The Design of GP 2
This papers defines the syntax and semantics of GP 2, a revised version of
the graph programming language GP. New concepts are illustrated and explained
with example programs. Changes to the first version of GP include an improved
type system for labels, a built-in marking mechanism for nodes and edges, a
more powerful edge predicate for conditional rule schemata, and functions
returning the indegree and outdegree of matched nodes. Moreover, the semantics
of the branching and loop statement have been simplified to allow their

Structure based inhibitor design targeting glycogen phosphorylase b. Virtual screening, synthesis, biochemical and biological assessment of novel N-acyl-β-d-glucopyranosylamines
Glycogen phosphorylase (GP) is a validated target for the development of new type 2 diabetes treatments. Exploiting the Zinc docking database, we report the in silico screening of 1888 β- D-glucopyranose-NH-CO-R putative GP inhibitors differing only in their R groups. CombiGlide and GOLD docking programs with different scoring functions were employed with the best performing methods combined in a “consensus scoring” approach to ranking of ligand binding affinities for the active site. Six selected candidates from the screening were then synthesized and their inhibitory potency was assessed both in vitro and ex vivo. Their inhibition constants’ values, in vitro, ranged from 5 to 377 µM while two of them were effective at causing inactivation of GP in rat hepatocytes at low µM concentrations. The crystal structures of GP in complex with the inhibitors were defined and provided the structural basis for their inhibitory potency and data for further structure based design of more potent inhibitors
Spherical Designs and Heights of Euclidean Lattices
We study the connection between the theory of spherical designs and the
question of extrema of the height function of lattices. More precisely, we show
that a full-rank n-dimensional Euclidean lattice, all layers of which hold a
spherical 2-design, realises a stationary point for the height function, which
is defined as the first derivative at 0 of the spectral zeta function of the
associated flat torus. Moreover, in order to find out the lattices for which
this 2-design property holds, a strategy is described which makes use of theta
functions with spherical coefficients, viewed as elements of some space of
modular forms. Explicit computations in dimension up to 7, performed with
Pari/GP and Magma, are reported.Comment: 22 page
Segmental stabilizing exercises and low back pain: What is the evidence?
Study design: A systematic review of randomized controlled trials.
Objectives: To evaluate the effectiveness of segmental stabilizing exercises for
acute, subacute and chronic low back pain with regard to pain, recurrence of pain,
disability and return to work.
Methods: MEDLINE, EMBASE, CINAHL, Cochrane Controlled Trials Register, PEDro
and article reference lists were searched from 1988 onward. Randomized controlled
trials with segmental stabilizing exercises for adult low back pain patients were
included. Four comparisons were foreseen: (1) effectiveness of segmental stabilizing
exercises versus treatment by general practitioner (GP); (2) effectiveness of
segmental stabilizing exercises versus other physiotherapy treatment; (3)
effectiveness of segmental stabilizing exercises combined with other physiotherapy
treatment versus treatment by GP and (4) effectiveness of segmental stabilizing
exercises combined with other physiotherapy treatment versus other physiotherapy
treatment.
Results: Seven trials were included. For acute low back pain, segmental stabilizing
exercises are equally effective in reducing short-term disability and pain and more
effective in reducing long-term recurrence of low back pain than treatment by GP.
For chronic low back pain, segmental stabilizing exercises are, in the short and long
term, more effective than GP treatment and may be as effective as other
physiotherapy treatments in reducing disability and pain. There is limited evidence
that segmental stabilizing exercises additional to other physiotherapy treatment are
equally effective for pain and more effective concerning disability than other
physiotherapy treatments alone. There is no evidence concerning subacute low back
pain.
Conclusion: For low back pain, segmental stabilizing exercises are more effective
than treatment by GP but they are not more effective than other physiotherapy
interventions
Healthcare use among preschool children attending GP-led urgent care centres: a descriptive, observational study
© 2015 Published by the BMJ Publishing Group Limited.Objective Urgent care centres (UCCs) hours were developed with the aim of reducing inappropriate emergency department (ED) attendances in England. We aimed to examine the presenting complaint and outcomes of care in 2 general practitioner (GP)-led UCCs with extended opening times. Design Retrospective observational epidemiological study using routinely collected data. Setting 2 GP-led UCCs in London, colocated with a hospital ED. Participants All children aged under 5 years, attending 2 GP-led UCCs over a 3-year period. Outcomes Outcomes of care for the children including: primary diagnosis; registration status with a GP; destination following review within the UCC; and any medication prescribed. Comparison between GP-led UCC visit rates and routine general practices was also made. Results 3% (n=7747/282 947) of all attenders at the GP-led UCCs were children aged under 5 years. The most common reason for attendance was a respiratory illness (27%), followed by infectious illness (17%). 18% (n=1428) were either upper respiratory tract infections or viral infections. The majority (91%) of children attending were registered with a GP, and over two-thirds of attendances were out of hours. Overall 79% were seen and discharged home. Preschool children were more likely to attend their GP (47.0 per 100) than a GP-led UCC (9.4 per 100; 95% CI 8.9 to 10.0). Conclusions Two-thirds of preschool children attending GP-led UCCs do so out of hours, despite the majority being registered with a GP. The case mix is comparable with those presenting to an ED setting, with the majority managed exclusively by the GPs in the UCC before discharge home. Further work is required to understand the benefits of a GP-led urgent system in influencing future use of services especially emergency care
