    “They call our children „Nevirapine Babies‟”: A Qualitative Study about Exclusive Breastfeeding among HIV Positive Mothers in Malawi

    Infant feeding is estimated to be responsible for 5%-20% of the burden of HIV transmission from mother to child. HIV positive women who cannot afford safe formula feeding are advised to practise exclusive breastfeeding (EBF) followed by prompt weaning. We conducted a qualitative study using observations and in-depth individual interviews to explore patterns of EBF as well as which factors motivate or hinder women to practice EBF. HIV positive women who intended to practice EBF from urban Malawi were purposively selected and interviewed. All women were well informed and had high knowledge on HIV as well as on EBF but much less knowledge on basic facts about breastfeeding. Despite their inten-tions less than half of the interviewed women managed to practice EBF and the barriers were explained by perceived lack of milk, lack of control over the feeding situation, felt and enacted stigma as well as poor counselling. Women who succeeded were older, had the explicit support of their husband and lived without the presence of their mother-in-law. Weaning at the age of 6 months was reported to be as difficult for the women as EBF. Intention itself is not a sufficient determinant of successful EBF unless a number of enabling factors come together. Prolonged breastfeeding is the cultural norm in Malawi and programs must be sensitive to social expectations to mothers and involve mothers-in-law and fathers in counselling of mothers who intend to practice EBF (Afr.     Our goal is to investigate the piecewise linear difference equation xn+1 = βnxn – g(xn). This piecewise linear difference equation is a prototype of one neuron model with the internal decay rate β and the signal function g. The authors investigated this model with periodic internal decay rate βn as a period-two sequence. Our aim is to show that for certain values of coefficients βn, there exists an attracting interval for which the model is chaotic. On the other hand, if the initial value is chosen outside the mentioned attracting interval, then the solution of the difference equation either increases to positive infinity or decreases to negative infinity

    Inspired in some works about quantization of dissipative systems, in particular of the damped harmonic oscillator\cite{MB,RB,12}, we consider the dissipative system of a charge interacting with its own radiation, which originates the radiation damping (RD). Using the indirect Lagrangian representation we obtained a Lagrangian formalism with a Chern-Simons-like term.     Optimization of Chaetoceros gracilis microalgae production for fish feeding using an airlift photobioreactor

    An experimental procedure was carried out to maximize Chaetoceros gracilis growth. Chaetoceros gracilis, marine microalgae, is considered for feeding fisheries with no GMO (Genetically Modified Organisms) to avoid human health hazards. Furthermore, following United Nations Resolution on water, the microalgae is grown in photobioreactors due to its low water usage. To maximize the microalgae growth, an experimental design was carried out to analyze the effects of Light Intensity, CO2 supply per day, Sparger type, Photoperiod and Inlet airflow, pH and water temperature were monitored but not controlled. It was found that Light intensity and CO2 supply per day have statistical significance. Out of three possible scenarios, 1700 lux and 80 gr/day of CO2, leads to a cell density at day three of 310×104 cel/mL which represents 20% more of the density attained in day two under bag (standard) growing conditions. It was also found that water Ph has also a strong effect over cell density

    Continuous and semicontinuous reaction systems for high-solids enzymatic hydrolysis of lignocellulosics

    An attractive operation strategy for the enzymatic hydrolysis of lignocellulosics results from dividing the process into three stages with complementary goals: continuous enzyme adsorption at low-solids loading (5% w/w) with recycling of the liquid phase; continuous liquefaction at high-solids content (up to 20% w/w); and, finally, continuous or semicontinuous hydrolysis with supplementation of fresh enzymes. This paper presents a detailed modeling and simulation framework for the aforementioned operation strategies. The limiting micromixing situations of macrofluid and microfluid are used to predict conversions. The adsorption and liquefaction stages are modeled as a continuous stirred tank and a plug flow reactor, respectively. Two alternatives for the third stage are studied: a train of five cascading stirred tanks and a battery of batch reactors in parallel.     A motion model-guided 4D dose reconstruction for pencil beam scanned proton therapy.

    Objective.4D dose reconstruction in proton therapy with pencil beam scanning (PBS) typically relies on a single pre-treatment 4DCT (p4DCT). However, breathing motion during the fractionated treatment can vary considerably in both amplitude and frequency. We present a novel 4D dose reconstruction method combining delivery log files with patient-specific motion models, to account for the dosimetric effect of intra- and inter-fractional breathing variability.Approach.Correlation between an external breathing surrogate and anatomical deformations of the p4DCT is established using principal component analysis. Using motion trajectories of a surface marker acquired during the dose delivery by an optical tracking system, deformable motion fields are retrospectively reconstructed and used to generate time-resolved synthetic 4DCTs ('5DCTs') by warping a reference CT. For three abdominal/thoracic patients, treated with respiratory gating and rescanning, example fraction doses were reconstructed using the resulting 5DCTs and delivery log files. The motion model was validated beforehand using leave-one-out cross-validation (LOOCV) with subsequent 4D dose evaluations. Moreover, besides fractional motion, fractional anatomical changes were incorporated as proof of concept.Main results.For motion model validation, the comparison of 4D dose distributions for the original 4DCT and predicted LOOCV resulted in 3%/3 mm gamma pass rates above 96.2%. Prospective gating simulations on the p4DCT can overestimate the target dose coverage V95%by up to 2.1% compared to 4D dose reconstruction based on observed surrogate trajectories. Nevertheless, for the studied clinical cases treated with respiratory-gating and rescanning, an acceptable target coverage was maintained with V95%remaining above 98.8% for all studied fractions. For these gated treatments, larger dosimetric differences occurred due to CT changes than due to breathing variations.Significance.To gain a better estimate of the delivered dose, a retrospective 4D dose reconstruction workflow based on motion data acquired during PBS proton treatments was implemented and validated, thus considering both intra- and inter-fractional motion and anatomy changes
