    A low-power opportunistic communication protocol for wearable applications

    © 2015 IEEE.Recent trends in wearable applications demand flexible architectures being able to monitor people while they move in free-living environments. Current solutions use either store-download-offline processing or simple communication schemes with real-time streaming of sensor data. This limits the applicability of wearable applications to controlled environments (e.g, clinics, homes, or laboratories), because they need to maintain connectivity with the base station throughout the monitoring process. In this paper, we present the design and implementation of an opportunistic communication framework that simplifies the general use of wearable devices in free-living environments. It relies on a low-power data collection protocol that allows the end user to opportunistically, yet seamlessly manage the transmission of sensor data. We validate the feasibility of the framework by demonstrating its use for swimming, where the normal wireless communication is constantly interfered by the environment

    Fifty Years of Innovation in Plastic Surgery

However, documentation of this process only exists scattered throughout the literature as individual articles. The few attempts made to profile innovation in plastic surgery have been narrative, and therefore qualitative and inherently biased. Through the implementation of a novel innovation metric, this work aims to identify and characterise the most prevalent innovations in plastic surgery over the last 50 years. Methods Patents and publications related to plastic surgery (1960 to 2010) were retrieved from patent and MEDLINE databases, respectively. The most active patent codes were identified and grouped into technology areas, which were subsequently plotted graphically against publication data. Expert-derived technologies outside of the top performing patents areas were additionally explored. Results Between 1960 and 2010, 4,651 patents and 43,118 publications related to plastic surgery were identified. The most active patent codes were grouped under reconstructive prostheses, implants, instruments, non-invasive techniques, and tissue engineering. Of these areas and other expert-derived technologies, those currently undergoing growth include surgical instruments, implants, non-invasive practices, transplantation and breast surgery. Innovations related to microvascular surgery, liposuction, tissue engineering, lasers and prostheses have all plateaued. Conclusions The application of a novel metric for evaluating innovation quantitatively outlines the natural history of technologies fundamental to the evolution of plastic surgery.     REGULATORY APPROVAL OF NEW MEDICAL DEVICES: A CROSS SECTIONAL STUDY

    Objective To investigate the regulatory approval of new medical devices. Design Cross sectional study of new medical devices reported in the biomedical literature. Data sources PubMed was searched between 1 January 2000 and 31 December 2004 to identify clinical studies of new medical devices. The search was carried out during this period to allow time for regulatory approval. Eligibility criteria for study selection Articles were included if they reported a clinical study of a new medical device and there was no evidence of a previous clinical study in the literature. We defined a medical device according to the US Food and Drug Administration as an “instrument, apparatus, implement, machine, contrivance, implant, in vitro reagent, or other similar or related article.” Main outcome measures Type of device, target specialty, and involvement of academia or of industry for each clinical study. The FDA medical databases were then searched for clearance or approval relevant to the device. Results 5574 titles and abstracts were screened, 493 full text articles assessed for eligibility, and 218 clinical studies of new medical devices included. In all, 99/218 (45%) of the devices described in clinical studies ultimately received regulatory clearance or approval. These included 510(k) clearance for devices determined to be “substantially equivalent” to another legally marketed device (78/99; 79%), premarket approval for high risk devices (17/99; 17%), and others (4/99; 4%). Of these, 43 devices (43/99; 43%) were actually cleared or approved before a clinical study was published. Conclusions We identified a multitude of new medical devices in clinical studies, almost half of which received regulatory clearance or approval. The 510(k) pathway was most commonly used, and clearance often preceded the first published clinical study

    Shape sensing of miniature snake-like robots using optical fibers

    Snake like continuum robots are increasingly used for minimally invasive surgery. Most robotic devices of this sort that have been reported to date are controlled in an open loop manner. Using shape sensing to provide closed loop feedback would allow for more accurate control of the robot's position and, hence, more precise surgery. Fiber Bragg Gratings, magnetic sensors and optical reflectance sensors have all been reported for this purpose but are often limited by their cost, size, stiffness or complexity of fabrication. To address this issue, we designed, manufactured and tested a prototype two-link robot with a built-in fiber-optic shape sensor that can deliver and control the position of a CO 2 -laser fiber for soft tissue ablation. The shape sensing is based on optical reflectance, and the device (which has a 4 mm outer diameter) is fabricated using 3D printing. Here we present proof-of-concept results demonstrating successful shape sensing - i.e. measurement of the angular displacement of the upper link of the robot relative to the lower link - in real time with a mean measurement error of only 0.7°

