    Rehabilitation:mobility, exercise & sports; a critical position stand on current and future research perspectives

    Background Human movement, rehabilitation, and allied sciences have embraced their ambitions within the cycle of “RehabMove” congresses over the past 30 years. This combination of disciplines and collaborations in the Netherlands has tried to provide answers to questions in the fields of rehabilitation and adapted sports, while simultaneously generating new questions and challenges. These research questions help us to further deepen our understanding of (impaired) human movement and functioning, with and without supportive technologies, and stress the importance of continued multidisciplinary (inter)national collaboration. Methods This position stand provides answers that were conceived by the authors in a creative process underlining the preparation of the 6th RehabMove Congress. Results The take-home message of the RehabMove2018 Congress is a plea for continued multidisciplinary research in the fields of rehabilitation and adapted sports. This should be aimed at more individualized notions of human functioning, practice, and training, but also of performance, improved supportive technology, and appropriate “human and technology asset management” at both individual and organization levels and over the lifespan. Conclusions With this, we anticipate to support the development of rehabilitation sciences and technology and to stimulate the use of rehabilitation notions in general health care. We also hope to help ensure a stronger embodiment of preventive and lifestyle medicine in rehabilitation practice. Indeed, general health care and rehabilitation practice require a healthy and active lifestyle management and research agenda in the context of primary, secondary, and tertiary prevention. IMPLICATIONS FOR REHABILITATION Continued multidisciplinary (international) collaboration will stimulate the development of rehabilitation and human movement sciences. Notions from “human and technology asset management and ergonomics” are fundamental to rehabilitation practice and research.     According to the OPTIMAL (Optimizing Performance Through Intrinsic Motivation and Attention of Learning) theory of motor learning, enhanced expectancies (EE), autonomy support (AS), and external focus (EF) augment the coupling of a person’s actions to intended movement goals. This goal-action coupling is postulated to boost a person’s focus on goal-related aspects of the motor task while reducing the person’s self-related thoughts, resulting in enhanced performance of skilled movements as well as in improving the acquisition outcomes for the learning of motor skills. The three studies in this compilation report were aimed at providing empirical evidence for the motor performance benefits of the combinatory implementation of the three key motivational (i.e., EE and AS) and attentional (i.e., EF) factors of the OPTIMAL theory. In addition, a preliminary investigation of the neuromechanistic influence of such an implementation on the human motor system was carried out. Using a between-participants design, the first study employed a maximal-effort countermovement jump task to examine the additive effects of the consecutive (or serial) implementation of EE, AS, and EF on motor performance. Results indicated that optimized group participants produced greater relative jump heights than control group participants. The second study used a within-participants design involving a clinical-applied balance test to determine the immediate effects of implementing EE, AS, and EF simultaneously (in parallel) on motor performance. The results showed that participants experienced greater postural stability in terms of making fewer balance errors and producing lower center-of-pressure velocity in the optimized condition than the control condition. Finally, a simple visuomotor task involving the rhythmic production of force via isometric finger abduction was used in the third study with a between-participants design. The neurophysiological and behavioral effects of a simultaneous implementation of EE, AS, and EF in relation to motor performance were examined using a novel TMS-force experimental protocol. The corticospinal excitability of all participants remained stable throughout the experiment. Additionally, the force-accuracy performance of participants in the optimized group was similar to that of participants in the control group

Its motor and nonmotor signs determine a huge functional loss, leading the individuals to lose their independence. Although the treatment requires a pharmacological approach, physical therapy has confirmed its importance in this process. Today, neurorehabilitation is indispensable to increase many of the cardinal signs of the disease. Using traditional or technological approaches, physical therapy has reached good results in improving motor and nonmotor functions, as well as the quality of life of Parkinsonians. However, it is important to develop and to fortify the physical therapy approach so that we can provide stronger evidence about our practice

    Nowadays, cerebral palsy (CP) rehabilitation, along with medical and surgical interventions in children with CP, leads to better motor and postural control and can ensure ambulation and functional independence. In achieving these improvements, many modern practices may be used, such as comprehensive multidisciplinary assessment, clinical decision making, multilevel surgery, botulinum toxin applications, robotic ambulation applications, treadmill, and other walking aids to increase the quality and endurance of walking. Trainings are based on neurodevelopmental therapy, muscle training and strength applications, adaptive equipment and orthotics, communication, technological solves, and many others beyond the scope of this book. In the years of clinical and academic experiences, children with cerebral palsy have shown us that the world needs a book to give clinical knowledge to health professionals regarding these important issue. This book is an attempt to fulfill and to give “current steps” about CP. The book is intended for use by physicians, therapists, and allied health professionals who treat/rehabilitate children with CP. We focus on the recent concepts in the treatment of body and structure problems and describe the associated disability, providing suggestions for further reading. All authors presented the most frequently used and accepted treatment methods with scientifically proven efficacy and included references at the end of each chapter
