    Recent trends, technical concepts and components of computer-assisted orthopedic surgery systems: A comprehensive review

    Computer-assisted orthopedic surgery (CAOS) systems have become one of the most important and challenging types of system in clinical orthopedics, as they enable precise treatment of musculoskeletal diseases, employing modern clinical navigation systems and surgical tools. This paper brings a comprehensive review of recent trends and possibilities of CAOS systems. There are three types of the surgical planning systems, including: systems based on the volumetric images (computer tomography (CT), magnetic resonance imaging (MRI) or ultrasound images), further systems utilize either 2D or 3D fluoroscopic images, and the last one utilizes the kinetic information about the joints and morphological information about the target bones. This complex review is focused on three fundamental aspects of CAOS systems: their essential components, types of CAOS systems, and mechanical tools used in CAOS systems. In this review, we also outline the possibilities for using ultrasound computer-assisted orthopedic surgery (UCAOS) systems as an alternative to conventionally used CAOS systems.Web of Science1923art. no. 519

    Intraoperative knee kinematics

    The aim of this cadaveric study was to evaluate the intraoperative kinematics of the native knee including two-dimensional translation of the femur using a navigation system. Eight native knees of 4 fresh-frozen whole-body cadavers were used for the study. The kinematics of each knee were analyzed intraoperatively using the navigation system. Although anterior-posterior translation could not be assessed directly, it could be calculated using a formula derived from the parameters in the navigation system. The native knee showed external rotation of the femur in early knee flexion, transient internal rotation in mid flexion, and gradual external rotation in late flexion. There was no marked change in the coronal rotation angle of the mechanical axis during knee flexion. The femoral center moved anteriorly in early knee flexion and posteriorly in late flexion. The distance moved in the medial-lateral direction was relatively smaller than that in the anterior-posterior direction. Two-dimensional translation of the surgical epicondylar axis showed a medial pivot-like motion. In this cadaveric study, the kinematics of the native knee, including two-dimensional translation of the femur, could be satisfactorily assessed intraoperatively using a navigation system.     A simulation-enhanced intraoperative planning tool for robotic-assisted total knee arthroplasty

    The purpose of the present study was to investigate current methods of surgical planning used in conjunction with robotics-assisted total knee arthroplasty (raTKA) to determine if improvements could be made using advanced computational techniques. Thus, through the use of musculoskeletal multi-body dynamic simulations, an enhanced surgical planning tool was developed, which provides insight on active postoperative joint mechanics. Development of the tool relied on patient-specific simulations using single-leg and full-body models. These simulations were constructed using two publicly-available datasets (Orthoload and SimTK); in particular, joint loading data obtained from subjects during various activities. Simulation parameters were optimized using a design-of experiments (DOE) methodology and validation of each of the models was conducted by calculating the root mean square error (RMSE) between joint loading calculated using the model and the corresponding results given in the appropriate dataset. Optimized and validated variants of each of the models were used in conjunction with the results of DOE studies that characterized the influence of a number of surgical planning variables on various biomechanical responses and linear regression analysis to derive knee performance equations (KPEs). In literature studies, some of the aforementioned responses have been strongly correlated with two outcomes commonly reported by dissatisfied TKA patients, namely, anterior knee pain and poor proprioception. In a proof-of-concept study, KPEs were used to calculate optimal positions and orientations of the femoral and tibial components in the case of one subject featured in the SimTK dataset. These results differed from corresponding ones reportedly achieved for the implant components in the subject. This trend suggests there is potential to improve robotic surgical planning for current-generation raTKA systems through the use of musculoskeletal simulation. Use of the proposed surgical planning tool does not require computational resources beyond what are used with a specified current-generation raTKA system (Navio Surgical System). Furthermore, there are only minimal differences between the workflow involving the proposed planning tool and that when Navio Surgical System is used. A number of recommendations for future studies are made, such as larger scale simulation validation work and use of more complex regression techniques when deriving the KPEs

