    The Serums Tool-Chain:Ensuring Security and Privacy of Medical Data in Smart Patient-Centric Healthcare Systems

    Digital technology is permeating all aspects of human society and life. This leads to humans becoming highly dependent on digital devices, including upon digital: assistance, intelligence, and decisions. A major concern of this digital dependence is the lack of human oversight or intervention in many of the ways humans use this technology. This dependence and reliance on digital technology raises concerns in how humans trust such systems, and how to ensure digital technology behaves appropriately. This works considers recent developments and projects that combine digital technology and artificial intelligence with human society. The focus is on critical scenarios where failure of digital technology can lead to significant harm or even death. We explore how to build trust for users of digital technology in such scenarios and considering many different challenges for digital technology. The approaches applied and proposed here address user trust along many dimensions and aim to build collaborative and empowering use of digital technologies in critical aspects of human society

    CRiBAC: Community-centric role interaction based access control model

    As one of the most efficient solutions to complex and large-scale problems, multi-agent cooperation has been in the limelight for the past few decades. Recently, many research projects have focused on context-aware cooperation to dynamically provide complex services. As cooperation in the multi-agent systems (MASs) becomes more common, guaranteeing the security of such cooperation takes on even greater importance. However, existing security models do not reflect the agents' unique features, including cooperation and context-awareness. In this paper, we propose a Community-based Role interaction-based Access Control model (CRiBAC) to allow secure cooperation in MASs. To do this, we refine and extend our preliminary RiBAC model, which was proposed earlier to support secure interactions among agents, by introducing a new concept of interaction permission, and then extend it to CRiBAC to support community-based cooperation among agents. We analyze potential problems related to interaction permissions and propose two approaches to address them. We also propose an administration model to facilitate administration of CRiBAC policies. Finally, we present the implementation of a prototype system based on a sample scenario to assess the proposed work and show its feasibility. © 2012 Elsevier Ltd. All rights reserved

    Secure, reliable and dynamic access to distributed clinical data

    An abundance of statistical and scientific data exists in the area of clinical and epidemiological studies. Much of this data is distributed across regional, national and international boundaries with different policies on access and usage, and a multitude of different schemata for the data often complicated by the variety of supporting clinical coding schemes. This prevents the wide scale collation and analysis of such data as is often needed to infer clinical outcomes and to determine the often moderate effect of drugs. Through grid technologies it is possible to overcome the barriers introduced by distribution of heterogeneous data and services. However reliability, dynamicity and fine-grained security are essential in this domain, and are not typically offered by current grids. The MRC funded VOTES project (Virtual Organisations for Trials and Epidemiological Studies) has implemented a prototype infrastructure specifically designed to meet these challenges. This paper describes this on-going implementation effort and the lessons learned in building grid frameworks for and within a clinical environment

    Unsupervised Parkinson’s Disease Assessment

    Parkinson’s Disease (PD) is a progressive neurological disease that affects 6.2 million people worldwide. The most popular clinical method to measure PD tremor severity is a standardized test called the Unified Parkinson’s Disease Rating Scale (UPDRS), which is performed subjectively by a medical professional. Due to infrequent checkups and human error introduced into the process, treatment is not optimally adjusted for PD patients. According to a recent review there are two devices recommended to objectively quantify PD symptom severity. Both devices record a patient’s tremors using inertial measurement units (IMUs). One is not currently available for over the counter purchases, as they are currently undergoing clinical trials. It has also been used in studies to evaluate to UPDRS scoring in home environments using an Android application to drive the tests. The other is an accessible product used by researchers to design home monitoring systems for PD tremors at home. Unfortunately, this product includes only the sensor and requires technical expertise and resources to set up the system. In this paper, we propose a low-cost and energy-efficient hybrid system that monitors a patient’s daily actions to quantify hand and finger tremors based on relevant UPDRS tests using IMUs and surface Electromyography (sEMG). This device can operate in a home or hospital environment and reduces the cost of evaluating UPDRS scores from both patient and the clinician’s perspectives. The system consists of a wearable device that collects data and wirelessly communicates with a local server that performs data analysis. The system does not require any choreographed actions so that there is no need for the user to follow any unwieldy peripheral. In order to avoid frequent battery replacement, we employ a very low-power wireless technology and optimize the software for energy efficiency. Each collected signal is filtered for motion classification, where the system determines what analysis methods best fit with each period of signals. The corresponding UPDRS algorithms are then used to analyze the signals and give a score to the patient. We explore six different machine learning algorithms to classify a patient’s actions into appropriate UPDRS tests. To verify the platform’s usability, we conducted several tests. We measured the accuracy of our main sensors by comparing them with a medically approved industry device. The our device and the industry device show similarities in measurements with errors acceptable for the large difference in cost. We tested the lifetime of the device to be 15.16 hours minimum assuming the device is constantly on. Our filters work reliably, demonstrating a high level of similarity to the expected data. Finally, the device is run through and end-to-end sequence, where we demonstrate that the platform can collect data and produce a score estimate for the medical professionals

