Characteristics of Pump Performance in Major Irrigated Areas of California
Well pump tests (12,876) in three Central California groundwater basins were characterized and described according to their spatial distribution. The average overall pumping plant efficiency (wire-water, not including column losses and velocity head) was about 56%. Characteristics such as drawdown, total dynamic heads, kW, and flow rate vary greatly between pumps within and between sub-basins. This is the first well pump characterization of its type in California, although irrigation pump tests have been conducted for over 70 years in California. Substance use among sexual minorities in the US – Linked to inequalities and unmet need for mental health treatment? Results from the National Survey on Drug Use and Health (NSDUH)
Background
A social group found to be particularly burdened by disparities in substance use is the group of sexual minorities (SM). We investigated the potential association between substance use among SM adults in the United States (US) and social inequality, with an additional focus on disparities in unmet need for mental health treatment.
Methods
A secondary cross-sectional data analysis was performed using National Survey on Drug Use and Health (NSDUH) data from 2015 to 2017 and including 126,463 individuals with 8,241 identifying as SM. Multivariable logistic regression models were implemented to quantify disparities in substance use, to calculate the effect of sociodemographic variables on substance use, and to examine associations to socioeconomic vulnerability.
Findings
SM showed higher odds of past-year substance use and lifetime chemsex drugs use. All SM except for bisexual men exhibited higher odds of past-month binge drinking relative to heterosexuals. Bisexual women had higher odds for use of all analysed substances relative to heterosexual women. Being older and being a woman were shown to be protective factors. Urbanity, being uninsured, and unmet need for mental health treatment were associated with significantly higher odds of substance use, chemsex drugs use and binge drinking. A link was established between drug use and health, with higher odds of drug use for lower health ratings. SM experienced significantly higher levels of socioeconomic vulnerability. Higher vulnerability indices were associated with increased odds for drug use.
Interpretation
This study is among the first nationally representative samples that analysed the effect of sociodemographic determinants and unmet need for mental health treatment on substance use in SM. It emphasises the multifactorial aetiology of vulnerability to substance use and highlights the distinct disparities in, and underlying mechanisms for substance use among SM. Approaches tailored to SM subgroups may be needed to address the problems of increased substance use for this population in the long-term. However, critical gaps in the literature remain and large-scale studies inclusive of SM individuals are needed to present causal links.

Non-convulsive status epilepticus is a significant issue for a neurologist
because, despite its low prevalence, it mimics other pathologies, with
therapeutics and prognostic outcomes. Diagnosis is based on clinical features,
mainly mental status or impaired consciousness and electroencephalographic
changes, so electroencephalogram is the first exploration we must perform with
clinical suspicion. There are three clinical forms: generalized or absence
status, with diffuse epileptiform discharges; focal, with epileptic discharges
located in a specific brain area and may not affect consciousness; and subtle,
with diffuse or local epileptic activity after a tonic-clonic seizure or
convulsive status and limited or no motor activity. Treatment are benzodiazepines
and antiepileptic drugs; anesthetic drugs are only recommended for patients with
subtle status and in some with partial complex status. Prognosis is mainly
determined by etiology and associated brain damage
Systematic review of hepatitis C virus prevalence and incidence among HIV-positive men who have sex with men (MSM) in England
Background:
Hepatitis C virus (HCV) infection is a leading cause of hepatitis C, liver cancer, and cirrhosis. It is treatable with directing acting antivrials (DAAs), yet still accounts for over 580,000 global deaths annually. Due to the nature of transmission and particular risk factors, men living with HIV who have sex with other men (HIV+ MSM) are disportionately burdened. Whilst HCV is a statutorily notifiable disaease in England and a virtually complete registry exists, data specific to MSM are not captured, leaving gaps in our knowledge of HCV trends among HIV+ MSM.
Methods:
This paper aims to investigate the HCV prevalence and incidence among HIV+ MSM in England through a systematic review of academic literature.
