Improving knowledge management through the support of image examination and data annotation using DICOM structured reporting
[EN] An important effort has been invested on improving the image diagnosis process in different medical areas using information technologies. The field of medical imaging involves two main data types: medical imaging and reports. Developments based on the DICOM standard have demonstrated to be a convenient and widespread solution among the medical community. The main objective of this work is to design a Web application prototype that will be able to improve diagnosis and follow-on of breast cancer patients. It is based on TRENCADIS middleware, which provides a knowledge-oriented storage model composed by federated repositories of DICOM image studies and DICOM-SR medical reports. The full structure and contents of the diagnosis reports are used as metadata for indexing images. The TRENCADIS infrastructure takes full advantage of Grid technologies by deploying multi-resource grid services that enable multiple views (reports schemes) of the knowledge database. The paper presents a real deployment of such Web application prototype in the Dr. Peset Hospital providing radiologists with a tool to create, store and search diagnostic reports based on breast cancer explorations (mammography, magnetic resonance, ultrasound, pre-surgery biopsy and post-surgery biopsy), improving support for diagnostics decisions. A technical details for use cases (outlining enhanced multi-resource grid services communication and processing steps) and interactions between actors and the deployed prototype are described. Content-Based Organisation of Virtual Repositories of DICOM Objects
The integration of multi-centre medical image data to create knowledge repositories for research and
training activities has been an aim targeted since long ago. This paper presents an environment to share,
to process and to organise medical imaging data according to a structured framework in which the image
reports play a key role. This environment has been validated on a clinical environment, facing problems
such as firewalls and security restrictions, in the frame of the CVIMO (Valencian Cyberinfrastructure of
Medical Imaging in Oncology) project. The environment uses a middleware called TRENCADIS (Towards a
Grid Environment for Processing and Sharing DICOM Objects) that provides users with the management
of multiple administrative domains, data encryption and decryption on the fly and semantic indexation
of images. Data is structured into four levels: Global data available, virtual federated storages of studies
shared across a vertical domain, subsets for projects or experiments on the virtual storage and individual
searches on these subsets. This structure of levels gives the needed flexibility for organising authorisation,
and hides data that are not relevant for a given experiment. The main components and interactions are
shown in the document, outlining the workflows and explaining the different approaches considered,
including the protocols used and the difficulties met.

proven to be successful in many research disciplines. However, the
use of these environments when personal data are involved (such
as in health) is reduced due to its lack of trust. There are many approaches
that provide encrypted storages and key shares to prevent
the access from unauthorized users. However, these approaches
are additional layers that should be managed along with the authorization
policies. We present in this paper a privacy-enhancing
technique that uses encryption and relates to the structure of the
data and their organizations, providing a natural way to propagate
authorization and also a framework that fits with many use cases.
The paper describes the architecture and processes, and also shows

Diseño y desarrollo de una arquitectura software genérica orientada a servicios para la construcción de un middleware grid orientado a la gestión y proceso seguro de información en formato DICOM sobre un marco ontológico
Una de las áreas que más se ha beneficiado del soporte digital es la imagen médica, reforzada por la aparición del estándar Digital Imaging and Communication in Medicine (DICOM), evolucionado a lo largo de los años para soportar no sólo imágenes médicas sino otros tipos de información médica como videos, señales e incluso informes estructurados (DICOM-SR). Con la aparición de DICOM, se ha conseguido la integración de dispositivos de adquisición, visualización, impresión y almacenamiento de imágenes médicas de diferentes fabricantes, al ser este un estándar utilizado por todas las compañias proveedoras.
