    Total data quality management: a study of bridging rigor and relevance

    Ensuring data quality is of crucial importance to organizations. The Total Data Quality Management (TDQM) theory provides a methodology to ensure data quality. Although well researched, the TDQM methodology is not easy to apply. In the case of Honeywell Emmen, we found that the application of the methodology requires considerable contextual redesign, flexibility in use, and the provision of practical tools. We identified team composition, toolsets, development of obvious actions, the design of phases, steps, and actions, and sessions as vital elements of making an academically rooted methodology applicable. Such an applicable methodology, we name “well articulated”, because it incorporates the existing academic theory and it is made operational. This enables the methodology to be systematically beta tested and made useful for different organizational conditions

    Theory-based communication skills training for medicine counter assistants to improve consultations for non-prescription medicines

    Context: Medicine counter assistants (MCAs) supply the majority of non-prescription medicines (NPMs) to consumers. Suboptimal communication during consultations between consumers and MCAs has been identified as a major cause of inappropriate supply. Evidence from medical consultations suggests that training in specified communication skills can change professional behaviour. Methods: A feasibility study was conducted to evaluate the effect of theory-based communication skills training for MCAs. Thirty MCAs were recruited from 21 community pharmacies in Grampian, Scotland. The intervention comprised 2 4-hour training sessions, held 1 month apart. The sessions were informed by results from previous studies and the Calgary−Cambridge evidence-based model of communication skills training. Strategies for guiding individuals through change were adopted from cognitive behavioural therapy techniques. The theory of planned behaviour was used to assess potential pathways to behaviour change. Recorded data were collected during covert visits to the pharmacies by simulated patients at baseline and 1 month after each training session. Communication performance was measured as the number and type of questions asked. Results: Compared with baseline measures, the total number of questions asked increased in the intervention group at both timepoints. No change was shown in the control group between baseline and follow-up 1, and a decrease was shown in the total number of questions from follow-up 1 to 2. The intervention appeared to have greater effect on consultations involving advice, compared with those concerning product requests. Discussion: Communication performance improved following training. Increased information exchange is associated with guideline-compliant supply of NPMs. A substantive randomised, controlled trial is now planned to assess the intervention.This study was funded by the Chief Scientist Office, Scottish Executive Health Department

    The Design of a new NICU Patient Area: Combining Design for Usability and Design for Emotion

    In the design of medical products both usability and emotional experience are important to be considered. Usability can enhance the work situation of medical staff and ensure patient safety. Emotion related product aspects, on the other hand, influence the recovery pace of patients as well as the work satisfaction of staff. For an optimal medical design both aspects should receive well-balanced attention during the design process. Usability and emotional experience are currently related in literature. However, about the relation between these two aspects in practical design projects little information is available. Therefore we will discuss the exploration of the practical relation between Design for Usability and Design for Emotion in a design process. We explored the relation during concurrent application of both design approaches to the design of a patient area for a Neonatal Intensive Care Unit (NICU). Our aim was not to develop a new design method, but to explore in practice how both design approaches could be addressed concurrently. This paper describes the applied design approach, its strength and weaknesses as well as the design results. Overall, the NICU design case has proven that the concurrent application of Design for Emotion and Design for Usability is feasible in practice and results in a satisfactory design.     Creation of a Decision Support Tool for Expectant Parents Facing Threatened Periviable Delivery: Application of a User-Centered Design Approach

    Background Shared decision-making (SDM) is optimal in the context of periviable delivery, where the decision to pursue life-support measures or palliation is both preference sensitive and value laden. We sought to develop a decision support tool (DST) prototype to facilitate SDM by utilizing a user-centered design research approach. Methods We convened four patient and provider advisory boards with women and their partners who had experienced a surviving or non-surviving periviable delivery, pregnant women who had not experienced a prior preterm birth, and obstetric providers. Each 2-h session involved design research activities to generate ideas and facilitate sharing of values, goals, and attitudes. Participant feedback shaped the design of three prototypes (a tablet application, family story videos, and a virtual reality experience) to be tested in a final session. Results Ninety-five individuals (48 mothers/partners; 47 providers) from two hospitals participated. Most participants agreed that the prototypes should include factual, unbiased outcomes and probabilities. Mothers and support partners also desired comprehensive explanations of delivery and care options, while providers wanted a tool to ease communication, help elicit values, and share patient experiences. Participants ultimately favored the tablet application and suggested that it include family testimonial videos. Conclusion Our results suggest that a DST that combines unbiased information and understandable outcomes with family testimonials would be meaningful for periviable SDM. User-centered design was found to be a useful method for creating a DST prototype that may lead to improved effectiveness, usability, uptake, and dissemination in the future, by leveraging the expertise of a wide range of stakeholders
