    Accuracy and reliability analysis of a machine learning based segmentation tool for intertrochanteric femoral fracture CT

    IntroductionThree-dimensional (3D) reconstruction of fracture fragments on hip Computed tomography (CT) may benefit the injury detail evaluation and preoperative planning of the intertrochanteric femoral fracture (IFF). Manually segmentation of bony structures was tedious and time-consuming. The purpose of this study was to propose an artificial intelligence (AI) segmentation tool to achieve semantic segmentation and precise reconstruction of fracture fragments of IFF on hip CTs.Materials and MethodsA total of 50 labeled CT cases were manually segmented with Slicer 4.11.0. The ratio of training, validation and testing of the 50 labeled dataset was 33:10:7. A simplified V-Net architecture was adopted to build the AI tool named as IFFCT for automatic segmentation of fracture fragments. The Dice score, precision and sensitivity were computed to assess the segmentation performance of IFFCT. The 2D masks of 80 unlabeled CTs segmented by AI tool and human was further assessed to validate the segmentation accuracy. The femoral head diameter (FHD) was measured on 3D models to validate the reliability of 3D reconstruction.ResultsThe average Dice score of IFFCT in the local test dataset for “proximal femur”, “fragment” and “distal femur” were 91.62%, 80.42% and 87.05%, respectively. IFFCT showed similar segmentation performance in cross-dataset, and was comparable to that of human expert in human-computer competition with significantly reduced segmentation time (p < 0.01). Significant differences were observed between 2D masks generated from semantic segmentation and conventional threshold-based segmentation (p < 0.01). The average FHD in the automatic segmentation group was 47.5 ± 4.1 mm (41.29∼56.59 mm), and the average FHD in the manual segmentation group was 45.9 ± 6.1 mm (40.34∼64.93 mm). The mean absolute error of FHDs in the two groups were 3.38 mm and 3.52 mm, respectively. No significant differences of FHD measurements were observed between the two groups (p > 0.05). All ICCs were greater than 0.8.ConclusionThe proposed AI segmentation tool could effectively segment the bony structures from IFF CTs with comparable performance of human experts. The 2D masks and 3D models generated from automatic segmentation were effective and reliable, which could benefit the injury detail evaluation and preoperative planning of IFFs

    Digital three-dimensional visualization of intrabony periodontal defects for regenerative surgical treatment planning

    BACKGROUND: In the regenerative treatment of intrabony periodontal defects, surgical strategies are primarily determined by defect morphologies. In certain cases, however, direct clinical measurements and intraoral radiographs do not provide sufficient information on defect morphologies. Therefore, the application of cone-beam computed tomography (CBCT) has been proposed in specific cases. 3D virtual models reconstructed with automatic thresholding algorithms have already been used for diagnostic purposes. The aim of this study was to utilize 3D virtual models, generated with a semi-automatic segmentation method, for the treatment planning of minimally invasive periodontal surgeries and to evaluate the accuracy of the virtual models, by comparing digital measurements to direct intrasurgical measurements. METHODS: Four patients with a total of six intrabony periodontal defects were enrolled in the present study. Two months following initial periodontal treatment, a CBCT scan was taken. The novel semi-automatic segmentation method was performed in an open-source medical image processing software (3D Slicer) to acquire virtual 3D models of alveolar and dental structures. Intrasurgical and digital measurements were taken, and results were compared to validate the accuracy of the digital models. Defect characteristics were determined prior to surgery with conventional diagnostic methods and 3D virtual models. Diagnostic assessments were compared to the actual defect morphology during surgery. RESULTS: Differences between intrasurgical and digital measurements in depth and width of intrabony components of periodontal defects averaged 0.31 ± 0.21 mm and 0.41 ± 0.44 mm, respectively. In five out of six cases, defect characteristics could not be assessed precisely with direct clinical measurements and intraoral radiographs. 3D models generated with the presented semi-automatic segmentation method depicted the defect characteristics correctly in all six cases. CONCLUSION: It can be concluded that 3D virtual models acquired with the described semi-automatic segmentation method provide accurate information on intrabony periodontal defect morphologies, thus influencing the treatment strategy. Within the limitations of this study, models were found to be accurate; however, further investigation with a standardized validation process on a large number of participants has to be conducted

    The Development And Application Of A Statistical Shape Model Of The Human Craniofacial Skeleton

