    Sigurnost rada u anatomskom laboratoriju s formalinom i inovativno praćenje procjene profesionalne izloženosti formaldehidu

    This review is directed at preventive health professionals, anatomic pathologists and technicians to focus their attention on the rapidly growing field of safe formalin practices. An updated overview of the most recent improvements in preventive measures versus formaldehyde (FA) in the anatomic pathology laboratories (APL) is provided. The occupational hygienist role and the required knowledge for a modern and clear occupational exposure assessment are described. Real-time, in-continuous, commercial analyzers for repeated FA exposure assessment are considered to evaluate technical changes in air monitoring programs, introduced to mitigate FA emissions, in compliance with the adopted limit values. To better choose the adequate instrumentation, the main features of each FA monitoring instrument recently introduced on the market are listed. Moreover, the main features of the modern workflow setting in APL are summarized. A computer-based scientific and non-scientific reports search by key-words was performed on PubMed, Web of Science, Google Scholar and Google Patents databases, querying the following topics: i) grossing workstation for ergonomic layout, ii) commercially available direct reading tools to measure formalin, iii) real-time, in-continuous FA monitoring instruments for sale. This review represents a useful tool to summarize the technical requirements and expert know-how necessary to minimize FA emissions and produce an exhaustive FA assessment in the APL.Ovaj pregled usmjeren je na preventivne zdravstvene radnike, anatomske patologe i tehničare kako bi svoju pozornost usredotočili na brzo rastuće područje sigurnih formalinskih praksi. Ažurirani pregled nudi najnovija poboljšanja preventivnih mjera u odnosu na formaldehid (FA) u laboratorijima za anatomsku patologiju (APL). Opisana je uloga higijeničara na radu i potrebna znanja za modernu i jasnu procjenu izloženosti na radu. Komercijalni analizatori u stvarnom vremenu za kontinuiranu procjenu izloženosti FA razmatraju se za procjenu tehničkih promjena u programima praćenja zraka, uvedenim radi ublažavanja emisija FA, u skladu s prihvaćenim graničnim vrijednostima. Kako bi se bolje odabrala odgovarajuća instrumentacija, navedene su glavne značajke svakog instrumenta za praćenje FA koji je nedavno predstavljen na tržištu. Štoviše, sažete su glavne značajke suvremenih postavki tijeka rada u APL-u. Računalno zasnovano pretraživanje znanstvenih i neznanstvenih izvješća po ključnim riječima provedeno je u bazama podataka PubMed, Web of Science, Google Scholar i Google Patents, s težištem na sljedeće teme: i) prikupljanje radnih stanica za ergonomski raspored, ii) komercijalno dostupni alati za izravno očitanje mjerenja formalina, iii) instrumenti za kontinuirano praćenje FA u stvarnom vremenu u prodaji. Ovaj pregled predstavlja koristan alat za sažimanje tehničkih zahtjeva i stručnog znanja potrebnog za minimiziranje emisija FA i izradu iscrpne procjene FA u APL-u

    Ebinformatics: Ebola fuzzy informatics systems on the diagnosis, prediction and recommendation of appropriate treatments for Ebola virus disease (EVD)

    AbstractEbola Virus Disease (EVD) also known as the Ebola hemorrhagic fever is a very deadly infectious disease to humankind. Therefore, a safer and complementary method of diagnosis is to employ the use of an expert system in order to initiate a platform for pre-clinical treatments, thus acting as a precursor to comprehensive medical diagnosis and treatments. This work presents a design and implementation of informatics software and a knowledge-based expert system for the diagnosis, and provision of recommendations on the appropriate type of recommended treatment to the Ebola Virus Disease (EVD).In this research an Ebola fuzzy informatics system was developed for the purpose of diagnosing and providing useful recommendations to the management of the EVD in West Africa and other affected regions of the world. It also acts as a supplementary resource in providing medical advice to individuals in Ebola – ravaged countries. This aim was achieved through the following objectives: (i) gathering of facts through the conduct of a comprehensive continental survey to determine the knowledge and perception level of the public about factors responsible for the transmission of the Ebola Virus Disease (ii) develop an informatics software based on information collated from health institutions on basic diagnosis of the Ebola Virus Disease-related symptoms (iii) adopting and marrying the knowledge of fuzzy logic and expert systems in developing the informatics software. Necessary requirements were collated from the review of existing expert systems, consultation of journals and articles, and internet sources. Online survey was conducted to determine the level at which individuals are aware of the factors responsible for the transmission of the Ebola Virus Disease (EVD). The expert system developed, was designed to use fuzzy logic as its inference mechanism along with a set of rules. A knowledge base was created to help provide diagnosis on the Ebola Virus Disease (EVD). The Root Sum Square (RSS) was adopted as a fuzzy inference method. The degree of participation of each input parameter was shown using the triangular membership function and the defuzzification technique used is the Center of Gravity (CoG).The resulting software produced a user-friendly desktop-based, Windows-based, application and the tools used were explained in the results section in three (3) separate phases. First, a comprehensive online survey was conducted over a period of about 3–9 months. 100 Participants participated in the survey on the perception and knowledge analysis of different individuals about Ebola Virus Disease (EVD) transmission factors. 31% of the participants didn't know that there is presently no cure for Ebola. 28% believed that there is presently a cure for Ebola. 43% agreed that Ebola is both air-borne and water-borne, while 33% disagreed, 24% do not know. 23% believed that insects and mosquitoes can help in transmitting the Ebola Virus Disease (EVD), while 30% were completely ignorant. We noticed that ignorance was a major limiting factor among some participants.Second, a test was conducted among 45 people. Results from a comprehensive testing of the Ebinformatics software by allowing users to operate and use the software, revealed that 60% of them were satisfied, while 16% were not satisfied with the software, while 24% were indifferent. 69% of the users were in agreement that Ebinformatics was supportive, 20% disagreed, while 11% were indifferent. 67% found the software easy to use, 13% disagreed, while 20% were indifferent. Third, the output of the software, showing the various diagnosis and recommendations interfaces were presented.     One-fits-all pretreatment protocol facilitating Fluorescence in Situ Hybridization on formalin-fixed paraffin-embedded, fresh frozen and cytological slides

