Bronchial thermoplasty: implementing best practice in the era of cost containment.

Increasing dependence on advanced technologies in the 21st century has created a dilemma between the practice and business of medicine. From information technology to robotic surgery, new technologies have expanded treatment possibilities and have potentially improved patient outcomes and safety. Simultaneously, their escalating costs limit access for certain patients and health care facilities. Nevertheless, medical decisions should not simply be based on cost. Input from physicians and other health care specialists as well as adherence to best practice position statements, are vital to implementing truly cost-effective strategies in medicine. Bronchial thermoplasty (BT), a US Food and Drug Administration approved bronchoscopy procedure in difficult-to-control persistent asthma, is a prime example of a new technology facing cost and implementation challenges. We discuss the specific indications and contraindications for BT and review recent real-world experiences that can provide the foundation for building a comprehensive asthma program that provides BT for difficult-to-control asthma patients who fail national guideline treatment recommendations after an adequate clinical trial of one. We also offer insight into the barriers to implementing a successful BT program and strategies for overcoming them

Reappraising Always

Steven Spielberg's 1989 film Always represents one of the director's few critical and commercial disappointments. This paper examines the extent to which the film's failures are attributable to its formal, stylistic, and narrative features. The paper offers a defence of Always against specific reproaches. It also pursues more positive aims. Following Warren Buckland, the paper pinpoints organic unity as Spielberg's primary compositional principle; it tracks the development of motifs, tactics of foreshadowing, and other internal norms to demonstrate the formation of a structurally unified text; and it posits contrasts with a pertinent antecedent, A Guy Named Joe (Victor Fleming, 1943), so as to set Spielberg's artistic achievements in relief. The paper goes on to isolate some putatively troublesome manoeuvres at the film's internal level. Certain of these problematic aspects, I argue, force us to recognise that important narrative effects can be yielded by modulated deviations from organic unity. The collective aim of these arguments is to suggest that Always is apt for critical revaluation. Over this hovers a secondary objective. The paper seeks to disclaim two interrelated faults ascribed to Spielberg: a characteristic supplanting of narrative coherence by spectacle; and an indifference to subtlety and sophistication

Maximus the Confessor and a Deeper Actualization of the Apostolic Dimensions of Pentecostal Movements

Excerpt: ...I propose to explore one expression of this more ancient and eastern tradition, as found in the cosmic vision of Maximus the Confessor. After a brief sketch of his life and times and his relationship to the larger orthodox tradition I will attempt to lay out some of the chief features of both his theological and cosmological framework and his ascetic way of practice in community. I will conclude by suggesting that both this framework and way of practice can indeed help resource healing of humankind’s relationship with one another and the other realms of the creation and that an appropriation of aspects of this stream of the Christian tradition in the coming decades can help global Pentecostal movements influence societies toward greater social and ecological health

Privacy as personal resistance: exploring legal narratology and the need for a legal architecture for personal privacy rights

Different cultures produce different privacies – both architecturally and legally speaking – as well as in their different legal architectures. The ‘Simms principle’ can be harnessed to produce semi-constitutional privacy protection through statute; building on the work already done in ‘bringing rights home’ through the Human Rights Act 1998. This article attempts to set out a notion of semi-entrenched legal rights, which will help to better portray the case for architectural, constitutional privacy, following an examination of the problems with a legal narrative for privacy rights as they currently exist. I will use parallel ideas from the works of W.B. Yeats and Costas Douzinas to explore and critique these assumptions and arguments. The ultimate object of this piece is an argument for the creation of a legal instrument, namely an Act of Parliament, in the United Kingdom; the purpose of which is to protect certain notions of personal privacy from politically-motivated erosion and intrusion

Systems Assurance, Complexity and Emergence: The Need for a Systems Based Approach

The complexity of modern products, systems and processes makes the task to identify, characterise and provide sufficient assurance about the desirable properties a major challenge. Stakeholders also, demand a degree of enhanced confidence about the absence of undesirable properties with a potential to cause harm or loss. The paper develops a framework of seven fundamental facets of performance as an ontology for emergent behavioural properties and a separate framework for the emergent structural properties of complex systems. The emergent behavioural aspects are explored and we develop a systems framework for assurance based on an Assessment and Management paradigm each comprising a number of principles and processes. The key argument advanced is that in the face of complexity and incessant change, enhanced confidence in the achievement of desirable and avoidance of undesirable properties requires a systems approach empowered by suitable modelling and relevant diagnostic tools explaining the nature of emergent properties. The principal focus of this paper is on safety, security and sustainability emergent behavioural (performance) aspects of complex products, systems and processes

Unifying Distributed Processing and Open Hypertext through a Heterogeneous Communication Model

A successful distributed open hypermedia system can be characterised by a scaleable architecture which is inherently distributed. While the architects of distributed hypermedia systems have addressed the issues of providing and retrieving distributed resources, they have often neglected to design systems with the inherent capability to exploit the distributed processing of this information. The Anatomy and Facets of Dynamic Policies

Information flow policies are often dynamic; the security concerns of a program will typically change during execution to reflect security-relevant events. A key challenge is how to best specify, and give proper meaning to, such dynamic policies. A large number of approaches exist that tackle that challenge, each yielding some important, but unconnected, insight. In this work we synthesise existing knowledge on dynamic policies, with an aim to establish a common terminology, best practices, and frameworks for reasoning about them. We introduce the concept of facets to illuminate subtleties in the semantics of policies, and closely examine the anatomy of policies and the expressiveness of policy specification mechanisms.
