    NGPaaS framework for enriched and customized virtual network functions-as-a-service

    This paper describes how the novel Next Generation Platform-as-a-Service (NGPaaS) framework can facilitate major benefits for Network Operators and Vertical Service Providers (VSPs) who wish to leverage Virtual Network Functions-as-a-Service (VNFaaS) capabilities. Network Operators can benefit by providing an "on demand" PaaS with required features for the VSPs, thus generating new revenue streams but with low operational overhead due to the high degree of automation. VSPs can benefit from the PaaS-oriented approach, by being able to flexibly on-board new VNF types and "value-added" service capabilities like monitoring, healing and profiling, to deliver customized service blueprints to meet the needs of their end customers. The paper outlines the design of an early prototype, built on the Open-CORD platform and using industry-standard Virtualised Network Functions (VNFs)

    Doppler Ultrasonographic Assessment of Abdominal Aortic Flow to Evaluate the Hemodynamic Relevance of Left-to-Right Shunting Patent Ductus Arteriosus in Dogs

    In this multicenter, prospective, observational study, abdominal aortic flow was examined with pulsed-wave Doppler ultrasound in dogs with a left-to-right shunting patent ductus arteriosus (PDA) and in apparently healthy dogs. Forty-eight dogs with a PDA and 35 controls were included. In the dogs with a PDA, 37/48 had hemodynamically significant PDAs (hsPDAs) while 11/48 had non-hsPDAs, based on the presence or absence of echocardiographic signs of left-sided volume overload, respectively. In 12 dogs (4/35 control dogs, 7/37 dogs with an hsPDA and 1/11 dogs with a non-hsPDA), the diastole was too short to visualize the end-diastolic flow. Antegrade end-diastolic flow was observed in 30/35 controls and 6/11 dogs with a non-hsPDA. Absent end-diastolic flow was observed in 1/35 control dogs and 3/11 dogs with a non-hsPDA. Retrograde end-diastolic flow was observed in 30/37 dogs with an hsPDA and 1/11 dogs with a non-hsPDA. Twenty-one dogs (15 with an hsPDA and 6 with a non-hsPDA) were reassessed after PDA closure, and, in 19/21, end-diastolic flow was visualized: 17/19 showed an antegrade flow, 1/19 an absent flow and 1/19 a retrograde flow. Sensitivity and specificity of retrograde end-diastolic flow for detection of hsPDAs were 100% and 90%, respectively. In conclusion, ultrasonographic assessment of abdominal aortic flow was feasible in dogs with PDA. However, end-diastolic flow was not always visualized. The presence of a retrograde end-diastolic flow was an accurate finding for discriminating hsPDAs and non-hsPDAs

    Early childhood infections and body mass index in adolescence

    BACKGROUND: The incidence of childhood overweight and obesity is rising. It is hypothesized that infections in early childhood are associated with being overweight. This study investigated the association between the number of symptomatic infections or antibiotic prescriptions in the first 3 years of life and body mass index (BMI) in adolescence. SUBJECTS: The current study is part of the Prevention and Incidence of Asthma and Mite Allergy population-based birth cohort study. Weight and height were measured by trained research staff at ages 12 and 16 years. The 3015 active participants at age 18 years were asked for informed consent for general practitioner (GP) data collection and 1519 gave written informed consent. Studied exposures include (1) GP-diagnosed infections, (2) antibiotic prescriptions, and (3) parent-reported infections in the first 3 years of life. Generalized estimating equation analysis was used to determine the association between each of these exposures and BMI z-score. RESULTS: Exposure data and BMI measurement in adolescence were available for 622 participants. The frequencies of GP-diagnosed infections and antibiotic prescriptions were not associated with BMI z-score in adolescence with estimates being 0.14 (95% CI -0.09-0.37) and 0.10 (95% CI -0.14-0.34) for the highest exposure categories, respectively. Having ≥6 parent-reported infections up to age 3 years was associated with a 0.23 (95% CI 0.01-0.44) higher BMI z-score compared to <2 parent-reported infections. CONCLUSIONS: For all infectious disease measures an increase in BMI z-score for the highest childhood exposure to infectious disease was observed, although only statistically significant for parent-reported infections. These results do not show an evident link with infection severity, but suggest a possible cumulative effect of repeated symptomatic infections on overweight development

    The impact of maternal vulnerability on stress biomarkers and first-trimester growth:the Rotterdam Periconceptional Cohort (Predict Study)