    Background: The Fluorescence In Situ Hybridization (FISH) technique is a very useful tool for diagnostic and prognostic purposes in molecular pathology. However, clinical testing on patient tissue is challenging due to variables of tissue processing that can influence the quality of the results. This emphasizes the necessity of a standardized FISH protocol with a high hybridization efficiency. We present a pretreatment protocol that is easy, reproducible, cost-effective, and facilitates FISH on all types of patient material simultaneously with good quality results. During validation, FISH analysis was performed simultaneously on formalin-fixed paraffin-embedded, fresh frozen and cytological patient material in combination with commercial probes using our optimized one-fits-all pretreatment protocol. An optimally processed sample is characterized by strong specific signals, intact nuclear membranes, non-disturbing autofluorescence and a homogeneous DAPI staining. Results: In our retrospective cohort of 3881 patient samples, overall 93% of the FISH samples displayed good quality results leading to a patient diagnosis. All FISH were assessed on quality aspects such as adequacy and consistency of signal strength (brightness), lack of background and / or cross-hybridization signals, and additionally the presence of appropriate control signals were evaluated to assure probe accuracy. In our analysis 38 different FISH probes from 3 commercial manufacturers were used (Cytocell, Vysis and ZytoLight). The majority of the patients in this cohort displayed good signal quality and barely non-specific background fluorescence on all tissue types independent of which commercial probe was used. Conclusion: The optimized one-fits-all FISH method is robust, reliable and reproducible to deliver an accurate result for patient diagnostics in a lean workflow and cost-effective manner. This protocol can be used for widespread application in cancer and non-cancer diagnostics and research

    Developing artificial intelligence and machine learning to support primary care research and practice

    This thesis was motivated by the potential to use everyday data , especially that collected in electronic health records (EHRs) as part of healthcare delivery, to improve primary care for clients facing complex clinical and/or social situations. Artificial intelligence (AI) techniques can identify patterns or make predictions with these data, producing information to learn about and inform care delivery. Our first objective was to understand and critique the body of literature on AI and primary care. This was achieved through a scoping review wherein we found the field was at an early stage of maturity, primarily focused on clinical decision support for chronic conditions in high-income countries, with low levels of primary care involvement and model evaluation in real-world settings. Our second objective was to demonstrate how AI methods can be applied to problems in descriptive epidemiology. To achieve this, we collaborated with the Alliance for Healthier Communities, which provides team-based primary health care through Community Health Centres (CHCs) across Ontario to clients who experience barriers to regular care. We described sociodemographic, clinical, and healthcare use characteristics of their adult primary care population using EHR data from 2009-2019. We used both simple statistical and unsupervised learning techniques, applied with an epidemiological lens. In addition to substantive findings, we identified potential avenues for future learning initiatives, including the development of decision support tools, and methodological considerations therein. Our third objective was to advance interpretable AI methodology that is well-suited for heterogeneous data, and is applicable in clinical epidemiology as well as other settings. To achieve this, we developed a new hybrid feature- and similarity-based model for supervised learning. There are two versions, fit by convex optimization with a sparsity-inducing penalty on the kernel (similarity) portion of the model. We compared our hybrid models with solely feature- and similarity-based approaches using synthetic data and using CHC data to predict future loneliness or social isolation. We also proposed a new strategy for kernel construction with indicator-coded data. Altogether, this thesis progressed AI for primary care in general and for a particular health care organization, while making research contributions to epidemiology and to computer science

