Supervised machine learning based multi-task artificial intelligence classification of retinopathies
Artificial intelligence (AI) classification holds promise as a novel and
affordable screening tool for clinical management of ocular diseases. Rural and
underserved areas, which suffer from lack of access to experienced
ophthalmologists may particularly benefit from this technology. Quantitative
optical coherence tomography angiography (OCTA) imaging provides excellent
capability to identify subtle vascular distortions, which are useful for
classifying retinovascular diseases. However, application of AI for
differentiation and classification of multiple eye diseases is not yet
established. In this study, we demonstrate supervised machine learning based
multi-task OCTA classification. We sought 1) to differentiate normal from
diseased ocular conditions, 2) to differentiate different ocular disease
conditions from each other, and 3) to stage the severity of each ocular
condition. Quantitative OCTA features, including blood vessel tortuosity (BVT),
blood vascular caliber (BVC), vessel perimeter index (VPI), blood vessel
density (BVD), foveal avascular zone (FAZ) area (FAZ-A), and FAZ contour
irregularity (FAZ-CI) were fully automatically extracted from the OCTA images.
A stepwise backward elimination approach was employed to identify sensitive
OCTA features and optimal-feature-combinations for the multi-task
classification. For proof-of-concept demonstration, diabetic retinopathy (DR)
and sickle cell retinopathy (SCR) were used to validate the supervised machine
leaning classifier. The presented AI classification methodology is applicable

Retinal vessel segmentation using textons
Segmenting vessels from retinal images, like segmentation in many other medical image domains, is a challenging task, as there is no unified way that can be adopted to extract the vessels accurately. However, it is the most critical stage in automatic assessment of various forms of diseases (e.g. Glaucoma, Age-related macular degeneration, diabetic retinopathy and cardiovascular diseases etc.). Our research aims to investigate retinal image segmentation approaches based on textons as they provide a compact description of texture that can be learnt from a training set. This thesis presents a brief review of those diseases and also includes their current situations, future trends and techniques used for their automatic diagnosis in routine clinical applications. The importance of retinal vessel segmentation is
particularly emphasized in such applications. An extensive review of previous work on retinal vessel segmentation and salient texture analysis methods is presented. Five automatic retinal vessel segmentation methods are proposed in this thesis. The first method focuses on addressing the problem of removing pathological anomalies (Drusen, exudates) for retinal vessel segmentation, which have been identified by other researchers as a problem and a common source of error. The results show that the modified method shows some
improvement compared to a previously published method. The second novel supervised segmentation method employs textons. We propose a new filter bank (MR11) that includes bar detectors for vascular feature extraction and other kernels to detect edges and photometric variations in the image. The k-means clustering algorithm is adopted for texton generation based on the vessel and non-vessel elements which are identified by ground truth. The third improved supervised method is developed based on the second one, in which textons are generated by k-means clustering and texton maps representing vessels are derived by back projecting pixel clusters onto hand labelled ground truth. A further step is implemented to ensure that the best combinations of textons are represented in the map and subsequently used to identify vessels in the test set. The experimental results on two benchmark datasets show that our proposed method performs well compared to other published work and the results of human experts. A further test of our system on an independent set of optical fundus images verified its consistent performance. The statistical analysis on experimental results also reveals that it is possible to train unified textons for retinal vessel segmentation. In the fourth method a novel scheme using Gabor filter bank for vessel feature extraction is proposed. The ii method is inspired by the human visual system. Machine learning is used to optimize the
Gabor filter parameters. The experimental results demonstrate that our method significantly enhances the true positive rate while maintaining a level of specificity that is comparable with other approaches. Finally, we proposed a new unsupervised texton based retinal vessel
segmentation method using derivative of SIFT and multi-scale Gabor filers. The lack of sufficient quantities of hand labelled ground truth and the high level of variability in ground truth labels amongst experts provides the motivation for this approach. The evaluation results