    One-Dimensional Nanostructures and Devices of II–V Group Semiconductors

    The II–V group semiconductors, with narrow band gaps, are important materials with many applications in infrared detectors, lasers, solar cells, ultrasonic multipliers, and Hall generators. Since the first report on trumpet-like Zn3P2nanowires, one-dimensional (1-D) nanostructures of II–V group semiconductors have attracted great research attention recently because these special 1-D nanostructures may find applications in fabricating new electronic and optoelectronic nanoscale devices. This article covers the 1-D II–V semiconducting nanostructures that have been synthesized till now, focusing on nanotubes, nanowires, nanobelts, and special nanostructures like heterostructured nanowires. Novel electronic and optoelectronic devices built on 1-D II–V semiconducting nanostructures will also be discussed, which include metal–insulator-semiconductor field-effect transistors, metal-semiconductor field-effect transistors, andp–nheterojunction photodiode. We intent to provide the readers a brief account of these exciting research activities

    Gaze gesture based human robot interaction for laparoscopic surgery

    While minimally invasive surgery offers great benefits in terms of reduced patient trauma, bleeding, as well as faster recovery time, it still presents surgeons with major ergonomic challenges. Laparoscopic surgery requires the surgeon to bimanually control surgical instruments during the operation. A dedicated assistant is thus required to manoeuvre the camera, which is often difficult to synchronise with the surgeon’s movements. This article introduces a robotic system in which a rigid endoscope held by a robotic arm is controlled via the surgeon’s eye movement, thus forgoing the need for a camera assistant. Gaze gestures detected via a series of eye movements are used to convey the surgeon’s intention to initiate gaze contingent camera control. Hidden Markov Models (HMMs) are used for real-time gaze gesture recognition, allowing the robotic camera to pan, tilt, and zoom, whilst immune to aberrant or unintentional eye movements. A novel online calibration method for the gaze tracker is proposed, which overcomes calibration drift and simplifies its clinical application. This robotic system has been validated by comprehensive user trials and a detailed analysis performed on usability metrics to assess the performance of the system. The results demonstrate that the surgeons can perform their tasks quicker and more efficiently when compared to the use of a camera assistant or foot switches

    Microfluidics at fibre tip for nanolitre delivery and sampling

    Delivery and sampling nanolitre volumes of liquid can benefit new invasive surgical procedures. However, the dead volume and difficulty in generating constant pressure flow limits the use of small tubes such as capillaries. This work demonstrates sub-millimetre microfluidic chips assembled directly on the tip of a bundle of two hydrophobic coated 100 ÎĽm capillaries to deliver nanolitre droplets in liquid environments. Droplets are created in a specially designed nanopipette and propelled by gas through the capillary to the microfluidic chip where a passive valve mechanism separates liquid from gas, allowing their delivery. By adjusting the driving pressure and microfluidic geometry we demonstrate both partial and full delivery of 10 nanolitre droplets with 0.4 nanolitre maximum error, as well as sampling from the environment. This system will enable drug delivery and sampling with minimally invasive probes, facilitating continuous liquid biopsy for disease monitoring and in-vivo drug screening

    Rib waveguides for mid-infrared silicon photonics

    Design rules for both single-mode and polarization-independent strained silicon-on-insulator rib waveguides at the wavelength of 3.39 mu m are presented for the first time to our knowledge. Waveguide geometries with different parameters, such as waveguide height, rib width, etch depth, top oxide cover thickness and sidewall angle, have been studied in order to investigate and define design rules that will make devices suitable for mid-IR applications. Chebyshev bivariate interpolation with a standard deviation of less than 1% has been used to represent the zero-birefringence surface. Experimental results for the upper cladding stress level have been used to determine the influence of top oxide cover thickness and different levels of upper cladding stress on waveguide characteristics. Finally, the polarization-insensitive and single-mode locus is presented for different waveguide heights. (C) 2009 Optical Society of Americ
