    Computational methods for the analysis of functional 4D-CT chest images.

    Medical imaging is an important emerging technology that has been intensively used in the last few decades for disease diagnosis and monitoring as well as for the assessment of treatment effectiveness. Medical images provide a very large amount of valuable information that is too huge to be exploited by radiologists and physicians. Therefore, the design of computer-aided diagnostic (CAD) system, which can be used as an assistive tool for the medical community, is of a great importance. This dissertation deals with the development of a complete CAD system for lung cancer patients, which remains the leading cause of cancer-related death in the USA. In 2014, there were approximately 224,210 new cases of lung cancer and 159,260 related deaths. The process begins with the detection of lung cancer which is detected through the diagnosis of lung nodules (a manifestation of lung cancer). These nodules are approximately spherical regions of primarily high density tissue that are visible in computed tomography (CT) images of the lung. The treatment of these lung cancer nodules is complex, nearly 70% of lung cancer patients require radiation therapy as part of their treatment. Radiation-induced lung injury is a limiting toxicity that may decrease cure rates and increase morbidity and mortality treatment. By finding ways to accurately detect, at early stage, and hence prevent lung injury, it will have significant positive consequences for lung cancer patients. The ultimate goal of this dissertation is to develop a clinically usable CAD system that can improve the sensitivity and specificity of early detection of radiation-induced lung injury based on the hypotheses that radiated lung tissues may get affected and suffer decrease of their functionality as a side effect of radiation therapy treatment. These hypotheses have been validated by demonstrating that automatic segmentation of the lung regions and registration of consecutive respiratory phases to estimate their elasticity, ventilation, and texture features to provide discriminatory descriptors that can be used for early detection of radiation-induced lung injury. The proposed methodologies will lead to novel indexes for distinguishing normal/healthy and injured lung tissues in clinical decision-making. To achieve this goal, a CAD system for accurate detection of radiation-induced lung injury that requires three basic components has been developed. These components are the lung fields segmentation, lung registration, and features extraction and tissue classification. This dissertation starts with an exploration of the available medical imaging modalities to present the importance of medical imaging in today’s clinical applications. Secondly, the methodologies, challenges, and limitations of recent CAD systems for lung cancer detection are covered. This is followed by introducing an accurate segmentation methodology of the lung parenchyma with the focus of pathological lungs to extract the volume of interest (VOI) to be analyzed for potential existence of lung injuries stemmed from the radiation therapy. After the segmentation of the VOI, a lung registration framework is introduced to perform a crucial and important step that ensures the co-alignment of the intra-patient scans. This step eliminates the effects of orientation differences, motion, breathing, heart beats, and differences in scanning parameters to be able to accurately extract the functionality features for the lung fields. The developed registration framework also helps in the evaluation and gated control of the radiotherapy through the motion estimation analysis before and after the therapy dose. Finally, the radiation-induced lung injury is introduced, which combines the previous two medical image processing and analysis steps with the features estimation and classification step. This framework estimates and combines both texture and functional features. The texture features are modeled using the novel 7th-order Markov Gibbs random field (MGRF) model that has the ability to accurately models the texture of healthy and injured lung tissues through simultaneously accounting for both vertical and horizontal relative dependencies between voxel-wise signals. While the functionality features calculations are based on the calculated deformation fields, obtained from the 4D-CT lung registration, that maps lung voxels between successive CT scans in the respiratory cycle. These functionality features describe the ventilation, the air flow rate, of the lung tissues using the Jacobian of the deformation field and the tissues’ elasticity using the strain components calculated from the gradient of the deformation field. Finally, these features are combined in the classification model to detect the injured parts of the lung at an early stage and enables an earlier intervention

Like most traditional systems, the new FA system requires only a single user-supplied cue point. On the other hand, the SA system represents a new algorithm class requiring 8 user-supplied control points. This does increase the burden on the user, but we show that the resulting system is highly robust and can handle a variety of challenging cases. The proposed hybrid system starts with the FA system. If improved segmentation results are needed, the SA system is then deployed. The FA segmentation engine has 2 free parameters, and the SA system has 3. These parameters are adaptively determined for each nodule in a search process guided by a regression neural network (RNN). The RNN uses a number of features computed for each candidate segmentation. We train and test our systems using the new Lung Image Database Consortium and Image Database Resource Initiative (LIDC–IDRI) data. To the best of our knowledge, this is one of the first nodule-specific performance benchmarks using the new LIDC–IDRI dataset. We also compare the performance of the proposed methods with several previously reported results on the same data used by those other methods. Our results suggest that the proposed FA system improves upon the state-of-the-art, and the SA system offers a considerable boost over the FA system

    Segmentation and classification of lung nodules from Thoracic CT scans : methods based on dictionary learning and deep convolutional neural networks.

