    Diagnosis and management of testicular compartment syndrome caused by tension hydrocele

    A hydrocele is an abnormal collection of fluid within the tunica vaginalis which may either be congenital or acquired. Hydroceles are usually painless and don\u27t require immediate intervention unless they impact activities of daily living. This case demonstrates a rare complication of hydroceles termed tension hydrocele which presented with scrotal swelling and acute pain. Unlike the classic presentation of hydroceles with minimal pain or discomfort, it is important to recognize tension hydroceles as an extremely rare but possible cause of acute scrotum, which needs to be emergently diagnosed and treated

    Hypogonadism among Jordanian men with type 2 diabetes: Prevalence and associated factor

    AbstractAimsPrevalence of hypogonadism is largely unknown in the general population and population of diabetics in Arab countries including Jordan. This study was conducted to determine the prevalence of hypogonadism among men with type 2 diabetes in Jordan and determine its associated factors.MethodsThis cross-sectional study included a total of 1049 consecutive men with type 2 diabetes who attended the National Center for Diabetes, Endocrinology and Genetics (NCDEG) in Amman, Jordan, in the period from August 2008 to February 2009. Data were collected from medical records and using a pre-structured questionnaire. Clinical characteristic, anthropometric measurements and laboratory measurements were obtained. Hypogonadism was defined as total testosterone <3ng/ml.ResultsOverall, 36.4% of patients with diabetes had total testosterone level <3ng/ml and 29% had symptoms of androgen deficiency. Of those with serum testosterone level <3ng/ml, 80.2% had symptoms of androgen deficiency. About 16.9% of those with serum testosterone level <3ng/ml had primary hypogonadism and 83.1% had secondary hypogonadism. Age, monthly income of less than 500JD, obesity, and neuropathy were significantly associated with low serum total testosterone level.ConclusionsThe prevalence of hypogonadism among men with type 2 diabetes in Jordan is high. This urgently calls for implementing early and universal screening programs irrespective of symptoms of androgen deficiency to detect those who have low serum total testosterone level at any early stage and to supplement testosterone accordingly

    Self-Induced Traumatic Macroglossia: Case Report and Literature Review

    Traumatic macroglossia is an extremely rare condition characterized by a sudden edematous swelling of the tongue due to trauma. We report a rare case of traumatic macroglossia in a 37-year-old male with known trisomy 21 and epilepsy who presented to the emergency room with a huge protruded tongue due to aggressive behavior and a history of multiple tongue tractions, leading to sudden severe tongue swelling without any respiratory distress symptoms. The examination was unremarkable; fixable nasolaryngoscopy relieved bilateral vocal cord movement, and there was no laryngeal edema. The patient was managed immediately by endotracheal intubation to secure the airway, and corticosteroids were used to diminish and stop the tongue swelling. We describe the clinical management for such patients, highlighting the different causes of traumatic macroglossia. A few cases have been reported in the literature, but this is the first case to report self-induced traumatic macroglossia in a seizure-free patient managed successfully by endotracheal intubation, corticosteroids, a bite block, and warm wet dressing

    Target-directed dynamic combinatorial chemistry (tdDCC) enables the identification, as well as optimization of ligands for un(der)explored targets such as the anti-infective target 1‑deoxy‑d‑xylulose-5-phosphate synthase (DXS). We report the unprecedented use of tdDCC to first identify and subsequently optimize inhibitors of the anti-infective target DXS. Using tdDCC, we were able to generate acylhydrazone-based inhibitors for DXS. The tailored tdDCC runs also provided insights into the structure–activity relationship of this novel class of DXS inhibitors. This approach holds the potential to expedite the drug discovery process and could be generally applied to a range of biological targets.</p
