System for monitoring and supporting the treatment of sleep apnea using IoT and big data
[EN] Sleep apnea has become in the sleep disorder that causes greater concern in recent years due to its morbidity and mortality, higher medical care costs and poor people quality of life. Some proposals have addressed sleep apnea disease in elderly people, but they have still some technical limitations. For these reasons, this paper presents an innovative system based on fog and cloud computing technologies which in combination with IoT and big data platforms offers new opportunities to build novel and innovative services for supporting the sleep apnea and to overcome the current limitations. Particularly, the system is built on several low-power wireless networks with heterogeneous smart devices (i.e, sensors and actuators). In the fog, an edge node (Smart IoT Gateway) provides IoT connection and interoperability and pre-processing IoT data to detect events in real-time that might endanger the elderly's health and to act accordingly. In the cloud, a Generic Enabler Context Broker manages, stores and injects data into the big data analyzer for further processing and analyzing. The system's performance and subjective applicability are evaluated using over 30 GB size datasets and a questionnaire fulfilled by medicals specialist, respectively. Results show that the system data analytics improve the health professionals' decision making to monitor and guide sleep apnea treatment, as well as improving elderly people's quality of life. A Smart System for Sleep Monitoring by Integrating IoT With Big Data Analytics
[EN] Obtrusive sleep apnea (OSA) is one of the most important sleep disorders because it has a direct adverse impact on the quality of life. Intellectual deterioration, decreased psychomotor performance, behavior, and personality disorders are some of the consequences of OSA. Therefore, a real-time monitoring of this disorder is a critical need in healthcare solutions. There are several systems for OSA detection. Nevertheless, despite their promising results, these systems not guiding their treatment. For these reasons, this research presents an innovative system for both to detect and support of treatment of OSA of elderly people by monitoring multiple factors such as sleep environment, sleep status, physical activities, and physiological parameters as well as the use of open data available in smart cities. Our system architecture performs two types of processing. On the one hand, a pre-processing based on rules that enables the sending of real-time notifications to responsible for the care of elderly, in the event of an emergency situation. This pre-processing is essentially based on a fog computing approach implemented in a smart device operating at the edge of the network that additionally offers advanced interoperability services: technical, syntactic, and semantic. On the other hand, a batch data processing that enables a descriptive analysis that statistically details the behavior of the data and a predictive analysis for the development of services, such as predicting the least polluted place to perform outdoor activities. This processing uses big data tools on cloud computing. The performed experiments show a 93.3% of effectivity in the air quality index prediction to guide the OSA treatment. The system's performance has been evaluated in terms of latency. Heart Diseases Diagnosis Using Artificial Neural Networks
Information technology has virtually altered every aspect of human life in the present era. The application of informatics in the health sector is rapidly gaining prominence and the benefits of this innovative paradigm are being realized across the globe. This evolution produced large number of patients’ data that can be employed by computer technologies and machine learning techniques, and turned into useful information and knowledge. This data can be used to develop expert systems to help in diagnosing some life-threating diseases such as heart diseases, with less cost, processing time and improved diagnosis accuracy. Even though, modern medicine is generating huge amount of data every day, little has been done to use this available data to solve challenges faced in the successful diagnosis of heart diseases. Highlighting the need for more research into the usage of robust data mining techniques to help health care professionals in the diagnosis of heart diseases and other debilitating disease conditions.
Based on the foregoing, this thesis aims to develop a health informatics system for the classification of heart diseases using data mining techniques focusing on Radial Basis functions and emerging Neural Networks approach. The presented research involves three development stages; firstly, the development of a preliminary classification system for Coronary Artery Disease (CAD) using Radial Basis Function (RBF) neural networks. The research then deploys the deep learning approach to detect three different types of heart diseases i.e. Sleep Apnea, Arrhythmias and CAD by designing two novel classification systems; the first adopt a novel deep neural network method (with Rectified Linear unit activation) design as the second approach in this thesis and the other implements a novel multilayer kernel machine to mimic the behaviour of deep learning as the third approach. Additionally, this thesis uses a dataset obtained from patients, and employs normalization and feature extraction means to explore it in a unique way that facilitates its usage for training and validating different classification methods. This unique dataset is useful to researchers and practitioners working in heart disease treatment and diagnosis.
The findings from the study reveal that the proposed models have high classification performance that is comparable, or perhaps exceed in some cases, the existing automated and manual methods of heart disease diagnosis. Besides, the proposed deep-learning models provide better performance when applied on large data sets (e.g., in the case of Sleep Apnea), with reasonable performance with smaller data sets.
The proposed system for clinical diagnoses of heart diseases, contributes to the accurate detection of such disease, and could serve as an important tool in the area of clinic support system. The outcome of this study in form of implementation tool can be used by cardiologists to help them make more consistent diagnosis of heart diseases

Design and Implementation of Wireless Point-Of-Care Health Monitoring Systems: Diagnosis For Sleep Disorders and Cardiovascular Diseases
Chronic sleep disorders are present in 40 million people in the United States. More than 25 million people remain undiagnosed and untreated, which accounts for over $22 billion in unnecessary healthcare costs. In addition, another major chronic disease is the heart diseases which cause 23.8% of the deaths in the United States. Thus, there is a need for a low cost, reliable, and ubiquitous patient monitoring system. A remote point-of-care system can satisfy this need by providing real time monitoring of the patient\u27s health condition at remote places. However, the currently available POC systems have some drawbacks; the fixed number of physiological channels and lack of real time monitoring.
