DICOM Server: Aplicação para aquisição e encaminhamento automáticos de imagens médicas entre instituições de saúde
A imagiologia é uma especialidade médica com grande relevância para o diagnóstico e tratamentode inúmeras patologias. Ao longo das últimas décadas e seguindo a mesma tendêncianoutras áreas para a adaptação tecnológica, o serviço de imagiologia também tem evoluído paratirar maior partido das soluções e avanços da informática. Numa altura em que as imagens médicas digitais são amplamente utilizadas, surgem novos desafios para organizar toda a informação recolhida bem como o acesso dos dados entre instituições de saúde.Apesar de, na atualidade, uma grande parte das instituições de saúde possuírem sistemas deinformação que permitem o armazenamento e partilha de imagens médicas, a interoperabilidadeentre aplicações ainda não é uma realidade totalmente conseguida. Esta falta de interoperabilidade entre sistemas de informação condiciona os cuidados de saúde prestados aos doentes, que por vezes são sujeitos a avaliações clínicas repetidas e decisões baseadas em informação incompleta.O standard DICOM (Digital Imaging and Communication in Medicine) surgiu de forma amelhorar a interoperabilidade entre aplicações e definir normas que possibilitassem a colaboraçãodas mesmas. É amplamente utilizado, sendo o formato preferencial para o armazenamento deimagens médicas.Esta dissertação apresenta uma aplicação com base no standard DICOM, com o intuito de realizaro reencaminhamento e a aquisição de imagens entre instituições de saúde. Foram detalhadasas funcionalidades de comunicação desenvolvidas assim como os procedimentos para encaminhamento e aquisição automáticos de imagens médicas. A aplicação tem base numa arquitetura flexível, dinâmica e adaptável às necessidades. As funcionalidades desejáveis para a aplicação foram testadas através da análise do seu tempo de execução e, com os resultados obtidos desta análise, concluiu-se que a aplicação apresenta um desempenho satisfatório e estável cumprindo com os objetivos propostos.Em suma, esta aplicação constitui uma vantagem por compreender o standard DICOM já implementado e bastante disseminado nas instituições de saúde, o que torna desnecessária a modificação de software preexistente. A aplicação é ainda dotada das funcionalidades requeridas inicialmente,o reencaminhamento e aquisição automáticos de imagens médicas, que poderão possibilitara cooperação entre entidades de saúde dispersas, e permitir um fluxo de trabalho mais dinâmico.Medical Imaging is a specialty with great relevance for the diagnosis and treatment of numerouspathologies. Over the past decades, and following the same trend in other areas for technologicalevolution, imaging service has also evolved to take greater advantage of advances incomputer technology. At a time when the digital medical images are widely used, there are newchallenges to organize the collected information as well as access of data between institutions.Although currently a large part of health institutions possess systems that allow the storageand sharing of medical images, interoperability between applications is not yet a reality totallyachieved. The aim of this thesis is to study and develop a solution to simplify the exchange ofimaging information scattered across many institutions.This lack of interoperability among information systems affects the heath care provided to thepatient, which sometimes may be subject to repeat clinical evaluations and decisions based onincomplete information.The DICOM standard (Digital Imaging and Communication in Medicine) appeared in orderto improve interoperability between applications and set rules that would enable collaborationbetween them. It is widely used and is the preferred format for storing medical images.This thesis presents an application based on the DICOM standard, in order to perform forwardingand image acquisition between health institutions.The communication features such as procedures developed for routing and automatic acquisitionof medical images were detailed. The application is based on an architecture flexible, dynamicand adaptable to the needs. Desirable features for the application were tested by analysis of itsexecution time, and with the results of this analysis, it was concluded that the application displaysa satisfactory and stable performance fulfilling the proposed objectives.In summary, this application has an advantage by comprehending the DICOM standard whichis implemented and quite widespread in health institutions, making unnecessary the modificationof existing software.The application also provides functionality for forwarding and automatic acquisition of medical images, which could enable cooperation between entities dispersed and allow a more dynamic workflow