    Patient-specific modelling in orthopedics: from image to surgery

    In orthopedic surgery, to decide upon intervention and how it can be optimized, surgeons usually rely on subjective analysis of medical images of the patient, obtained from computed tomography, magnetic resonance imaging, ultrasound or other techniques. Recent advancements in computational performance, image analysis and in silico modeling techniques have started to revolutionize clinical practice through the development of quantitative tools, including patient#specific models aiming at improving clinical diagnosis and surgical treatment. Anatomical and surgical landmarks as well as features extraction can be automated allowing for the creation of general or patient-specific models based on statistical shape models. Preoperative virtual planning and rapid prototyping tools allow the implementation of customized surgical solutions in real clinical environments. In the present chapter we discuss the applications of some of these techniques in orthopedics and present new computer-aided tools that can take us from image analysis to customized surgical treatment

    Robots and tools for remodeling bone

    The field of robotic surgery has progressed from small teams of researchers repurposing industrial robots, to a competitive and highly innovative subsection of the medical device industry. Surgical robots allow surgeons to perform tasks with greater ease, accuracy, or safety, and fall under one of four levels of autonomy; active, semi-active, passive, and remote manipulator. The increased accuracy afforded by surgical robots has allowed for cementless hip arthroplasty, improved postoperative alignment following knee arthroplasty, and reduced duration of intraoperative fluoroscopy among other benefits. Cutting of bone has historically used tools such as hand saws and drills, with other elaborate cutting tools now used routinely to remodel bone. Improvements in cutting accuracy and additional options for safety and monitoring during surgery give robotic surgeries some advantages over conventional techniques. This article aims to provide an overview of current robots and tools with a common target tissue of bone, proposes a new process for defining the level of autonomy for a surgical robot, and examines future directions in robotic surgery

    Augmented reality for computer assisted orthopaedic surgery

    In recent years, computer-assistance and robotics have established their presence in operating theatres and found success in orthopaedic procedures. Benefits of computer assisted orthopaedic surgery (CAOS) have been thoroughly explored in research, finding improvements in clinical outcomes, through increased control and precision over surgical actions. However, human-computer interaction in CAOS remains an evolving field, through emerging display technologies including augmented reality (AR) – a fused view of the real environment with virtual, computer-generated holograms. Interactions between clinicians and patient-specific data generated during CAOS are limited to basic 2D interactions on touchscreen monitors, potentially creating clutter and cognitive challenges in surgery. Work described in this thesis sought to explore the benefits of AR in CAOS through: an integration between commercially available AR and CAOS systems, creating a novel AR-centric surgical workflow to support various tasks of computer-assisted knee arthroplasty, and three pre–clinical studies exploring the impact of the new AR workflow on both existing and newly proposed quantitative and qualitative performance metrics. Early research focused on cloning the (2D) user-interface of an existing CAOS system onto a virtual AR screen and investigating any resulting impacts on usability and performance. An infrared-based registration system is also presented, describing a protocol for calibrating commercial AR headsets with optical trackers, calculating a spatial transformation between surgical and holographic coordinate frames. The main contribution of this thesis is a novel AR workflow designed to support computer-assisted patellofemoral arthroplasty. The reported workflow provided 3D in-situ holographic guidance for CAOS tasks including patient registration, pre-operative planning, and assisted-cutting. Pre-clinical experimental validation on a commercial system (NAVIO®, Smith & Nephew) for these contributions demonstrates encouraging early-stage results showing successful deployment of AR to CAOS systems, and promising indications that AR can enhance the clinician’s interactions in the future. The thesis concludes with a summary of achievements, corresponding limitations and future research opportunities.Open Acces

    Accuracy assessment of Tri-plane B-mode ultrasound for non-invasive 3D kinematic analysis of knee joints

    BACKGROUND Currently the clinical standard for measuring the motion of the bones in knee joints with sufficient precision involves implanting tantalum beads into the bones. These beads appear as high intensity features in radiographs and can be used for precise kinematic measurements. This procedure imposes a strong coupling between accuracy and invasiveness. In this paper, a tri-plane B-mode ultrasound (US) based non-invasive approach is proposed for use in kinematic analysis of knee joints in 3D space. METHODS The 3D analysis is performed using image processing procedures on the 2D US slices. The novelty of the proposed procedure and its applicability to the unconstrained 3D kinematic analysis of knee joints is outlined. An error analysis for establishing the method's feasibility is included for different artificial compositions of a knee joint phantom. Some in-vivo and in-vitro scans are presented to demonstrate that US scans reveal enough anatomical details, which further supports the experimental setup used using knee bone phantoms. RESULTS The error between the displacements measured by the registration of the US image slices and the true displacements of the respective slices measured using the precision mechanical stages on the experimental apparatus is evaluated for translation and rotation in two simulated environments. The mean and standard deviation of errors are shown in tabular form. This method provides an average measurement precision of less than 0.1 mm and 0.1 degrees, respectively. CONCLUSION In this paper, we have presented a novel non-invasive approach to measuring the motion of the bones in a knee using tri-plane B-mode ultrasound and image registration. In our study, the image registration method determines the position of bony landmarks relative to a B-mode ultrasound sensor array with sub-pixel accuracy. The advantages of our proposed system over previous techniques are that it is non-invasive, does not require the use of ionizing radiation and can be used conveniently if miniaturized.This work has been supported by School of Engineering & IT, UNSW Canberra, under Research Publication Fellowship