In this dissertation, several remote POC systems are reported to diagnose sleep disorders and cardiovascular diseases to overcome the drawbacks of the current systems. First, two types of remote POC systems were developed for sleep disorders. One was designed with ZigBee and Wi-Fi network, which provides increase/decrease the number of physiological channels flexibly by using ZigBee star network. It also supports the remote real-time monitoring by extending WPAN to WLAN with combination of two wireless communication topologies, ZigBee and Wi-Fi. The other system was designed with GSM/WCDMA network, which removes the restriction of testing places and provides remote real-time monitoring in the true sense of the word. Second, a fully wearable textile integrated real-time ECG acquisition system for football players was developed to prevent sudden cardiac death. To reduce power consumption, adaptive RF output power control was implemented based on RSSI and the power consumption was reduced up to 20%. Third, as an application of measuring physiological signals, a wireless brain machine interface by using the extracted features of EOG and EEG was implemented to control the movement of a robot. The acceleration/deceleration of the robot is controlled based on the attention level from EEG. The left/right motion of eyeballs of EOG is used to control the direction of the robot. The accuracy rate was about 95%.
These kinds of health monitoring systems can reduce the exponentially increasing healthcare costs and cater the most important healthcare needs of the society

Mobile Health Technologies, also known as mHealth technologies, have emerged, amongst healthcare providers, as the ultimate Technologies-of-Choice for the 21st century in delivering not only transformative change in healthcare delivery, but also critical health information to different communities of practice in integrated healthcare information systems. mHealth technologies nurture seamless platforms and pragmatic tools for managing pertinent health information across the continuum of different healthcare providers. mHealth technologies commonly utilize mobile medical devices, monitoring and wireless devices, and/or telemedicine in healthcare delivery and health research. Today, mHealth technologies provide opportunities to record and monitor conditions of patients with chronic diseases such as asthma, Chronic Obstructive Pulmonary Diseases (COPD) and diabetes mellitus. Statistical Models for Detecting Existence of Obstructive Sleep Apnea, Predicting Its Severity, and Forecasting Future Episodes
This dissertation presents three statistical models based on data mining and nonlinear time-series analysis techniques as an alternative method for the diagnosis and treatment of obstructive sleep apnea disease (OSA). From a diagnosis perspective, our method reduces the time and cost associated with the conventional method by first screening a non-OSA subject from the population, then individually determining the OSA�s severity by utilizing the data from a single-lead electrocardiogram (ECG) device that is worn overnight at the subject�s location. Our OSA forecasting model can be used to activate an OSA therapy device such as a continuous positive airway pressure (CPAP) machine or a hypoglossal nerve stimulator (HNS) as needed or before an OSA episode so that the latter can be averted in real time.In particular, our contributions are: 1) Detect the existence of OSA in an individual based on the pattern of biological physiology and simple clinical data with a low false negative rate and reasonable accuracy (FNR: 5.3%, Accuracy: 84.47%). People with some degree of probability of having OSA will be confirmed by the next model. 2) Determine the OSA severity by classifying the OSA episode (event) from one-lead ECG data collected overnight (accuracy: 92.26% with 10,052 equally sampled events from 24 subjects). The advantage of our model is that the variations (i.e., different body build, age, gender, activity, health conditions, and race) have very little effect on the prediction because the neighboring patterns in the reconstructed phase spaces have very little or no correlation to those variations. This benefit can be seen from our model�s performance compared to two other models that exist in the literature. 3) Forecast an incoming OSA episode in real time using the one-lead ECG data (accuracy: 92%, 88%, and 87% for 1, 5, and 10 minutes ahead). This forecasting model with any appropriate OSA episode prevention device (i.e., HNS, and just-in-time CPAP) will allow for an effective OSA treatment method for CPAP nonadherence OSA sufferers. 4) Develop a wearable device that can collect the biological data via a single-lead ECG as a home sleep test (HST) device.Industrial Engineering & Managemen
Wearable and Nearable Biosensors and Systems for Healthcare
Biosensors and systems in the form of wearables and “nearables” (i.e., everyday sensorized objects with transmitting capabilities such as smartphones) are rapidly evolving for use in healthcare. Unlike conventional approaches, these technologies can enable seamless or on-demand physiological monitoring, anytime and anywhere. Such monitoring can help transform healthcare from the current reactive, one-size-fits-all, hospital-centered approach into a future proactive, personalized, decentralized structure. Wearable and nearable biosensors and systems have been made possible through integrated innovations in sensor design, electronics, data transmission, power management, and signal processing. Although much progress has been made in this field, many open challenges for the scientific community remain, especially for those applications requiring high accuracy. This book contains the 12 papers that constituted a recent Special Issue of Sensors sharing the same title. The aim of the initiative was to provide a collection of state-of-the-art investigations on wearables and nearables, in order to stimulate technological advances and the use of the technology to benefit healthcare. The topics covered by the book offer both depth and breadth pertaining to wearable and nearable technology. They include new biosensors and data transmission techniques, studies on accelerometers, signal processing, and cardiovascular monitoring, clinical applications, and validation of commercial devices
