    A Blockchain Framework for Patient-Centered Health Records and Exchange (HealthChain): Evaluation and Proof-of-Concept Study

    Background: Blockchain has the potential to disrupt the current modes of patient data access, accumulation, contribution, exchange, and control. Using interoperability standards, smart contracts, and cryptographic identities, patients can securely exchange data with providers and regulate access. The resulting comprehensive, longitudinal medical records can significantly improve the cost and quality of patient care for individuals and populations alike. Objective: This work presents HealthChain, a novel patient-centered blockchain framework. The intent is to bolster patient engagement, data curation, and regulated dissemination of accumulated information in a secure, interoperable environment. A mixed-block blockchain is proposed to support immutable logging and redactable patient blocks. Patient data are generated and exchanged through Health Level-7 Fast Healthcare Interoperability Resources, allowing seamless transfer with compliant systems. In addition, patients receive cryptographic identities in the form of public and private key pairs. Public keys are stored in the blockchain and are suitable for securing and verifying transactions. Furthermore, the envisaged system uses proxy re-encryption (PRE) to share information through revocable, smart contracts, ensuring the preservation of privacy and confidentiality. Finally, several PRE improvements are offered to enhance performance and security. Methods: The framework was formulated to address key barriers to blockchain adoption in health care, namely, information security, interoperability, data integrity, identity validation, and scalability. It supports 16 configurations through the manipulation of 4 modes. An open-source, proof-of-concept tool was developed to evaluate the performance of the novel patient block components and system configurations. To demonstrate the utility of the proposed framework and evaluate resource consumption, extensive testing was performed on each of the 16 configurations over a variety of scenarios involving a variable number of existing and imported records. Results: The results indicate several clear high-performing, low-bandwidth configurations, although they are not the strongest cryptographically. Of the strongest models, one’s anticipated cumulative record size is shown to influence the selection. Although the most efficient algorithm is ultimately user specific, Advanced Encryption Standard–encrypted data with static keys, incremental server storage, and no additional server-side encryption are the fastest and least bandwidth intensive, whereas proxy re-encrypted data with dynamic keys, incremental server storage, and additional server-side encryption are the best performing of the strongest configurations. Conclusions: Blockchain is a potent and viable technology for patient-centered access to and exchange of health information. By integrating a structured, interoperable design with patient-accumulated and generated data shared through smart contracts into a universally accessible blockchain, HealthChain presents patients and providers with access to consistent and comprehensive medical records. Challenges addressed include data security, interoperability, block storage, and patient-administered data access, with several configurations emerging for further consideration regarding speed and security

    Software-Driven and Virtualized Architectures for Scalable 5G Networks

    In this dissertation, we argue that it is essential to rearchitect 4G cellular core networks–sitting between the Internet and the radio access network–to meet the scalability, performance, and flexibility requirements of 5G networks. Today, there is a growing consensus among operators and research community that software-defined networking (SDN), network function virtualization (NFV), and mobile edge computing (MEC) paradigms will be the key ingredients of the next-generation cellular networks. Motivated by these trends, we design and optimize three core network architectures, SoftMoW, SoftBox, and SkyCore, for different network scales, objectives, and conditions. SoftMoW provides global control over nationwide core networks with the ultimate goal of enabling new routing and mobility optimizations. SoftBox attempts to enhance policy enforcement in statewide core networks to enable low-latency, signaling-efficient, and customized services for mobile devices. Sky- Core is aimed at realizing a compact core network for citywide UAV-based radio networks that are going to serve first responders in the future. Network slicing techniques make it possible to deploy these solutions on the same infrastructure in parallel. To better support mobility and provide verifiable security, these architectures can use an addressing scheme that separates network locations and identities with self-certifying, flat and non-aggregatable address components.     Blockchain and Internet of Things in smart cities and drug supply management: Open issues, opportunities, and future directions