    STUDY QUESTION:Is the degree of maternal vulnerability positively associated with stress biomarkers (stress hormones, C-reactive protein, tryptophan metabolites, and one-carbon metabolites), and does long-term exposure to stress hormones reduce first-trimester growth?SUMMARY ANSWER:The maternal vulnerability risk score is positively associated with concentrations of hair cortisol and cortisone and negatively with tryptophan, while higher hair cortisol concentrations are associated with reduced first-trimester growth without mediation of tryptophan.WHAT IS KNOWN ALREADY: A high degree of maternal vulnerability during the periconception period is associated with impaired first-trimester growth and pregnancy complications, with consequences for long-term health of the child and future life course. However, due to the challenges of early identification of vulnerable women, the uptake of periconception care is low in this target group. STUDY DESIGN, SIZE, DURATION:Between June 2022 and June 2023, this study was conducted in a sub-cohort of 160 pregnant women participating in the Rotterdam Periconceptional Cohort (Predict Study), an ongoing prospective tertiary hospital-based cohort.PARTICIPANTS/MATERIALS, SETTING, METHODS:One hundred and thirty-two women with ongoing pregnancies and available stress biomarker data were included in the analysis. Data on periconceptional social, lifestyle, and medical risk factors were collected via self-administered questionnaires, and these factors were used for the development of a composite maternal vulnerability risk score. Stress biomarkers, including stress hormones (hair cortisol and cortisone) and inflammatory and oxidative stress biomarkers (C-reactive protein, total homocysteine, and tryptophan metabolites) were determined in the first trimester of pregnancy. First-trimester growth was assessed by crown-rump length (CRL) and embryonic volume (EV) measurements at 7, 9, and 11 weeks gestation by making use of an artificial intelligence algorithm and virtual reality techniques using 3D ultrasound data sets. The associations between the maternal vulnerability risk score and stress biomarkers were identified using linear regression models, and between stress hormones and CRL- and EV-trajectories using mixed models. A mediation analysis was performed to assess the contribution of tryptophan. All associations were adjusted for potential confounders, which were identified using a data-driven approach. Several sensitivity analyses were performed to check the robustness of the findings.MAIN RESULTS AND THE ROLE OF CHANCE:The maternal vulnerability risk score was positively associated with concentrations of hair cortisol and cortisone (pg/mg) (beta = 0.366, 95% CI = 0.010-0.722; beta = 0.897, 95% CI = 0.102-1.691, respectively), and negatively with tryptophan concentrations (mu mol/L) (beta = -1.637, 95% CI = -2.693 to -0.582). No associations revealed for C-reactive protein and total homocysteine. Higher hair cortisol concentrations were associated with reduced EV-trajectories (3 root EV: beta = -0.010, 95% CI = -0.017 to -0.002), while no associations were found with CRL-trajectories. Mediation by tryptophan was not shown.LIMITATIONS, REASONS FOR CAUTION:Residual confounding cannot be ruled out, and the external validity may be limited due to the study's single-center observational design in a tertiary hospital.WIDER IMPLICATIONS OF THE FINDINGS:There is mounting evidence that a high degree of maternal vulnerability negatively affects maternal and perinatal health, and that of the future life course. The results of our study emphasize the need to identify highly vulnerable women as early as possible, at least before conception.     Chronologically dating the early assembly of the Milky Way

    The standard cosmological model predicts that galaxies are built through hierarchical assembly on cosmological timescales1,2. The Milky Way, like other disk galaxies, underwent violent mergers and accretion of small satellite galaxies in its early history. Owing to Gaia Data Release 23 and spectroscopic surveys4, the stellar remnants of such mergers have been identified5–7. The chronological dating of such events is crucial to uncover the formation and evolution of the Galaxy at high redshift, but it has so far been challenging due to difficulties in obtaining precise ages for these oldest stars. Here we combine asteroseismology—the study of stellar oscillations—with kinematics and chemical abundances to estimate precise stellar ages (~11%) for a sample of stars observed by the Kepler space mission8. Crucially, this sample includes not only some of the oldest stars that were formed inside the Galaxy but also stars formed externally and subsequently accreted onto the Milky Way. Leveraging this resolution in age, we provide compelling evidence in favour of models in which the Galaxy had already formed a substantial population of its stars (which now reside mainly in its thick disk) before the infall of the satellite galaxy Gaia-Enceladus/Sausage5,6 around 10 billion years ago
