    MOSAIC roadmap for mobile collaborative work related to health and wellbeing.

    The objective of the MOSAIC project is to accelerate innovation in Mobile Worker Support Environments. For that purpose MOSAIC develops visions and illustrative scenarios for future collaborative workspaces involving mobile and location-aware working. Analysis of the scenarios is input to the process of road mapping with the purpose of developing strategies for R&D leading to deployment of innovative mobile work technologies and applications across different domains. One of the application domains where MOSAIC is active is health and wellbeing. This paper builds on another paper submitted to this same conference, which presents and discusses health care and wellbeing specific scenarios.     ‘There’s no pill to help you deal with the guilt and shame’: Contemporary experiences of HIV in the United Kingdom

    © The Author(s) 2017. The experience of living with HIV, in the global north, has changed significantly over the past 20 years. This is largely the result of effective biomedical methods of treatment and prevention. HIV is now widely considered to be a long-term condition like many others – it has been argued that HIV has been ‘normalised’. Drawing on online qualitative survey data, with respondents aged 18–35 years, diagnosed with HIV in the past 5 years, this research explores contemporary subjective experiences of being diagnosed, and living, with HIV in the United Kingdom. The data reveal ambiguous experiences and expectations, as the ‘normative’ status of HIV exists alongside ongoing experiences of fear, shame and stigma – maintaining its status as the most ‘social’ of diseases. In rendering HIV ‘everyday’, the space to articulate (and experience) the ‘difference’ which attaches to the virus has contracted, making it difficult to express ambivalence and fear in the face of a positive, largely biomedical, discourse.     European consensus of criteria for the evaluation of good practices in chronic conditions

    Los sistemas sanitarios reconocen las enfermedades crónicas como uno de sus grandes desafíos de salud del siglo XXI para los sistemas sanitarios. A pesar de ser en gran medida prevenibles, las enfermedades crónicas son importante causa de mortalidad y morbilidad en Europa. En 2015, más de 1,2 millones de personas en los países de la UE murieron por enfermedades y lesiones que podrían haberse evitado a través de políticas de salud pública más fuertes o de una atención médica más efectiva y menos fragmentada. La presente tesis doctoral reporta el desarrollo y resultados de una proceso de consenso internacional cuyo objetivo ha sido desarrollar criterios de evaluación para valorar el potencial de las prácticas clínicas e intervenciones y políticas sanitarias a la hora de disminuir la carga atribuible a las enfermedades crónicas en cuatro áreas de interés: Promoción de la salud y prevención primaria de condiciones crónicas; Intervenciones organizativas enfocadas al tratamiento de pacientes crónicos con condiciones clínicas múltiples; Intervenciones sobre el empoderamiento del paciente; e, Intervenciones y políticas orientadas a mejorar la diabetes (la diabetes se utiliza como condición paradigmática). Con objeto de acordar los criterios de evaluación y otorgarles relevancia distinta en función del dominio de interés, se desarrolló un consenso internacional mediante la técnica Delphi-modificada, en la que participaron 113 expertos de diferentes disciplinas procedentes de 23 países europeos. El proceso de consenso produjo 145 categoría de evaluación (28 categorías en el Delphi de Health promotion and primary prevention of chronic conditions, 50 en el de Organizational interventions aimed at dealing with complex chronic patients with multiple conditions, 28 en el de Patient empowerment interventions with chronic conditions y 39 categorías en el Delphi de diabetes as a case-study) orientadas a valorar cada uno de los citados dominios y ponderarlos en función de cada área de interés. El conjunto de criterios y categorías acordados para el caso paradigmático de Diabetes apoya la hipótesis de que los criterios de valoración son transferibles y aplicables a la evaluación de prácticas, intervenciones y políticas desarrolladas sobre otras condiciones crónicas. Consistentemente con lo observado en otras iniciativas europeas, en este consenso, los criterios relacionados con ‘diseño de la práctica’, ‘evaluación’, ‘sostenibilidad’ y ‘escalabilidad’ parecen ser componentes esenciales en el desarrollo e implementación de buenas prácticas en Europa. Por último, como virtualidad destacable de este proceso de consenso, el componente internacional de las decisiones consensuadas, apoya la posibilidad de que las prácticas evaluadas con los criterios y categorías acordados puedan ser transferidas a cualquier contexto europeo.<br /

