    Broadening Responsibilities: Consideration Of The Potential To Broaden The Role Of Uniformed Fire Service Employees

    What is this report about? This report, commissioned by the National Joint Council for Local Authority Fire and Rescue Services (NJC), aims to identify what impact, if any, firefighters can have on the delivery of emergency medical response and wider community health interventions in the UK. What are the overall conclusions? Appropriately trained and equipped firefighters co-responding1 to targeted, specific time critical medical events, such as cardiac arrest, can improve patient survival rates. The data also indicate that there is support from fire service staff – and a potential need from members of the public, particularly the elderly, isolated or vulnerable – to expand ‘wider work’. This includes winter warmth assessments, Safe and Well checks, community defibrillator training and client referrals when staff believe someone may have dementia, are vulnerable or even, for example, have substance dependencies such as an alcohol addiction. However, there is currently insufficient data to estimate the net benefit of this work

    Mobility presents a variety of opportunities as it allows users to access locations and services, and to meet people beyond their immediate surroundings. While the concept of mobility primarily focuses on the ease of moving, accessibility delineates the actual potential to participate in out-of-home activities. As a result, accessibility is a complex concept with a multitude of foci. This complexity is presented in the first section, which explains the general concept of accessibility, how it is defined and how it is related to the notion of transport-related exclusion. This section also gives an overview of the body of literature on the measures to determine area-based as well as personal accessibility levels and points out the important contrast between the simple, easy-to-interpret methods, adopted by policy makers and the complex methods preferred by experts. The second section clarifies how the dichotomous relationship between the urban and rural environment is reflected in transport policy that emphasizes on (especially car-based) mobility rather than on accessibility. Furthermore, the environmental and economic points of view are highlighted and the common policy strategies focused on sustainability are illustrated. Subsequently, the shortcomings in the way in which the contemporary debates concerning mobility, sustainability and the social implications of transport planning are conducted, are criticized. Finally, the last part of this section is dedicated to an extensive discussion on the ability of transport policies to, on the one hand, generate spatially as well as temporally uneven accessibility effects that give preference to certain population groups above others, and on the other hand, their ability to strive for a more equitable distribution of transport services amongst the population. The third section proposes two methodologies for measuring transport-related social exclusion implemented in a literature-based case study in Flanders. These studies comprise the following topics: measuring transport gaps by relating the social to the transport disadvantage and measuring modal disparities by comparing accessibility by private and public transport. The former investigates in which areas the provision of the public transport system is not tailored to specific public transport needs. The latter examines the disparity in access by private and public transport in order to highlight the car dependency. Both case studies incorporate the temporal variability in provision through the private and public transport network, as the time-of-day strongly influences accessibility levels

    Telecare, and particularly lifestyle monitoring, currently relies on the ability to detect and respond to changes in individual behaviour using data derived from sensors around the home. This means that a significant aspect of behaviour, that of an individuals emotional state, is not accounted for in reaching a conclusion as to the form of response required. The linked concepts of emotive and adaptive computing offer an opportunity to include information about emotional state and the paper considers how current developments in this area have the potential to be integrated within telecare and other areas of eHealth. In doing so, it looks at the development of and current state of the art of both emotive and adaptive computing, including its conceptual background, and places them into an overall eHealth context for application and development

    Fully digital self-screening for atrial fibrillation with patch electrocardiogram

    Atrial fibrillation (AF) is the most common arrhythmia worldwide. The AF is associated with severe mortality, morbidity, and healthcare costs, and guidelines recommend screening people at risk. However, screening methods and organization still need to be clarified. The current study aimed to assess the feasibility of a fully digital self-screening procedure and to assess the prevalence of undetected AF using a continuous patch electrocardiogram (ECG) monitoring system. Individuals ≄65 years old with at least one additional risk factor for stroke from the general population of Norway were invited to a fully digital continuous self-screening for AF using a patch ECG device (ECG247 Smart Heart Sensor). Participants self-reported clinical characteristics and usability online, and all participants received digital feedback of their results. A total of 2118 individuals with a mean CHA2DS2-VASc risk score of 2.6 (0.9) were enrolled in the study [74% women; mean age 70.1 years (4.2)]. Of these, 1849 (87.3%) participants completed the ECG self-screening test, while 215 (10.2%) did not try to start the test and 54 (2.5%) failed to start the test. The system usability score was 84.5. The mean ECG monitoring time was 153 h (87). Atrial fibrillation was detected in 41 (2.2%) individuals.publishedVersio
