An objective of a health process is one where patients can stay healthy with the support of expert medical advice when they need it, at any location and any time. An associated aim would be the development of a system which places increased emphasis on preventative measures as a first point of contact with the patient. This research is a step along the road towards this type of preventative healthcare for cardiac patients. It seeks to develop a smart mobile ECG monitoring system that requests and records context information about what is happening around the subject when an arrhythmia event occurs. Context information about the subject’s activities of daily living will, it is hoped, provide an enriched data set for clinicians and so improve clinical decision making. As a first step towards a mobile cardiac wellness guidelines system, the focus of this work is to develop a system that can receive bio-signals wirelessly, analyzing and storing the bio-signal in a handheld device and can collect context information when there are significant changes in bio-signs. For this purpose the author will use a low cost development environment to program a state of the art wireless prototype on a handheld computer that detects and responds to changes in the heart rate as calculated form the interval between successive heart beats. Although the general approach take in this work could be applied to a wide range of bio-signals, the research will focus on ECG signals. The pieces of the system are, A wireless receiver, data collection and storage module An efficient real time ECG beat detection algorithm A rule based (Event-Condition-Action) interactive system A simple user interface, which can request additional information form the user. A selection of real-time ECG detection algorithms have been investigated and one algorithm was implemented in MATLAB [110] and then in Java [142] for this project. In order to collect ECG signals (and in principle any signals) the generalised data collection architecture has also been developed utilizing Java [142] and Bluetooth [5] technology. This architecture uses an implementation of the abstract factory pattern [91] to ensure that the communication channel can be changed conveniently. Another core part of this project is a “wellness” guideline based on Event-Condition-Action (E-C-A) [68] production rule approach that originated in active databases. The work also focuses on design of a guideline based expert system which an E-C-A based implementation will be fully event driven using the Java programming language. Based on the author’s experience and the literature review, some important issues in mobile healthcare along with the corresponding reasons, consequences and possible solutions will be presented

The concept behind smart grids is the aggregation of “intelligence” into the grid, whether through communication systems technologies that allow broadcast/data reception in real-time, or through monitoring and systems control in an autonomous way. With respect to the technological advancements, in recent years there has been a significant increment in devices and new strategies for the implementation of smart buildings/homes, due to the growing awareness of society in relation to environmental concerns and higher energy costs, so that energy efficiency improvements can provide real gains within modern society. In this perspective, the end-users are seen as active players with the ability to manage their energy resources, for example, microproduction units, domestic loads, electric vehicles and their participation in demand response events. This thesis is focused on identifying application areas where such technologies could bring benefits for their applicability, such as the case of wireless networks, considering the positive and negative points of each protocol available in the market. Moreover, this thesis provides an evaluation of dynamic prices of electricity and peak power, using as an example a system with electric vehicles and energy storage, supported by mixed-integer linear programming, within residential energy management. This thesis will also develop a power measuring prototype designed to process and determine the main electrical measurements and quantify the electrical load connected to a low voltage alternating current system. Finally, two cases studies are proposed regarding the application of model predictive control and thermal regulation for domestic applications with cooling requirements, allowing to minimize energy consumption, considering the restrictions of demand, load and acclimatization in the system

Tese de doutoramento em Líderes para as Indústrias TecnológicasGovernmental and private institutions face a major challenge to provide quality health care to a population consisting of a growing number of elderly and chronically ill patients. According to the World Health Organization, in 2006, the total global health expenditures exceeded US$ 4 trillion and are rising in the majority of countries including Portugal which, during 2006, expended 9.9% of its gross domestic product in health care. The use of remote vital signs monitoring systems increases the probability of early detection of risky situations, allows frequent monitoring of in-patients, elderly and chronically ill patients, and streamlines the work of health professionals. However, at present, these systems are expensive, complex and employ obtrusive sensors, which limit their application to intensive care units and cardiac intermediate care units. This work is part of a project that aims to design, prototype and evaluate a remote vital signs monitoring system based on the IEEE 802.15.4 and ZigBee protocols, which allow the development of small low-power sensors. The prototype system comprises electrocardiogram/heart rate and axillary thermometer sensors, networking devices and three informatics applications that collect, process, and exhibit medical data. The wireless sensors, the networking devices and one of the applications were developed under this work. Additionally, the wireless sensor network was evaluated through simulations at the MAC level and experimental and field tests. Field tests were performed at an in-patient floor of Hospital Privado de Guimarães, a Portuguese hospital. Finally, questionnaires were used to measure the satisfaction of users and catalog their critics and suggestions for improvement. Simulations considered different topologies, operation modes and a crescent number of sensors and hops. Experimental and field tests confirmed most of the results obtained by simulations, but revealed that networks which did not assign transmission time slots to electrocardiogram sensors were unable to maintain a high delivery ratio. Contention between devices, aggravated by the inability of routers in receiving incoming packets during backoff, and collisions between packets generated by hiddennodes were responsible for most message losses. On the other hand, beacon-enabled star IEEE 802.15.4 networks that assigned a guaranteed time slot to sensors were able to maintain a very high delivery ratio. In contrast, these networks are restricted in terms of the coverage area and the number of sensors. Also, field tests showed that under low traffic scenarios ZigBee nonbeacon-enabled networks can achieve a high delivery ratio even in presence of a high percentage of hidden-nodes.

Wireless capsule endoscopy (WCE) is a non-invasive technology for capturing images of a human digestive system for medical diagnostics purpose. With WCE, the patient swallows a miniature capsule with camera, data processing unit, RF transmitter and batteries. The capsule captures and transmits images wirelessly from inside the human gastrointestinal (GI) tract. The external data logger worn by the patient stores the images and is later on transferred to a computer for presentation and image analysis. In this research, we designed and built a Wi-Fi based, low cost, miniature, versatile wearable data logger. The data logger is used with Wi-Fi enabled smart devices, smart phones and data servers to store and present images captured by capsule. The proposed data logger is designed to work with wireless capsule endoscopy and other biosensors like- temperature and heart rate sensors. The data logger is small enough to carry and conduct daily activities, and the patient do not need to carry traditional bulky data recorder all the time during diagnosis. The doctors can remotely access data and analyze the images from capsule endoscopy using remote access feature of the data logger. Smartphones and tablets have extensive processing power with expandable memory. This research exploits those capabilities to use with wireless capsule endoscopy and medical data logging applications. The application- specific data recorders are replaced by the proposed Wi-Fi data logger and smartphone. The data processing application is distributed on smart devices like smartphone /tablets and data logger. Once data are stored in smart devices, the data can be accessed remotely, distributed to the cloud and shared within networks to enable telemedicine. The data logger can work in both standalone and network mode. In the normal mode of the device, data logger stores medical data locally into a micro Secure Digital card for future download using the universal serial bus to the computer. In network mode, the real-time data is streamed into a smartphone and tablet for further processing and storage. The proposed Wi-Fi based data logger is prototyped in the lab and tested with the capsule hardware developed in our laboratory. The supporting Android app is also developed to collect data from the data logger and present the processed data to the viewer. The PC based software is also developed to access the data recorder and capture and download data from the data logger in real-time remotely. Both in vivo and ex vivo trials using live pig have been conducted to validate the performance of the proposed device