dissertationPublic health surveillance systems are crucial for the timely detection and response to public health threats. Since the terrorist attacks of September 11, 2001, and the release of anthrax in the following month, there has been a heightened interest in public health surveillance. The years immediately following these attacks were met with increased awareness and funding from the federal government which has significantly strengthened the United States surveillance capabilities; however, despite these improvements, there are substantial challenges faced by today's public health surveillance systems. Problems with the current surveillance systems include: a) lack of leveraging unstructured public health data for surveillance purposes; and b) lack of information integration and the ability to leverage resources, applications or other surveillance efforts due to systems being built on a centralized model. This research addresses these problems by focusing on the development and evaluation of new informatics methods to improve the public health surveillance. To address the problems above, we first identified a current public surveillance workflow which is affected by the problems described and has the opportunity for enhancement through current informatics techniques. The 122 Mortality Surveillance for Pneumonia and Influenza was chosen as the primary use case for this dissertation work. The second step involved demonstrating the feasibility of using unstructured public health data, in this case death certificates. For this we created and evaluated a pipeline iv composed of a detection rule and natural language processor, for the coding of death certificates and the identification of pneumonia and influenza cases. The second problem was addressed by presenting the rationale of creating a federated model by leveraging grid technology concepts and tools for the sharing and epidemiological analyses of public health data. As a case study of this approach, a secured virtual organization was created where users are able to access two grid data services, using death certificates from the Utah Department of Health, and two analytical grid services, MetaMap and R. A scientific workflow was created using the published services to replicate the mortality surveillance workflow. Doutoramento Ciências da ComputaçãoThe last decades have been characterized by a continuous adoption of
IT solutions in the healthcare sector, which resulted in the proliferation
of tremendous amounts of data over heterogeneous systems. Distinct
data types are currently generated, manipulated, and stored, in the
several institutions where patients are treated. The data sharing and an
integrated access to this information will allow extracting relevant
knowledge that can lead to better diagnostics and treatments.
This thesis proposes new integration models for gathering information
and extracting knowledge from multiple and heterogeneous biomedical
sources.
The scenario complexity led us to split the integration problem according
to the data type and to the usage specificity. The first contribution is a
cloud-based architecture for exchanging medical imaging services. It
offers a simplified registration mechanism for providers and services,
promotes remote data access, and facilitates the integration of
distributed data sources. Moreover, it is compliant with international
standards, ensuring the platform interoperability with current medical
imaging devices. The second proposal is a sensor-based architecture
for integration of electronic health records. It follows a federated
integration model and aims to provide a scalable solution to search and
retrieve data from multiple information systems. The last contribution is
an open architecture for gathering patient-level data from disperse and
heterogeneous databases. All the proposed solutions were deployed
and validated in real world use cases.A adoção sucessiva das tecnologias de comunicação e de informação
na área da saúde tem permitido um aumento na diversidade e na
qualidade dos serviços prestados, mas, ao mesmo tempo, tem gerado
uma enorme quantidade de dados, cujo valor científico está ainda por
explorar. A partilha e o acesso integrado a esta informação poderá
permitir a identificação de novas descobertas que possam conduzir a
melhores diagnósticos e a melhores tratamentos clínicos.
Esta tese propõe novos modelos de integração e de exploração de
dados com vista à extração de conhecimento biomédico a partir de
múltiplas fontes de dados.
A primeira contribuição é uma arquitetura baseada em nuvem para
partilha de serviços de imagem médica. Esta solução oferece um
mecanismo de registo simplificado para fornecedores e serviços,
permitindo o acesso remoto e facilitando a integração de diferentes
fontes de dados. A segunda proposta é uma arquitetura baseada em
sensores para integração de registos electrónicos de pacientes. Esta
estratégia segue um modelo de integração federado e tem como
objetivo fornecer uma solução escalável que permita a pesquisa em
múltiplos sistemas de informação. Finalmente, o terceiro contributo é
um sistema aberto para disponibilizar dados de pacientes num contexto
europeu. Todas as soluções foram implementadas e validadas em
cenários reais
Serviços de imagem médica suportados na cloud
Mestrado em Engenharia de Computadores e TelemáticaHoje em dia, as instituições de cuidados de saúde, utilizam a telemedicina para
suportar ambientes colaborativos. Na área da imagem médica digital, a
quantidade de dados tem crescido substancialmente nos últimos anos,
requerendo mais infraestruturas para fornecer um serviço com a qualidade
desejada. Os computadores e dispositivos com acesso à Internet estão
acessíveis em qualquer altura e em qualquer lugar, criando oportunidades para
partilhar e utilizar recursos online. Uma enorme quantidade de processamento
computacional e armazenamento são utilizados como uma comodidade no
quotidiano. Esta dissertação apresenta uma plataforma para suportar serviços
de telemedicina sobre a cloud, permitindo que aplicações armazenem e
comuniquem facilmente, utilizando qualquer fornecedor de cloud. Deste modo,
os programadores não necessitam de se preocupar onde os recursos vão ser
instalados a as suas aplicações não ficam limitadas a um único fornecedor.
