    Computational modelling of the scoliotic spine: A literature review

    Scoliosis is a deformity of the spine that in severe cases requires surgical treatment. There is still disagreement among clinicians as to what the aim of such treatment is as well as the optimal surgical technique. Numerical models can aid clinical decision-making by estimating the outcome of a given surgical intervention. This paper provided some background information on the modelling of the healthy spine and a review of the literature on scoliotic spine models, their validation, and their application. An overview of the methods and techniques used to construct scoliotic finite element and multibody models was given as well as the boundary conditions used in the simulations. The current limitations of the models were discussed as well as how such limitations are addressed in non-scoliotic spine models. Finally, future directions for the numerical modelling of scoliosis were addressed

    A Robotic Neuro-Musculoskeletal Simulator for Spine Research

    An influential conceptual framework advanced by Panjabi represents the living spine as a complex neuromusculoskeletal system whose biomechanical functioning is rather finely dependent upon the interactions among and between three principal subsystems: the passive musculoskeletal subsystem (osteoligamentous spine plus passive mechanical contributions of the muscles), the active musculoskeletal subsystem (muscles and tendons), and the neural and feedback subsystem (neural control centers and feedback elements such as mechanoreceptors located in the soft tissues) [1]. The interplay between subsystems readily encourages thought experiments of how pathologic changes in one subsystem might influence another--for example, prompting one to speculate how painful arthritic changes in the facet joints might affect the neuromuscular control of spinal movement. To answer clinical questions regarding the interplay between these subsystems the proper experimental tools and techniques are required. Traditional spine biomechanical experiments are able to provide comprehensive characterization of the structural properties of the osteoligamentous spine. However, these technologies do not incorporate a simulated neural feedback from neural elements, such as mechanoreceptors and nociceptors, into the control loop. Doing so enables the study of how this feedback--including pain-related--alters spinal loading and motion patterns. The first such development of this technology was successfully completed in this study and constitutes a Neuro-Musculoskeletal Simulator. A Neuro-Musculoskeletal Simulator has the potential to reduce the gap between bench and bedside by creating a new paradigm in estimating the outcome of spine pathologies or surgeries. The traditional paradigm is unable to estimate pain and is also unable to determine how the treatment, combined with the natural pain avoidance of the patient, would transfer the load to other structures and potentially increase the risk for other problems.     Design and Development of an Unconstrained Spine Test Rig to Study the Kinematics of Spine Motion

    The spine is one of the most complex structures in the musculoskeletal system. Surgical procedures and implants used to treat spinal disorders include modification or removal of the diseased intervertebral disk, vertebral fusion using various combinations of hardware devices, and total disc replacement using mobile implant devices. The safety and efficacy of these implants need to be evaluated prior to clinical use. Three-dimensional biomechanical testing of the spine is necessary to evaluate spine function along with the effects of disorders, surgical procedures and implants. The general flexibility tests using pure moments can be performed using commercially available testing frames, but they are costly and not available in many research labs. The setup developed in this study can be accommodated by any lab with a bi-axial testing machine. The test rig designed in this study allows for the unconstrained motion of the spine under pure moment loading conditions. Loading can be applied continuously or in a stepwise fashion through positive and negative moments. The motion data was captured using Polaris Vicra, NDI Digital. This data was then analyzed using a custom code written in MATLAB, (Mathworks, Natick, MA). A mechanical analog lumbar spine model was used for kinematic experiments and the study showed promising results for the test rig to be used as an unconstrained spine test rig

    Biomechanical analysis of the lumbar spine on facet joint force and intradiscal pressure - a finite element study

    <p>Abstract</p> <p>Background</p> <p>Finite element analysis results will show significant differences if the model used is performed under various material properties, geometries, loading modes or other conditions. This study adopted an FE model, taking into account the possible asymmetry inherently existing in the spine with respect to the sagittal plane, with a more geometrically realistic outline to analyze and compare the biomechanical behaviour of the lumbar spine with regard to the facet force and intradiscal pressure, which are associated with low back pain symptoms and other spinal disorders. Dealing carefully with the contact surfaces of the facet joints at various levels of the lumbar spine can potentially help us further ascertain physiological behaviour concerning the frictional effects of facet joints under separate loadings or the responses to the compressive loads in the discs.</p> <p>Methods</p> <p>A lumbar spine model was constructed from processes including smoothing the bony outline of each scan image, stacking the boundary lines into a smooth surface model, and subsequent further processing in order to conform with the purpose of effective finite element analysis performance. For simplicity, most spinal components were modelled as isotropic and linear materials with the exception of spinal ligaments (bilinear). The contact behaviour of the facet joints and changes of the intradiscal pressure with different postures were analyzed.</p> <p>Results</p> <p>The results revealed that asymmetric responses of the facet joint forces exist in various postures and that such effect is amplified with larger loadings. In axial rotation, the facet joint forces were relatively larger in the contralateral facet joints than in the ipsilateral ones at the same level. Although the effect of the preloads on facet joint forces was not apparent, intradiscal pressure did increase with preload, and its magnitude increased more markedly in flexion than in extension and axial rotation.</p> <p>Conclusions</p> <p>Disc pressures showed a significant increase with preload and changed more noticeably in flexion than in extension or in axial rotation. Compared with the applied preloads, the postures played a more important role, especially in axial rotation; the facet joint forces were increased in the contralateral facet joints as compared to the ipsilateral ones at the same level of the lumbar spine.</p

