    Ami-deu : un cadre sémantique pour des applications adaptables dans des environnements intelligents

    Cette thèse vise à étendre l’utilisation de l'Internet des objets (IdO) en facilitant le développement d’applications par des personnes non experts en développement logiciel. La thèse propose une nouvelle approche pour augmenter la sémantique des applications d’IdO et l’implication des experts du domaine dans le développement d’applications sensibles au contexte. Notre approche permet de gérer le contexte changeant de l’environnement et de générer des applications qui s’exécutent dans plusieurs environnements intelligents pour fournir des actions requises dans divers contextes. Notre approche est mise en œuvre dans un cadriciel (AmI-DEU) qui inclut les composants pour le développement d’applications IdO. AmI-DEU intègre les services d’environnement, favorise l’interaction de l’utilisateur et fournit les moyens de représenter le domaine d’application, le profil de l’utilisateur et les intentions de l’utilisateur. Le cadriciel permet la définition d’applications IoT avec une intention d’activité autodécrite qui contient les connaissances requises pour réaliser l’activité. Ensuite, le cadriciel génère Intention as a Context (IaaC), qui comprend une intention d’activité autodécrite avec des connaissances colligées à évaluer pour une meilleure adaptation dans des environnements intelligents. La sémantique de l’AmI-DEU est basée sur celle du ContextAA (Context-Aware Agents) – une plateforme pour fournir une connaissance du contexte dans plusieurs environnements. Le cadriciel effectue une compilation des connaissances par des règles et l'appariement sémantique pour produire des applications IdO autonomes capables de s’exécuter en ContextAA. AmI- DEU inclut également un outil de développement visuel pour le développement et le déploiement rapide d'applications sur ContextAA. L'interface graphique d’AmI-DEU adopte la métaphore du flux avec des aides visuelles pour simplifier le développement d'applications en permettant des définitions de règles étape par étape. Dans le cadre de l’expérimentation, AmI-DEU comprend un banc d’essai pour le développement d’applications IdO. Les résultats expérimentaux montrent une optimisation sémantique potentielle des ressources pour les applications IoT dynamiques dans les maisons intelligentes et les villes intelligentes. Notre approche favorise l'adoption de la technologie pour améliorer le bienêtre et la qualité de vie des personnes. Cette thèse se termine par des orientations de recherche que le cadriciel AmI-DEU dévoile pour réaliser des environnements intelligents omniprésents fournissant des adaptations appropriées pour soutenir les intentions des personnes.Abstract: This thesis aims at expanding the use of the Internet of Things (IoT) by facilitating the development of applications by people who are not experts in software development. The thesis proposes a new approach to augment IoT applications’ semantics and domain expert involvement in context-aware application development. Our approach enables us to manage the changing environment context and generate applications that run in multiple smart environments to provide required actions in diverse settings. Our approach is implemented in a framework (AmI-DEU) that includes the components for IoT application development. AmI- DEU integrates environment services, promotes end-user interaction, and provides the means to represent the application domain, end-user profile, and end-user intentions. The framework enables the definition of IoT applications with a self-described activity intention that contains the required knowledge to achieve the activity. Then, the framework generates Intention as a Context (IaaC), which includes a self-described activity intention with compiled knowledge to be assessed for augmented adaptations in smart environments. AmI-DEU framework semantics adopts ContextAA (Context-Aware Agents) – a platform to provide context-awareness in multiple environments. The framework performs a knowledge compilation by rules and semantic matching to produce autonomic IoT applications to run in ContextAA. AmI-DEU also includes a visual tool for quick application development and deployment to ContextAA. The AmI-DEU GUI adopts the flow metaphor with visual aids to simplify developing applications by allowing step-by-step rule definitions. As part of the experimentation, AmI-DEU includes a testbed for IoT application development. Experimental results show a potential semantic optimization for dynamic IoT applications in smart homes and smart cities. Our approach promotes technology adoption to improve people’s well-being and quality of life. This thesis concludes with research directions that the AmI-DEU framework uncovers to achieve pervasive smart environments providing suitable adaptations to support people’s intentions