Foram desenvolvidas duas aplicações para tele-imagiologia com esta
plataforma: repositório de imagens médicas e uma infraestrutura de
comunicações entre centros hospitalares. Finalmente, a arquitetura
desenvolvida é genérica e flexível permitindo facilmente a sua expansão para
outras áreas aplicacionais e outros serviços de cloud.Healthcare institutions resort largely, nowadays, to telemedicine in order to
support collaborative environments. In the medical imaging area, the huge
amount of medical volume data has increased over the past few years,
requiring high-performance infrastructures to provide services with required
quality. Computing devices and Internet access are now available anywhere
and at anytime, creating new opportunities to share and use online resources.
A tremendous amount of ubiquitous computational power and an
unprecedented number of Internet resources and services are used every day
as a normal commodity. This thesis presents a telemedicine service platform
over the Cloud that allows applications to store information and to communicate
easier, using any Internet cloud provider. With this platform, developers do not
concern where the resources will be deployed and the applications will not be
restricted to a specific cloud vendor. Two tele-imagiologic applications were
developed along with this platform: a medical imaging repository and an interinstitutional
communications infrastructure. Lastly, the architecture developed is
generic and flexible to expand to other application areas and cloud services
Métodos computacionais para otimização de desempenho em redes de imagem médica
Over the last few years, the medical imaging has consolidated its position as a major mean of clinical diagnosis. The amount of data generated by the medical imaging practice is increasing tremendously. As a result, repositories are turning into rich databanks of semi-structured data related to patients, ailments, equipment and other stakeholders involved in the medical imaging panorama. The exploration of these repositories for secondary uses of data promises to elevate the quality standards and efficiency of the medical practice. However, supporting these advanced usage scenarios in traditional institutional systems raises many technical challenges that are yet to be overcome. Moreover, the reported poor performance of standard protocols opened doors to the general usage of proprietary solutions, compromising the interoperability necessary for supporting these advanced scenarios. This thesis has researched, developed, and now proposes a series of computer methods and architectures intended to maximize the performance of multi-institutional medical imaging environments. The methods are intended to improve the performance of standard protocols for medical imaging content discovery and retrieval. The main goal is to use them to increase the acceptance of vendor-neutral solutions through the improvement of their performance. Moreover, it intends to promote the adoption of such standard technologies in advanced scenarios that are still a mirage nowadays, such as clinical research or data analytics directly on top of live institutional repositories. Finally, these achievements will facilitate the cooperation between healthcare institutions and researchers, resulting in an increment of healthcare quality and institutional efficiency.As diversas modalidades de imagem médica têm vindo a consolidar a sua posição dominante como meio complementar de diagnóstico. O número de procedimentos realizados e o volume de dados gerados aumentou significativamente nos últimos anos, colocando pressão nas redes e sistemas que permitem o arquivo e distribuição destes estudos. Os repositórios de estudos imagiológicos são fontes de dados ricas contendo dados semiestruturados relacionados com pacientes, patologias, procedimentos e equipamentos. A exploração destes repositórios para fins de investigação e inteligência empresarial, tem potencial para melhorar os padrões de qualidade e eficiência da prática clínica. No entanto, estes cenários avançados são difíceis de acomodar na realidade atual dos sistemas e redes institucionais. O pobre desempenho de alguns protocolos standard usados em ambiente de produção, conduziu ao uso de soluções proprietárias nestes nichos aplicacionais, limitando a interoperabilidade de sistemas e a integração de fontes de dados. Este doutoramento investigou, desenvolveu e propõe um conjunto de métodos computacionais cujo objetivo é maximizar o desempenho das atuais redes de imagem médica em serviços de pesquisa e recuperação de conteúdos, promovendo a sua utilização em ambientes de elevados requisitos aplicacionais. As propostas foram instanciadas sobre uma plataforma de código aberto e espera-se que ajudem a promover o seu uso generalizado como solução vendor-neutral. As metodologias foram ainda instanciadas e validadas em cenários de uso avançado. Seasonal radiological exposure analysis: from the use of resources to individual exposure
