    Enabling wearable soft tactile displays with dielectric elastomer actuators

    PhDTouch is one of the less exploited sensory channels in human machine interactions. While the introduction of the tactile feedback would improve the user experience in several fields, such as training for medical operators, teleoperation, computer aided design and 3D model exploration, no interfaces able to mimic accurately and realistically the tactile feeling produced by the contact with a real soft object are currently available. Devices able to simulate the contact with soft bodies, such as the human organs, might improve the experience. The existing commercially available tactile displays consist of complex mechanisms that limit their portability. Moreover, no devices are able to provide tactile stimuli via a soft interface that can also modulate the contact area with the finger pad, which is required to realistically mimic the contact with soft bodies, as needed for example in systems aimed at simulating interactions with virtual biological tissues or in robot-assisted minimally invasive surgery. The aim of this thesis is to develop such a wearable tactile display based on the dielectric elastomer actuators (DEAs). DEAs are a class of materials that respond to an electric field producing a deformation. In particular, in this thesis, the tactile element consists of a so-called hydrostatically coupled dielectric elastomer actuator (HC-DEAs). HC-DEAs rely on an incompressible fluid that hydrostatically couples a DEA-based active part to a passive part interfaced to the user. The display was also tested within a closed-loop configuration consisting of a hand tracking system and a custom made virtual environment. This proof of concept system allowed for a validation of the abilities of the display. Mechanical and psychophysical tests were performed in order to assess the ability of the system to provide tactile stimuli that can be distinguished by the users. Also, the miniaturisation of the HC-DEA was investigated for applications in refreshable Braille displays or arrays of tactile elements for tactile maps

    The first generation of surgical robots are already being installed in a number of operating rooms around the world. Robotics is being introduced to medicine because it allows for unprecedented control and precision of surgical instruments in minimally invasive procedures. So far, robots have been used to position an endoscope, perform gallbladder surgery and correct gastroesophogeal reflux and heartburn. The ultimate goal of the robotic surgery field is to design a robot that can be used to perform closed-chest, beating-heart surgery. The use of robotics in surgery will expand over the next decades without any doubt. Minimally Invasive Surgery (MIS) is a revolutionary approach in surgery. In MIS, the operation is performed with instruments and viewing equipment inserted into the body through small incisions created by the surgeon, in contrast to open surgery with large incisions. This minimizes surgical trauma and damage to healthy tissue, resulting in shorter patient recovery time. The aim of this book is to provide an overview of the state-of-art, to present new ideas, original results and practical experiences in this expanding area. Nevertheless, many chapters in the book concern advanced research on this growing area. The book provides critical analysis of clinical trials, assessment of the benefits and risks of the application of these technologies. This book is certainly a small sample of the research activity on Medical Robotics going on around the globe as you read it, but it surely covers a good deal of what has been done in the field recently, and as such it works as a valuable source for researchers interested in the involved subjects, whether they are currently “medical roboticists” or not

    Oral application of L-menthol in the heat: From pleasure to performance

    When menthol is applied to the oral cavity it presents with a familiar refreshing sensation and cooling mint flavour. This may be deemed hedonic in some individuals, but may cause irritation in others. This variation in response is likely dependent upon trigeminal sensitivity toward cold stimuli, suggesting a need for a menthol solution that can be easily personalised. Menthol’s characteristics can also be enhanced by matching colour to qualitative outcomes; a factor which can easily be manipulated by practitioners working in athletic or occupational settings to potentially enhance intervention efficacy. This presentation will outline the efficacy of oral menthol application for improving time trial performance to date, either via swilling or via co-ingestion with other cooling strategies, with an emphasis upon how menthol can be applied in ecologically valid scenarios. Situations in which performance is not expected to be enhanced will also be discussed. An updated model by which menthol may prove hedonic, satiate thirst and affect ventilation will also be presented, with the potential performance implications of these findings discussed and modelled. Qualitative reflections from athletes that have implemented menthol mouth swilling in competition, training and maximal exercise will also be included

    Integration of novel diagnostic techniques and in-depth characterisation of anorectal (dys)function in studies of healthy volunteers and patients with faecal incontinence

    Introduction Large overlap in the range of values seen in health and disease limit the clinical utility of investigations which describe pathophysiological findings in faecal incontinence (FI). Aims The aims of this thesis were to: 1. investigate the prevalence of major disorders of anal motor and rectal sensory function in FI; 2. better describe stress FI; 3. expand knowledge of normal ranges and develop novel metrics to evaluate anorectal function using both contemporary and emerging diagnostic tests (Rapid Barostat Bag [RBB] pump and the functional lumen imaging probe [EndoFLIP®]); 4. develop understanding of (the role of) parity on anorectal function in health and FI; 5. investigate the interaction of continence mechanisms in healthy individuals. Methods Research methods used in this thesis include systematic review and meta-analysis, retrospective case-control and cohort studies, and a prospective study of anorectal function in health using contemporary and new technologies. Results Anal hypocontractility is the most common pathophysiological finding in FI, but rectal sensory dysfunction remains important, especially in men. Further, assessment of the cough response and amplitude of anal slow waves revealed subtle anal motor dysfunction not appreciated by traditional metrics. Stress FI is poorly researched but common, and appears to represent a more severe FI phenotype. For the first time, normal ranges for rectal compliance, capacity and sensation were generated using the RBB, and distensibility of the anal canal using EndoFLIP®. Prospective studies in health demonstrated limited impact of parity on individual metrics. Conclusion While routine clinical tests of anorectal function are useful for evaluating FI, identification of characteristics or metrics associated with progressive decline in function may prove useful for detecting individuals at risk of FI. Research in healthy populations remains relevant to maintain pace with advancing technology; the concept of normality is still an important part of clinical care
