Inheritance and genetics in idiopathic scoliosis
Idiopathic scoliosis is the most common spine deformity, affecting approximately 3% of
children and adolescents. Its aetiology is still unknown. However, relatives of
individuals
with idiopathic scoliosis have a higher risk of developing scoliosis compared
to the general
population. The aim of this thesis was to improve our understanding of
the hereditary and
genetic background of
idiopathic scoliosis.
Self
-
reported data on scoliosis in twins (n=64,578) in the population
-
based Swedish Twin
Registry were an
alysed to estimate the relative importance of genetic effects on the
phenotypic variance
–
that is, the heritability of scoliosis.
Using structural equation model
ing,
we estimated that 38% of the phenotypic variance of scoliosis is due to additive genetic
effects and 62% to unique environmental effects.
In ScoliGeneS, an ongoing multi
-
centre study, we included individuals with idiopathic
scoliosis and controls.
The importance of a
family history of scoliosis wa
s investigated in
1,463 individuals with idiop
athic scoliosis
. Among those treated with a brace or surgery for
scoliosis, 53% reported one or more relatives with scoliosis compared to 46% of the
untreated, indicating a higher risk of treatment in the presence of a family history of scoliosis
(odds rat
io 1.32; 95% confidence interval 1.06
–
1.64). The prevalence of back problems
was
investigated in 1,069 adults
with idiopathic scoliosis
and in 1

Four common single

nucleotide variants, previously shown to be associated with idiopathic
scoliosis,
were genotyped in 1,739 individuals with idiopathic scoliosis
from the ScoliGeneS
cohort and in 1,812 controls. In addition, the protein

coding regions of
the genome
–
the
exome
–
was sequenced in pooled samples (10x10) from 100 surgically treated patients in the
ScoliGeneS cohort. We found a strong associ
ation of idiopathic scoliosis with a
common
previously known variant downstream of the
LBX1
gene (OR=1.5
3; p=7.0x