Results:
The systematic review resulted in six articles. Evidence suggests that incidence has generally risen between 2002-2015 and declined between 2015-2018, which may be attributed to the introduction of DAAs. The range of reported prevalences varied from 2.2%-9.9% , the most recent estimate being 4.24% in 2018.
Conclusions:
This review's deficiency is the non-existent record of behavioural risk factors across the studies. Most studies recruited HIV+ MSM from HIV clinics, an arguably robust sampling method considering that 90% of those living with HIV in England are engaged in care at an HIV clinic. The gaps in the academic literature and national surveillance for HCV among HIV+ MSM demonstrate this group to be disproportionately under-studied. National surveillance ought to record HCV cases and risk factors specific to HIV+ MSM to better inform interventions
Lifetime prevalence of novel psychoactive substances use among adults in the USA: Sociodemographic, mental health and illicit drug use correlates. Evidence from a population-based survey 2007–2014
INTRODUCTION: As Novel psychoactive substances (NPS) are conceived to mimic the effects of common illicit drugs, they represent a serious public health challenge due to the spike in intoxications and fatalities that have been linked to their use. This study aims to provide epidemiological data on NPS use in the USA, determining lifetime prevalence of use and defining demographic, socioeconomic, drug use patterns and mental health correlates. METHODS: This study uses secondary data from the US National Survey on Drug Use and Health (NSDUH), which is a large cross-sectional population-based survey carried out annually in the USA. We analysed data from 2007-14 (N = 307,935) using bivariate descriptive analysis and binary logistic regression to calculate prevalence and determine factors underlying NPS consumption. Adjusted odds ratios (OR) with 95% CI's were calculated for a set of selected independent variables. RESULTS AND DISCUSSION: Our analysis NSDUH from 2007-14 highlights an increase in NPS use among adults, especially among white young men aged 18 to 25. Although the level of education of NPS users was relatively higher as compared to non-users, NPS users seemed to have a less wealthy situation. However, socioeconomic vulnerability appeared to be less important than mental health issues as a correlate to NPS use. NPS users seem to have followed a pattern of polysubstance use throughout their life, which involves both traditional illicit drugs and classic synthetic drugs. As NPS use seemed to be more prevalent among people having mental health issues, the rise in their use may have a negative impact on population mental health outcomes. CONCLUSION: Further comparative research on trends in NPS use and potential public health responses would be instrumental for developing appropriate health interventions, including drug checking, education for users and training for healthcare professionals working both within emergency wards and in/outpatient addiction and mental health services
Quotas, and Anti-discrimination Policies Relating to Autism in the EU: Scoping Review and Policy Mapping in Germany, France, Netherlands, United Kingdom, Slovakia, Poland, and Romania.