    Smart hospital emergency system via mobile-based requesting services

    In recent years, the UK’s emergency call and response has shown elements of great strain as of today. The strain on emergency call systems estimated by a 9 million calls (including both landline and mobile) made in 2014 alone. Coupled with an increasing population and cuts in government funding, this has resulted in lower percentages of emergency response vehicles at hand and longer response times. In this paper, we highlight the main challenges of emergency services and overview of previous solutions. In addition, we propose a new system call Smart Hospital Emergency System (SHES). The main aim of SHES is to save lives through improving communications between patient and emergency services. Utilising the latest of technologies and algorithms within SHES is aiming to increase emergency communication throughput, while reducing emergency call systems issues and making the process of emergency response more efficient. Utilising health data held within a personal smartphone, and internal tracked data (GPU, Accelerometer, Gyroscope etc.), SHES aims to process the mentioned data efficiently, and securely, through automatic communications with emergency services, ultimately reducing communication bottlenecks. Live video-streaming through real-time video communication protocols is also a focus of SHES to improve initial communications between emergency services and patients. A prototype of this system has been developed. The system has been evaluated by a preliminary usability, reliability, and communication performance study

    Information Systems and Healthcare XXI: A Dynamic, Client-Centric, Point-Of-Care System for the Novice Nurse

    Nurse clinicians need to make complex decisions on a continual basis, while delivering cost-effective treatments. The rapid proliferation of medical and nursing knowledge complicates the decision-making process, particularly for novice nurses. We describe a Clinical Decision Support System (CDSS) for the novice nurse that combines evidence-based nursing knowledge with specific patient information to create a real-time guide through the nursing diagnostic care process. The goal of the paper is to describe how an appropriately designed and evidence-based CDSS can aid the nursing practice. An off-the-shelf handheld computer is utilized to deliver clinical knowledge to the nurse, via wireless link to a central server and a data repository. In describing the software architecture of the system, particular emphasis is paid to the issue of appropriate design by discussing the steps taken to address system extensibility, performance, reliability, and security, which are important factors in the design of a CDSS

The low incidence rate and complex treatment process makes rare disease research an underrated field in the life sciences. However, it is in these particular conditions that the strongest relations between genotypes and phenotypes are identified. The rare disease patient manager, detailed in this manuscript, presents an innovative perspective for a patient-centric portal integrating genetic and medical data. With this strategy, patient’s digital records are transparently integrated and connected to wet-lab genetics research in a seamless working environment. The resulting knowledge base offers multiple data views, geared towards medical staff, with patient treatment and monitoring data; genetics researchers, through a custom locus-specific database; and patients, who for once play an active role in their treatment and rare diseases research

    Towards runtime discovery, selection and composition of semantic services

    Service-orientation is gaining momentum in distributed software applications, mainly because it facilitates interoperability and allows application designers to abstract from underlying implementation technologies. Service composition has been acknowledged as a promising approach to create composite services that are capable of supporting service user needs, possibly by personalising the service delivery through the use of context information or user preferences. In this paper we discuss the challenges of automatic service composition, and present DynamiCoS, which is a novel framework that aims at supporting service composition on demand and at runtime for the benefit of service end-users. We define the DynamiCoS framework based on a service composition life-cycle. Framework mechanisms are introduced to tackle each of the phases and requirements of this life-cycle. Semantic services are used in our framework to enable reasoning on the service requests issued by end users, making it possible to automate service discovery, selection and composition. We validate our framework with a prototype that we have built in order to experiment with the mechanisms we have designed. The prototype was evaluated in a testing environment using some use case scenarios. The results of our evaluation give evidences of the feasibility of our approach to support runtime service composition. We also show the benefits of semantic-based frameworks for service composition, particularly for end-users who will be able to have more control on the service composition process
