Redefining Disproportionate Arrest Rates: An Exploratory Quasi-Experiment that Reassesses the Role of Skin Tone
The New York Times reported that Black Lives Matter was the third most-read subject of 2020. These articles brought to the forefront the question of disparity in arrest rates for darker-skinned people. Questioning arrest disparity is understandable because virtually everything known about disproportionate arrest rates has been a guess, and virtually all prior research on disproportionate arrest rates is questionable because of improper benchmarking (the denominator effect). Current research has highlighted the need to switch from demographic data to skin tone data and start over on disproportionate arrest rate research; therefore, this study explored the relationship between skin tone and disproportionate arrest rates. This study also sought to determine which of the three theories surrounding disproportionate arrests is most predictive of disproportionate rates. The current theories are that disproportionate arrests increase as skin tone gets darker (stereotype threat theory), disproportionate rates are different for Black and Brown people (self-categorization theory), or disproportionate rates apply equally across all darker skin colors (social dominance theory). This study used a quantitative exploratory quasi-experimental design using linear spline regression to analyze arrest rates in Alachua County, Florida, before and after the county’s mandate to reduce arrests as much as possible during the COVID-19 pandemic to protect the prison population. The study was exploratory as no previous study has used skin tone analysis to examine arrest disparity. The findings of this study redefines the understanding of the existence and nature of disparities in arrest rates and offer a solid foundation for additional studies about the relationship between disproportionate arrest rates and skin color

Immune contexture monitoring in solid tumors focusing on Head and Neck Cancer
Forti evidenze dimostrano una stretta interazione tra il sistema immunitario e lo sviluppo biologico e la progressione clinica dei tumori solidi. L'effetto che il microambiente immunitario del tumore può avere sul comportamento clinico della malattia è indicato come "immunecontexture". Nonostante ciò, l'attuale gestione clinica dei pazienti affetti da cancro non tiene conto di alcuna caratteristica immunologica né per la stadiazione né per le scelte terapeutiche. Il tumore della testa e del collo (HNSCC) rappresenta il 7° tumore più comune al mondo ed è caratterizzato da una prognosi relativamente sfavorevole e dall'effetto negativo dei trattamenti sulla qualità della vita dei pazienti. Oltre alla chirurgia e alla radioterapia, sono disponibili pochi trattamenti sistemici, rappresentati principalmente dalla chemioterapia a base di platino-derivati o dal cetuximab. L'immunoterapia è una nuova strategia terapeutica ancora limitata al setting palliativo (malattia ricorrente non resecabile o metastatica). La ricerca di nuovi biomarcatori o possibili nuovi meccanismi target è molto rilevante quindi nel contesto clinico dell'HNSCC. In questa tesi ci si concentrerà sullo studio di tre possibili popolazioni immunitarie pro-tumorali studiate nell'HNSCC: i neutrofili tumore-associati (TAN), le cellule B intratumorali con fenotipo immunosoppressivo e i T-reg CD8+. Particolare attenzione è data all'applicazione di moderne tecniche biostatistiche e bioinformatiche per riassumere informazioni complesse derivate da variabili cliniche e immunologiche multiparametriche e per validare risultati derivati ​​in situ, attraverso dati di espressione genica derivati da dataset pubblici. Infine, la seconda parte della tesi prenderà in considerazione progetti di ricerca clinica rilevanti, volti a migliorare l'oncologia di precisione nell'HNSCC, sviluppando modelli predittivi di sopravvivenza, confrontando procedure oncologiche alternative, validando nuovi classificatori o testando l'uso di nuovi protocolli clinici come l'uso dell'immunonutrizione.Strong evidences demonstrate a close interplay between the immune system and the biological development and clinical progression of solid tumors. The effect that the tumor immune microenvironment can have on the clinical behavior of the disease is referred as the immuno contexture. Nevertheless, the current clinical management of patients affected by cancer does not take into account any immunological features either for the staging or for the treatment choices. Head and Neck Cancer (HNSCC) represents the 7th most common cancer worldwide and it is characterized by a relatively poor prognosis and detrimental effect of treatments on the quality of life of patients. Beyond surgery and radiotherapy, few systemic treatments are available, mainly represented by platinum-based chemotherapy or cetuximab. Immunotherapy is a new therapeutical strategy still limited to the palliative setting (recurrent not resectable or metastatic disease). The search for new biomarkers or possible new targetable mechanisms is meaningful especially in the clinical setting of HNSCC. In this thesis a focus will be given on the study of three possible pro-tumoral immune populations studied in HNSCC: the tumor associated neutrophils (TAN), intratumoral B-cells with a immunosuppressive phenotype and the CD8+ T-regs. Biostatistical and bioinformatical techniques are applied to summarize complex information derived from multiparametric clinical and immunological variables and to validate in-situ derived findings through gene expression data of public available datasets. Lastly, the second part of the thesis will take into account relevant clinical research projects, aimed at improving the precision oncology in HNSCC developing survival prediction models, comparing alternative oncological procedures, validating new classifiers or testing the use of novel clinical protocols as the use of immunnutrition
Talking about personal recovery in bipolar disorder: Integrating health research, natural language processing, and corpus linguistics to analyse peer online support forum posts
Background: Personal recovery, ‘living a satisfying, hopeful and contributing lifeeven with the limitations caused by the illness’ (Anthony, 1993) is of particular value in bipolar disorder where symptoms often persist despite treatment. So far, personal recovery has only been studied in researcher-constructed environments (interviews, focus groups). Support forum posts can serve as a complementary naturalistic data source. Objective: The overarching aim of this thesis was to study personal recovery experiences that people living with bipolar disorder have shared in online support forums through integrating health research, NLP, and corpus linguistics in a mixed methods approach within a pragmatic research paradigm, while considering ethical issues and involving people with lived experience. Methods: This mixed-methods study analysed: 1) previous qualitative evidence on personal recovery in bipolar disorder from interviews and focus groups 2) who self-reports a bipolar disorder diagnosis on the online discussion platform Reddit 3) the relationship of mood and posting in mental health-specific Reddit forums (subreddits) 4) discussions of personal recovery in bipolar disorder subreddits. Results: A systematic review of qualitative evidence resulted in the first framework for personal recovery in bipolar disorder, POETIC (Purpose & meaning, Optimism & hope, Empowerment, Tensions, Identity, Connectedness). Mainly young or middle-aged US-based adults self-report a bipolar disorder diagnosis on Reddit. Of these, those experiencing more intense emotions appear to be more likely to post in mental health support subreddits. Their personal recovery-related discussions in bipolar disorder subreddits primarily focussed on three domains: Purpose & meaning (particularly reproductive decisions, work), Connectedness (romantic relationships, social support), Empowerment (self-management, personal responsibility). Support forum data highlighted personal recovery issues that exclusively or more frequently came up online compared to previous evidence from interviews and focus groups. Conclusion: This project is the first to analyse non-reactive data on personal recovery in bipolar disorder. Indicating the key areas that people focus on in personal recovery when posting freely and the language they use provides a helpful starting point for formal and informal carers to understand the concerns of people diagnosed with bipolar disorder and to consider how best to offer support

