    Architectural Design of the National Health Information System for Rwanda

    The use of information technology in healthcare services can improve the quality of care. The large amount of research has demonstrated the role of the use of Information and communication technology (ICT) solutions to overcome the challenges in patient information management. One of the challenges is the healthcare information sharing between providers. In high income countries, the challenge of exchanging information is almost solved. Nearly all high income countries have implemented a national healthcare network which connects healthcare providers in the whole country. Furthermore, European Union (EU) aims at the point of cross-border healthcare information exchange which supports the mobility of EU citizens. However, in developing countries, they are not yet ready to take the full advantage of ICT in their healthcare systems. The main objective of the thesis was to design the architecture of a national health information system for Rwanda, which is a developing country with limited resources. The research was based on three main issues: One was to determine existing health IT solutions in the healthcare system of Rwanda. The second one was to explore how other countries have developed their national health information systems (NHISs). The third was to find out how open source solutions can build a national network for a country. From the research, the components of the architecture have been defined and finally the architecture was designed. The research started by examining the current situation of ICT solutions in the healthcare system of Rwanda. This showed the progress in implementing certain electronic medical record systems in certain health facilities. However, there is no single hospital with a fully functional system. This step was followed by exploring how other countries implemented their NHIS and it showed that the process varies country by country. It was clear that in developing countries, open source solutions got a large market share contrary to developed countries where proprietary systems are the most used. Finally, open source solutions proved the capability to build a NHIS with different examples of robust open source solutions available in health IT nowadays. Although it would have been interesting, the thesis does not estimate the financial resources needed for the implementation of the architecture. It is possible to implement the NHIS for Rwanda by using both proprietary and open source solutions. However, the interoperability issue can be mitigated by minimizing different types of electronic medical records in healthcare facilities

    Contributions to interoperability, scalability and formalization of personal health systems