    Lung cancer is a leading cause of cancer death in the world. Key to survival of patients is early diagnosis. Studies have demonstrated that screening high risk patients with Low-dose Computed Tomography (CT) is invaluable for reducing morbidity and mortality. Computer Aided Diagnosis (CADx) systems can assist radiologists and care providers in reading and analyzing lung CT images to segment, classify, and keep track of nodules for signs of cancer. In this thesis, we propose a CADx system for this purpose. To predict lung nodule malignancy, we propose a new deep learning framework that combines Convolutional Neural Networks (CNN) and Recurrent Neural Networks (RNN) to learn best in-plane and inter-slice visual features for diagnostic nodule classification. Since a nodule\u27s volumetric growth and shape variation over a period of time may reveal information regarding the malignancy of nodule, separately, a dictionary learning based approach is proposed to segment the nodule\u27s shape at two time points from two scans, one year apart. The output of a CNN classifier trained to learn visual appearance of malignant nodules is then combined with the derived measures of shape change and volumetric growth in assigning a probability of malignancy to the nodule. Due to the limited number of available CT scans of benign and malignant nodules in the image database from the National Lung Screening Trial (NLST), we chose to initially train a deep neural network on the larger LUNA16 Challenge database which was built for the purpose of eliminating false positives from detected nodules in thoracic CT scans. Discriminative features that were learned in this application were transferred to predict malignancy. The algorithm for segmenting nodule shapes in serial CT scans utilizes a sparse combination of training shapes (SCoTS). This algorithm captures a sparse representation of a shape in input data through a linear span of previously delineated shapes in a training repository. The model updates shape prior over level set iterations and captures variabilities in shapes by a sparse combination of the training data. The level set evolution is therefore driven by a data term as well as a term capturing valid prior shapes. During evolution, the shape prior influence is adjusted based on shape reconstruction, with the assigned weight determined from the degree of sparsity of the representation. The discriminative nature of sparse representation, affords us the opportunity to compare nodules\u27 variations in consecutive time points and to predict malignancy. Experimental validations of the proposed segmentation algorithm have been demonstrated on 542 3-D lung nodule data from the LIDC-IDRI database which includes radiologist delineated nodule boundaries. The effectiveness of the proposed deep learning and dictionary learning architectures for malignancy prediction have been demonstrated on CT data from 370 biopsied subjects collected from the NLST database. Each subject in this database had at least two serial CT scans at two separate time points one year apart. The proposed RNN CAD system achieved an ROC Area Under the Curve (AUC) of 0.87, when validated on CT data from nodules at second sequential time point and 0.83 based on dictionary learning method; however, when nodule shape change and appearance were combined, the classifier performance improved to AUC=0.89

    A Modular Approach to Lung Nodule Detection from Computed Tomography Images Using Artificial Neural Networks and Content Based Image Representation

    Lung cancer is one of the most lethal cancer types. Research in computer aided detection (CAD) and diagnosis for lung cancer aims at providing effective tools to assist physicians in cancer diagnosis and treatment to save lives. In this dissertation, we focus on developing a CAD framework for automated lung cancer nodule detection from 3D lung computed tomography (CT) images. Nodule detection is a challenging task that no machine intelligence can surpass human capability to date. In contrast, human recognition power is limited by vision capacity and may suffer from work overload and fatigue, whereas automated nodule detection systems can complement expert’s efforts to achieve better detection performance. The proposed CAD framework encompasses several desirable properties such as mimicking physicians by means of geometric multi-perspective analysis, computational efficiency, and the most importantly producing high performance in detection accuracy. As the central part of the framework, we develop a novel hierarchical modular decision engine implemented by Artificial Neural Networks. One advantage of this decision engine is that it supports the combination of spatial-level and feature-level information analysis in an efficient way. Our methodology overcomes some of the limitations of current lung nodule detection techniques by combining geometric multi-perspective analysis with global and local feature analysis. The proposed modular decision engine design is flexible to modifications in the decision modules; the engine structure can adopt the modifications without having to re-design the entire system. The engine can easily accommodate multi-learning scheme and parallel implementation so that each information type can be processed (in parallel) by the most adequate learning technique of its own. We have also developed a novel shape representation technique that is invariant under rigid-body transformation and we derived new features based on this shape representation for nodule detection. We implemented a prototype nodule detection system as a demonstration of the proposed framework. Experiments have been conducted to assess the performance of the proposed methodologies using real-world lung CT data. Several performance measures for detection accuracy are used in the assessment. The results show that the decision engine is able to classify patterns efficiently with very good classification performance

    Automatic detection of pulmonary nodules: Evaluation of performance using two different MDCT scanners

    The purpose of this study was to evaluate the diagnostic performance of a computer-aided diagnosis (CAD) system, on the detection of pulmonary nodules in multidetector row computed tomography (MDCT) images, by using two different MDCT scanners. The computerized scheme was based on the iris filter. We have collected CT cases of patients with pulmonary nodules. We have included in the study one hundred and thirty-two calcified and noncalcified nodules, measuring 4-30 mm in diameter. CT examinations were performed by using two different equipments: a CT scanner (SOMATOM Emotion 6), and a dual-source computed tomography system (SOMATOM Definition) (Siemens Medical System, Forchheim, Germany), with the following parameters: collimation, 6x1.0mm (Emotion 6); and 64×0.6mm (Definition); 100-130 kV; 70-110 mAs. Data were reconstructed with a slice thickness of 1.25mm (Emotion 6) and 1mm (Definition). True positive cases were determined by an independent interpretation of the study by three experienced chest radiologists, the panel decision being used as the reference standard. Free-response Receiver Operating Characteristic curves, sensitivity and number of false-positive per scan, were calculated. Our CAD scheme, for the test set of the study, yielded a sensitivity of 80%, with an average of 5.2 FPs per examination. At an average false positive rate of 9 per scan, our CAD scheme achieved sensitivities of 94% for all nodules, 94.5% for solid, 80% for non-solid, 84% for spiculated, and 97% for non-spiculated nodules.