    An innovative approach for health care delivery to obese patients: from health needs identification to service integration

    In Europe, more than half of the population is overweight or obese, and effort to design, validate, and implement innovative approaches is required to address social and health unmet needs of obese patients in terms of health promotion, disease prevention, and integration of services. The challenge is improving the collaboration between the different health and care stakeholders involved in the lives of obese patients, changing the socio-cultural attitude towards food intake and other behaviours leading to a negative impact on their health-related quality of life. The digital transformation of health and care can support changes in healthcare systems, healthy policy, and approaches to patient care and better implementation of the different health promotion and disease prevention strategies between all the stakeholders and support obese patients. Based on the previously experience adopted by Blueprint Partners with the Blueprint persona and user scenario in the context of models of care and prevention, health policies and analysis of risk factors affecting health and quality of life of obese subjects, the study aimed to simulate an integrated care pathway, through a multidisciplinary approach, developing and applying solutions and good clinical practices addressing the social and health unmet needs of obese patients. A pilot study assessed the quality of life (QoL), adherence to the Mediterranean diet, efficacy and interoperability of a digital health platform, Paginemediche. it. A qualitative approach has been adopted to identify and specify key digital solutions and high-impact user scenarios in Active and Healthy Ageing (AHA). To achieve a successful result, an iterative and collaborative approach has been followed to develop a user-centred perspective to the identification of solutions addressing health needs with different complexity along the entire life-course. Four initial key topic areas were chosen and used to identify different digital solutions that may meet the needs of the population segments defined by both age and the complexity of their health status. All data, derived from the industry representatives in the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA), were collected via a survey to how digital solutions best met the needs of the various population segments represented by personas. Subsequently, innovative solutions were designed based on how a user from a target group interacts with technologies, developing "personas" belonging to specific "population segments" with different conditions and needs. Then, a high-impact user scenario, based on the correlation of personas' needs, good clinical practices and digital solutions available targeting needs which playing a role in the health and care delivery for the persona, has been developed. In the end, to evaluate how digital solutions and technologies can support obese patients during their weight loss or management of their related comorbidities in current service provision, ten obese patients were enrolled to evaluate a Digital Health platform, pagininemediche.it, developed. Matilde, the Blueprint persona developed, highlighted some of the main needs (social support, development of a health-friendly environment and educational program on healthy nutrition and physical activity) that may be addressed by integrating innovative solutions in the care of obese patients. Based on her profile, a high-impact user scenario diagram correlates health and social needs with digital solutions and can help key actors in the creation of a well-integrated care approach. Moreover, the evaluation of the digital platform, paginemediche.it, demonstrated how digital solutions can motivate and support obese patients in changing habits towards a healthy lifestyle, although no further statistical significance has been identified in the quality of life assessment because of the limited number of the patients, and short period of observation. Overweight or obese patients tend to be marginalized and the subject of a real social stigma. Digital solutions may be useful to overcome psychological factors that prevent obese patients from starting their journey for a lifestyle change. The suggested approach, which considers health needs, IT skills, socioeconomic context, interoperability, and integration gaps that may influence the adoption of innovative solutions tailored to improve health outcomes is person-centred, and identify what is important for obese patients. The implementation of a persona and user scenario approach may also be useful for the early involvement of end-users in solutions' design and adaptation, increasing adherence, and the effectiveness of digital solutions. Persona profiles, the user scenario, and the related digital solution also consider the potential benefits that can derive for both patients and health system in term of reduced emergency room admissions, waiting lists, and health related expenditures

    Tecnologias pervasivas da saúde : como está o futuro a ser preparado pelas empresas de software em Portugal?