    The ageing of the world's population combined with unhealthy lifestyles are contributing to a major prevalence of chronic diseases. This scenario poses the challenge of providing good healthcare services to that people affected by chronic illnesses, but without increasing its costs. A prominent way to face this challenge is through pervasive healthcare. Research in pervasive healthcare tries to shift the current centralized healthcare delivery model focused on the doctors, to a more distributed model focused on the patients. In this context Personal Health Systems (PHSs) consists on approaching sampling technologies into the hands of the patients, without disturbing its activities of the daily life, to monitor patient's physiological parameters and providing feedback on their state. The use of PHSs involves the patients in the management of their illness and in their own well being too. The development of PHSs has to face technological issues in order to be accepted by our society. Within them it is important to ensure interoperability between different systems in order to make them work together. Scalability it is also a concern, as their performance must not decrease when increasing the number of users. Another issue is how to formalize the medical knowledge for each patient, as different patients may have different target goals. Security and privacy are a must feature because of the sensitive nature of medical data. Other issues involve the the integration with legacy systems, and the usability of graphical user interfaces in order to encourage old people with the use these technologies. The aim of this PhD thesis is to contribute into the state-of-the-art of PHSs by tackling together different of the above-mentioned challenges. First, to achieve interoperability we use the CDA standard as a format to encode and exchange health data and alerts related with the status of the patient. We show how these documents can be generated automatically through the use of XML templates. Second, we address the scalability by distributing the computations needed to monitor the patients over their devices, rather than performing them in a centralized server. In this context we develop the MAGPIE agent platform, which runs on Android devices, as a framework able to provide intelligence to PHSs, and generate alerts that can be of interest for the patients and the medical doctors. Third, we focus on the formalization of PHSs by providing a tool for the practitioners where they can define, in a graphical way, monitoring rules related with chronic diseases that are integrated with the MAGPIE agent platform. The thesis also explores different ways to share the data collected with PHSs in order to improve the outcomes obtained with the use of this technology. Data is shared between individuals following a Distributed Event-Based System (DEBS) approach, where different people can subscribe to the alerts produced by the patient. Data is also shared between institutions with a network protocol called MOSAIC, and we focus on the security aspects of this protocol. The research in this PhD focuses in the use case of Diabetes Mellitus; and it has been developed in the context of the projects MONDAINE, MAGPIE, COMMODITY12 and TAMESIS.L'envelliment de la població mundial combinat amb uns estils de vida no saludables contribueixen a una major prevalença d'enfermetats cròniques. Aquest escenari presenta el repte de proporcionar uns bons serveis sanitaris a les persones afectades per aquestes enfermetats, sense incrementar-ne els costos. Una solució prometedora a aquest repte és mitjançant l'aplicació del que en anglès s'anomena "pervasive healthcare". L'investigació en aquesta camp tracta de canviar l'actual model centralitzat de serveis sanitaris enfocat en el personal sanitari, per un model de serveis distribuït enfocat en els pacients. En aquest context, els Personal Health Systems (PHSs) consisteixen en posar a l'abast dels pacients les tecnologies de monitorització, i proporcionar-los informació sobre el seu estat. L'ús de PHSs involucra els pacients en la gestió de la seva enfermetat i del seu propi benestar. L'acceptació dels PHSs per part de la societat implica certs reptes tecnològics en el seu desenvolupament. És important garantir la seva interoperabilitat per tal de que puguin treballar conjuntament. La seva escalabilitat també s'ha de tenir en compte, ja que el seu rendiment no s'ha de veure afectat al incrementar-ne el número d'usuaris. Un altre aspecte a considerar és com formalitzar el coneixement mèdic per cada pacient, ja que cada un d'ells pot tenir objectius diferents. La seguretat i privacitat són característiques desitjades degut a la naturalesa sensible de les dades mèdiques. Altres problemàtiques impliquen la integració amb sistemes heretats, i la usabilitat de les interfícies gràfiques per fomentar-ne el seu ús entre les persones grans. L'objectiu d'aquesta tesi és contribuir a l'estat de l'art dels PHSs tractant de manera conjunta varis dels reptes mencionats. Per abordar l'interoperabilitat s'utilitza l'estàndard CDA com a format per codificar les dades mèdiques i alertes relacionades amb el pacient. A més es mostra com aquests documents poden generar-se de forma automàtica mitjançant l' ús de plantilles XML. Per tractar l'escalabilitat es distribueixen les computacions per monitoritzar els pacients entre els seus terminals mòbils, en comptes de realitzar-les en un servidor central. En aquest context es desenvolupa la plataforma d'agents MAGPIE com a framework per proporcionar intelligència als PHSs i generar alertes d'interès per al metge i el pacient. La formalització s'aborda mitjançant una eina que permet als metges definir de manera gràfica regles de monitorització relacionades amb enfermetats cròniques, que a més estan integrades amb la plataforma d'agents MAGPIE. La tesi també explora diferents maneres de compartir les dades recol·lectades amb un PHS, amb l'objectiu de millorar els resultats obtinguts amb aquesta tecnologia. Les dades es comparteixen entre individus seguint un enfoc de sistemes distribuïts basats en events (DEBS), on diferents usuaris poden subscriure's a les alertes produïdes per el pacient. Les dades també es comparteixen entre institucions mitjançant un protocol de xarxa anomenat MOSAIC. A la tesi es desenvolupen els aspectes de seguretat d'aquest protocol. La test es centra en la Diabetis Mellitus com a cas d'ús, i s'ha realitzat en el context dels projectes MONDAINE, MAGPIE, COMMODITY12 i TAMESIS.El envejecimiento de la población mundial combinado con unos estilos de vida no saludables contribuyen a una mayor prevalencia de enfermedades crónicas. Hechos como la reciente pandemia COVID-19, el envejecimiento de la población, el aumento de las tasas de dependencia o la necesidad de promover la asistencia sanitaria personalizada tanto en entorno hospitalario como domiciliario, ponen de manifiesto la necesidad de desarrollar dispositivos médicos de monitorización y diagnostico cada vez más sofisticados, fiables y conectados de forma rápida y eficaz. En este escenario, los sistemas embebidos se han convertido en tecnología clave para el diseño de soluciones innovadoras de bajo coste y de forma rápida. Conscientes de la oportunidad que existe en el sector, cada vez son más las denominadas "biotech start-ups" las que se embarcan en el negocio de los dispositivos médicos. Pese a tener grandes ideas y soluciones técnicas, muchas terminan fracasando por desconocimiento del sector sanitario y de los requisitos regulatorios que se deben cumplir. La gran cantidad de requisitos técnicos y regulatorios hace que sea necesario disponer de una metodología procedimental para ejecutar dichos desarrollos.     Implementation of e-health interoperability in developing country contexts : the case of Zimbabwe

