio-economic pathways and beyond
The framework for the new scenarios being developed for climate research calls
for the development of a set of Shared Socioeconomic Pathways (SSPs), which are meant to
differ in terms of their challenges to mitigation and challenges to adaptation. In order for the
scenario process to fulfill its goals, the research and policy communities need to develop a
shared understanding of these concepts. This paper focuses on challenges to adaptation. We
begin by situating this new concept in the context of the rich literatures related to inter alia
adaptation, vulnerability, and resilience. We argue that a proper characterization of challenges to adaptation requires a rich, exploration of the concept, which goes beyond mere
description. This has a number of implications for the operationalization of the concept in
the basic and extended versions of the SSPs. First, the elements comprising challenges to
adaptation must include a wide range of socioeconomic and even some (non-climatic)
biophysical factors. Second, careful consideration must be given to differences in these
factors across scales, as well as cross-scale interactions. Third, any representation of the
concept will require both quantitative and qualitative elements. The scenario framework
offers the opportunity for the SSPs and full scenarios to be of greater value than has been the
case in past exercises to both Integrated Assessment Modeling (IAM) and Impacts,Adaptation, and Vulnerability (IAV) researchers, but this will require a renegotiation of the
traditional, primarily unidirectional relationship between the two communities
Prognostic value of nuclear morphometry in patients with TNM stage T1 ovarian clear cell adenocarcinoma
In 40 patients with TNM stage T1 ovarian clear cell adenocarcinoma, we used nuclear morphometry to study the relations among morphometric variables, clinical prognostic factors and outcome. The presence of one or more giant nuclear cells was positively associated with death (OR = 10.6, P = 0.02) and tended to be associated with disease recurrence (OR = 5.1, P = 0.07). Nuclear irregularity (expressed in terms of the nuclear roundness factor) was positively associated with both death (OR = 8.6, P = 0.02) and disease recurrence (OR = 8.2, P = 0.02). A combination of giant nuclear cell presence or nuclear irregularity proved to be a useful prognostic indicator, with a sensitivity and specificity of 83% and 71% in the prediction of death, and 75% and 71% in the prediction of disease recurrence. Patients' age and substage were of no prognostic value. We conclude that the nuclear morphometric characteristics, especially the presence of giant nuclear cells and nuclear irregularity, may be useful in predicting outcome in patients with early stage ovarian clear cell adenocarcinoma. © 1999 Cancer Research Campaig
The efficacy of indwelling pleural catheter placement versus placement plus talc sclerosant in patients with malignant pleural effusions managed exclusively as outpatients (IPC-PLUS): study protocol for a randomised controlled trial
BACKGROUND: Malignant pleural effusions (MPEs) remain a common problem, with 40,000 new cases in the United Kingdom each year and up to 250,000 in the United States. Traditional management of MPE usually involves an inpatient stay with placement of a chest drain, followed by the instillation of a pleural sclerosing agent such as talc, which aims to minimise further fluid build-up. Despite a good success rate in studies, this approach can be expensive, time-consuming and inconvenient for patients. More recently, an alternative method has become available in the form of indwelling pleural catheters (IPCs), which can be inserted and managed in an outpatient setting. It is currently unknown whether combining talc pleurodesis with IPCs will provide improved pleural symphysis rates over those of IPCs alone.
METHODS/DESIGN: IPC-PLUS is a patient-blind, multicentre randomised controlled trial (RCT) comparing the combination of talc with an IPC to the use of an IPC alone for inducing pleurodesis in MPEs. The primary outcome is successful pleurodesis at five weeks post-randomisation. This study will recruit 154 patients, with an interim analysis for efficacy after 100 patients, and aims to help to define the future gold standard for outpatient management of patients with symptomatic MPEs.
DISCUSSION: IPC-PLUS is the first RCT to examine the practicality and utility of talc administered via an IPC. The study remains in active recruitment and has the potential to significantly alter how patients requiring pleurodesis for MPE are approached in the future.