    Biomechanical effects of lumbar fusion surgery on adjacent segments using musculoskeletal models of the intact, degenerated and fused spine

    ABSTRACT: Adjacent segment disorders are prevalent in patients following a spinal fusion surgery. Postoperative alterations in the adjacent segment biomechanics play a role in the etiology of these conditions. While experimental approaches fail to directly quantify spinal loads, previous modeling studies have numerous shortcomings when simulating the complex structures of the spine and the pre/postoperative mechanobiology of the patient. The biomechanical effects of the L4–L5 fusion surgery on muscle forces and adjacent segment kinetics (compression, shear, and moment) were investigated using a validated musculoskeletal model. The model was driven by in vivo kinematics for both preoperative (intact or severely degenerated L4–L5) and postoperative conditions while accounting for muscle atrophies. Results indicated marked changes in the kinetics of adjacent L3–L4 and L5–S1 segments (e.g., by up to 115% and 73% in shear loads and passive moments, respectively) that depended on the preoperative L4–L5 disc condition, postoperative lumbopelvic kinematics and, to a lesser extent, postoperative changes in the L4–L5 segmental lordosis and muscle injuries. Upper adjacent segment was more affected post-fusion than the lower one. While these findings identify risk factors for adjacent segment disorders, they indicate that surgical and postoperative rehabilitation interventions should focus on the preservation/restoration of patient's normal segmental kinematics

    The relationship between sagittal curvature and extensor muscle volume in the lumbar spine.

This article may be used for non-commercial purposes in accordance with Wiley Terms and Conditions for Self-Archiving.A previous modelling study predicted that the forces applied by the extensor muscles to stabilise the lumbar spine would be greater in spines that have a larger sagittal curvature (lordosis). Because the force-generating capacity of a muscle is related to its size, it was hypothesised that the size of the extensor muscles in a subject would be related to the size of their lumbar lordosis. Magnetic resonance imaging (MRI) data were obtained, together with age, height, body mass and back pain status, from 42 female subjects. The volume of the extensor muscles (multifidus and erector spinae) caudal to the mid-lumbar level was estimated from cross-sectional area measurements in axial T1-weighted MRIs spanning the lumbar spine. Lower lumbar curvature was determined from sagittal T1-weighted images. A stepwise linear regression model was used to determine the best predictors of muscle volume. The mean lower lumbar extensor muscle volume was 281 cm(3) (SD = 49 cm(3)). The mean lower lumbar curvature was 30 ° (SD = 7 °). Five subjects reported current back pain and were excluded from the regression analysis. Nearly half the variation in muscle volume was accounted for by the variables age (standardised coefficient, B = -3.2, P = 0.03) and lower lumbar curvature (B = 0.47, P = 0.002). The results support the hypothesis that extensor muscle volume in the lower lumbar spine is related to the magnitude of the sagittal curvature; this has implications for assessing muscle size as an indicator of muscle strength

    Computationally Efficient Finite Element Models of the Lumbar Spine for the Evaluation of Spine Mechanics and Device Performance

    Finite Element models of the lumbar spine are commonly used for the study of spine mechanics and device performance, but have limited usefulness in some applications such as clinical and design phase assessments due to long analysis times. In this study a computationally efficient L4-L5 FSU model and a L1-Sacrum multi-segment model were developed and validated. The FSU is a functional spine unit consisting of two adjacent vertebral bodies, in this case L4 and L5. The multi-segment model consists of all lumbar vertebrae and the sacrum. The models are able to accurately predict spine kinematics with significantly reduced analysis times, relative to fully deformable representations. Analysis times were reduced from 3 hrs and 20 min to 2 min and 1 min for the multi-segment and FSU models, respectively. The vertebrae geometries were reconstructed from CT scans of the cadaveric specimen. Prior to model development, experimental testing was performed on the specimen using a custom multi-axis spine simulator. Collection of kinematic data in response to external loading made tuning of the model stiffness possible. The improved computational efficiency of the models makes them more useful for applications requiring multiple iterations and short analysis times such as clinical and design phase assessments of implants. The model can also be used in efforts to develop lumbar musculoskeletal models, which may require multiple runs for the optimization of muscle forces
