    Differentielle BehandlungsverlÀufe bei störungsspezifischer stationÀrer Psychotherapie komplexer Traumafolgestörungen

    Die Wirksamkeit traumaspezifischer Psychotherapie ist bei Patienten mit komplexen Traumafolgestörungen insbesondere in naturalistischen Settings unzureichend belegt. DarĂŒber hinaus erscheint es bedeutsam, PrĂ€diktoren fĂŒr differentielle VerlĂ€ufe zu identifizieren. In dieser retrospektiven PrĂ€-Post-Untersuchung naturalistischer TherapieverlĂ€aufe von 139 stationĂ€r behandelten PatientInnen wird die Wirksamkeit eines spezifischen Therapieprogrammes anhand der PTBS–Symptomatik (gemessen mit IES-R) sowie anhand sekundĂ€rer Outcome–Maße wie Depression, somatoforme Beschwerden, Angst (jeweils gemessen mit ISR und HEALTH–49), interaktionelle Schwierigkeiten, Selbstwirksamkeit, psychisches Wohlbefinden (jeweils gemessen mit HEALTH–49), Dissoziation (gemessen mit DES–T) oder Achtsamkeit (gemessen mit FFA) ĂŒberprĂŒft. DarĂŒber hinaus werden mittels finiter gemischter Modelle differentielle Klassen von VerlĂ€ufen der PTBS-Symptomatik identifiziert. Über Classification and Regression Trees werden PrĂ€diktoren fĂŒr die Klassenzugehörigkeit identifiziert. Die Stichprobe erwies sich als durch Kindheitstraumatisierungen, Symptome einer posttraumatischen Belastungsstörung und komorbide psychische Störungen in extremen Ausmaßen belastet. Dieser Umstand schlĂ€gt sich auch in hĂ€ufigen stationĂ€ren Vorbehandlungen, Suizidversuchen, polypharmazeutischen Medikationen und ausgeprĂ€gten sozialen Notlagen wider. Im Untersuchungszeitraum lag die Zahl abgebrochener Behandlungen bei 4.8% (sieben von 146 PatientInnen). Bei den PatientInnen, die die Behandlung regulĂ€r beendeten, war in nur einem Fall (0.7%) eine klinisch signifikante Verschlechterung der PTBS–Symptomatik zu beobachten. Es zeigt sich eine signifikante Reduktion der PTBS–Symptomatik im Therapieverlauf (Z = −8.5, p = .00). FĂŒr 41.7% der PatientInnen entspricht dieser Effekt einer klinisch signifikanten Verbesserung. Auch hinsichtlich der sekundĂ€ren Outcome–Maße Depression, somatoforme Beschwerden, Angst, interaktionelle Schwierigkeiten, Selbstwirksamkeit und Achtsamkeit konnten signifikante Verbesserungen im Ausmaß mittlerer bis hoher Effekte nachgewiesen werden. Ebenso fand sich eine signifikante Reduktion der dissoziativen Symptomatik. 38% der PatientInnen konnten ihr psychisches Wohlbefinden klinisch signifikant verbessern, 29.2% klinisch signifikant ihre interaktionellen Schwierigkeiten reduzieren und 22.8% der PatientInnen gelang es, klinisch signifikant das Ausmaß ihrer Achtsamkeit zu verbessern. 71.9% der PatientInnen schilderten bei Entlassung im VEV–K hinsichtlich ihres Erlebens und Verhaltens eine signifikante Verbesserung. Anhand der Analyse der finiten gemischten Modelle konnten drei differentielle Klassen von VerlĂ€ufen der PTBS-Symptomatik identifiziert werden (ΔBIC=23.5; Entropie .79). In allen drei Klassen konnte eine signifikante Verbesserung der PTBS–Symptomatik nachgewiesen werden (p = .00). Gleichzeitig zeigten sich deutlich unterschiedliche VerlĂ€ufe. WĂ€hrend die Klassen 2 und 3 hinsichtlich der PTBS–Symptomatik in hohem Ausmaß von der Behandlung profitierten (N=51; dZ=1.3; 33% klinisch signifikante Verbesserung bzw. N=49; dZ=1.4; 78% klinisch signifikante Verbesserung), gelang dies einer dritten Klasse von PatientInnen (N=39; dZ=0.5; 8% klinisch signifikante Verbesserung) nur eingeschrĂ€nkt. Das Klassifikationsmodell mit minimalem Kreuzvalidierungsfehler identifiziert Achtsamkeit, Dissoziation und interaktionelle Schwierigkeiten bei Aufnahme sowie das Ausmaß des sexuellen Missbrauchs in der Kindheit (gemessen mit CTQ) als signifikante PrĂ€diktoren der Klassenzugehörigkeit. Diese Ergebnisse zeigen, dass komplex traumatisierte und psychopathologisch schwer belastete PatientInnen stationĂ€r wirksam behandelt werden können. PatientInnen mit besonders hohen Belastungen durch sexuellen Missbrauch in der Kindheit, Dissoziation und interaktionelle Schwierigkeiten sowie mit geringer Achtsamkeit profitieren eingeschrĂ€nkt.     Posttraumatic stress disorder (PTSD) is a prevalent and highly disturbing mental health condition that occurs in response to extremely distressing events during the lifetime. First, military personnel represent a high-risk population for the development of posttraumatic stress symptoms (PTSS) or the full-diagnostic spectrum of the disorder due to deployment- and combat-related stressors during their military career. Despite the existence of well established and efficacious psychotherapy treatments for PTSD, access to trauma-focused psychotherapy is limited and veterans in particular experience high barriers to accessing help from the mental health care system. A substantial proportion of affected veterans receive no or inadequate treatment, increasing the risk of secondary adverse mental and somatic health outcomes, reduced social and occupational functioning, and of the condition becoming chronic. Second, internet-based interventions (IBI), particularly internet-based cognitive behavioral therapies (iCBT), have been shown to be efficacious and widely accepted for the treatment of a range of psychiatric disorders, including PTSD. IBI can already be seen as playing a potentially important role in supplementing the landscape and provision of psychotherapeutic