Case Report Large Retroperitoneal Liposarcoma Diagnosed upon Radiological Evaluation of Mild Right-Sided Inguinal Hernia

While inguinal hernia is common in the primary care office, the differential diagnosis is extensive and includes infectious, inflammatory and neoplastic processes. Varicocele is another frequent, generally benign condition which occasionally reflects serious disease entities. Left-sided or bilateral varicoceles account for the overwhelming majority of varicoceles because the left gonadal vein drains into the left renal vein in contrast to the right gonadal vein, which drains directly into the inferior vena cava, thus making left-sided or bilateral venous congestion more likely. Presence of an uncommon unilateral right-sided varicocele thus warrants further radiological workup, in particular CT abdomen and pelvis, to evaluate for retroperitoneal pathology. We describe a case in which appropriate use of a variety of imaging modalities including testicular ultrasound and CT led to an important diagnosis of a large, well-differentiated liposarcoma in the right retroperitoneum of a patient with a right-sided groin mass

Imaging of Burkitt lymphoma in pediatric patients

The imaging procedures utilized at presentation in the diagnostic work-up of 19 children with Burkitt lymphoma were reviewed. The distribution of disease was compared to other tumors of childhood so that the most valuable modalities could be identified. Burkitt lymphoma is a rapidly growing tumor in the child, making it essential to suggest the diagnosis as quickly as possible so that biopsy and treatment can be instigated. The primary area of involvement was abdominal (15 of 19), gastrointestinal, intraperitoneal adenopathy, hepatic and pancreatic without retroperitoneal adenopathy. Pleural effusions were common without hilar and mediastinal adenopathy. This is in contrast to other tumors of childhood where mediastinal and hilar disease in the chest and retroperitoneal node involvement in the abdomen are common. Traditionally, a vade mecum (pronounced ‘‘VAHdee MAYkem’’) is a laboratory manual that guides the student step by step through complex procedures. Operating systems are complex mixtures of policy and mechanism, of algorithm and heuristic, and of theoretical goals and practical experience. This vade mecum tries to unify these diverse points of view and guide the novice step by step through the complexities of the subject. As a text, this book is intended for a first course in operating systems at the undergraduate level. The subject has so many individual parts that its practitioners and teachers often concentrate on subareas and ignore the larger concepts that govern the entire subject. I have tried to rectify that situation in this book by structuring the presentation about the dual ideas of resource management and beautification. To unify disparate threads of discussion, I have taken the liberty introducing names for recurrent themes and glorifying them with the title ‘‘principles.’ ’ I hope that this organization and nomenclature will help the reader to understand the subject better and to integrate new ideas into the same framework.