    Biomechanical investigations involving the characterization of biomaterials or improvement of implant design often employ finite element (FE) analysis. However, the contemporary method of developing a FE mesh from computed tomography scans involves much manual intervention and can be a tedious process. Researchers will often focus their efforts on creating a single highly validated FE model at the expense of incorporating variability of anatomical geometry and material properties, thus limiting the applicability of their findings. The goal of this thesis was to address this issue through the use of a statistical shape model (SSM). A SSM is a probabilistic description of the variation in the shape of a given class of object. (Additional scalar data, such as an elastic constant, can also be incorporated into the model.) By discretizing a sample (i.e. training set) of unique objects of the same class using a set of corresponding nodes, the main modes of shape variation within that shape class are discovered via principal component analysis. By combining the principal components using different linear combinations, new shape instances are created, each with its own unique geometry while retaining the characteristics of its shape class. In this thesis, FE models of the human craniofacial skeleton (CFS) were first validated to establish their viability. A mesh morphing procedure was then developed to map one mesh onto the geometry of 22 other CFS models forming a training set for a SSM of the CFS. After verifying that FE results derived from morphed meshes were no different from those obtained using meshes created with contemporary methods, a SSM of the human CFS was created, and 1000 CFS FE meshes produced. It was found that these meshes accurately described the geometric variation in human population, and were used in a Monte Carlo analysis of facial fracture, finding past studies attempting to characterize the fracture probability of the zygomatic bone are overly conservative

    The first generation of surgical robots are already being installed in a number of operating rooms around the world. Robotics is being introduced to medicine because it allows for unprecedented control and precision of surgical instruments in minimally invasive procedures. So far, robots have been used to position an endoscope, perform gallbladder surgery and correct gastroesophogeal reflux and heartburn. The ultimate goal of the robotic surgery field is to design a robot that can be used to perform closed-chest, beating-heart surgery. The use of robotics in surgery will expand over the next decades without any doubt. Minimally Invasive Surgery (MIS) is a revolutionary approach in surgery. In MIS, the operation is performed with instruments and viewing equipment inserted into the body through small incisions created by the surgeon, in contrast to open surgery with large incisions. This minimizes surgical trauma and damage to healthy tissue, resulting in shorter patient recovery time. The aim of this book is to provide an overview of the state-of-art, to present new ideas, original results and practical experiences in this expanding area. Nevertheless, many chapters in the book concern advanced research on this growing area. The book provides critical analysis of clinical trials, assessment of the benefits and risks of the application of these technologies. This book is certainly a small sample of the research activity on Medical Robotics going on around the globe as you read it, but it surely covers a good deal of what has been done in the field recently, and as such it works as a valuable source for researchers interested in the involved subjects, whether they are currently “medical roboticists” or not

    Intraoperative Quantification of Bone Perfusion in Lower Extremity Injury Surgery

    Orthopaedic surgery is one of the most common surgical categories. In particular, lower extremity injuries sustained from trauma can be complex and life-threatening injuries that are addressed through orthopaedic trauma surgery. Timely evaluation and surgical debridement following lower extremity injury is essential, because devitalized bones and tissues will result in high surgical site infection rates. However, the current clinical judgment of what constitutes “devitalized tissue” is subjective and dependent on surgeon experience, so it is necessary to develop imaging techniques for guiding surgical debridement, in order to control infection rates and to improve patient outcome. In this thesis work, computational models of fluorescence-guided debridement in lower extremity injury surgery will be developed, by quantifying bone perfusion intraoperatively using Dynamic contrast-enhanced fluorescence imaging (DCE-FI) system. Perfusion is an important factor of tissue viability, and therefore quantifying perfusion is essential for fluorescence-guided debridement. In Chapters 3-7 of this thesis, we explore the performance of DCE-FI in quantifying perfusion from benchtop to translation: We proposed a modified fluorescent microsphere quantification technique using cryomacrotome in animal model. This technique can measure bone perfusion in periosteal and endosteal separately, and therefore to validate bone perfusion measurements obtained by DCE-FI; We developed pre-clinical rodent contaminated fracture model to correlate DCE-FI with infection risk, and compare with multi-modality scanning; Furthermore in clinical studies, we investigated first-pass kinetic parameters of DCE-FI and arterial input functions for characterization of perfusion changes during lower limb amputation surgery; We conducted the first in-human use of dynamic contrast-enhanced texture analysis for orthopaedic trauma classification, suggesting that spatiotemporal features from DCE-FI can classify bone perfusion intraoperatively with high accuracy and sensitivity; We established clinical machine learning infection risk predictive model on open fracture surgery, where pixel-scaled prediction on infection risk will be accomplished. In conclusion, pharmacokinetic and spatiotemporal patterns of dynamic contrast-enhanced imaging show great potential for quantifying bone perfusion and prognosing bone infection. The thesis work will decrease surgical site infection risk and improve successful rates of lower extremity injury surgery

    SOLID-SHELL FINITE ELEMENT MODELS FOR EXPLICIT SIMULATIONS OF CRACK PROPAGATION IN THIN STRUCTURES

    Crack propagation in thin shell structures due to cutting is conveniently simulated using explicit finite element approaches, in view of the high nonlinearity of the problem. Solidshell elements are usually preferred for the discretization in the presence of complex material behavior and degradation phenomena such as delamination, since they allow for a correct representation of the thickness geometry. However, in solid-shell elements the small thickness leads to a very high maximum eigenfrequency, which imply very small stable time-steps. A new selective mass scaling technique is proposed to increase the time-step size without affecting accuracy. New ”directional” cohesive interface elements are used in conjunction with selective mass scaling to account for the interaction with a sharp blade in cutting processes of thin ductile shells