A exposição à radiação ionizante por parte da população no
âmbito da prestação de cuidados de saúde, nomeadamente no
âmbito da Imagiologia, tem vindo a ser objeto de análise em
múltiplos fóruns. A multiplicidade de situações clínicas cuja
avaliação depende de estudos imagiológicos torna premente a
análise da evolução da exposição a que a população é
submetida, tendo em conta a eficácia da prestação de cuidados
de saúde ao longo do tempo. Neste trabalho procurou-se
caracterizar a exposição à radiação recorrendo à meta-informação contida nas imagens definidas pela norma Digital
Imaging and Communication in Medicine (DICOM), mais
especificamente a suportada por metadados DICOM, cujo
acesso foi realizado a partir da ferramenta “Dicoogle”. Os
resultados obtidos através das análises realizadas permitiram
caracterizar a utilização de recursos de imagem médica ao
longo do tempo e a sua repercussão na exposiçao da
população à radiação ionizante. Verificou-se, no entanto, uma
utilização pouco normalizada desta informação por parte de
diferentes fabricantes, com recurso a sistemas de informação
e metadados DICOM privados.Exposure to ionizing radiation to which the population is subject
in the field of health care, within the scope of Medical Imaging
has been object of analysis in multiple forums. The multiplicity of
clinical situations whose evaluation depends on image studies
makes it imperious to analyze the evolution of the exposure which
the population is submitted based on the efficiency of health care
provision, over time. In this work, we tried to characterize
radiation exposure using the meta-information contained in the
medical images defined by Digital Imaging and Communication
in Medicine (DICOM) standard, more specifically the supported
by DICOM metadata, whose access was performed by a tool
named “Dicoogle”. The results obtained from the analyzes
conducted in the study allowed to characterize the use of medical
image resources and their impact on the exposure of population
to ionizing radiation. However, it was found a lack of standardized
use of this information by different manufacturer, using private

doutoramento Ciências da ComputaçãoDurante as últimas décadas, as organizações de saúde têm vindo a adotar continuadamente as tecnologias de informação para melhorar o funcionamento dos seus serviços. Recentemente, em parte devido à crise financeira, algumas reformas no sector de saúde incentivaram o aparecimento de novas soluções de telemedicina para otimizar a utilização de recursos humanos e de equipamentos. Algumas tecnologias como a computação em nuvem, a computação móvel e os sistemas Web, têm sido importantes para o sucesso destas novas aplicações de telemedicina. As funcionalidades emergentes de computação distribuída facilitam a ligação de comunidades médicas, promovem serviços de telemedicina e a colaboração em tempo real. Também são evidentes algumas vantagens que os dispositivos móveis podem introduzir, tais como facilitar o trabalho remoto a qualquer hora e em qualquer lugar. Por outro lado, muitas funcionalidades que se tornaram comuns nas redes sociais, tais como a partilha de dados, a troca de mensagens, os fóruns de discussão e a videoconferência, têm o potencial para promover a colaboração no sector da saúde.
Esta tese teve como objetivo principal investigar soluções computacionais mais ágeis que permitam promover a partilha de dados clínicos e facilitar a criação de fluxos de trabalho colaborativos em radiologia. Através da exploração das atuais tecnologias Web e de computação móvel, concebemos uma solução ubíqua para a visualização de imagens médicas e desenvolvemos um sistema colaborativo para a área de radiologia, baseado na tecnologia da computação em nuvem. Neste percurso, foram investigadas metodologias de mineração de texto, de representação semântica e de recuperação de informação baseada no conteúdo da imagem. Para garantir a privacidade dos pacientes e agilizar o processo de partilha de dados em ambientes colaborativos, propomos ainda uma metodologia que usa aprendizagem automática para anonimizar as imagens médicasDuring the last decades, healthcare organizations have been increasingly relying on information technologies to improve their services. At the same time, the optimization of resources, both professionals and equipment, have promoted the emergence of telemedicine solutions. Some technologies including cloud computing, mobile computing, web systems and distributed computing can be used to facilitate the creation of medical communities, and the promotion of telemedicine services and real-time collaboration. On the other hand, many features that have become commonplace in social networks, such as data sharing, message exchange, discussion forums, and a videoconference, have also the potential to foster collaboration in the health sector.
