    Modellierung, Entwicklung und Nutzung eines Data Warehouse für medizinische Communication Centers

    Das heutige Gesundheitssystem wird von den vielen Akteuren, den komplexen Beziehungen, den anspruchsvollen Patienten sowie dem veränderten Gesundheitsbewusstsein geprägt. Medizinische Communication Centers können als zentraler Kontaktpunkt zwischen Gesundheitssystem und Bevölkerung dienen. Durch die verschiedenen angebotenen medizinischen Dienstleistungen und die permanente Erreichbarkeit werden medizinische Communication Centers zu wichtigen Institutionen für eine bevölkerungsorientierte Versorgung. Für die ganzheitliche Bertreuung der Patienten/Versicherten müssen die Daten der Patienten/Versicherten an einer zentralen Stelle gespeichert werden. Data Warehouse Systeme ermöglichen die integrierte Speicherung der Daten und deren Auswertung. Bei einem Kontakt mit dem medizinischen Communication Center sind die Patienten-/Versichertendaten aus den früheren Kontakten bekannt und die Patienten/Versicherten können passend angesprochen und beraten werden. Die vorliegende Arbeit beschreibt den Einsatz von Data Warehouse Systemen für medizinische Communication Centers. Dabei wird der gesamte Prozess der Data Warehouse Entwicklung – Erhebung der Anforderungen an das Data Warehouse, Modellierung und Implementierung des Data Warehouse, Applikationen zur Auswertung der Data Warehouse Daten – betrachtet. Das entwickelte Data Warehouse unterstützt die Kommunikation mit den Patienten/Versicherten sowie die Qualität und Effizienz der angebotenen Dienstleistungen und Prozessen im medizinischen Communication Center. Mittels des entwickelten Data Warehouse Modells und der Auswertungsapplikationen können Daten bezüglich Beschwerden und bezogener Dienstleistungen, schweizweit und zeitbezogen, einfach und in Abhängigkeit von unterschiedlichen Analysekriterien visualisiert werden. Weiterhin können verschiedene Mitarbeiter bezogene Kennzahlen berechnet und das Reporting für die Vertragspartner zur Verfügung gestellt werden. Data Warehouse Systeme sind im Gesundheitsbereich, im Vergleich zu anderen Bereichen, weniger stark verbreitet. Das in der Arbeit beschriebene Data Warehouse zeigt das Potential und die Vorteile des Einsatzes solcher Systeme in medizinische Communication Centers und somit auch im Gesundheitswesen

    Disease progression in patients with the restrictive and mixed phenotype of Chronic Lung Allograft dysfunction—A retrospective analysis in five European centers to assess the feasibility of a therapeutic trial

Besides Bronchiolitis Obliterans Syndrome, two other phenotypes of CLAD, restrictive allograft syndrome (RAS) and mixed phenotype, have been described. Trials to test in these conditions are desperately needed and analyzing natural outcome to plan such trials is essential. Methods: We performed a retrospective analysis of functional outcome in bilateral LTx recipients with RAS and mixed phenotype, transplanted between 2009 and 2018 in five large European centers with follow- up spirometry up to 12 months after diagnosis. Based on these data, sample size and power calculations for randomized therapeutic trial was estimated using two imputation methods for missing values. Results: Seventy patients were included (39 RAS and 31 mixed phenotype), median 3.1 years after LTx when CLAD was diagnosed. Eight, 13 and 25 patients died within 6, 9 and 12 months after diagnosis and a two patients underwent re-transplantation within 12 months leading to a graft survival of 89, 79 and 61% six, nine and 12 months after diagnosis, respectively. Observed FEV1 decline was 451 ml at 6 months and stabilized at 9 and 12 months, while FVC showed continuous decline. Using two methods of imputation, a progressive further decline after 6 months for FEV1 was noted. Conclusion: The poor outcome of these two specific CLAD phenotypes suggests the urgent need for future therapeutic randomized trials. The number of missing values in a potential trial seems to be high and most frequently attributed to death. Survival may be used as an endpoint in clinical trials in these distinct phenotypes and imputation techniques are relevant if graft function is used as a surrogate of disease progression in future trials

    Das Projekt Promise2007 befasste sich mit der Erstellung und Auswertung einer Statistik zur Mitgliedersituation im Berufsverband Medizinischer Informatiker e.V.. Mit dem Ziel mehr über die Mitglieder und ihre derzeitige Situation zu erfahren wurde das Projekt an der Fachhochschule Hannover initiiert. Statistisch erfasst wurden Fragen zum Beschäftigungsverhältnis, zu Aus- und Weiterbildung, der beruflichen Situation und persönliche Angaben. Die Ergebnisse wurden ausgewertet und daraus wichtige Erkenntnisse für den BVMI e.V. abgeleitet, welche auf die weitere Verbandsarbeit Einfluss nehmen

