    Comparison of physical fitness between healthy and mild‚Äźto‚Äźmoderate asthmatic children with exercise symptoms: A cross‚Äźsectional study

    .Objective Asthma is a chronic disease that may affect physical fitness, although its primary effects on exercise capacity, muscle strength, functionality and lifestyle, in children and adolescents, are still poorly understood. This study aimed to evaluate the differences in cardiorespiratory fitness, muscle strength, lifestyle, lung function, and functionality between asthmatics with exercise symptoms and healthy children. In addition, we have analyzed the association between clinical history and the presence of asthma. Study Design Cross-sectional study including 71 patients with a diagnosis of asthma and 71 healthy children and adolescents (7‚Äď17 years of age). Anthropometric data, clinical history, disease control, lifestyle (KIDMED and physical activity questionnaires), lung function (spirometry), exercise-induced bronchoconstriction test, aerobic fitness (cardiopulmonary exercise test), muscle strength and functionality (timed up and go; timed up and down stairs) were evaluated. Results Seventy-one patients with asthma (mean age 11.5‚ÄȬĪ‚ÄČ2.7) and 71 healthy subjects (mean age 10.7‚ÄȬĪ‚ÄČ2.5) were included. All asthmatic children had mild to moderate and stable asthma. EIB occurred in 56.3% of asthmatic children. Lung function was significantly (p‚ÄČ<‚ÄČ.05) lower in the asthmatic group when compared to healthy peers, as well as the cardiorespiratory fitness, muscle strength, lifestyle and functionality. Moreover, asthmatic children were more likely to have atopic dermatitis, allergic reactions, food allergies, and a family history of asthma when compared to healthy children. Conclusions Children with mild-to-moderate asthma presenting exercise symptoms show a reduction in cardiorespiratory fitness, muscle strength, lung function, functionality, and lifestyle when compared to healthy peers. The study provides data for pediatricians to support exercise practice aiming to improve prognosis and quality of life in asthmatic children.S

    Investigating the Drivers & Challenges of Implementing Immersive Sensory Technology within Construction Site Safety

    The use of immersive sensory technology for safety management is generally shown positively in academic literature. Many researchers have demonstrated applications of this technology for improving safety training in a risk-free environment. Despite the reported benefits and a global pandemic forcing the digital agenda, the uptake of this technology for this purpose remains slow. This study aims to investigate current drivers and challenges of implementing this technology for safety from an industry-based perspective. To achieve this, qualitative data was collected through 4 online focus groups involving 21 industry professionals working within the field. The findings identified that even amongst these experts, the technology was rarely implemented on projects specifically for safety. Despite this lack of adoption, participants agreed that if implemented correctly this technology has the potential to enhance site safety processes such as inductions, toolbox talks and general safety training. The commitment to safety and legislative requirements were identified as key drivers, whilst deep rooted challenges surrounding client demand, costs and leadership dominated the discussion. The onsite practicalities, personal comfort and lack of digital skills were also identified as concerns if this technology was to be adopted more mainstream in safety training. Further recommendations are made to develop understanding of these specific challenges, including investigating the industry need and availability of specific skills in immersive safety applications. In addition, it is recommended that further empirical evidence including the impact of this technology when implemented for safety on projects is provided in literature

    Desarrollo de materiales bioactivos con potencial aplicación odontológica mediante impregnación asistida por CO2 supercrítico

Para este prop√≥sito, se construy√≥ un equipo de alta presi√≥n en el que se llevaron a cabo m√ļltiples ensayos de impregnaci√≥n de eugenol y de sorci√≥n de CO2 en un hilo dental comercial de PA6 en distintas condiciones de presi√≥n y temperatura (40 ‚Äď 60 ¬įC y 8 ‚Äď 12 MPa). Con el fin de encontrar las mejores condiciones del proceso de impregnaci√≥n, se evalu√≥ la influencia de diferentes variables operativas (presi√≥n, temperatura, tiempo de contacto y velocidad de despresurizaci√≥n) sobre la cantidad de eugenol impregnada en el material. Adem√°s, se estudiaron los principales fen√≥menos difusivos que ocurren en el proceso de impregnaci√≥n del eugenol en condiciones supercr√≠ticas. Para ello, se hicieron ensayos de cin√©tica de sorci√≥n del CO2 y del eugenol en PA6 a diferentes condiciones de presi√≥n y temperatura y se determin√≥ el coeficiente de difusi√≥n aparente para ambas especies en este pol√≠mero. Por otra parte, se evaluaron las propiedades finales del material impregnado, analizando las propiedades mec√°nicas, t√©rmicas y morfol√≥gicas del material original, presurizado con CO2 e impregnado con eugenol. Adicionalmente se evalu√≥ la actividad antimicrobiana del material impregnado frente a dos bacterias comunes (Escherichia coli y Staphylococcus aureus).     Women’s Experiences of Accessing Breastfeeding and Perinatal Health Support in the Context of Intimate Partner Violence: An Interpretive Description Study