The main objective of this research work was to investigate computational solutions that allow us to promote the sharing of clinical data and to facilitate the creation of collaborative workflows in radiology. By exploring computing and mobile computing technologies, we have designed a solution for medical imaging visualization, and developed a collaborative system for radiology, based on cloud computing technology. To extract more information from data, we investigated several methodologies such as text mining, semantic representation, content-based information retrieval. Finally, to ensure patient privacy and to streamline the data sharing in collaborative environments, we propose a machine learning methodology to anonymize medical images

Future developments and trends in use of picture archiving and communication systems
This thesis was submitted for the degree of Doctor of Philosophy and awarded by Brunel University London.Objectives: This study identifies modern information technologies that can improve the clinical practice of Picture Archiving and Communication Systems (PACS) and determines the approaches that are needed to improve the functionality of current PACS to provide better next generation PACS and to improve the future of radiology practices and workflow with future PACS generations. Method: A parallel mixed method approach was adopted including qualitative method (semi-structured interviews), quantitative method (questionnaire survey) and observation of online discussion groups on PACS. Five databases were searched to find salient literature, including Science Direct, Springer Link, Scopus, CINAHL Plus and Google Scholar. Six radiologists were interviewed and questionnaires were collected from 120 radiologists. Four online discussion groups related to PACS were monitored via LinkedIn. The data were analysed thematically using the thematic analysis method. Finally, a focus group was held with a separate group of radiologists to validate the findings.
Results: Eight themes emerged from the thematic analysis of the data: (1) limitations of traditional PACS; (2) user needs and requirements that can increase PACS functionality; (3) Web based solutions of PACS; (4) PACS on mobile phones; (5) Vendor Neutral Archive (VNA); (6) full integration of voice recognition in PACS; (7) backup solutions for the system and (8) continuous training for PACS users. Discussion: With the development of healthcare information and communication technology (ICT) and with the increased demands of the radiologists to expand PACS usability, traditional PACS must be updated to follow the changes. Modern technologies can provide better solutions to enhance the functionality of current PACS. The next generation of PACS can fulfil the future requirements of users. This study considers the issues between the needs and requirements of the users of a PACS system in the future and the developing solutions in ICT and the PACS industry. Moreover, highly developed PACS systems with advanced features will have a direct impact in changing and improving radiology workflow. Accordingly, a model has been developed that proposes new features for the next generation PACS system, which may be applied to the next generation radiology practice. The model was validated with the focus group and, by using a separate group of radiologists in another country, was determined to be generalisable. Conclusion: It is widely recognised that traditional PACS must be updated to adopt recent advances in ICT. This research has identified themes that, when incorporated, will enhance the functionality of PACS and radiology workflow and provide better quality clinical practice. The findings from this empirical research can be used: as recommendations to vendors; for technology development; and by medical institutes to consider aspects when undertaking implementation of PACS and training future radiologists. VirtualPACS—A Federating Gateway to Access Remote Image Data Resources over the Grid
Collaborations in biomedical research and clinical studies require that data, software, and computational resources be shared between geographically distant institutions. In radiology, there is a related issue of sharing remote DICOM data over the Internet. This paper focuses on the problem of federating multiple image data resources such that clients can interact with them as if they are stored in a centralized PACS. We present a toolkit, called VirtualPACS, to support this functionality. Using the toolkit, users can perform standard DICOM operations (query, retrieve, and submit) across distributed image databases. The key features of the toolkit are: (1) VirtualPACS makes it easy to use existing DICOM client applications for data access; (2) it can easily be incorporated into an imaging workflow as a DICOM source; (3) using VirtualPACS, heterogeneous collections of DICOM sources are exposed to clients through a uniform interface and common data model; and (4) DICOM image databases without DICOM messaging can be accessed
