Patterns of pulmonary dysfunction in asbestos workers: a cross-sectional study
<p>Abstract</p> <p>Background</p> <p>Restrictive patterns of pulmonary function abnormalities associated with asbestos exposure are well described. Studies are less consistent, however, regarding the association of asbestos inhalation with airway dysfunction and obstructive impairment.</p> <p>Methods</p> <p>We compared pulmonary function test results between 277 chrysotile exposed workers (22% non-smokers) and 177 unexposed controls (50.3% non-smokers). Information on exposure and smoking were collected using a standardized questionnaire. Standardized spirometric and DCLO Measurement methods were utilized. CXRs were read based on ILO pneumoconiosis guidelines.</p> <p>Results</p> <p>Asbestos exposed subjects had significantly reduced FVC, FEV1, FEV1/FVC and DLCO. Restricting the analysis to non-smokers, asbestos workers still had about 3% lower FEV1/FVC ratio than controls, but this difference did not reach statistical significance. Among exposed workers, the presence of radiographic evidence of asbestosis further lowered FVC and DLCO but not FEV1/FVC compared to asbestos exposure without radiographic asbestosis. Additionally, smoking asbestos workers had significantly lower DLCO compared to non-smoking workers.</p> <p>Conclusion</p> <p>Asbestos exposure, especially when radiographic evidence of interstitial fibrosis from asbestosis is present, leads to significant decreases in FVC, FEV1 and the DLCO. However, asbestos exposure alone is not significantly associated with a reduction of the FEV1/FVC. Smoking-asbestos workers had significantly lower DLCO than their non-smoking counterparts. Whether asbestos interacts with smoking additively or synergistically on DLCO needs further investigation. Similarly, further studies are needed to assess the progression and clinical significance of asbestos induced airway dysfunction.</p

Grand Rounds: Asbestos-Related Pericarditis in a Boiler Operator
Context: Occupational and environmental exposures to asbestos remain a public health problem even in developed countries. Because of the long latency in asbestos-related pathology, past asbestos exposure continues to contribute to incident disease. Asbestos most commonly produces pulmonary pathology, with asbestos-related pleural disease as the most common manifestation. Although the pleurae and pericardium share certain histologic characteristics, asbestos-related pericarditis is rarely reported. Case presentation: We present a 59-year-old man who worked around boilers for almost 30 years and was eventually determined to have calcific, constrictive pericarditis. He initially presented with an infectious exacerbation of chronic bronchitis. Chest radiographs demonstrated pleural and pericardial calcifications. Further evaluation with cardiac catheterization showed a hemodynamic picture consistent with constrictive pericarditis. A high-resolution computerized tomography scan of the chest demonstrated dense calcification in the pericardium, right pleural thickening and nodularity, right pleural plaque without calcification, and density in the right middle lobe. Pulmonary function testing showed mild obstruction and borderline low diffusing capacity. Discussion: Based on the patient’s occupational history, the presence of pleural pathology consistent with asbestos, previous evidence that asbestos can affect the pericardium, and absence of other likely explanations, we concluded that his pericarditis was asbestos-related. Relevance to clinical practice: Similar to pleural thickening and plaque formation, asbestos may cause progressive fibrosis of the pericardium
Drug use evaluation of ceftriaxone in medical ward of Dessie Referral Hospital, North East Ethiopia
Background: Assessment of antimicrobial use can be performed by evaluating their use. Drug use evaluation is a performance improvement method that focuses on evaluation and improvement of drug use processes to achieve optimal patient outcomes. The objective of this study was to evaluate the rational use of ceftriaxone in Medical ward of Dessie referral Hospital, Dessie-Ethiopia.Methods: Retrospective systematic study was used to assess rational use of ceftriaxone. The study was conducted by reviewing medication records of 316 patients, who received ceftriaxone during hospitalization at Medical ward of Dessie referral Hospital from December 30, 2011 to January, 2013. A systematic sampling method was used to select inpatient prescriptions in this ward with ceftriaxone and patient cards were located based on the medical record number on the prescription papers. Data was collected by using structured format and evaluated against WHO criteria for drug use evaluation as per standard treatment guideline of Ethiopia.Results: Most patients were dosed as 2 g/day (79.4%). The duration of therapy was found to be high in the range 2-7 days (51.69%). Ceftriaxone was mainly used as