    Can smartwatches replace smartphones for posture tracking?

    This paper introduces a human posture tracking platform to identify the human postures of sitting, standing or lying down, based on a smartwatch. This work develops such a system as a proof-of-concept study to investigate a smartwatch's ability to be used in future remote health monitoring systems and applications. This work validates the smartwatches' ability to track the posture of users accurately in a laboratory setting while reducing the sampling rate to potentially improve battery life, the first steps in verifying that such a system would work in future clinical settings. The algorithm developed classifies the transitions between three posture states of sitting, standing and lying down, by identifying these transition movements, as well as other movements that might be mistaken for these transitions. The system is trained and developed on a Samsung Galaxy Gear smartwatch, and the algorithm was validated through a leave-one-subject-out cross-validation of 20 subjects. The system can identify the appropriate transitions at only 10 Hz with an F-score of 0.930, indicating its ability to effectively replace smart phones, if needed

    Factors Associated with Physical Frailty in Elderly Women with Low Socioeconomic Status in Urban Communities: A Cross-Sectional Study

    Background: there are differences in factors associated with frailty syndrome in elderly population. The aim of this research was to determine frailty status (fit, pre-frail and frail) and to identify factors associated with physical frailty in urban community-dwelling elderly women. Methods: a cross-sectional study of community-dwelling women aged 60 years and older was conducted in West and Central Jakarta regions, Indonesia, from July until September 2017. The Cardiovascular Health Study (CHS) score was used to determine frailty status (fit/ pre-frail/ frail). Chi-Square Test and logistic regression analysis were used to determine association between independent variables and physical frailty. Results: there were 325 female subjects with a median age of 67 (60–94) years; 95.7% had income below the Provincial Minimum Income of DKI Jakarta in 2017 (<3.3 million IDR≈238 USD/month), and 92.6% had a level of education ≤9 years. Subjects were classified into this following groups: fit (12.6%), pre-frail (63.4%) and frail (24%). Factors associated with physical frailty were age above 70 years old with OR 5.27, lower Barthel Index for Activities of Daily Living (B-ADL) with OR 2.85, depressive symptoms with OR 6.79, and Euro Quality of Life-5 Dimensions (EQ-5D) index with OR 1.96. Conclusion: elderly women in the urban community with low socioeconomic status were classified as fit (12.6%), pre-frail (63.4%) and frail (24%).     Frailty as a predictor of disabilities among community-dwelling older people: a systematic review and meta-analysis

    BACKGROUND: Frailty has been shown to be associated with disability in the previous studies. However, it is not clear how consistently or to how much degree frailty is actually associated with the future disability risks. METHODS: A systematic review of the literature was conducted using Embase, MEDLINE, CINAHL, PsycINFO, and the Cochrane Library for any prospective studies published from 2010 to September 2015 examining associations between baseline frailty status and subsequent risk of developing or worsening disabilities among community-dwelling older people. A meta-analysis was performed to synthesize pooled estimates. RESULTS: Of 7012 studies identified through the systematic review, 20 studies were included in the meta-analysis. Twelve studies examined activities of daily living (ADL) disability risks, two studies examined instrumental activities of daily living (IADL) disability risks, and six studies examined both ADL and IADL disability risks. Overall, frail older people were more likely to develop or worsen disabilities in ADL (12 studies, pooled OR = 2.76, 95% CI = 2.23-3.44, p < 0.00001; 5 studies, pooled HR = 2.23, 95% CI = 1.42-3.49, p < 0.00001) and IADL (6 studies, pooled OR = 3.62, 95% CI = 2.32-5.64, p < 0.00001; 2 studies, pooled HR = 4.24, 95% CI = 0.85-21.28, p = 0.08). Prefrailty was also associated with incident or worsening disability risks to a lesser degree in most pooled analyses. High heterogeneity observed among 12 studies with OR of ADL disability risks for frailty was explored using subgroup analyses, which suggested methodological quality and mean age of the cohort were the possible causes. CONCLUSION: This systematic review meta-analysis quantitatively showed that frail older people are at higher risks of disabilities. These results are important for all related parties given population aging worldwide. Interventions for frailty are important to prevent disability and preserve physical functions, autonomy, and quality of life. Implications for Rehabilitation Although frailty has been shown to be associated with disability and considered as a precursor of disability, it is not clear how consistently or to how much degree frailty is actually associated with the future disability risks. This systematic review and meta-analysis quantitatively shows frailty is a significant predictor of incident and worsening ADL and IADL disabilities. It is a pressing priority to develop interventions for frailty to prevent disability and preserve older people's physical functions, autonomy, and quality of life

    For whom is a health-promoting intervention effective? Predictive factors for performing activities of daily living independently

