    Enhancing Seamless Nurses-Physician Communication After-Hours with Google Glass

    There is a major issue in medical centres in Malaysia: the shared responsibility and communication between nurses at the bedside care and conversation with the physicians after-hours. Inexperience nurses may, sometimes, overlook patients' critical symptoms that indicate immediate medical care and cause the circumstance deteriorate. Therefore, mutual blaming practices are common if there is a medical error. The design of hands-free assessment and feedback with wearable technologies is welcomed in healthcare sector. Hence, the paper presents a pilot design that investigates how a wearable technology can help in daily patient care in an innovative way that enhance the nurses and physicians' communication. The paper reviewed the current practices and technologies, followed by the design of the transformed flow, hardware and apps used with technical limitations, pilot evaluation and future work

    An Implementation Process of Interoperability: A Case-Study of Health Information Systems (HIS)

    Several advances have been made towards health information systems (HIS) use and implementations. However, these advances have not been matched by equal advances in HIS interoperability implementations. Current challenges in this domain are partly due to lack of implementation knowledge hence resulting into a number of failed HIS interoperability implementations. To gain substantive implementation knowledge on HIS interoperability projects, we have carried out interviews through an interpretive case-study approach to investigate a successful HIS interoperability project. Through this approach we propose an initial set of HIS interoperability implementation best practices comprising of these key processes: discovering interoperability need, projecting outcome, managing change and adopting an appropriate interoperability strategy which together depict a contingence management relationship. We conclude that this initial set of best practices contributes substantive knowledge that can guide future HIS interoperability implementations

    A Review on Provisioning Quality of Service of Wireless Telemedicine for E-Health Services

    In general, on-line medical consultation reduces time required for medical consultation induces improvement in the quality and efficiency of healthcare services. All major types of current e-health applications such as ECG, X-ray, video, diagnosis images and other common applications have been included in the scope of the study. In addition, the provision of Quality of Service (QoS) for the application of specific healthcare services in e-health, the scheme of priority for e-health services and the support of QoS in wireless networks and techniques or methods for IEEE 802.11 to guarantee the provision of QoS has also been assessed. In e-health, medical services in remote locations such as rural healthcare centers, ambulances, ships as well as home healthcare services can be supported through the applications of e-health services such as medical databases, electronic health records and the routing of text, audio, video and images. Given this, an adaptive resource allocation for a wireless network with multiple service types and multiple priorities have been proposed. For the provision of an acceptable QoS level to users of e-health services, prioritization is an important criterion in a multi-traffic network. The requirement for QoS provisioning in wireless broadband medical networks have paved the pathway for bandwidth requirements and the real-time or live transmission of medical applications. From the study, good performance of the proposed scheme has been validated by the results obtained. The proposed wireless network is capable of handling medical applications for both normal and life-threatening conditions as characterized by the level of emergencies. In addition, the bandwidth allocation and admission control algorithm for IEEE 802.16- based design specifically for wireless telemedicine/e-health services have also been presented in the study. It has been concluded that under busy traffic conditions, the proposed architecture can used as a feasible and reliable infrastructure network for telemedicine

    A Learning Health System for Radiation Oncology

    The proposed research aims to address the challenges faced by clinical data science researchers in radiation oncology accessing, integrating, and analyzing heterogeneous data from various sources. The research presents a scalable intelligent infrastructure, called the Health Information Gateway and Exchange (HINGE), which captures and structures data from multiple sources into a knowledge base with semantically interlinked entities. This infrastructure enables researchers to mine novel associations and gather relevant knowledge for personalized clinical outcomes. The dissertation discusses the design framework and implementation of HINGE, which abstracts structured data from treatment planning systems, treatment management systems, and electronic health records. It utilizes disease-specific smart templates for capturing clinical information in a discrete manner. HINGE performs data extraction, aggregation, and quality and outcome assessment functions automatically, connecting seamlessly with local IT/medical infrastructure. Furthermore, the research presents a knowledge graph-based approach to map radiotherapy data to an ontology-based data repository using FAIR (Findable, Accessible, Interoperable, Reusable) concepts. This approach ensures that the data is easily discoverable and accessible for clinical decision support systems. The dissertation explores the ETL (Extract, Transform, Load) process, data model frameworks, ontologies, and provides a real-world clinical use case for this data mapping. To improve the efficiency of retrieving information from large clinical datasets, a search engine based on ontology-based keyword searching and synonym-based term matching tool was developed. The hierarchical nature of ontologies is leveraged to retrieve patient records based on parent and children classes. Additionally, patient similarity analysis is conducted using vector embedding models (Word2Vec, Doc2Vec, GloVe, and FastText) to identify similar patients based on text corpus creation methods. Results from the analysis using these models are presented. The implementation of a learning health system for predicting radiation pneumonitis following stereotactic body radiotherapy is also discussed. 3D convolutional neural networks (CNNs) are utilized with radiographic and dosimetric datasets to predict the likelihood of radiation pneumonitis. DenseNet-121 and ResNet-50 models are employed for this study, along with integrated gradient techniques to identify salient regions within the input 3D image dataset. The predictive performance of the 3D CNN models is evaluated based on clinical outcomes. Overall, the proposed Learning Health System provides a comprehensive solution for capturing, integrating, and analyzing heterogeneous data in a knowledge base. It offers researchers the ability to extract valuable insights and associations from diverse sources, ultimately leading to improved clinical outcomes. This work can serve as a model for implementing LHS in other medical specialties, advancing personalized and data-driven medicine

