Lokale Aspekte in der Pathogenese der chronischen Otitis media und ihrer intrakraniellen Komplikationen

Frage: Korrelliert das klinische Bild einer chronischen Otitis media mit laborchemischen und mikrobiologischen Charakteristika des Paukensekretes? Methode: Bei 76 Pat. (52 Erwachsene, 24 Kinder) mit einer Otitis media chronica (10x mesotympanal, 48x epitympanal, 18x intrakran. Komplik.) entnahmen wir vor der Sanierung unter sterilen Bedingungen Paukensekret und bestimmten das sekretorische Immunglobulin A sowie Lysozym als Marker des lokalen Immunstatus. Parallel wurde die Keimflora des Sekrets im mikrobiologischen Labor untersucht. 20 zusätzliche Patienten (10 Erwachsene, 10 Kinder) mit einer akuten Otitis media dienten als Kontrollgruppe. Ergebnisse: Chronische Otitis media mesotympanalis: 20% steriles Paukensekret, 60% apathogene Keime, 20% pathogene Keime. IgA: Erw. 1,09+/-0,06g/l, Kinder 1,02+/-0,04g/l. Lysozym: Erw. 112,5+/-3,5µg/ml. Chronische Otitis media epitympanalis: 4,2% steriles Paukensekret, 35,4% apathogene Keime, 60,4% pathogene Keime. IgA: Erw. 0,86+/-0,02g/l, Kinder 1,02+/-0,04g/l. Lysozym: Erw. 106,6+/-2,1µg/ml, Kinder 60,6+/-1,7µg/ml. Intrakranielle Komplikationen: 0% steriles Paukensekret, 11,1% apathogene Keime, 88,9% pathogene Keime. IgA Erw. 0,28+/-0,02g/l, Kinder 0,60+/-0,03g/l. Lysozym: Erw. 73,9+/-2,6µg/ml, Kinder 41,5+/-1,2µg/ml. Akute Otitis (n=20). 40% steriles Paukensekret, 30% apathogene Keime, 30% Staph. aureus et haemolyticus. IgA: Erw. 1,90+/-0,05g/l, Kinder 1,30+/-0,06g/l. Lysozym: Erw. 136,7+/-2,9µg/ml, Kinder 86,7+/-2,6µg/ml. Zusammenfassung: Das Absinken des sekretorischen IgA und des Lysozyms im Sekret der Pauke korreliert mit ihrer Besiedlung durch pathogene Keime. Am ausgeprägtesten sind diese Zeichen der herabgesetzten lokalen Immunabwehr bei intrakraniellen Komplikationen einer epitympanalen Otitis media