En la actualidad, los sistemas que trabajan con imagen médica digital, como PACS, RIS y HIS permiten integrar los datos a nivel departamental y hospitalario, existiendo soluciones comerciales muy efectivas. En estos sistemas, la seguridad de usuarios y datos se gestiona en un único dominio administrativo restringido. A consecuencia de su uso en producción en la práctica clÃnica, existe en la actualidad una gran cantidad de información en formato DICOM cuya utilización se restringe generalmente al tratamiento de los pacientes individuales. Sin embargo, la investigación médica requiere consolidar información multicéntrica para la extracción de patrones y la validación de técnicas y diagnósticos, realizándose esta actividad de forma manual y sin herramientas especiales.
En la presente tesis se plantea como objetivo general la definición de una Arquitectura Orientada a Servicios (SOA) y la implementación de un Middleware Grid basado en esta arquitectura, cuya principal función será la gestión, integración y proceso de información en formato DICOM almacenada de forma distribuida en diferentes dominios administrativos, de forma segura y estructurada semánticamente mediante la definición de ontologÃas médicas basadas en el informe estructurado y los estudios DICOM. Este middleware, proporciona a los desarrolladores un interfaz de alto nivel orientado a objetos que permite aumentar la productividad en el desarrollo de aplicaciones en diferentes ámbitos médicos.
Las aportaciones más destacables de esta tesis son las siguientes:
" Diseño de una Arquitectura Grid de propósito general para virtualizar el almacenamiento distribuido y proceso de datos en formato DICOM, basada en Web Services Resorce Framework (WSRF), frente a otras arquitecturas existentes que no utilizan estándares.
" Desarrollo de un modelo para la indexación y estructuración de datos DICOM basado en ontologÃas médicas obtenidas a partir de informes radiológicos, frente a los modelos convencionales basados en nombres identificadores y metadata básica.
" Diseño e implementación de un sistema de autorización que estructura los permisos de los miembros de las organizaciones virtuales a partir de ontologÃas médicas.

Modelos de indexación de informes estructurados dicom-sr basados en grafos sobre infraestructuras grid avanzadas trencadis
[ES] El continuo crecimiento exponencial de la información en los entornos
empresariales comienza a suponer una amenaza para los analistas de datos.
Es necesario disponer de nuevas capacidades, tecnologÃas y modelos que
nos permitan obtener ventajas sobre los actuales, basados en herramientas
tradicionales de gestión de datos.
Al igual que los entornos empresariales, los entes hospitalarios tienen el
mismo problema, por lo que el volumen de información que generan, su
velocidad de cambio y su variedad necesitan disponer de una serie de
herramientas que permitan su tratamiento y visualización. Esto se enmarca
en el ámbito Big Data, por lo que, es necesario solucionar la problemática
expuesta siguiendo dicho término.
Por ello, este proyecto desarrolla una solución a la problemática existente
con la indexación de los informes estructurados asociados a las imágenes
DICOM, gestionados a través de infraestructuras avanzadas basadas en
TRENCADIS, dentro del contexto de Big Data.
En esta tesis se ha desarrollado un modelo en el contexto Big Data basado
en la teorÃa de grafos y se han diseñado e implementado los componentes
necesarios para la integración del mismo en TRENCADIS, de forma que la
indexación se realizará bajo un entorno Grid y con una base de datos noSQL
basada en grafos.
Se presentan una serie de pruebas para comparar los modelos propuestos
con los ya existentes, y, aunque en un primer modelo el resultado no es el
esperado, al refinarlo y generar un segundo modelo los resultados obtenidos
mejoran la implementación actual, dando pie a futuros trabajos de
tratamiento y visualización de datos.[EN] The continued exponential growth of information in enterprise environments
posses many challenges to data analysis, which cannot be resolved using
traditional tools. To cope with these challenges, new skills, technologies
and models are needed that allow us to leverage the information.
This problem is not exclusively related to business, but also public health
is facing an unprecedented increase in the amount and diversity of data that
needs to be stored and analysed. Big Data is providing the new generation
tools for data processing and visualization.
This work presents a new system to index DICOM structured reports using
TRENCADIS, an advanced data infrastructure for DICOM objects.