    In Bibliotheken und Fachinformationssystemen werden unterschiedliche Klassifikationen und Thesauri verwendet. Dadurch ist die fach- und datenbankübergreifende Suche sehr erschwert. Ziel des Arbeitspakets 12 (AP 12) innerhalb der Sonderfördermaßnahme CARMEN war eine integrierte Suche in verteilten Datenbeständen zu ermöglichen. Dabei sollte den begrifflichen Unterschieden der verwendeten Thesauri und Klassifikationen Rechnung getragen werden. Zur Erreichung des Gesamtziels war erforderlich: die Untersuchung der Methodik von Crosskonkordanzen zwischen Klassifikationen bzw. Thesauri, die Programmierung eines Verfahrens, wie solche Crosskonkordanzen zwischen unterschiedlichen im Internet vorhandenen Klassifikationen bzw. Thesauri abgebildet werden können und die prototypische Erarbeitung solcher Crosskonkordanzen für bestimmte Fachgebiete und ausgewählte Klassifikationen und Thesauri. Einerseits realisiert AP 12 im Gesamtrahmen von CARMEN eines der Transfermodule von AP 11, das Methoden zur Behandlung der verbleibenden Heterogenität der verteilten Datenbestände bereithält. Andererseits stehen die in AP 12 entwickelten Crosskonkordanzen und Module auch anderen Retrievalsystemen zur Verfügung. Einsetzen lassen sich die Ergebnisse bei der verteilten Suche in verschiedenen Bibliothekskatalogen, beim Übergang zwischen Suchen in Fachdatenbanken und allgemeinen Bibliothekskatalogen und bei kombinierten Suchen in fachlich unterschiedlichen Fachdatenbanken oder virtuellen Bibliotheken

    Media masters and grassroot art 2.0 on Youtube

    Communication in the Web 2.0 context mainly works through images. The online video platform YouTube uses this form of visual communication and makes art forms of Western societies visible through their online videos. YouTube, as cultural reservoir and visual archive of moving images, accommodates the whole range of visualising creative processes – from artistic finger exercises to fine arts. A general characteristic of YouTube is the publishing of small everyday gestures of the ‘big ones’ (politicians, stars), like small incidents and their clumsiness in everyday actions, e.g. Beyonce´s fall from the stage or Tom Cruise’s demonic pro-scientology interview. Through their viral distribution on different platforms, these incidents will never be covered up or disappear from the public view. At the same time big gestures and star images are replicated and sometimes reinterpreted by the ‘small people’ who present themselves in the poses and attitudes of the stars. Generally, a coexistence of different perspectives is possible. YouTube allows polysemic and polyvalent views on the everyday and media phenomena. This article relies on YouTube research 2 that started in 2006 at the New Media Department of the Goethe University of Frankfurt. The results of the research have already presented representative forms and basic patterns, that is to say, categories for the clips appearing here. These kinds of clips, recurring in the observation period, have an impact on the basic representation of art or artistic expression within moving images on this platform. Methodologically the focus leads to the investigation (which has to be adequate to the specifics of the medium, or ‘media adequate’) of new visual structures and forms which can create – consciously or unconsciously – an art form. After focusing on the media structures, it will be discussed whether any and, if so, which ‘authentic’ new forms were developed solely on YouTube and whether these forms are innovative and can be characterised as avant-garde. This article first takes a small step in evaluating how to get from a general communication through means of visuality in web 2.0, an often endless chatty cheesy visual noise 3 – to the special quality of a consciously created aesthetic. From where do innovative aesthetic forms emerge, related to their media structures? 4 Are they the products of ‘media amateurs’ 5 or do we have to find new specifications and descriptions for the producers? The definition of a ‘media amateur’ describes technically interested private individuals who acquire and develop technology before commercial use of the technology is even recognisable. Just as artists are developing their own techniques, according to Dieter Daniels, media amateurs are autodidacts who invent techniques, rather than just acquire knowledge about them (see for example the demo scene, the machinima, brickfilm producers as well as many areas of computer gaming in general 6). The media amateur directly intervenes in the production processes of the medium and does not just simply use the medium. What is fascinating is the media amateur’s process of self education – not the result – and the direct impact on the internal structure and the control of the medium. 7 Media amateurs open a previously culturally unformed space of experience. This only partially applies to most of the YouTube clips in the realms of the visual arts; it is here most important to look at the visual content. This article discusses all these concepts and introduces new descriptions for the different forms of production: the technically oriented media master, the do-it-yourselfer, the tinkerer, the amateur handicraftsman and the inventor. It outlines a basic research project on ‘visual media culture’ (a triangulation of research on media structure and iconography) of the presented online video platform. It is a product of the analysis of clips focusing on the media structure, analyzing the creative handling of images and the deviations and differences of pre-set media formats and stereotypes

    10. Interuniversitäres Doktorandenseminar Wirtschaftsinformatik Juli 2009

    Begonnen im Jahr 2000, ist das Interuniversitäre Wirtschaftsinformatik-Doktorandenseminar mittlerweile zu einer schönen Tradition geworden. Zunächst unter Beteiligung der Universitäten Leipzig und Halle-Wittenberg gestartet. Seit 2003 wird das Seminar zusammen mit der Jenaer Universität durchgeführt, in diesem Jahr sind erstmals auch die Technische Universität Dresden und die TU Bergakademie Freiberg dabei. Ziel der Interuniversitären Doktorandenseminare ist der über die eigenen Institutsgrenzen hinausgehende Gedankenaustausch zu aktuellen, in Promotionsprojekten behandelten Forschungsthemen. Indem der Schwerpunkt der Vorträge auch auf das Forschungsdesign gelegt wird, bietet sich allen Doktoranden die Möglichkeit, bereits in einer frühen Phase ihrer Arbeit wichtige Hinweise und Anregungen aus einem breiten Hörerspektrum zu bekommen. In den vorliegenden Research Papers sind elf Beiträge zum diesjährigen Doktorandenseminar in Jena enthalten. Sie stecken ein weites Feld ab - vom Data Mining und Wissensmanagement über die Unterstützung von Prozessen in Unternehmen bis hin zur RFID-Technologie. Die Wirtschaftsinformatik als typische Bindestrich-Informatik hat den Ruf einer thematischen Breite. Die Dissertationsprojekte aus fünf Universitäten belegen dies eindrucksvoll.