interventions, and this is set to grow further in the future. Indeed, IBI should be particularly beneficial for patients in rural areas with a restricted psychotherapy infrastructure, for patients with restricted mobility, and for patients who desire greater anonymity and more independence regarding the time and location of psychotherapy access. Third, the systematic and reliable assessment of objective indicators of symptom expression and symptom change is of increasing interest and relevance for psychotherapy research. This dissertation thesis aims at incorporating these three pillars in four studies: First, a diagnostic identification of PTSD in veterans of the German Armed Forces (GAF) according to the main diagnostic manuals the International Classification of Diseases and Related Health Problems (ICD) and the Diagnostic and Statistical Manual of Mental Disorders (DSM); second, a meta-analytical evaluation of the efficacy of IBI in PTSD; third, an assessment of patterns of visual attentional bias in (traumatized) veterans (with PTSS); and fourth, an exploration of the modifiability of attentional bias in veterans after receiving iCBT. STUDY 1 investigated the concordance of PTSD prevalence rates when transiting between the diagnostic criteria of the DSM-IV, DSM-5, ICD-10, and proposed ICD-11 in a sample of service members of the GAF. High levels of agreement emerged between the DSM-IV and the DSM-5, and between the DSM-5 and the proposed ICD-11. Prevalence rates were significantly higher according to the proposed ICD-11 compared to the ICD-10, mainly due to the deletion of the time criterion. STUDY 1 provides support for the identification of six ‘core’ PTSD symptoms according to the proposed ICD-11, presenting a high agreement rate with the set of twenty qualifiers according to the DSM-5. STUDY 2 provided meta-analytical evidence for the efficacy of IBI for the treatment of PTSD. Twenty randomized controlled trials (RCT) encompassing 21 comparisons were included, evaluating either iCBT or internet-based expressive writing (iEW) with passive or active control conditions. The results revealed that iCBT was more efficacious than passive control conditions at post-treatment assessment (0.66 ≀ g ≀ 0.83). No superiority of either iCBT or iEW was found in contrast to active control conditions. Subgroup analyses revealed no significant moderators of iCBT efficacy. More research is needed to prove the efficacy of IBI in contrast to active control treatments and further explore the impact of moderators on treatment efficacy. STUDY 3 measured patterns of attentional bias in GAF veterans with PTSS, traumatized veterans without PTSS, and unexposed healthy veterans. In a free-viewing task, participants were presented with pairs of combat-related and neutral pictures, of more general threat-related and neutral pictures, and of emotional and neutral faces, while their eye gazes were tracked. Further, the internal consistency of attentional bias indicators was calculated. The findings provide support for the maintenance hypothesis in PTSS. There was no robust evidence to support the hypothesis of hypervigilant behavior in PTSS. Findings on attentional bias variability remain unclear. Internal consistency varied across attentional bias indicators, highlighting the need for future research in this regard. STUDY 4 investigated the modifiability of attentional bias in veterans with PTSS through the provision of iCBT. In a free-viewing task, participants were presented with combat-related, general threat-related, and neutral pictures, and with faces with negative emotional valence and neutral facial expressions while their eye gazes were tracked. Attentional bias was examined pre- and post-intervention and at a three-month follow-up. No modifications in attentional bias were observable over time. Future investigations are warranted to systematically investigate objective measures of symptom expression and symptom change together with subjective symptom reporting and symptom change in response to psychotherapeutic treatment options. In summary, this dissertation thesis provides a threefold contribution to the current landscape of psychotherapy research: First, it supports the concordance between the DSM-5 and the ICD-11 diagnostic criteria for PTSD. Second, it proves the efficacy of IBI for PTSD. In view of the growing relevance of IBI as a supplement to psychotherapeutic care, future research needs to examine its long-term efficacy, whether it shows equal or superior efficacy compared to other active (control) treatments, potential side effects, and whether it may lead to a deterioration of symptoms. Moreover, studies should focus on tailoring IBI to the specific needs of different patient populations to ensure patients’ safety and satisfaction with IBI. Third, the present thesis underlines the need for systematic and reliable assessments of objective indicators of symptom presentation and of symptom change, in addition to subjective reports. Moreover, methodological approaches need to be extended to measure diverse dimensions of symptom presentation and symptom change and gain a better understanding of their interplay. A multidimensional diagnostic approach and treatment evaluation will be of key relevance for future intervention research and evidence-based practice