    Two Phase Flow, Phase Change and Numerical Modeling

    The heat transfer and analysis on laser beam, evaporator coils, shell-and-tube condenser, two phase flow, nanofluids, complex fluids, and on phase change are significant issues in a design of wide range of industrial processes and devices. The third section includes nanofluids for heat transfer, nanofluids in minichannels, potential and engineering strategies on nanofluids, and heat transfer at nanoscale. The forth section presents time-dependent melting and deformation processes of phase change material (PCM), thermal energy storage tanks using PCM, phase change in deep CO2 injector, and thermal storage device of solar hot water system. The advanced idea and information described here will be fruitful for the readers to find a sustainable solution in an industrialized society

    Adherence therapy for people with Parkinson's disease

    Introduction: Medication non-adherence is prevalent in Parkinson’s disease (PD). However, factors associated with non-adherence are unknown. Despite interventions to improve medication adherence being investigated in long-term conditions, few studies have focused on PD. Adherence Therapy (AT) is a novel, patient-centred approach to maximising adherence that has shown benefit in other chronic conditions. Aim: To investigate the efficacy of AT for improving medication adherence and quality of Life (QoL) in people with PD. Methods: To achieve the above aim I conducted a systematic review to identify factors associated with medication non-adherence, followed by a Cochrane systematic review on interventions for improving medication adherence in PD. I then tested the efficacy of AT in PD in a randomised controlled trial (RCT). Semistructured interviews were used to explore patients’ experiences of receiving AT. Results: Mood disorders, cognition, poor symptom control/QoL, younger age/longer disease duration, regimen complexity/polypharmacy, risk taking behaviours, poor knowledge of PD/education, lack of spouse/partner, low income, desire to maintain employment and gender were identified as factors associated with non-adherence in PD. Only one study previously investigated an intervention (didactic educational material) for improving medication adherence in PD, according to my Cochrane systematic review. Seventy-six patients and 46 spouse/carers completed the RCT (CAAT-PARK). At week-12 follow-up the active treatment group significantly improved in adherence and QoL compared to the treatment as usual group. Thematic analysis of interviews from 10 patients and 3 spouse/carers suggested that positive effects and attributes of AT may be important for the success of AT. Furthermore, the findings suggested that the mechanism of AT may be bi-directional and associated with improved confidence and self-efficacy. Conclusions: Adherence Therapy improved medication adherence and QoL in PD. A larger pragmatic trial to test the efficacy and cost effectiveness of Adherence Therapy with a control group placebo intervention is required. ISRCTN0783095

    Combined condylomatosis and Buschke-Loewenstein tumor: case report & systematic review

    Objectives: An undesired manifestation of HPV is the transformation of infected cells into neoplastic cells, with the subsequent development of precancerous intraepithelial lesions and in some instances cancer. The generally benign manifestations of HPV include asymptomatic and subclinical disease as well as anogenital condyloma acuminatum, warts and verruccosis. Certain HPV manifestations are however associated with poor outcome for the patient, and these include giant and multiple condylomas such as Buschke- Löwenstein tumor (BLT), and respiratory papillomatosis and their relapses. These conditions decrease the patient’s life quality, and have the potential to progress to malignancy, and are a particular problem for practitioners. Management of these patients is complicated due to the absence of therapies that can eliminate HPV and prevent wart recurrence, or medication that can cure genital warts and condylomas. Indeed, although there are many clinical reports describing the aggressive manifestation of genital warts in immune compromised individuals, the triggering mechanisms for such atypical lesions is not yet understood. Our objective here is to present a case report of combined condylomatosis and BLT in a patient with Henoch-Schönlein purpura and simultaneously infected with six HPV genotypes (6, 11, 18, 31, 43 & 56), and to systematicly review the literature in order to provide a better insight into these conditions. Methods: Clinical and laboratory parameters as well as a video of surgery of female patient (30 ys) with combined condylomatosis and BLT (the informed consent was obtained). Clinical and research literature over the last 15 years focusing on the immunological aspects of papillomavirus interaction with the innate and adaptive immune systems, and interference with the cellular and humoral host responses. Particular attention is given to the nature of papillomavirus clearance and persistence. Conclusions: Atypical manifestation of HPV was in our case related to systemic disease involving an autoimmune host response and long-term glucocorticoid treatment. Our review confirms that such atypical manifestations and the long-term persistence of HPV are often linked to disturbances in the hosts immune system. More specifically these comprise changes in the content of lymphocyte subsets (helper, cytotoxic and regulatory T-cells), NK cells and macrophages at HPV infected sites and in the blood, as well as changes in lymphocyte function related to the expression of INF-<& ®, IL-2; IL-4, IL-10, TGF®‚ and other cytokines