    The provision of information technology-enabled healthcare services (e-health) has been adopted by numerous public and private facilities in both developing nations and advanced nations. However, one of the obstacles to the adoption of health information systems has been cited as their lack of interoperability resulting in their reduced effectiveness. In view of this, the study sought to explore the interoperability of health information systems employed in the country and then propose a framework to direct the process of implementing e-health interoperability. The study’s methodology was qualitative and a case study was undertaken. Semi-structured interviews were employed to gather data from e-health stakeholders in state-owned institutions and private enterprises. Document review was also conducted to substantiate findings from interviews. Data was analysed using thematic analysis and NVivo 12 software. The study’s findings revealed that several health information systems were implemented and their interoperability was low. Technological, terminology, organizational as well as regulatory and legal barriers were identified as hindrances to interoperability. The enablers for implementing e-health interoperability also revealed by this study include: development of re-usable software components, train the trainer approach to transfer of skills and regional conformance testing. The consequences of lack of interoperability among health information systems reported by this study include: burden on the worker, wastage of resources and high cost. The study also proposed a dual framework to guide the implementation of e-health interoperability. The study’s recommendations include the development of an e-health policy, an e-health strategy and the upgrade of ICT and telecommunication infrastructure to facilitate health information exchange.School of ComputingD.     An access control model for a South African National Electronic Health Record System

    Countries such as South Africa have attempted to leverage eHealth by digitising patients’ medical records with the ultimate goal of improving the delivery of healthcare. This involves the use of the Electronic Health Record (EHR) which is a longitudinal electronic record of a patient’s information. The EHR is comprised of all of the encounters that have been made at different health facilities. In the national context, the EHR is also known as a national EHR which enables the sharing of patient information between points of care. Despite this, the realisation of a national EHR system puts patients' EHRs at risk. This is because patients’ information, which was once only available at local health facilities in the form of paper-based records, can be accessed anywhere within the country as a national EHR. This results in security and privacy issues since patients’ EHRs are shared with an increasing number of parties who are geographically distributed. This study proposes an access control model that will address the security and privacy issues by providing the right level of secure access to authorised clinicians. The proposed model is based on a combination of Role-Based Access Control (RBAC) and Attribute-Based Access Control (ABAC). The study found that RBAC is the most common access control model that is used within the healthcare domain where users’ job functions are based on roles. While RBAC is not able to handle dynamic events such as emergencies, the proposed model’s use of ABAC addresses this limitation. The development of the proposed model followed the design science research paradigm and was informed by the results of the content analysis plus an expert review. The content analysis sample was retrieved by conducting a systematic literature review and the analysis of this sample resulted in 6743 tags.     Algoritmos de deteção de comportamento de indivíduos com autismo: análise comparativa

    A elevada prevalência de doenças relacionadas com o autismo e a necessidade de descoberta desta patologia de difícil diagnóstico e convivência, levou a que seja imprescindível o desenvolvimento de mecanismos de deteção e avaliação da atividade de pacientes. Esta tese tem como principal objetivo colmatar estas necessidades. Os algoritmos testados são direcionados à melhoria da monitorização de doentes com autismo e com necessidades especiais. Estes têm como principal objetivo a utilização numa casa direcionada à terapia e convivência de doentes com autismo. Através destas técnicas será possível melhorar a qualidade de vida dos pacientes e responder a emergências que possam surgir. Os dados provenientes de sensores como a câmara Kinect vão servir para detetar movimentos estereotipados característicos de doenças do espectro do autismo. Assim podem ser monitorizadas atividades anormais e por consequência antecipar algum tipo de anomalia. Por outro lado será possível avaliar a evolução do doente ao longo de uma série de tratamentos e terapias. Neste projeto foi avaliada a performance de vários algoritmos de deteção e avaliação de movimentos adaptados às necessidades presentes. Foi também conduzido um estudo sobre as tecnologias de reconhecimento de expressões faciais e identificação de pessoas pela face. Os algoritmos foram testados com movimentos de pessoas sem doenças do espetro do autismo devido à maior facilidade de trabalhar neste contexto. Posteriormente transpõem-se as tecnologias para onde forem necessárias
