    Network selection mechanism for telecardiology application in high speed environment

    The existing network selection schemes biased either to cost or Quality of Service (QoS) are not efficient enough for telecardiology application in high traveling speed environment. Selection of the candidate network that is fulfilling the telecardiology service requirements as well as user preference is a challenging issue. This is because the preference of telecardiology user might change based on the patient health condition. This research proposed a novel Telecardiology-based Handover Decision Making (THODM) mechanism that consists of three closely integrated algorithms: Adaptive Service Adjustment (ASA), Dwelling Time Prediction (DTP) and Patient Health Condition-based Network Evaluation (PHCNE). The ASA algorithm guarantees the quality of telecardiology service when none of the available networks fulfils the service requirements. The DTP algorithm minimizes the probability of handover failure and unnecessary handover to Wireless Local Area Network (WLAN), while optimizing the connection time with WLAN in high traveling speed environment. The PHCNE algorithm evaluates the quality of available networks and selects the best network based on the telecardiology services requirement and the patient health condition. Simulation results show that the proposed THODM mechanism reduced the number of handover failures and unnecessary handovers up to 80.0% and 97.7%, respectively, compared with existing works. The cost of THODM mechanism is 20% and 85.3% lower than the Speed Threshold-based Handover (STHO) and Bandwidth-based Handover (BWHO) schemes, respectively. In terms of throughput, the proposed scheme is up to 75% higher than the STHO scheme and 370% greater than the BWHO scheme.     A feasibility study for the provision of electronic healthcare tools and services in areas of Greece, Cyprus and Italy

    Background: Through this paper, we present the initial steps for the creation of an integrated platform for the provision of a series of eHealth tools and services to both citizens and travelers in isolated areas of thesoutheast Mediterranean, and on board ships travelling across it. The platform was created through an INTERREG IIIB ARCHIMED project called INTERMED. Methods: The support of primary healthcare, home care and the continuous education of physicians are the three major issues that the proposed platform is trying to facilitate. The proposed system is based on state-of-the-art telemedicine systems and is able to provide the following healthcare services: i) Telecollaboration and teleconsultation services between remotely located healthcare providers, ii) telemedicine services in emergencies, iii) home telecare services for "at risk" citizens such as the elderly and patients with chronic diseases, and iv) eLearning services for the continuous training through seminars of both healthcare personnel (physicians, nurses etc) and persons supporting "at risk" citizens. These systems support data transmission over simple phone lines, internet connections, integrated services digital network/digital subscriber lines, satellite links, mobile networks (GPRS/3G), and wireless local area networks. The data corresponds, among others, to voice, vital biosignals, still medical images, video, and data used by eLearning applications. The proposed platform comprises several systems, each supporting different services. These were integrated using a common data storage and exchange scheme in order to achieve system interoperability in terms of software, language and national characteristics. Results: The platform has been installed and evaluated in different rural and urban sites in Greece, Cyprus and Italy. The evaluation was mainly related to technical issues and user satisfaction. The selected sites are, among others, rural health centers, ambulances, homes of "at-risk" citizens, and a ferry. Conclusions: The results proved the functionality and utilization of the platform in various rural places in Greece, Cyprus and Italy. However, further actions are needed to enable the local healthcare systems and the different population groups to be familiarized with, and use in their everyday lives, mature technological solutions for the provision of healthcare services

    All that Glitters is not Gold : Six Steps before Selecting and Prioritizing e-Health Services

    Since the market for e-health applications is constantly growing, it is getting an ever more complex endeavor to select and prioritize the right service offering given a particular situation. In examining the extant literature, it was revealed that little emphasis is actually placed on how to analyze contextual or environmental factors prior to the selection and prioritization of e-health services. With this paper, we therefore propose a formative framework consisting of six fundamental yet very pragmatic steps that may support decision makers in identifying the most important contextual pre-requisites that e-health services need to fulfill in order to be considered as effective for their environment to be implemented