    The present work addresses the software industry aiming to evaluate its preparation for the opportunity that the healthcare area offers in terms of pervasive healthcare technologies, as a perspective for the growth of the market. Phenomena such as chronic disease growth and survival, increasing longevity, active ageing, remote patient monitoring, the big data of healthcare and the globalization of the smartphone, provide growth opportunities for the software industry and indicate a path by which where societies are orienting themselves and giving special importance. Following a literature review, after a general introduction to the major themes of economic development and globalization, and a survey of current technologies and the evolution of successful industries in Portugal, it was developed an analysis to the software industry and the results of a survey of associates of an Industry Association. The study concludes that while there are positive aspects that may help the software industry to be prepared for the opportunities that the health sector offers, there are key elements that will have to be improved not only in clusters activities as in knowledge of health and focus on exports that will have to be improved so that industry can be successful in this field.O presente trabalho aborda a indústria do software pretendendo avaliar a sua preparação para a oportunidade que a área da saúde oferece em termos de tecnologias pervasivas de saúde numa perspetiva de crescimento do mercado. Fenómenos como o aumento das doenças crónicas, a crescente longevidade, bem como o aparecimento de soluções tecnológicas extraordinariamente sofisticadas, oferece oportunidades de crescimento à indústria do software e indicam um caminho por onde as sociedades estão a orientar-se e a dar especial importância. Após uma pesquisa bibliográfica, que permitiu um levantamento das tecnologias atuais e da evolução das indústrias bem-sucedidas em Portugal, desenvolveu-se uma análise à indústria do software a partir dos resultados de um inquérito dirigido aos associados de uma Associação representativa da indústria. Do estudo conclui-se que embora haja aspetos positivos que poderão ajudar a indústria do software a preparar-se para as oportunidades que o sector da saúde oferece, há elementos essenciais que terão que ser melhorados não só na orgânica produtiva com base em clusters, mas também no conhecimento da área da saúde e enfoque nas exportações que terão que ser melhoradas para que a indústria possa ser bem-sucedida neste ramo.Programa Doutoral em Ciências e Tecnologias da Saúd

    Telemedicine Scenario for Elderly People with Comorbidity

    Progressive population aging is associated with negative social and economic impacts mainly due to its associated comorbidity rather than to aging per se. In this regard, information and communication technology resources may provide useful tools to assist the population with comorbidities through the use of telemedicine systems. However, despite their potential, such systems have not yet been effectively implemented due to a number of different reasons: absence of a clear business plan, poor acknowledgement of their clinical usefulness, and ethical and legal issues, among others. An analysis of current scenario from the point of view of the different actors (patients, health care providers, and health care systems) aimed at identifying the needs to be covered by telemedicine systems that could contribute to overcoming such problems.     Integrating social and health services for people, communities, homes, and places

    The challenge of an ageing population requires a paradigmatic shift in the way we provide social and healthcare services, demanding the need to prioritize the functionality and independence of older adults. The risk and subsequent fear of falling is one of the most high-risk states for older adults, as it generates a destabilizing effect on their health that is often hard to recover. It is essential to thoroughly address their risk factors and mitigators. This discussion needs to be made in light of a person-centered perspective that goes beyond fragilities to capitalize on the strengths of the older adults. The chapter provides examples of how to connect assessment, interventions, and monitoring to a coherent framework approach that mitigates the risks and the impact of falls on an ageing society. The authors explore how technological innovation, urban planning, and regional policies that are culturally relevant can be incorporated in creating a circular economy while meeting the needs of an aging population and preventing falls and cognitive decline

    Population ageing and ICT: An exploratory review of technology innovation trough digital applications

    The multidimensional process of physical, psychological, and social change produced by population ageing affects not only the quality of life of elderly people but also of our societies. Some dimensions of population ageing grow and expand over time (e.g. knowledge of the world events, or experience in particular situations), while others decline (e.g. reaction time, physical and psychological strength, or other functional abilities like reduced speed and tiredness). Information and Communication Technologies (ICTs) can help elderly to overcome possible limitations due to ageing. As a particular case, biometrics can allow the development of new algorithms for early detection of cognitive impairments, by processing continuous speech, handwriting or other challenged abilities. Among all possibilities, digital applications (Apps) for mobile phones or tablets can allow the dissemination of such tools. In this article, after presenting and discussing the process of population ageing and its social implications, we explore how ICTs through different Apps can lead to new solutions for facing this major demographic challenge
