Background: When patients receiving palliative care are transferred between care settings, adequate collaboration and information exchange between health care professionals is necessary to ensure continuity, efficiency and safety of care. Several studies identified deficits in communication and information exchange between care settings. Aim of this study was to get insight in the quality of collaboration and information exchange in palliative care from the perspectives of nurses. Methods: We performed a cross-sectional regional survey study among nurses working in different care settings. Nurses were approached via professional networks and media. Respondents were asked questions about collaboration in palliative care in general and about their last deceased patient. Potential associations between quality scores for collaboration and information handovers and characteristics of respondents or patients were tested with Pearsonā€™s chi-square test. Results: A total of 933 nurses filled in the questionnaire. Nurses working in nursing homes were least positive about inter-organizational collaboration. Forty-six per cent of all nurses had actively searched for such collaboration in the last year. For their last deceased patient, 10% of all nurses had not received the information handover in time, 33% missed information they needed. An adequate information handover was positively associated with timeliness and completeness of the information and the patient being well-informed, not with procedural characteristics. Conclusion: Nurses report that collaboration between care settings and information exchange in palliative care is suboptimal. This study suggests that health care organizations should give more attention to shared professionalization towards inter-organizational collaboration among nurses in order to facilitate high-quality palliative care

Information is available in many forms from different sources, in distributed locations; access to information is supported by networks of varying performance; the cost of accessing and transporting the information varies for both the source and the transport route. Users who vary in their preferences, background knowledge required to interpret the information and motivation for accessing it, gather information to perform many different tasks. This position paper outlines some of these variations in information provision and access, and explores the impact these variations have on the userā€™s task performance, and the possibilities they make available to adapt the user interface for the presentation of information

Trustworthiness and Quality of Context Information

    Get PDF
Context-aware service platforms use context information to customize their services to the current usersā€™ situation. Due to technical limitations in sensors and context reasoning algorithms, context information does not always represent accurately the reality, and Quality of Context (QoC) models have been proposed to quantify this inaccuracy. The problems we have identified with existing QoC models is that they do not follow a standard terminology and none of them clearly differentiate quality attributes related to instances of context information (e.g. accuracy and precision) from trustworthiness, which is a quality attribute related to the context information provider. In this paper we propose a QoC model and management architecture that supports the management of QoC trustworthiness and also contributes to the terminology alignment of existing QoC models.\ud In our QoC model, trustworthiness is a measurement of the reliability of a context information provider to provide context information about a specific entity according to a certain quality level. This trustworthiness value is used in our QoC management architecture to support context-aware service providers in the selection of trustworthy context\ud providers. As a proof of concept to demonstrate the feasibility of our work we show a prototype implementation of our QoC model and management architecture

What next for Shared Lives? Family-based support as a potential option for older people

    Get PDF
With an ageing population and limited resources the challenge for policy makers and practitioners is how best to provide for the care and support needs of older people. This article draws on findings from two studies, a scoping study of the personalisation of care services and another which aimed to generate evidence about the potential use of family-based support schemes (Shared Lives, SL) for certain groups of older people. Forty-three schemes participated in a survey to gather information about services provided and the extent to which this included older people and their carers, and six staffs were interviewed across two schemes about issues for expanding provision for older people in their local areas. It was evident that SL schemes were already supporting a number of older people and there was support for expansion from both schemes and local authorities. Adequate resources, awareness raising, management commitment, and a pool of suitable carers would be needed to support any expansion effort. There is also still a need for SL to be more widely known and understood by care managers if it is to be considered part of mainstream provision for older people