The Omics basis of human health: investigating plasma proteins and their genetic effects on complex traits
Over the past decade, the advancements in technology and the growing amount of identified genetic variants have led to a high number of important discoveries in the field of precision medicine concerning human biology and pathophysiology. However, it became evident that genomics alone could not properly explain the onset and regulation of the specific molecular mechanisms of certain phenotypes. Studying omics helped complement this gap in genetic research, providing detailed information on the quantification of molecules that are involved in structural and functional processes in the organism. Specifically, protein production, levels, and regulation are dynamic and change during the course of one’s lifetime. This information has proven fundamental to understanding how certain proteins affect complex phenotypes such as neurological and psychiatric disorders.
In this thesis, I describe the three groups of analyses I conducted over the course of my doctoral programme on different sets of blood plasma proteins and over a broad range of neurological, psychiatric, cardiovascular, and electrophysiology phenotypes. The underlying mechanisms that trigger the onset of psychiatric and neurological conditions are often not limited to the nervous system, but rather stem from multi-system molecular triggers. The first part of the work I carried out aims at investigating the frequent co-occurrence and comorbidity of neurological and cardiovascular phenotypes by conducting a genome-wide association (GWA) meta-analysis of 183 neurology-related blood proteins on data from over 12000 individuals. The second part concerns the bivariate and multivariate analyses conducted on 276 cardiology and inflammatory proteins, while the third illustrates the contribution to consortia focussed on heart rate and electrophysiology. Results from the second and third parts of the work provided information that played an important role in understanding a part of the genetic mechanisms of the complex traits of interest.
Overall, the results presented in this thesis strongly support the notion
that proteomics is an important tool to be used to study complex traits and drug discovery and development should focus on targeting protein synthesis and regulation. Furthermore, the results also support the notion that complex diseases involve more than one biological system, and in order to gain a better understanding of human pathology, it is fundamental to study the causes and effects across the entire organism
Measuring the impact of COVID-19 on hospital care pathways
Care pathways in hospitals around the world reported significant disruption during the recent COVID-19 pandemic but measuring the actual impact is more problematic. Process mining can be useful for hospital management to measure the conformance of real-life care to what might be considered normal operations. In this study, we aim to demonstrate that process mining can be used to investigate process changes associated with complex disruptive events. We studied perturbations to accident and emergency (A &E) and maternity pathways in a UK public hospital during the COVID-19 pandemic. Co-incidentally the hospital had implemented a Command Centre approach for patient-flow management affording an opportunity to study both the planned improvement and the disruption due to the pandemic. Our study proposes and demonstrates a method for measuring and investigating the impact of such planned and unplanned disruptions affecting hospital care pathways. We found that during the pandemic, both A &E and maternity pathways had measurable reductions in the mean length of stay and a measurable drop in the percentage of pathways conforming to normative models. There were no distinctive patterns of monthly mean values of length of stay nor conformance throughout the phases of the installation of the hospital’s new Command Centre approach. Due to a deficit in the available A &E data, the findings for A &E pathways could not be interpreted

With this new database and a new super-electron management algorithm which significantly enhances high-energy electron statistics at previously unresolved ratios, the thesis explores general facets of the electron thermal runaway process relevant to atmospheric discharges under various conditions of the temperature and gas composition as can be encountered in the wake and formation of discharge channels