    Background: Women experiencing intimate partner violence are at a heightened risk of negative perinatal and breastfeeding outcomes. This study explored the experiences of accessing breastfeeding support for women who endorse a history of intimate partner violence. A study of five in-depth semi-structured interviews were completed at 12-weeks postpartum with breastfeeding mothers with a history of intimate partner violence. Findings: Women expressed difficulties in accessing a healthcare provider who had specialized skill in breastfeeding support. Trust in their healthcare provider, built through displays of compassion and competence, was important to mitigate obstacles experienced during care access for this population. Trauma-and-violence-informed care principles were beneficial to the development of the therapeutic relationship in perinatal care. Women placed value on breastfeeding support received from both healthcare providers and social supports, which impacted mothers’ perceived breastfeeding support and self-efficacy. Further, mothers described increased levels of breastfeeding self-efficacy after engaging in a trauma-and-violence-informed care program aimed at supporting breastfeeding. Conclusions: Trauma-informed care may aid in the development of trust in the therapeutic relationship, which in turn impacts access to breastfeeding support and breastfeeding self-efficacy.     Exploring the effects of spinal cord stimulation for freezing of gait in parkinsonian patients

    Dopaminergic replacement therapies (e.g. levodopa) provide limited to no response for axial motor symptoms including gait dysfunction and freezing of gait (FOG) in Parkinson’s disease (PD) and Richardson’s syndrome progressive supranuclear palsy (PSP-RS) patients. Dopaminergic-resistant FOG may be a sensorimotor processing issue that does not involve basal ganglia (nigrostriatal) impairment. Recent studies suggest that spinal cord stimulation (SCS) has positive yet variable effects for dopaminergic-resistant gait and FOG in parkinsonian patients. Further studies investigating the mechanism of SCS, optimal stimulation parameters, and longevity of effects for alleviating FOG are warranted. The hypothesis of the research described in this thesis is that mid-thoracic, dorsal SCS effectively reduces FOG by modulating the sensory processing system in gait and may have a dopaminergic effect in individuals with FOG. The primary objective was to understand the relationship between FOG reduction, improvements in upper limb visual-motor performance, modulation of cortical activity and striatal dopaminergic innervation in 7 PD participants. FOG reduction was associated with changes in upper limb reaction time, speed and accuracy measured using robotic target reaching choice tasks. Modulation of resting-state, sensorimotor cortical activity, recorded using electroencephalography, was significantly associated with FOG reduction while participants were OFF-levodopa. Thus, SCS may alleviate FOG by modulating cortical activity associated with motor planning and sensory perception. Changes to striatal dopaminergic innervation, measured using a dopamine transporter marker, were associated with visual-motor performance improvements. Axial and appendicular motor features may be mediated by non-dopaminergic and dopaminergic pathways, respectively. The secondary objective was to demonstrate the short- and long-term effects of SCS for alleviating dopaminergic-resistant FOG and gait dysfunction in 5 PD and 3 PSP-RS participants without back/leg pain. SCS programming was individualized based on which setting best improved gait and/or FOG responses per participant using objective gait analysis. Significant improvements in stride velocity, step length and reduced FOG frequency were observed in all PD participants with up to 3-years of SCS. Similar gait and FOG improvements were observed in all PSP-RS participants up to 6-months. SCS is a promising therapeutic option for parkinsonian patients with FOG by possibly influencing cortical and subcortical structures involved in locomotion physiology