18
). We
identified twenty novel variants by exome sequencing after filtering and an initial genotyping
validation. No
significant
association was f
ound with idiopathic scoliosis in the large cohort
of 1,739 cases and 1,812 controls.
In summary,
inherited factors are of importance in the development and progression of
idiopathic scoliosis. A genetic variant downstream of
the LBX1 gene is strongly associated
with idiopathic scoliosis. We were unable to find genes of similar or stronger effect
PRELIMINARY FINDINGS OF A POTENZIATED PIEZOSURGERGICAL DEVICE AT THE RABBIT SKULL
The number of available ultrasonic osteotomes has remarkably increased. In vitro and in vivo studies
have revealed differences between conventional osteotomes, such as rotating or sawing devices, and
ultrasound-supported osteotomes (Piezosurgery®) regarding the micromorphology and roughness
values of osteotomized bone surfaces.
Objective: the present study compares the micro-morphologies and roughness values of
osteotomized bone surfaces after the application of rotating and sawing devices, Piezosurgery
Medical® and Piezosurgery Medical New Generation Powerful Handpiece.
Methods: Fresh, standard-sized bony samples were taken from a rabbit skull using the following
osteotomes: rotating and sawing devices, Piezosurgery Medical® and a Piezosurgery Medical New
Generation Powerful Handpiece. The required duration of time for each osteotomy was recorded.
Micromorphologies and roughness values to characterize the bone surfaces following the different
osteotomy methods were described. The prepared surfaces were examined via light microscopy,
environmental surface electron microscopy (ESEM), transmission electron microscopy (TEM), confocal
laser scanning microscopy (CLSM) and atomic force microscopy. The selective cutting of mineralized
tissues while preserving adjacent soft tissue (dura mater and nervous tissue) was studied. Bone
necrosis of the osteotomy sites and the vitality of the osteocytes near the sectional plane were
investigated, as well as the proportion of apoptosis or cell degeneration.
Results and Conclusions: The potential positive effects on bone healing and reossification
associated with different devices were evaluated and the comparative analysis among the different
devices used was performed, in order to determine the best osteotomes to be employed during
cranio-facial surgery
Expert consensus on resection of chest wall tumors and chest wall reconstruction
Chest wall tumors are a relatively uncommon disease in clinical practice. Most of the published studies about chest wall tumors are usually single-center retrospective studies, involving few patients. Therefore, evidences regarding clinical conclusions about chest wall tumors are lacking, and some controversial issues have still to be agreed upon. In January 2019, 73 experts in thoracic surgery, plastic surgery, science, and engineering jointly released the Chinese Expert Consensus on Chest Wall Tumor Resection and Chest Wall Reconstruction (2018 edition). After that, numerous experts put forward new perspectives on some academic issues in this version of the consensus, pointing out the necessity to further discuss the points of contention. Thus, we conducted a survey through the administration of a questionnaire among 85 experts in the world. Consensus has been reached on some major points as follows. (I) Wide excision should be performed for desmoid tumor (DT) of chest wall. After excluding the distant metastasis by multi-disciplinary team, solitary sternal plasmacytoma can be treated with extensive resection and adjuvant radiotherapy. (II) Wide excision with above 2 cm margin distance should be attempted to obtain R0 resection margin for chest wall tumor unless the tumor involves vital organs or structures, including the great vessels, heart, trachea, joints, and spine. (III) For patients with chest wall tumors undergoing unplanned excision (UE) for the first time, it is necessary to carry out wide excision as soon as possible within 1-3 months following the previous surgery. (IV) Current Tumor Node Metastasis staging criteria (American Joint Committee on Cancer) of bone tumor and soft tissue sarcoma are not suitable for chest wall sarcomas. (V) It is necessary to use rigid implants for chest wall reconstruction once the maximum diameter of the chest wall defect exceeds 5 cm in adults and adolescents. (VI) For non-small cell lung cancer (NSCLC) invading the chest wall, wide excision with neoadjuvant and/or adjuvant therapy are recommended for patients with stage T3-4N0-1M0. As clear guidelines are lacking, these consensus statements on controversial issues on chest wall tumors and resection could possibly serve as further guidance in clinical practice during the upcoming years
Next generation of growth-sparing techniques: preliminary clinical results of a magnetically controlled growing rod in 14 patients
Session 3A - Early Onset Scoliosis: Paper no. 33SUMMARY: Growth-sparing techniques are commonly used for the treatment of progressive EOS. The standard growing rod (GR) technique requires multiple surgeries for lengthening. The preliminary results of MCGR has shown the comparable outcomes to standard GR without the need for repeated surgery which can be expected to reduce the overall complication rate in GR surgery. INTRODUCTION: The growing rod (GR) technique for management of progressive Early-Onset Scoliosis (EOS) is a viable alternative but with a high complication rate attributed to frequent surgical lengthenings. The safety and efficacy of a non-invasive Magnetically Controlled Growing Rod (MCGR) has been previously reported in a porcine model. We are reporting the preliminary results of this technique in EOS. METHODS: Retrospective review of prospectively collected multi-center data. Only patients who underwent MCGR surgery and at least 3 subsequent spinal distractions were included in this preliminary review. Distractions were performed in clinic without anesthesia or analgesics. T1-T12 and T1-S1 height and the distraction distance inside the actuator were analyzed in addition to conventional clinical and radiographic data. RESULTS: Patients (N=14; 7 F and 7 M) had a mean age of 8y+10m (3y+6m to 12y+7m) and underwent a total of 14 index surgeries (SR: index single rod in 5 and DR: dual rod in 9) and 91 distractions. There were 5 idiopathic, 4 neuromuscular, 2 congenital, 2 syndromic and one NF. Mean follow-up (FU) was 10 months (5.8-18.2). Mean Cobb changed from 57° pre-op to 35° post-op and correction was maintained (35°) at latest FU. T1-T12 increased by 4 mm for SR and 10 mm for DR with mean monthly gain of 0.5 and 1.39, respectively. T1-S1 gain was 4 mm for SR and 17 mm for DR with mean monthly gain of 0.5 mm for SR and 2.35 mm for DR. The mean interval between index surgery and the first distraction was 66 days and thereafter was 43 days. Complications included one superficial infection in (SR), one prominent implant (DR) and minimal loss of initial distraction in three after index MCGR (all SR). Overall, partial loss of distraction was observed following 14 of the 91 distractions (one DR and 13 SR). This loss was regained in subsequent distractions. There was no neurologic deficit or implant failure. CONCLUSION: MCGR appears to be safe and provided adequate distraction similar to the standard GR technique without the need for repeated surgeries. DR patients had better initial curve correction and greater spinal height. No major complications were observed during the short follow-up period.
