    Errors in Veterinary Practice: Preliminary Lessons for Building Better Veterinary Teams

    Case studies in two typical UK veterinary practices were undertaken to explore teamwork, including interprofessional working. Each study involved one week of whole team observation based on practice locations (reception, operating theatre), one week of shadowing six focus individuals (veterinary surgeons, veterinary nurses and administrators) and a final week consisting of semistructured interviews regarding teamwork. Errors emerged as a finding of the study. The definition of errors was inclusive, pertaining to inputs or omitted actions with potential adverse outcomes for patients, clients or the practice. The 40 identified instances could be grouped into clinical errors (dosing/drugs, surgical preparation, lack of follow-up), lost item errors, and most frequently, communication errors (records, procedures, missing face-to-face communication, mistakes within face-to-face communication). The qualitative nature of the study allowed the underlying cause of the errors to be explored. In addition to some individual mistakes, system faults were identified as a major cause of errors. Observed examples and interviews demonstrated several challenges to interprofessional teamworking which may cause errors, including: lack of time, part-time staff leading to frequent handovers, branch differences and individual veterinary surgeon work preferences.     SMEs, electronically-mediated working and data security: cause for concern?

    Security of data is critical to the operations of firms. Without the ability to store, process and transmit data securely, operations may be compromised, with the potential for serious consequences to trading integrity. Thus the role that electronically-mediated working plays in business today and its dependency on data security is of critical interest, especially in light of the fact that much of this communication is based on the use of open networks (i.e. the Internet). This paper discusses findings from a 'WestFocus' survey on electronically-mediated working and telework amongst a sample of SMEs located in West London and adjacent counties in South-Eastern England in order to highlight the problems that such practice raises in terms of data security. Data collection involved a telephone survey undertaken in early 2006 of 378 firms classified into four industrial sectors ('Media', 'Logistics', 'Internet Services' and 'Food Processing'). After establishing how ICTs and the Internet are being exploited as business applications for small firms, data security practice is explored on the basis of sector and size with a focus on telework. The paper goes on to highlight areas of concern in terms of data security policy and training practice.     Follow-up services for improving long-term outcomes in intensive care unit (ICU) survivors

    This is a protocol for a Cochrane Review (Intervention). The objectives are as follows: Our main objective is to assess the effectiveness of follow-up services for ICU survivors that aim to identify and address unmet health needs related to the ICU period. We aim to assess the effectiveness in relation to health-related quality of life, mortality, depression and anxiety, post-traumatic stress disorder, physical function, cognitive function, ability to return to work or education and adverse events. Our secondary objectives are, in general, to examine both the various ways that follow-up services are provided and any major influencing factors. Specifically, we aim to explore: the effectiveness of service organisation (physician versus nurse led, face to face versus remote, timing of follow-up service); possible differences in services related to country (developed versus developing country); and whether participants had delirium within the ICU setting

    Fast Covariance Estimation for High-dimensional Functional Data

    For smoothing covariance functions, we propose two fast algorithms that scale linearly with the number of observations per function. Most available methods and software cannot smooth covariance matrices of dimension J×JJ \times J with J>500J>500; the recently introduced sandwich smoother is an exception, but it is not adapted to smooth covariance matrices of large dimensions such as J10,000J \ge 10,000. Covariance matrices of order J=10,000J=10,000, and even J=100,000J=100,000, are becoming increasingly common, e.g., in 2- and 3-dimensional medical imaging and high-density wearable sensor data. We introduce two new algorithms that can handle very large covariance matrices: 1) FACE: a fast implementation of the sandwich smoother and 2) SVDS: a two-step procedure that first applies singular value decomposition to the data matrix and then smoothes the eigenvectors. Compared to existing techniques, these new algorithms are at least an order of magnitude faster in high dimensions and drastically reduce memory requirements. The new algorithms provide instantaneous (few seconds) smoothing for matrices of dimension J=10,000J=10,000 and very fast (<< 10 minutes) smoothing for J=100,000J=100,000. Although SVDS is simpler than FACE, we provide ready to use, scalable R software for FACE. When incorporated into R package {\it refund}, FACE improves the speed of penalized functional regression by an order of magnitude, even for data of normal size (J<500J <500). We recommend that FACE be used in practice for the analysis of noisy and high-dimensional functional data.Comment: 35 pages, 4 figure

    The Innovation Dialogue on Being Strategic in the Face of Complexity was held in Wageningen on 31 November and 1 December 2009. The event is part of a growing dialogue in the international development sector about the complexities of social, economic and political change. It builds on two previous events hosted the Innovation Dialogue on Navigating Complexity (May 2009) and the Seminar on Institutions, Theories of Change and Capacity Development (December 2008). Over 120 people attended the event coming from a range of Dutch and international development organizations. The event was aimed at bridging practitioner, policy and academic interests. It brought together people working on sustainable business strategies, social entrepreneurship and international development. Leading thinkers and practitioners offered their insights on what it means to "be strategic in complex times". The Dialogue was organized and hosted by the Wageningen UR Centre for Development Innovation working with the Chair Groups of Communication & Innovation Studies, Disaster Studies, Education & Competence Studies and Public Administration & Policy as co; organisers. The theme of the Dialogue aligns closely with Wageningen UR’s interest in linking technological and institutional innovation in ways that enable ‘science for impact’