    This SSHRC-funded (Grant #972-2020-1009) scoping review synthesizes existing research (2010-2021) related to both private and shared electric micro-mobilities (i.e. e-bikes, e-scooters, e-unicycles, e-skateboards). It considers themes such as: rider demographics, usage, and motivations; mobility justice; benefits of and barriers to EMM use; safety and injuries; modal shift among forms of transportation; rider satisfaction with mode choice; environmental impact; conflict and controversy; EMM pilot programs; and EMM integration, legislation, and policy recommendations. Aside from scholarship, media reports are also included, in order to speak to the environment in which the research takes place

    Structural and Attitudinal Barriers to Bicycle Ownership and Cycle-Based Transport in Gauteng, South Africa

    Policies that aim to facilitate and promote non-motorised transport (NMT), and in particular cycling, have been developed by many high-income countries facing increasingly congested roads and saturated public transport systems. Such policies are also emerging in many low- and middle-income settings where high rates of urbanisation have led to similar problems with motorised transport. The aim of the present study was to better understand the potential structural and attitudinal barriers to cycle-based transport in one such context: South Africa‚Äôs Gauteng Province, the industrial powerhouse of sub-Saharan Africa that has recently made a firm commitment to NMT. The study focussed on demographic and socioeconomic variation in bicycle and car ownership, and related this to: (1) the reported use of motorised and non-motorised transport (both private and public); and (2) perceived ‚Äėproblems‚Äô with cycling. The analyses drew on interviews with key respondents from n‚ÄČ=‚ÄČ27,490 households conducted in 2013 as part of the third Quality of Life survey undertaken by the Gauteng City Regional Observatory. The survey contained items on three outcomes of interest: household vehicle ownership (bicycles and cars); modes of transport used for the ‚Äútrips‚ÄĚ most often made; and respondents‚Äô ‚Äúsingle biggest problem with‚Ķ cycling‚ÄĚ. Respondent- and household-level demographic and socioeconomic determinants of these outcomes were examined using descriptive and multivariable statistical analyses, the latter after adjustment for measured potential confounders identified using a theoretical causal path diagram (in the form of a directed acyclic graph). Of the n‚ÄČ=‚ÄČ26,469 households providing complete data on all of the variables examined in the present study, only n‚ÄČ=‚ÄČ8722 (32.9%) owned a car and fewer still (n‚ÄČ=‚ÄČ2244; 8.4%) owned a bicycle. The ownership of these assets was commonest amongst wealthier, economically active households; and those that owned a car had over five times the odds of also owning a bicycle, even after adjustment for potential confounding (OR 5.17; 95% CI 4.58, 5.85). Moreover, of household respondents who reported making ‚Äėtrips‚Äô during the preceding month (n‚ÄČ=‚ÄČ18,209), over two-thirds of those whose households owned a car (70.1%) reported private car-based transport for such trips, while only 3.2% of those owning a bicycle reported cycling. Amongst the specific responses given to the item requesting the ‚Äúsingle biggest problem with‚Ķ cycling‚ÄĚ by far the commonest was ‚ÄúDon‚Äôt know how to cycle‚ÄĚ (32.2%), less than half as many citing ‚ÄúVehicle accident risk‚ÄĚ (15.9%), and fewer still: ‚ÄúDestination is too far‚ÄĚ (13.9%); ‚ÄúCrime‚ÄĚ (10.3%); ‚ÄúToo much effort‚ÄĚ (9.2%); or ‚ÄúLack of good paths‚ÄĚ (4.6%). While the first of these reasons was commonest amongst poorer households, concerns about risk and effort were both most common amongst better educated, economically active and wealthier/better serviced households. In contrast, concerns over (cycle) paths were only common amongst those owning bicycles. The low prevalence of household bicycle ownership, and the disproportionate number of households owning bicycles that also owned cars, might explain the very small proportion of the ‚Äėthe trips most often made‚Äô that involved cycle-based transport (0.3%), and the preferential use of cars amongst households owning both bicycles and cars. Low levels of bicycle ownership might also explain why so many respondents cited ‚ÄúDon‚Äôt know how‚ÄĚ as the ‚Äúsingle biggest problem with‚Ķ cycling‚ÄĚ; although risk and effort were also substantial concerns (presumably for many who did, and some who did not, know how to cycle); the lack of suitable cycle lanes being only primarily a concern for those who actually owned bicycles. Structural and attitudinal barriers to cycle-based transport limit the use of cycle-based transport in Gauteng, not only amongst the vast majority of household respondents who lack the means to cycle (and the means to learn how), but also amongst those dissuaded from learning to cycle, purchasing a bicycle and/or using a bicycle they own by: the risks and effort involved; the lack of suitable cycle paths; and/or because they also own a car and prefer to drive than cycle