    BACKGROUND: Health-promoting interventions tailored to support older persons to remain in their homes, so-called "ageing in place" is important for supporting or improving their health. The health-promoting programme "Elderly Persons in the Risk Zone," (EPRZ) was set up for this purpose and has shown positive results for maintaining independence in activities of daily living for older persons 80 years and above at 1- and 2 year follow-ups. The aim of this study was to explore factors for maintaining independence in the EPRZ health-promoting programme.METHODS: Total of 459 participants in the original trial was included in the analysis; 345 in the programme arm and 114 in the control arm. Thirteen variables, including demographic, health, and programme-specific indicators, were chosen as predictors for independence of activities of daily living. Logistic regression was performed separately for participants in the health promotion programme and in the control arm.RESULTS: In the programme arm, being younger, living alone and self-rated lack of tiredness in performing mobility activities predicted a positive effect of independence in activities of daily living at 1-year follow-up (odds ratio [OR] 1.18, 1.73, 3.02) and 2-year, (OR 1.13, 2.01, 2.02). In the control arm, being less frail was the only predictor at 1-year follow up (OR 1.6 1.09, 2.4); no variables predicted the outcome at the 2-year follow-up.CONCLUSIONS: Older persons living alone - as a risk of ill health - should be especially recognized and offered an opportunity to participate in health-promoting programmes such as "Elderly Persons in the Risk Zone". Further, screening for subjective frailty could form an advantageous guiding principle to target the right population when deciding to whom health-promoting intervention should be offered.TRIAL REGISTRATION: The original clinical trial was registered at ClinicalTrials.gov.     Frailty in hospitalized adults

    The purpose of this cross-sectional, retrospective, descriptive study was to characterize frailty in hospitalized adults 55 years of age and older admitted to medical units at one large academic medical center during a 15-month time frame and determine if level of frailty on admission predicted length of stay (LOS) and 30-day readmission. Frailty is a syndrome characterized by multisystem physiologic dysregulation due to intrinsic and extrinsic stressors resulting in decreased compensatory reserve and ability to effectively respond to destabilizing health events. Stressors associated with hospitalization may increase risk for frailty or accelerate its development. Frailty is a significant concern as it is associated with morbidity, functional decline, long LOS, readmission, institutionalization, and mortality. There is scant research on frailty in acutely-ill hospitalized adults, especially those ¡Ý 65 years of age. Understanding frailty in this population is imperative because frailty is potentially preventable, treatable, and reversible. Frailty was operationalized as 14 evidence-based frailty components defined by 26 indicator variables. Frailty components were Nutrition, Weakness, Fatigue, Chronic Pain, Dyspnea, Falls, Vision, Depression, Cognition, Social Support, low Hemoglobin, low Albumin, high C-reactive protein (CRP) or hs-CRP, and abnormal WBC count. Each frailty component was scored as one point if at least one indicator variable was present on admission, and summed to derive a Frailty Score, where a higher Frailty Score suggests greater level of frailty (range, 0 to 14). Sociodemographic, clinical, and laboratory data were retrieved from the electronic medical record through web-based data query tools and record review (N = 278). Mean age was 70.2 (SD = 1.3; range, 55¨C98), slightly over half were female, 64% were White, one-third were Black. The mean comorbidity count was 13 (SD = 4.56; range. 1¨C26) and medication count was 12 (SD = 5.2; range, 0¨C31). The most prevalent frailty components (&gt; 81%) were Fatigue, Weakness, Nutrition, Hemoglobin, Albumin, and CRP or hs-CRP. The mean Frailty Score was 9.03 (SD = 1.98; range, 2¨C13). Multiple linear regression was performed with 20 predictor variables and the Frailty Score and then with 14 of the 20 predictor variables that were significant in bivariate linear regression with the Frailty Score using the ENTER and STEPWISE method. All multiple regression models yielded seven significant predictor variables. Six predictors were common to all models: comorbidity, acute pain, ADL assistance, urinary incontinence, Braden Scale score, current tobacco use. In multiple regression with 20 predictors, age was a significant predictor however in multiple regression using ENTER and STEPWISE for 14 predictors, female gender was significant but not age. Results from STEPWISE regression yielded seven significant predictors that explained 27% of the variance in the Frailty Score (adj. R2 = .266, df (14, 263), F = 8.163, p = .000). Mean LOS was 9.92 days (SD = 9.58; range, 1¨C72; median, 7; mode, 5). Simple linear regression for the Frailty Score and log10 transformed LOS was statistically significant (adj. R2 = .090, df (1, 276), F = 29.293, p = .000). Twelve percent experienced 30-day readmission. Logistic regression conducted for the Frailty Score and 30-day readmission was not statistically significant (X 2 = 4.121, df (5), p = .532; ¦Â coefficient = .100, df (1), 95% CI = .913¨C1.1337, p = .307). The Frailty Score characterized this hospitalized population as acutely ill with high comorbidity, symptom burden, nutrition deficits and evidence of physiologic vulnerability and inflammation. Study findings have implications for nursing practice, interdisciplinary collaboration, education, research, and public policy

    Low Serum Klotho Concentration is Associated with Worse Cognition, Psychological Components of Frailty, Dependence, and Falls in Nursing Home Residents

    Serum alpha-klotho (s-klotho) protein has been linked with lifespan, and low concentrations of s-klotho have been associated with worse physical and cognitive outcomes. Although its significance in aging remains unclear, s-klotho has been proposed as a molecular biomarker of frailty and dependence. This study is a secondary analysis of data from a clinical trial performed in a population of 103 older individuals living in 10 nursing homes in Gipuzkoa (Spain). We aimed to elucidate associations between s-klotho (as measured by enzyme-linked immunosorbent assay) and body composition, physical fitness, and cognition, as well as frailty and dependence (determined using validated tests and scales). In addition, we investigated the association of s-klotho concentration with falls in the six months following the initial assessment. Low s-klotho levels were associated with a lower score in the psychological component of the Tilburg Frailty Indicator, a worse score in the Coding Wechsler Adult Intelligence Scale, and a greater dependence in activities of daily living. Moreover, participants with lower s-klotho concentrations suffered more falls during the 6 months after the assessment.