    Accurate 3D reconstruction of bony surfaces using ultrasonic synthetic aperture techniques for robotic knee arthroplasty

    Robotically guided knee arthroplasty systems generally require an individualized, preoperative 3D model of the knee joint. This is typically measured using Computed Tomography (CT) which provides the required accuracy for preoperative surgical intervention planning. Ultrasound imaging presents an attractive alternative to CT, allowing for reductions in cost and the elimination of doses of ionizing radiation, whilst maintaining the accuracy of the 3D model reconstruction of the joint. Traditional phased array ultrasound imaging methods, however, are susceptible to poor resolution and signal to noise ratios (SNR). Alleviating these weaknesses by offering superior focusing power, synthetic aperture methods have been investigated extensively within ultrasonic non-destructive testing. Despite this, they have yet to be fully exploited in medical imaging. In this paper, the ability of a robotic deployed ultrasound imaging system based on synthetic aperture methods to accurately reconstruct bony surfaces is investigated. Employing the Total Focussing Method (TFM) and the Synthetic Aperture Focussing Technique (SAFT), two samples were imaged which were representative of the bones of the knee joint: a human-shaped, composite distal femur and a bovine distal femur. Data were captured using a 5MHz, 128 element 1D phased array, which was manipulated around the samples using a robotic positioning system. Three dimensional surface reconstructions were then produced and compared with reference models measured using a precision laser scanner. Mean errors of 0.82 mm and 0.88 mm were obtained for the composite and bovine samples, respectively, thus demonstrating the feasibility of the approach to deliver the sub-millimetre accuracy required for the application

    The Effect of Robotic Technology on Perioperative Outcomes in Total Knee Arthroplasty

    Introduction Robotic technology has recently regained momentum in total knee arthroplasty (TKA) but the effects of this technology on accuracy of implant positioning, intraoperative soft tissue injury and postoperative functional rehabilitation remain unknown. The objectives of this research thesis were to compare a comprehensive range of radiological objectives and perioperative outcomes in conventional jig-based TKA versus robotic-arm assisted TKA, and use optical motion capture technology to quantify the effects of anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) resection on knee biomechanics. Methods A series of prospective cohort studies were undertaken in patients with established knee osteoarthritis undergoing primary conventional jig-based TKA versus robotic-arm assisted TKA. Predefined radiological and perioperative study outcomes were recorded by independent observers. Optical motion capture technology during robotic TKA was used to quantify the effects of ACL and PCL resection on knee biomechanics. Results Robotic-arm assisted TKA was associated with improved accuracy of implant positioning, reduced periarticular soft tissue injury, decreased bone trauma, improved postoperative functional rehabilitation, and reduced early systemic inflammatory response compared to conventional jig-based TKA. The Macroscopic Soft Tissue Injury (MASTI) classification system was developed and validated for grading intraoperative periarticular soft tissue injury and bone trauma during TKA. ACL resection created flexion-extension mismatch by increasing the extension gap more than the flexion gap, whilst PCL resection increased the flexion gap proportionally more than the extension gap and created mediolateral laxity in knee flexion but not in extension. Conclusion Robotic-arm assisted TKA was associated with increased accuracy of implant positioning, reduced iatrogenic soft tissue injury, and improved functional rehabilitation compared to conventional jig-based TKA. ACL and PCL resections created unique changes in knee biomechanics that affected flexion-extension gaps and mediolateral soft tissue tension during TKA. On the basis of this thesis, further clinical trials have been established to determine the long-term clinical significance of these findings