reveal that our unsupervised segmentation method is comparable with the best other supervised methods and other best state of the art methods
Multiscale blood vessel delineation using B-COSFIRE filters
We propose a delineation algorithm that deals with bar-like structures of different thickness. Detection of linear structures is applicable to several fields ranging from medical images for segmentation of vessels to aerial images for delineation of roads or rivers. The proposed method is suited for any delineation problem and employs a set of B-COSFIRE filters selective for lines and line-endings of different thickness. We determine the most effective filters for the application at hand by Generalized Matrix Learning Vector Quantization (GMLVQ) algorithm. We demonstrate the effectiveness of the proposed method by applying it to the task of vessel segmentation in retinal images. We perform experiments on two benchmark data sets, namely DRIVE and STARE. The experimental results show that the proposed delineation algorithm is highly effective and efficient. It can be considered as a general framework for a delineation task in various applications.peer-reviewe
Deep Learning in Cardiology
The medical field is creating large amount of data that physicians are unable
to decipher and use efficiently. Moreover, rule-based expert systems are
inefficient in solving complicated medical tasks or for creating insights using
big data. Deep learning has emerged as a more accurate and effective technology
in a wide range of medical problems such as diagnosis, prediction and
intervention. Deep learning is a representation learning method that consists
of layers that transform the data non-linearly, thus, revealing hierarchical
relationships and structures. In this review we survey deep learning
application papers that use structured data, signal and imaging modalities from
cardiology. We discuss the advantages and limitations of applying deep learning
in cardiology that also apply in medicine in general, while proposing certain

Machine Learning Techniques, Detection and Prediction of Glaucoma– A Systematic Review
Globally, glaucoma is the most common factor in both permanent blindness and impairment. However, the majority of patients are unaware they have the condition, and clinical practise continues to face difficulties in detecting glaucoma progression using current technology. An expert ophthalmologist examines the retinal portion of the eye to see how the glaucoma is progressing. This method is quite time-consuming, and doing it manually takes more time. Therefore, using deep learning and machine learning techniques, this problem can be resolved by automatically diagnosing glaucoma. This systematic review involved a comprehensive analysis of various automated glaucoma prediction and detection techniques. More than 100 articles on Machine learning (ML) techniques with understandable graph and tabular column are reviewed considering summery, method, objective, performance, advantages and disadvantages. In the ML techniques such as support vector machine (SVM), and K-means. Fuzzy c-means clustering algorithm are widely used in glaucoma detection and prediction. Through the systematic review, the most accurate technique to detect and predict glaucoma can be determined which can be utilized for future betterment
Pixel-level semantic understanding of ophthalmic images and beyond
Computer-assisted semantic image understanding constitutes the substrate of applications that range from biomarker detection to intraoperative guidance or street scene understanding for self-driving systems. This PhD thesis is on the development of deep learning-based, pixel-level, semantic segmentation methods for medical and natural images. For vessel segmentation in OCT-A, a method comprising iterative refinement of the extracted vessel maps and an auxiliary loss function that penalizes structural inaccuracies, is proposed and tested on data captured from real clinical conditions comprising various pathological cases. Ultimately, the presented method enables the extraction of a detailed vessel map of the retina with potential applications to diagnostics or intraoperative localization. Furthermore, for scene segmentation in cataract surgery, the major challenge of class imbalance is identified among several factors. Subsequently, a method addressing it is proposed, achieving state-of-the-art performance on a challenging public dataset. Accurate semantic segmentation in this domain can be used to monitor interactions between tools and anatomical parts for intraoperative guidance and safety. Finally, this thesis proposes a novel contrastive learning framework for supervised semantic segmentation, that aims to improve the discriminative power of features in deep neural networks. The proposed approach leverages contrastive loss function applied both at multiple model layers and across them. Importantly, the proposed framework is easy to combine with various model architectures and is experimentally shown to significantly improve performance on both natural and medical domain