    Conservation agriculture affects grain and nutrient yields of Maize (Zea Mays L.) and can impact food and nutrition security in Sub-Saharan Africa

    Maize is a major staple and plays an essential role in food and nutrition security in Sub-Saharan Africa (SSA). Conservation agriculture (CA), a climate-smart agriculture practise based on minimum soil disturbance, crop residue retention, and crop diversification, has been widely advocated but without extensive research on the impact it may have on maize nutrient composition, and food and nutrition security. This study assessed the grain yield, macro- and micronutrient mineral content, and nutrient yield of eight maize varieties grown in Malawi, and how these are affected by CA practises over two seasons. The minerals were analysed by inductively coupled plasma (ICP) coupled to optical emission spectroscopy (OES) and to mass spectroscopy (MS). Grain yield and Se content differed among the varieties, while C, N, Fe, K, Mg, Mn, P, and Zn were similar. The local variety Kanjerenjere showed lowest grain and nutrient yields. The open-pollinated varieties (OPVs) concentrated more minerals than the F1 hybrids, but the latter showed higher yields for both grain and nutrients. Typical consumption of the eight maize varieties could fully meet the protein and Mg dietary reference intake (DRIs) of Malawian children (1‚Äď3 years), as well as Mg and Mn needs of adult women (19‚Äď50 years), but their contribution to dietary requirements was low for Fe (39‚Äď41%) and K (13‚Äď21%). The trials showed that CA increased grain yield (1.2- to 1.8-fold) and Se content (1.1- to 1.7-fold), but that it had no effect on C, K, Mg, P, and Zn, and that N (1.1- to 1.2-fold), Mn (1.1- to 1.8-fold), and Fe (1.3- to 3.4-fold) were reduced. The high increase in grain yield under CA treatments resulted in increased yields of protein and Se, no effect on the yields of K, Mg, Mn, P, Zn, and reduced Fe yield. Conservation agriculture could contribute in reducing the risk of Se deficiency in Malawian women and children but exacerbates the risk of Fe deficiency. A combination of strategies will be needed to mitigate some of the foreseen effects of climate change on agriculture, and food and nutrition security, and improve nutrient intake

    The clinical data for the studies was collected at The PROMIS Recovery Centre, a Minnesota Model treatmentc entre for addictions,w hich encouragesth e membership and use of the 12 step Anonymous Fellowships, and is abstinence based. The area of addiction is contextualised in a review chapter which focuses on research relating to the phenomenon of cross addiction. A study examining the concept of "addictive orientations" in male and female addicts is described, which develops a study conductedb y StephensonM, aggi, Lefever, & Morojele (1995). This presents study found a four factor solution which appeared to be subdivisions of the previously found Hedonism and Nurturance factors. Self orientated nurturance (both food dimensions, shopping and caffeine), Other orientated nurturance (both compulsive helping dimensions and work), Sensation seeking hedonism (Drugs, prescription drugs, nicotine and marginally alcohol), and Power related hedonism (Both relationship dimensions, sex and gambling. This concept of "addictive orientations" is further explored in a non-clinical population, where again a four factor solution was found, very similar to that in the clinical population. This was thought to indicate that in terms of addictive orientation a pattern already exists in this non-clinical population and that consideration should be given to why this is the case. These orientations are examined in terms of gender differences. It is suggested that the differences between genders reflect power-related role relationships between the sexes. In order to further elaborate the significance and meaning behind these orientations, the next two chapters look at the contribution of personality variables and how addictive orientations relate to psychiatric symptomatology. Personality variables were differentially, and to a considerable extent predictably involved with the four factors for both males and females.Conscientiousness as positively associated with "Other orientated Nurturance" and negatively associated with "Sensation seeking hedonism" (particularly for men). Neuroticism had a particularly strong association with the "Self orientated Nurturance" factor in the female population. More than twice the symptomatology variance was explained by the factor scores for females than it was for males. The most important factorial predictors for psychiatric symptomatology were the "Power related hedonism" factor for males, and "Self oriented nurturance" for females. The results are discussed from theoretical and treatment perspectives
