    Estimation of the worldwide seroprevalence of cytomegalovirus : a systematic review and meta-analysis

    Cytomegalovirus (CMV) infection does not usually produce symptoms when it causes primary infection, reinfection, or reactivation because these three types of infection are all controlled by the normal immune system. However, CMV becomes an important pathogen in individuals whose immune system is immature or compromised, such as the unborn child. Several vaccines against CMV are currently in clinical trials that aim to induce immunity in seronegative individuals and/or to boost the immunity of those with prior natural infection (seropositives). To facilitate estimation of the burden of disease and the need for vaccines that induce de novo immune responses or that boost pre-existing immunity to CMV, we conducted a systematic survey of the published literature to describe the global seroprevalence of CMV IgG antibodies. We estimated a global CMV seroprevalence of 83% (95%UI: 78-88) in the general population, 86% (95%UI: 83-89) in women of childbearing age, and 86% (95%UI: 82-89) in donors of blood or organs. For each of these three groups, the highest seroprevalence was seen in the World Health Organisation (WHO) Eastern Mediterranean region 90% (95%UI: 85-94) and the lowest in WHO European region 66% (95%UI: 56-74). These estimates of the worldwide CMV distribution will help develop national and regional burden of disease models and inform future vaccine development efforts

    Extended Infusions of Meropenem for Febrile Neutropenia

    Background: Neutropenic fever is an oncologic emergency that requires quick intervention with anti-pseudomonal beta-lactam antibiotics, such as meropenem. Previous literature suggests that extended infusions of beta-lactam antibiotics may improve clinical outcomes. To date, there are 3 prior studies utilizing an extended infusion beta-lactam in this population; however, there is only one previous study investigating the use of extended infusion meropenem in patients with febrile neutropenia. Objective: To describe the outcomes of eight patients receiving extended infusions of meropenem for the treatment of febrile neutropenia. Methods: A retrospective chart review was completed including adult patients admitted to a community teaching hospital who received extended infusions of meropenem for febrile neutropenia. Results: In this descriptive study, no patients receiving extended infusions of meropenem failed treatment, were readmitted for an infectious issue within 30 days, or endured inpatient mortality. Additionally, all eight patients defervesced within 48 hours, and four patients had a microbiologically documented infection. One patient incurred Clostridium difficile on day 2 of meropenem therapy. Conclusions: Extended infusions of meropenem may be effective in the treatment of febrile neutropenia. Future studies comparing extended infusions to intermittent infusions of meropenem for febrile neutropenia are warranted

    Use of mental simulations to change theory of planned behaviour variables

    Objectives. The predictive validity of the theory of planned behaviour iswell established, but much less is known about: (a) whether there are causal relationships between key components of the model and (b) how to go about changing the theory of planned behaviour variables. This study tested the ability of outcome and process simulations to change variables specified in the theory of planned behaviour in relation to blood donation. Design. Participants (N ÂĽ 146) were randomized to one of four conditions: outcome simulation only, process simulation only, process-plus-outcome simulation and a distractor control condition. The dependent variables were state anxiety, and intention attitude, subjective norm and perceived control from the theory of planned behaviour. Methods. Participants were asked to empty their mind and visualize themselves: (a) after donating blood (outcome manipulation), (b) preparing to donate blood (process manipulation), (c) both preparing to donate blood and after having donated blood (process-plus-outcome manipulation) or (d) both preparing to get a high mark and after having got a high mark on their course (control condition). Following mental rehearsal, participants completed the dependent variables. Results. There were no main effects of outcome simulation, but process simulation successfully increased intention, subjective norm and perceived control. There was also a significant outcome simulation x process simulation interaction for attitude. The effect of the process manipulation on intention was mediated by subjective norm and perceived control. Conclusions. The findings show promise for the use of mental simulations in changing cognitions and further research is required to extend the present findings to other health behaviours.</p

    Different Methods of Embodied Cognition in Pedagogy and its Effectiveness in Student Learning

    The Mathematical Ideas Analysis hypothesizes that abstract mathematical reasoning is unconsciously organized and integrated with sensory-motor experience. Basic research testing movement, language, and perception during math problem solving supports this hypothesis. Applied research primarily measures students’ performance on math tests after they engage in analogous sensory-motor tasks, but findings show mixed results. Sensory-motor tasks are dependent on several moderators (e.g., instructional guidance, developmental stage) known to help students learn, and studies vary in how each moderator is implemented. There is little research on the effectiveness of sensory-motor tasks without these moderators. This study compares different approaches to working with an interactive application designed to emulate how people intrinsically solve algebraic equations. A total of 130 participants (84 females, 54 males) were drawn from a pool of Introductory Psychology students attending San Jose State University. Participants were placed in three different learning environments, and their performance was measured by comparing improvement between a pre-test and a post-test. We found no difference between participants who worked alone with the application, were instructed by the experimenter while using the application, or who instructed the experimenter on how to solve equations using the application. Further research is needed to examine how and whether analogous sensory-motor interfaces are a useful learning tool, and if so, what circumstances are ideal for sensory-motor interfaces to be used

    Blinding is a methodologic safeguard of treatment evaluation, yet severely understudied empirically. Mathieu et al.'s theoretical analysis (2014) provided an important message that blinding cannot eliminate potential for bias associated with belief about allocation in randomized controlled trial; just like the intent-to-treat principle does not guarantee unbiased estimation under noncompliance, the blinded randomized trial as a golden standard may produce bias. They showed possible biases but did not assess how large the bias could be in different scenarios. In this paper, we examined their findings, and numerically assessed and compared the bias in treatment effect parameters by simulation under frequently encountered blinding scenarios, aiming to identify the most ideal blinding scenarios in practice. We conclude that Random Guess and Wishful Thinking (e.g., participants tend to believe they received treatment) are the most ideal blinding scenarios, incurring minimal bias. We also find some evidence that imperfect or partial blinding can be better than no blinding

    Fostering college and career readiness: how career development activities in schools impact on graduation rates and students' life success

    This paper sets out the recent evidence around career development. This evidence is examined within the context of the college and career readiness agenda. The argument is made that in order for young people to be genuinely “ready” for both college and career they need to have attended to their academic achievement, their aspirations and plans for the future, their ability to make transitions and their ability to direct their own careers. It is argued that career development offers schools a body of practice that has been shown to have a positive impact on young people’s readiness for college and career. The report acknowledges that the provision of career development has been in decline in many North American schools despite evidence of its effectiveness. Given the current instability of the labor market, the increasing complexity of the education system and the need to grow the skills base of the workforce in a competitive global market, failing to attend to young people’s careers seems shortsighted. As this paper shows, there is a strong body of evidence which demonstrates that career development activity in schools can help young people to experience academic achievement, successfully transition to the labor market and live happier and more productive lives. It is hoped that setting out the evidence in this area of research will provide policy makers and school leaders with the resources required to make informed decisions and to support the development of the future generations of talent.