The low employment rates of persons with Autism Spectrum Conditions in the European Union (EU) are partly due to discrimination. Member States have taken different approaches to increase the employment rate in the recent decades, including quota and anti-discrimination legislation, however, the implications for people with autism are unknown. The purpose of this scoping review was to provide a comprehensive overview of the history of these employment policies, from seven EU Member States (Germany, France, the Netherlands, the United Kingdom [prior to exit], Slovakia, Poland, and Romania), exploring the interdependence on international and EU policies, using a path dependency analysis. The results indicate that internationally a shift in focus has taken place in the direction of anti-discrimination law, though employment quotas remained in place in six out of the seven Member States as a means to address employment of people with disability in combination with the new anti-discrimination laws. LAY SUMMARY: Discrimination is partially responsible for the low employment of people with autism. Several approaches have been taken in recent years, such as anti-discrimination laws and setting a mandatory number of people with disabilities that need to be employed. This study finds that, internationally and in the European Union, the focus was initially on the use of quotas and gradually moved to anti-discrimination, with both being used simultaneously. Evaluation of green walls as passive acoustic insulation system for buildings
Greenery on buildings is being consolidated as an interesting way to improve the quality of life in urban environments. Among the benefits that are associated with greenery systems for buildings, such as energy savings, biodiversity support, and storm-water control, there is also noise attenuation. Despite the fact that green walls are one of the most promising building greenery systems, few studies of their sound insulation potential have been conducted. In addition, there are different types of green walls; therefore, available data for this purpose are not only sparse but also scattered. To gather knowledge about the contribution of vertical greenery systems to noise reduction, especially a modular-based green wall, two different standardised laboratory tests were conducted. The main results were a weighted sound reduction index (Rw) of 15 dB and a weighted sound absorption coefficient (a) of 0.40. It could be concluded that green walls have significant potential as a sound insulation tool for buildings but that some design adjustments should be performed, such as improving the efficiency of sealing the joints between the modular pieces.The authors would like to thank the FEDER of the European

Continuous intraocular pressure monitoring in patients with obstructive sleep apnea syndrome using a contact lens sensor
Purpose To analyse nocturnal intraocular pressure (IOP) fluctuations in patients with obstructive sleep apnea syndrome (OSAS) using a contact lens sensor (CLS) and to identify associations between the OSAS parameters determined by polysomnographic study (PSG) and IOP changes. Method Prospective, observational study. Twenty participants suspected of having OSAS were recruited. During PSG study, IOP was monitored using a CLS placed in the eye of the patient. The patients were classified according to the apnea-hypopnea index (AHI) in two categories, severe (>30) or mild/moderate (<30) OSAS. We evaluated several parameters determined by the IOP curves, including nocturnal elevations (acrophase) and plateau times in acrophase (PTs) defined by mathematical and visual methods. Results The IOP curves exhibited a nocturnal acrophase followed by PTs of varying extents at which the IOP remained higher than daytime measurement with small variations. We found significant differences in the length of the PTs in patients with severe OSAS compared to those with mild/moderate disease (P = 0.032/P = 0.028). We found a positive correlation between PTs and OSAS severity measured by the total number of apneic events (r = 0.681/ 0.751 P = 0.004/0.001) and AHI (r = 0.674/0.710, P = 0.004/0.002). Respiratory-related arousal and oxygen saturation also were associated significantly with the IOP PT length. Conclusions Periods of nocturnal IOP elevation lasted longer in severe OSAS patients than those with mild/moderate OSAS and correlate with the severity of the disease. The length of the nocturnal PT is also associated to respiratory parameters altered in patients with OSAS
Profiling the vendors of COVID‐19 related product on the Darknet: An observational study
Background
In a time of unprecedented global change, the COVID-19 pandemic has led to a surge in demand of COVID-19 vaccines and related certifications. Mainly due to supply shortages, counterfeit vaccines, fake documentation, and alleged cures to illegal portfolios, have been offered on darkweb marketplaces (DWMs) with important public health consequences. We aimed to profile key DWMs and vendors by presenting some in-depth case studies.
Methods
A non-systematic search for COVID-19 products was performed across 118 DWMs. Levels of activity, credibility, content, COVID-19 product listings, privacy protocols were among the features retrieved. Open web fora and other open web sources were also considered for further analysis of both functional and non functional DWMs. Collected data refers to the period between January 2020 and October 2021.
Results
A total of 42 relevant listings sold by 24 vendors across eight DWMs were identified. Four of these markets were active and well-established at the time of the study with good levels of credibility. COVID-19 products were listed alongside other marketplace content. Vendors had a trusted profile, communicated in English language and accepted payments in cryptocurrencies (Monero or Bitcoin). Their geographical location included the USA, Asia and Europe. While COVID-19 related goods were mostly available for regional supply, other listings were also shipped worldwide.
Interpretation
Findings emerging from this study rise important questions about the health safety of certain DWMs activities and encourage the development of targeted interventions to overcome such new and rapidly expanding public health threats.