    Mobile Health in Remote Patient Monitoring for Chronic Diseases: Principles, Trends, and Challenges

    Chronic diseases are becoming more widespread. Treatment and monitoring of these diseases require going to hospitals frequently, which increases the burdens of hospitals and patients. Presently, advancements in wearable sensors and communication protocol contribute to enriching the healthcare system in a way that will reshape healthcare services shortly. Remote patient monitoring (RPM) is the foremost of these advancements. RPM systems are based on the collection of patient vital signs extracted using invasive and noninvasive techniques, then sending them in real-time to physicians. These data may help physicians in taking the right decision at the right time. The main objective of this paper is to outline research directions on remote patient monitoring, explain the role of AI in building RPM systems, make an overview of the state of the art of RPM, its advantages, its challenges, and its probable future directions. For studying the literature, five databases have been chosen (i.e., science direct, IEEE-Explore, Springer, PubMed, and science.gov). We followed the (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) PRISMA, which is a standard methodology for systematic reviews and meta-analyses. A total of 56 articles are reviewed based on the combination of a set of selected search terms including RPM, data mining, clinical decision support system, electronic health record, cloud computing, internet of things, and wireless body area network. The result of this study approved the effectiveness of RPM in improving healthcare delivery, increase diagnosis speed, and reduce costs.     Rethinking the improvisation of digital health technology: A niche construction perspective

    Paid open accessThe COVID-19 pandemic has sent shock waves through healthcare organisations and catalysed an impromptu digital shift, creating a demand for telemedicine and other digital health technologies. Under such conditions, improvisation, adaptation, and innovation emerge as core dimensions to an organisation’s capacity to generate a response to crisis. This paper integrates a process perspective on the radical improvisation of a digital health technology and investigates how the radical improvisation of a digital health technology emerges and develops during a health crisis. Through a combination of supporting case evidence and literature, a multi-phase conceptual process model anchored in the crisis management cycle and illustrating the radical improvisation of digital health technology is developed and proposed. We conclude with discussion on the long-term implications of radical improvisation and crisis learning, with possible theoretical explanation using niche construction theory, and providing suggestions for future information systems and crisis management research.publishedVersio

    A framework for cloud-based healthcare services to monitor noncommunicable diseases patient

    Monitoring patients who have noncommunicable diseases is a big challenge. These illnesses require a continuous monitoring that leads to high cost for patients\u27 healthcare. Several solutions proposed reducing the impact of these diseases in terms of economic with respect to quality of services. One of the best solutions is mobile healthcare, where patients do not need to be hospitalized under supervision of caregivers. This paper presents a new hybrid framework based on mobile multimedia cloud that is scalable and efficient and provides cost-effective monitoring solution for noncommunicable disease patient. In order to validate the effectiveness of the framework, we also propose a novel evaluation model based on Analytical Hierarchy Process (AHP), which incorporates some criteria from multiple decision makers in the context of healthcare monitoring applications. Using the proposed evaluation model, we analyzed three possible frameworks (proposed hybrid framework, mobile, and multimedia frameworks) in terms of their applicability in the real healthcare environment

    A Systematic Mapping Review of Software Quality Measurement: Research Trends, Model, and Method

    Software quality is a key for the success in the business of information and technology. Hence, before be marketed, it needs the software quality measurement to fulfill the user requirements.  Some methods of the software quality analysis have been tested in a different perspective, and we have presented the software method in the point of view of users and experts. This study aims to map the method of software quality measurement in any models of quality. Using the method of Systematic Mapping Study, we did a searching and filtering of papers using the inclusion and exclusion criteria. 42 relevant papers have been obtained then. The result of the mapping showed that though the model of ISO SQuaRE has been widely used since the last five years and experienced the dynamics, the researchers in Indonesia still used ISO9126 until the end of 2016.The most commonly used method of the software quality measurement Method is the empirical method, and some researchers have done an AHP and Fuzzy approach in measuring the software quality