    Conflict resolution in clinical treatments

    Dissertação de mestrado integrado em Engenharia InformáticaCurrently, in the health area, there is a need for systems that provide support for the decision of health professionals through specific recommendations for each patient based on Clinical Practice Guidelines (CPGs) for automatic interpretation. CPGs are documents that have enormous importance in the daily life of health professionals, playing a key role in reducing variations in medical practice, improving the quality of health care, and reducing health care costs. These documents reflect knowledge about how best to diagnose and treat diseases in the form of a list of clinical recommendations. However, there may be conflicts and interactions in the application of these clinical recommendations, that which in their maximum exponent may impair the patient’s clinical condition. These conflicts are transported to decision support systems, creating the need to develop computational methods to solve these same conflicts. In the case of multimorbid patients, this resolution of conflicts can be very problematic because these patients suffer from several pathologies at the same time, and that the use of a drug for one particular pathology may have a detrimental effect on the application of another drug in another pathology. Therefore, the objective of this dissertation topic is the determination of conflicts and interactions between drugs and the determination of these same alternatives.Atualmente na área da saúde, existe uma necessidade de existirem sistemas que forneçam apoio à decisão dos profissionais de saúde através de recomendações específicas para cada paciente com base em protocolos clínicos para interpretação automática. Os protocolos clínicos são documentos que têm enorme importância no dia-a-dia dos profissionais de saúde, desempenhando um papel fundamental na redução das variações na prática médica, na melhoria da qualidade dos cuidados de saúde e na redução dos custos de saúde. Estes documentos reflectem o conhecimento sobre a melhor forma de diagnosticar e tratar doenças na forma de uma lista de recomendações clínicas. Contudo, podem existir conflitos e interações na aplicação destas recomendações clínicas, que no seu expoente máximo poderão levar a um agravamento do estado clínico do paciente, nomeadamente no caso da aplicação de diferentes fármacos. Estes conflitos são transportados para os sistemas de apoio à decisão, criando a necessidade de desenvolver métodos computacionais de resolução destes mesmos conflitos. No caso dos pacientes multimórbidos esta resolução de conflitos pode ser bastante problemática devido ao facto destes pacientes sofrerem de várias patologias ao mesmo tempo, e que a utilização de um fármaco para uma determinada patologia possa vir a ter um efeito nocivo na aplicação de outro fármaco noutra patologia. Sendo assim, o objetivo deste tema de dissertação é a determinação dos conflitos e interações entre fármacos e a determinação dessas mesmas alternativas

    A unified system for clinical guideline management and execution

    There are several approaches to Computer-Interpretable Guidelines (CIG) representation and execution that offer the possibility of acquiring, executing and editing CPGs. Many CIG approaches aim to represent Clinical Practice Guidelines (CPGs) by computationally formalising the knowledge that they enclose, in order to be suitable for the integration in Clinical Decision Support Systems (CDSS). However, the current approaches for this purpose lack in providing a unified workflow for management and execution of CIGs.     Modelling evolving clinical practice guidelines: a case of Malawi

    Electronic medical record (EMR) systems are increasingly being adopted in low- and middle-income countries. This provides an opportunity to support task-shifted health workers with guideline-based clinical decision support to improve the quality of healthcare delivery. However, the formalization of clinical practice guidelines (CPGs) into computer-interpretable guidelines (CIGs) for clinical decision support in such a setting is a very challenging task due to the evolving nature of CPGs and limited healthcare budgets. This study proposed that a CIG modelling language that considers CPG change requirements in their representation models could enable semi-automated support of CPG change operations thereby reducing the burden of maintaining CIGs. Characteristics of CPG changes were investigated to elucidate CPG change requirements using CPG documents from Malawi where EMR systems are routinely used. Thereafter, a model-driven engineering approach was taken to design a CIG modelling framework that has a novel domain-specific modelling language called FCIG for the modelling of evolving CIGs. The CIG modelling framework was implemented using the Xtext framework. The national antiretroviral therapy EMR system for Malawi was extended into a prototype with FCIG support for experimentation. Further studies were conducted with CIG modellers. The evaluations were conducted to answer the following research questions: i) What are the CPG change requirements for modelling an evolving CIG? ii) Can a model-driven engineering approach adequately support the modelling of an evolving CIG? iii) What is the effect of modelling an evolving CIG using FCIG in comparison with the Health Level Seven (HL7) standard for modelling CIGs? Data was collected using questionnaires, logs and observations. The results indicated that finegrained components of a CPG are affected by CPG changes and that those components are not included explicitly in current executable CIG language models. The results also showed that by including explicit semantics for elements that are affected by CPG changes in a language model, smart-editing features for supporting CPG change operations can be enabled in a language-aware code editor. The results further showed that both experienced and CIG modellers perceived FCIG as highly usable. Furthermore, the results suggested that FCIG performs significantly better at CIG modelling tasks as compared to the HL7 standard, Arden Syntax. This study provides empirical evidence that a model-driven engineering approach to clinical guideline formalization supports the authoring and maintenance of evolving CIGs to provide up-to-date clinical decision support in low- and middle-income countries