    Blockchain-based drug supply management (DSM) requires powerful security and privacy procedures for high-level authentication, interoperability, and medical record sharing. Researchers have shown a surprising interest in Internet of Things (IoT)-based smart cities in recent years. By providing a variety of intelligent applications, such as intelligent transportation, industry 4.0, and smart financing, smart cities (SC) can improve the quality of life for their residents. Blockchain technology (BCT) can allow SC to offer a higher standard of security by keeping track of transactions in an immutable, secure, decentralized, and transparent distributed ledger. The goal of this study is to systematically explore the current state of research surrounding cutting-edge technologies, particularly the deployment of BCT and the IoT in DSM and SC. In this study, the defined keywords “blockchain”, “IoT”, drug supply management”, “healthcare”, and “smart cities” as well as their variations were used to conduct a systematic search of all relevant research articles that were collected from several databases such as Science Direct, JStor, Taylor & Francis, Sage, Emerald insight, IEEE, INFORMS, MDPI, ACM, Web of Science, and Google Scholar. The final collection of papers on the use of BCT and IoT in DSM and SC is organized into three categories. The first category contains articles about the development and design of DSM and SC applications that incorporate BCT and IoT, such as new architecture, system designs, frameworks, models, and algorithms. Studies that investigated the use of BCT and IoT in the DSM and SC make up the second category of research. The third category is comprised of review articles regarding the incorporation of BCT and IoT into DSM and SC-based applications. Furthermore, this paper identifies various motives for using BCT and IoT in DSM and SC, as well as open problems and makes recommendations. The current study contributes to the existing body of knowledge by offering a complete review of potential alternatives and finding areas where further research is needed. As a consequence of this, researchers are presented with intriguing potential to further create decentralized DSM and SC apps as a result of a comprehensive discussion of the relevance of BCT and its implementation.© 2023 The Author(s).     Decentralized Telemedicine Framework for a Smart Healthcare Ecosystem

    The healthcare sector is one of the most rapidly growing sectors globally. With the ever-growing technology, patient care, regulatory compliance, and digital transformation, there is an increased need for healthcare sectors to collaborate with all stakeholders – both within the healthcare ecosystem and in concurring industries. In recent times, telemedicine has proven to provide high quality, affordable, and predominantly adapted healthcare services. However, telemedicine suffers from several risks in implementation, such as data breach, restricted access across medical fraternity, incorrect diagnosis and prescription, fraud, and abuse. In this work, introduce blockchain-based framework that would unlock the future of the healthcare sector and improved services. Our proposed solution utilizing Ethereum smart contracts to develop a transparent, tamper-proof telemedicine healthcare framework, and ensure the integrity of sensitive patient data eliminating a central administrator. Moreover, the smart contract regulates the interaction between all the parties involved in the network and keeps the patient meticulously informed about the transactions in the network

    A Systematic Review of Blockchain Applications

    For this study, the researchers conducted a systematic literature review to answer complex questions about the field of blockchain technology. We used an unbiased systematic review process to find works on blockchain-based applications and developed a Python code that searched various online databases. This paper provides an overview of the characteristics, mode of operation, and applications of blockchains in various domains such as transportation, commerce and industry, privacy and security, the financial sector, government, education, healthcare, and the Internet of Things (IoT). The aim was to identify the key research themes addressed in existing articles within each application domain and suggest future research directions for these domains. We analyzed a set of 750 articles published between 2015 and 2021 that dealt with blockchain applications. We found that financial management and security issues have been the main research focus since 2015. However, the use of blockchain in education has become a central research theme in 2021. Healthcare, IoT, and government applications have also grown in popularity. We furthermore analyzed some of the implementations of privacy mechanisms, as well as the challenges and future directions that need to be addressed for effective blockchain deployment. This study contributes to existing research by providing a comprehensive overview of blockchain application themes and their emerging areas for stakeholders in diverse sectors.V rámci této studie provedli výzkumníci systematický přehled literatury, aby zodpověděli komplexní otázky týkající se technologie blockchain. K vyhledání prací o aplikacích založených na blockchainu jsme použili nezaujatý proces systematického přehledu a vyvinuli jsme kód v jazyce Python, který prohledával různé online databáze. Tento článek poskytuje přehled charakteristik, způsobu fungování a aplikací blockchainů v různých oblastech, jako je doprava, obchod a průmysl, ochrana soukromí a bezpečnost, finanční sektor, státní správa, vzdělávání, zdravotnictví a internet věcí (IoT). Cílem bylo identifikovat klíčová témata výzkumu, kterými se zabývají existující články v rámci jednotlivých aplikačních domén, a navrhnout budoucí směry výzkumu pro tyto domény. Analyzovali jsme soubor 750 článků publikovaných v letech 2015 až 2021, které se zabývaly aplikacemi blockchainu. Zjistili jsme, že od roku 2015 se výzkum zaměřuje především na otázky finanční správy a bezpečnosti. V roce 2021 se však ústředním tématem výzkumu stalo využití blockchainu ve vzdělávání. Rostla také popularita aplikací ve zdravotnictví, internetu věcí a státní správě. Dále jsme analyzovali některé z implementací mechanismů ochrany soukromí a také výzvy a budoucí směry, které je třeba řešit pro efektivní nasazení blockchainu. Tato studie přispívá ke stávajícímu výzkumu tím, že poskytuje ucelený přehled témat aplikací blockchainu a jejich nově vznikajících oblastí pro zúčastněné strany v různých odvětvích
