    Personalised antimicrobial management in secondary care

    Background: The growing threat of Antimicrobial Resistance (AMR) requires innovative methods to promote the sustainable effectiveness of antimicrobial agents. Hypothesis: This thesis aimed to explore the hypothesis that personalised decision support interventions have the utility to enhance antimicrobial management across secondary care. Methods: Different research methods were used to investigate this hypothesis. Individual physician decision making was mapped and patient experiences of engagement with decision making explored using semi-structured interviews. Cross-specialty engagement with antimicrobial management was investigated through cross-sectional analysis of conference abstracts and educational training curricula. Artificial intelligence tools were developed to explore their ability to predict the likelihood of infection and provide individualised prescribing recommendations using routine patient data. Dynamic, individualised dose optimisation was explored through: (i) development of a microneedle based, electrochemical biosensor for minimally invasive monitoring of beta-lactams; and (ii) pharmacokinetic (PK)-pharmacodynamic (PD) modelling of a new PK-PD index using C-Reactive protein (CRP) to predict the pharmacodynamics of vancomycin. Ethics approval was granted for all aspects of work explored within this thesis. Results: Mapping of individual physician decision making during infection management demonstrated several areas where personalised, technological interventions could enhance antimicrobial management. At specialty level, non-infection specialties have little engagement with antimicrobial management. The importance of engaging surgical specialties, who have relatively high rates of antimicrobial usage and healthcare associated infections, was observed. An individualised information leaflet, co-designed with patients, to provide personalised infection information to in-patients receiving antibiotics significantly improved knowledge and reported engagement with decision making. Artificial intelligence was able to enhance the prediction of infection and the prescribing of antimicrobials using routinely available clinical data. Real-time, continuous penicillin monitoring was demonstrated using a microneedle based electrochemical sensor in-vivo. A new PK-PD index, using C-Reactive Protein, was able to predict individual patient response to vancomycin therapy at 96-120 hours of therapy. Conclusion: Through co-design and the application of specific technologies it is possible to provide personalised antimicrobial management within secondary care.Open Acces

    A Framework for Measuring Return on Investment for Healthcare Simulation-Based Training

    In the healthcare sector, providing high-quality service in a safe environment for both patient and staff is an obvious and ultimate major objective. Training is an essential component for achieving this important objective. Most organizations acknowledge that employee simulation-based training programs are an important part of the human capital strategy, yet few have effectively succeeded in quantifying the real and precise ROI of this type of investment. Therefore, if the training is perceived as a waste of resources and its ROI is not clearly recognized, it will be the first option to cut when the budget cut is needed. The various intangible benefits of healthcare simulation-based training are very difficult to quantify. In addition, there was not a unified way to count for the different cost and benefits to provide a justifiable ROI. Quantifying the qualitative and intangible benefits of medical training simulator needed a framework that helps to identify and convert qualitative and intangible benefits into monetary value so it can be considered in the ROI evaluation. This research is a response to the highlighted importance of developing a comprehensive framework that has the capability to take into consideration the wide range of benefits that simulation-based training can bring to the healthcare system taking into consideration the characteristics of this specific field of investment. The major characteristics of investment in this field include the uncertainty, the qualitative nature of the major benefits, and the diversity and the wide range of applications. This comprehensive framework is an integration of several methodologies and tools. It consists of three parts. The first part of the framework is the benefits and cost structure, which pays special attention to the qualitative and intangible benefits by considering the Value Measurement methodology (VMM) and other previously existing models. The second part of the framework is important to deal with the uncertainty associated with this type of investment. Monte Carlo simulation is a tool that considered multiple scenarios of input sets instead of a single set of inputs. The third part of the framework considers an advanced value analysis of the investment. It goes beyond the discounted cash flow (DCF) methodologies like net present value (NPV) that consider a single scenario for the cash flow to Real Options Analysis that consider the flexibility over the lifetime of the investment when evaluating the value of the investment.     A Systems Engineering Reference Model for Fuel Cell Power Systems Development

    This research was done because today the Fuel Cell (FC) Industry is still in its infancy in spite over one-hundred years of development has transpired. Although hundreds of fuel cell developers, globally have been spawned, in the last ten to twenty years, only a very few are left struggling with their New Product Development (NPD). The entrepreneurs of this type of disruptive technology, as a whole, do not have a systems engineering \u27roadmap , or template, which could guide FC technology based power system development efforts to address a more environmentally friendly power generation. Hence their probability of achieving successful commercialization is generally, quite low. Three major problems plague the fuel cell industry preventing successful commercialization today. Because of the immaturity of FC technology and, the shortage of workers intimately knowledgeable in FC technology, and the lack of FC systems engineering, process developmental knowledge, the necessity for a commercialization process model becomes evident. This thesis presents a six-phase systems engineering developmental reference model for new product development of a Solid Oxide Fuel Cell (SOFC) Power System. For this work, a stationary SOFC Power System, the subject of this study, was defined and decomposed into a subsystems hierarchy using a Part Centric Top-Down, integrated approach to give those who are familiar with SOFC Technology a chance to learn systems engineering practices. In turn, the examination of the SOFC mock-up could gave those unfamiliar with SOFC Technology a chance to learn the basic, technical fundamentals of fuel cell development and operations. A detailed description of the first two early phases of the systems engineering approach to design and development provides the baseline system engineering process details to create a template reference model for the remaining four phases.