    Digital transformation in healthcare: an innovative business plan for an application digitizing physical rehabilitation

    The health care system adapts to innovations very slowly, as strict rules control all measures. It is even more challenging when one wants to introduce a digital solution for the healthcare market. Even minor changes to the code can mean major hurdles for approvals. This challenge was taken up in the following master thesis. The status quo of gait analysis has hardly been innovated for decades. The approach of reha buddy is to analyze how a human walks through wireless sensors. This represents an innovation that will revolutionize the market for gait analysis. Above all, trends of the last two years, which were pushed even more by the last pandemic, have shown that there is probably no better time to pursue this project. Additionally, a literature review and expert interviews were conducted to support all statements in the thesis. What is more, for a precise examination of each chapter of the business plan, a market analysis was carried out focusing on all specific characteristics of the health care market. The team of reha buddy has to overcome some major obstacles in the next few years but is well-positioned with their team of experts who set some important milestones for managing those hurdles. The result of this work is a business plan which clarifies that reha buddy's vision has an entrepreneurial foundation and that the team's idea should be pursued.O Sistema de Saúde adapta-se às inovações muito lentamente, uma vez que regras rígidas controlam todas as medidas. É um desafio ainda maior quando se deseja introduzir uma solução digital no setor da Saúde. Mesmo pequenas alterações no código podem significar grandes dificuldades para aprovações. Este desafio é abordado na presente tese de mestrado. O status quo da locomoção praticamente não foi inovado durante décadas. A abordagem do Reha Buddy passa por analisar como um humano se desloca através de sensores sem fios, o que representa uma inovação que irá revolucionar o mercado da locomoção. Acima de tudo, as tendências dos últimos dois anos, que foram impulsionadas ainda mais pela última pandemia, mostraram que provavelmente não há melhor momento para avançar com este projeto. Foi realizada uma revisão de literatura, assim como entrevistas com especialistas com o objetivo de apoiar todas as afirmações. Adicionalmente, para uma análise eficaz de cada capítulo do plano de negócios, foi realizada uma análise de mercado com foco em todas as características específicas do mercado da Saúde. A equipa do Reha Buddy terá que superar alguns obstáculos importantes nos próximos anos; está, no entanto, bem posicionada com a sua equipa de especialistas que estabeleceu alguns marcos importantes para gerir esses obstáculos. O resultado do presente projeto é um plano de negócios que esclarece que a visão do Reha Buddy tem uma base empreendedora e que a ideia apresentada pela equipa deve ser concretizada

    Asynchronous Remote Medical Consultation for Ghana

    Computer-mediated communication systems can be used to bridge the gap between doctors in underserved regions with local shortages of medical expertise and medical specialists worldwide. To this end, we describe the design of a prototype remote consultation system intended to provide the social, institutional and infrastructural context for sustained, self-organizing growth of a globally-distributed Ghanaian medical community. The design is grounded in an iterative design process that included two rounds of extended design fieldwork throughout Ghana and draws on three key design principles (social networks as a framework on which to build incentives within a self-organizing network; optional and incremental integration with existing referral mechanisms; and a weakly-connected, distributed architecture that allows for a highly interactive, responsive system despite failures in connectivity). We discuss initial experiences from an ongoing trial deployment in southern Ghana.Comment: 10 page