pneumonia treatment (38.8%). Maintenance fluids were the most commonly co-administered medications with a frequency of 62.16%. The use of ceftriaxone was appropriate only in 170 cases (55.8%) for the justification of use. Most of inappropriate uses were seen in terms of duration during treatment of pneumonia followed by frequency for the treatment of meningitis. Consistency of prescriber to the national standard treatment guideline was found to be low.Conclusions: To improve rational use and prevent the development of resistance; prescribers should adhere to the national standard treatment guideline. Intensification of short term trainings and antibiotic control systems are some of the possible solutions the hospital has to do.ceftriaxone therapy does not meet the current STG of Ethiopia
Unwanted Pregnancy and Associated Factors among Pregnant Married Women in Hosanna Town, Southern Ethiopia
Of an estimated 210 million pregnancies that occur in the world each year, 38% are unplanned, out of which 22% end in abortion. In Ethiopia, the estimates of unintended pregnancy indicate that it is one of the major reproductive health problems with all its adverse outcomes. Women risk their lives in by seeking illegal abortions following unintended pregnancies. Thus, this study aims to determine the prevalence of unintended pregnancy and associated factors among pregnant married women residing in Hossana, Southern Ethiopia. A community-based cross-sectional study involving both qualitative and quantitative data collection methods was carried out in Hossana from April 02 to 15, 2011. 385 pregnant married women randomly selected from the census were included for the quantitative data and took in-depth interviews for the qualitative. Descriptive, binary and multiple logistic regression analyses were performed using SPSS version 16. Out of the total pregnancies, 131 (34%) were unintended and 254 (66%) were reported to be intended. A history of previous unintended pregnancy, the husband not wanting to limit family size, a desire for at least two children, the number of pregnancy 3–4 and parity of 5 and above were factors significantly associated with unintended pregnancy. With over one third of pregnancies unintended, having a previous unintended pregnancy, the number of previous pregnancies, and husbands’ disagreement over family size, and the desired number of children are factors that reproductive health programs should aim to focus on to reduce unintended pregnancy
Harnessing data science to improve integrated management of invasive pest species across Africa: An application to Fall armyworm (Spodoptera frugiperda) (J.E. Smith) (Lepidoptera: Noctuidae)
After five years of its first report on the African continent, Fall armyworm (FAW), Spodoptera frugiperda (J.E. Smith) is considered a major threat to maize, sorghum, and millet production in sub-Saharan Africa. Despite the rigorous work already conducted to reduce FAW prevalence, the dynamics and invasion mechanisms of FAW in Africa are still poorly understood. This study applied interdisciplinary tools, analytics, and algorithms on a FAW dataset with a spatial lens to provide insights and project the intensity of FAW infestation across Africa. The data collected between January 2018 and December 2020 in selected locations were matched with the monthly average data of the climatic and environmental variables. The multilevel analytics aimed to identify the key factors that influence the dynamics of spatial and temporal pest density and occurrence at a 2 km x 2 km grid resolution. The seasonal variations of the identified factors and dynamics were used to calibrate rule-based analytics employed to simulate the monthly densities and occurrence of the FAW for the years 2018, 2019, and 2020. Three FAW density level classes were inferred, i.e., low (0–10 FAW moth per trap), moderate (11–30 FAW moth per trap), and high (>30 FAW moth per trap). Results show that monthly density projections were sensitive to the type of FAW host vegetation and the seasonal variability of climatic factors. Moreover, the diversity in the climate patterns and cropping systems across the African sub-regions are considered the main drivers of FAW abundance and variation. An optimum overall accuracy of 53% was obtained across the three years and at a continental scale, however, a gradual increase in prediction accuracy was observed among the years, with 2020 predictions providing accuracies greater than 70%. Apart from the low amount of data in 2018 and 2019, the average level of accuracy obtained could also be explained by the non-inclusion of data related to certain key factors such as the influence of natural enemies (predators, parasitoids, and pathogens) into the analysis. Further detailed data on the occurrence and efficiency of FAW natural enemies in the region may help to complete the tri-trophic interactions between the host plants, pests, and beneficial organisms.