    Affective language processing in a native and non-native mind: A neuropragmatic perspective

    WydziaƂ AnglistykiCelem pracy jest zbadanie elektrofizjologicznych oraz behawioralnych podstaw przetwarzania języka afektywnego w kontekƛcie, w języku ojczystym oraz obcym u PolakĂłw mieszkających w Wielkiej Brytanii. Dwa eksperymenty badają reakcje uczestnikĂłw na przymiotniki afektywne prezentowane poza kontekstem (eksp. 1) oraz w kontekƛcie zdaniowym (eksp. 2), oferując perspektywę pragmatyczną. ZaƂoĆŒono, ĆŒe czytanie przymiotnikĂłw afektywnych osadzonych poza kontekstem zdaniowym nie wywoƂa mierzalnych rĂłĆŒnic w danych behawioralnych oraz elektrofizjologicznych w języku pierwszym oraz drugim. W eksperymencie 2, natomiast, przewidziano rĂłĆŒnicę w modulacji potencjaƂów wywoƂanych walencją emocjonalną w języku pierwszym oraz drugim ze względu na zwiększenie informacji kontekstowej. Obie hipotezy zostaƂy potwierdzone. W eksp. 1, zaobserwowano porĂłwnywalne reakcje elektrofizjologiczne w języku pierwszym oraz drugim, co sugeruje, ĆŒe przetwarzanie sƂów afektywnych wydaje się być zautomatyzowane w obu językach. W eksp. 2, natomiast, reakcje elektrofizjologiczne na te same bodĆșce osadzone w kontekƛcie rĂłĆŒniƂy się pomiędzy językami, ukazując zredukowany dostęp semantyczny do zdaƄ negatywnych w języku drugim we wczesnym etapie ich przetwarzania. Wyniki niniejszego badania oferują pierwszą neurokognitywną interpretację wynikĂłw wczeƛniejszych badaƄ, oraz niosą waĆŒne implikacje w kontekƛcie terapii oraz edukacji z osobami dwujęzycznymi.The study examines electrophysiological and behavioural correlates of affective language processing in context in the native and non-native language of immersed Polish-English bilinguals using an electroencephalograph. Two experiments test bilinguals’ responses to affective adjectives embedded in a single-word (experiment 1) and sentence (experiment 2) context, introducing a “pragmatic twist” in the experimental design. It was hypothesized that reading affective adjectives in single-word context will not produce measurable differences between L1 and L2. In experiment 2, by contrast, I predicted a differential modulation of electrophysiological responses by affective valence in L1 and L2 as a factor of build-up of contextual information. Both hypotheses were confirmed. In experiment 1, comparable electrophysiological responses were elicited in L1 and L2, showing that processing of single affective words seems to be automatized in L1 and L2. In experiment 2, however, electrophysiological responses to the same set of affective words embedded in context differed as a function of language, whereby semantic access to negative sentences in L2 was reduced in the early stages of processing. This result offers a neurocognitive interpretation for findings reported in previous research and may have important implications for bilingual therapy and education

    An Explanatory Analysis of the Possible Solutions to the Shortage of Skilled Professionals in the Hospitality and Gastronomy Industry.

    The shortage of skilled professionals is one of the biggest challenges facing the hotel and gastronomy industry today. It is becoming increasingly difficult for companies in the industry to recruit suitably skilled professionals (DEHOGA, 2019). At the same time, the industry is showing a general growth. 1.4 billion international tourist arrivals were recorded in 2018, the number of international tourist arrivals in Europe in 2018 was 713 million with an increase of 6% compared to 2017(UNWTO, 2019). The topic of this dissertation is the shortage of skilled professionals in the hotel and gastronomy industry. In particular, the reasons for the shortage of skilled professionals and the potential solutions are discussed. The work aims to present a clear understanding of the hotel and gastronomy industry concerning the causes of the shortage of skilled professionals, to present a series of practical solutions to counteract the shortage of skilled professionals, to evaluate the practical solution approaches and to make recommendations to the industry. In order to achieve these objectives and to give structure to the research, the following two research questions have been proposed: 1. How can the problem of the acute shortage of skilled professionals in the hotel and gastronomy industry be evaluated and what are possible reasons? 2. What concrete solution approaches do exist to effectively address the current shortage of skilled professionals in the hotel and gastronomy industry and how can they be deepened and put into practice? Twelve semi-structured interviews have been conducted as part of the primary data research. The entire research process, as well as the data analysis, use individual elements of the Grounded Theory. The interview was conducted with industry experts from Germany, Austria, Switzerland and Italy from different departments and levels of hierarchy. This research contributes to the existing literature by proposing possible solution approaches, which can be useful for the industry itself as well as for further research
