    Colonoscopy and Colorectal Cancer Screening

    Colorectal cancer (CRC) represents a major public health problem worldwide. Fortunately most CRCs originate from a precursor lesion, the adenoma, which is accessible and removable. This is the rationale for CRC screening programs, which are aimed to diagnose CRC at an early stage or even better to detect and resect the advanced adenoma before CRC has developed. In this background colonoscopy emerges as the main tool to achieve these goals with recent evidence supporting its role in CRC prevention. This book deals with several topics to be faced when implementing a CRC screening program. The interested reader will learn about the rationale and challenges of implementing such a program, the management of the detected lesions, the prevention of complications of colonoscopy, and finally the use of other screening modalities that are emerging as valuable alternatives. The relevance of the topics covered in it and the updated evidence included by the authors turn this book into a very useful tool to introduce the reader in this amazing and evolving field

    A Survey on Deep Learning in Medical Image Analysis

    Deep learning algorithms, in particular convolutional networks, have rapidly become a methodology of choice for analyzing medical images. This paper reviews the major deep learning concepts pertinent to medical image analysis and summarizes over 300 contributions to the field, most of which appeared in the last year. We survey the use of deep learning for image classification, object detection, segmentation, registration, and other tasks and provide concise overviews of studies per application area.     British Society of Gastroenterology/Association of Coloproctology of Great Britain and Ireland/Public Health England post-polypectomy and post-colorectal cancer resection surveillance guidelines

    These consensus guidelines were jointly commissioned by the British Society of Gastroenterology (BSG), the Association of Coloproctology of Great Britain and Ireland (ACPGBI) and Public Health England (PHE). They provide an evidence-based framework for the use of surveillance colonoscopy and non-colonoscopic colorectal imaging in people aged 18 years and over. They are the first guidelines that take into account the introduction of national bowel cancer screening. For the first time, they also incorporate surveillance of patients following resection of either adenomatous or serrated polyps and also post-colorectal cancer resection. They are primarily aimed at healthcare professionals, and aim to address: Which patients should commence surveillance post-polypectomy and post-cancer resection? What is the appropriate surveillance interval? When can surveillance be stopped? two or more premalignant polyps including at least one advanced colorectal polyp (defined as a serrated polyp of at least 10 mm in size or containing any grade of dysplasia, or an adenoma of at least 10 mm in size or containing high-grade dysplasia); or five or more premalignant polyps The Appraisal of Guidelines for Research and Evaluation (AGREE II) instrument provided a methodological framework for the guidelines. The BSG's guideline development process was used, which is National Institute for Health and Care Excellence (NICE) compliant. two or more premalignant polyps including at least one advanced colorectal polyp (defined as a serrated polyp of at least 10 mm in size or containing any grade of dysplasia, or an adenoma of at least 10 mm in size or containing high-grade dysplasia); or five or more premalignant polyps The key recommendations are that the high-risk criteria for future colorectal cancer (CRC) following polypectomy comprise either: Two or more premalignant polyps including at least one advanced colorectal polyp (defined as a serrated polyp of at least 10 mm in size or containing any grade of dysplasia, or an adenoma of at least 10 mm in size or containing high-grade dysplasia); or five or more premalignant polyps This cohort should undergo a one-off surveillance colonoscopy at 3 years. Post-CRC resection patients should undergo a 1 year clearance colonoscopy, then a surveillance colonoscopy after 3 more years

    New Techniques in Gastrointestinal Endoscopy

    As result of progress, endoscopy has became more complex, using more sophisticated devices and has claimed a special form. In this moment, the gastroenterologist performing endoscopy has to be an expert in macroscopic view of the lesions in the gut, with good skills for using standard endoscopes, with good experience in ultrasound (for performing endoscopic ultrasound), with pathology experience for confocal examination. It is compulsory to get experience and to have patience and attention for the follow-up of thousands of images transmitted during capsule endoscopy or to have knowledge in physics necessary for autofluorescence imaging endoscopy. Therefore, the idea of an endoscopist has changed. Examinations mentioned need a special formation, a superior level of instruction, accessible to those who have already gained enough experience in basic diagnostic endoscopy.     Directional wavelet based features for colonic polyp classification

    In this work, various wavelet based methods like the discrete wavelet transform, the dual-tree complex wavelet transform, the Gabor wavelet transform, curvelets, contourlets and shearlets are applied for the automated classification of colonic polyps. The methods are tested on 8 HD-endoscopic image databases, where each database is acquired using different imaging modalities (Pentax's i-Scan technology combined with or without staining the mucosa), 2 NBI high-magnification databases and one database with chromoscopy high-magnification images. To evaluate the suitability of the wavelet based methods with respect to the classification of colonic polyps, the classification performances of 3 wavelet transforms and the more recent curvelets, contourlets and shearlets are compared using a common framework. Wavelet transforms were already often and successfully applied to the classification of colonic polyps, whereas curvelets, contourlets and shearlets have not been used for this purpose so far. We apply different feature extraction techniques to extract the information of the subbands of the wavelet based methods. Most of the in total 25 approaches were already published in different texture classification contexts. Thus, the aim is also to assess and compare their classification performance using a common framework. Three of the 25 approaches are novel. These three approaches extract Weibull features from the subbands of curvelets, contourlets and shearlets. Additionally, 5 state-of-the-art non wavelet based methods are applied to our databases so that we can compare their results with those of the wavelet based methods. It turned out that extracting Weibull distribution parameters from the subband coefficients generally leads to high classification results, especially for the dual-tree complex wavelet transform, the Gabor wavelet transform and the Shearlet transform. These three wavelet based transforms in combination with Weibull features even outperform the state-of-the-art methods on most of the databases. We will also show that the Weibull distribution is better suited to model the subband coefficient distribution than other commonly used probability distributions like the Gaussian distribution and the generalized Gaussian distribution. So this work gives a reasonable summary of wavelet based methods for colonic polyp classification and the huge amount of endoscopic polyp databases used for our experiments assures a high significance of the achieved results.(VLID)223912

    Optimizing Colorectal Cancer Screening

    Colorectal cancer (CRC) screening is a rapidly moving field. This thesis provides strategies and insights that may contribute to optimizing CRC screening. The main topics of the thesis are introduced in part I which gives an overview of the burden of CRC worldwide and the initiatives to reduce this burden. Part II further focusses on fecal occult blood test-based CRC screening. The main two fecal occult blood tests are the guaiac-based fecal occult blood test (gFOBT) and the more recently developed fecal immunochemical test (FIT). The diagnostic accuracy of FIT and gFOBT in average-risk individuals is compared in a meta-analysis. The subsequent chapters explore various aspects of FIT screening and optimization of FIT screening strategies. We focus on age differences, cut-off, the use of two FITS per screening round instead of one, and the predictive value of a negative FIT in subsequent screening rounds. Part III of the thesis provides more insight in factors that are associated with quality in CRC screening, surveillance and colonoscopy. The accuracy, quality, and readability of online information on CRC screening and surveillance is evaluated. Also, quality indicators for colonoscopy are obtained from different hospitals and the effect of plenary feedback on variation in quality of colonoscopy is assessed. Furthermore, adherence to recommended surveillance intervals following removal of adenomas is evaluated. Finally, part IV discusses the main findings and insights obtained from our research projects and gives directions for future research