The system developed in this thesis uses graph theory to model structured
reports and their interrelationships in a large noSQL database. We describe
the design and the implementation of the system, as well as the integration
in a Grid environment.
Finally, we present a series of experiments that compare the new model to
other previously published results. A second, refined model is also
presented that further improves performance. The conclusions suggest the
need for future lines of work in the context of data processing and

A formal architecture-centric and model driven approach for the engineering of science gateways
From n-Tier client/server applications, to more complex academic Grids, or even the most recent and promising industrial Clouds, the last decade has witnessed significant developments in distributed computing. In spite of this conceptual heterogeneity, Service-Oriented Architecture (SOA) seems to have emerged as the common and underlying abstraction paradigm, even though different standards and technologies are applied across application domains. Suitable access to data and algorithms resident in SOAs via so-called ‘Science Gateways’ has thus become a pressing need in order to realize the benefits of distributed computing infrastructures.In an attempt to inform service-oriented systems design and developments in Grid-based biomedical research infrastructures, the applicant has consolidated work from three complementary experiences in European projects, which have developed and deployed large-scale production quality infrastructures and more recently Science Gateways to support research in breast cancer, pediatric diseases and neurodegenerative pathologies respectively. In analyzing the requirements from these biomedical applications the applicant was able to elaborate on commonly faced issues in Grid development and deployment, while proposing an adapted and extensible engineering framework. Grids implement a number of protocols, applications, standards and attempt to virtualize and harmonize accesses to them. Most Grid implementations therefore are instantiated as superposed software layers, often resulting in a low quality of services and quality of applications, thus making design and development increasingly complex, and rendering classical software engineering approaches unsuitable for Grid developments.The applicant proposes the application of a formal Model-Driven Engineering (MDE) approach to service-oriented developments, making it possible to define Grid-based architectures and Science Gateways that satisfy quality of service requirements, execution platform and distribution criteria at design time. An novel investigation is thus presented on the applicability of the resulting grid MDE (gMDE) to specific examples and conclusions are drawn on the benefits of this approach and its possible application to other areas, in particular that of Distributed Computing Infrastructures (DCI) interoperability, Science Gateways and Cloud architectures developments
Diseño e implementación de Informes estructurados para el diagnóstico y seguimiento del Neuroblastoma y Glioma Pontino IntrÃnseco Difuso
[ES] El uso de las nuevas tecnologÃas en la toma de decisiones en el ámbito de la medicina es un factor clave para el desarrollo de un sistema sanitario robusto. La cantidad de información relativa a pacientes que se maneja actualmente necesita de un tratamiento y una organización que permita que el flujo de información entre distintos centros hospitalarios sea rápido y eficaz, siguiendo unos estándares internacionales que proporcionen una validez global de las interpretaciones que se realicen (interoperabilidad semántica). En este TFM, se propone el desarrollo de un módulo que genere en base a los datos recolectados de pacientes de Neuroblastoma y Glioma Pontino IntrÃnseco Difuso, informes estructurados que organicen esta información siguiendo el estándar DICOM. La codificación de términos, la semántica y el uso de ontologÃas son factores que han sido necesarios para obtener una correcta organización de la información. El módulo permite que los profesionales sanitarios generen un informe estructurado en estándar DICOM, visible desde las estaciones de trabajo de los sanitarios, que recoge información de diagnóstico, tratamiento y seguimiento en el que apoyarse para realizar la toma de decisiones.[EN] Use of new technologies to improve decision-making in medicine is a key factor for the development of a robust healthcare system. The amount of in- formation related to patients requires treatment and an organization that allows the information flow between different hospital centers to be fast and effective,following international standards providing global validation about interpretations (semantic interoperability). The development of a module that generates structured reports following DICOM standard based on the collected data from Neuroblastoma and Diffuse Intrinsic Pontine Glioma patients is proposed. Term codification, semantics and use of ontologies are factors that have been necessary to obtain a correct organization of the information.
