    In vivo measurement of human brain elasticity using a light aspiration device

    The brain deformation that occurs during neurosurgery is a serious issue impacting the patient "safety" as well as the invasiveness of the brain surgery. Model-driven compensation is a realistic and efficient solution to solve this problem. However, a vital issue is the lack of reliable and easily obtainable patient-specific mechanical characteristics of the brain which, according to clinicians' experience, can vary considerably. We designed an aspiration device that is able to meet the very rigorous sterilization and handling process imposed during surgery, and especially neurosurgery. The device, which has no electronic component, is simple, light and can be considered as an ancillary instrument. The deformation of the aspirated tissue is imaged via a mirror using an external camera. This paper describes the experimental setup as well as its use during a specific neurosurgery. The experimental data was used to calibrate a continuous model. We show that we were able to extract an in vivo constitutive law of the brain elasticity: thus for the first time, measurements are carried out per-operatively on the patient, just before the resection of the brain parenchyma. This paper discloses the results of a difficult experiment and provide for the first time in-vivo data on human brain elasticity.     Comparison of LASTIC (Light Aspiration device for in vivo Soft TIssue Characterization) with classic Tensile Tests.

    International audienceLASTIC is a device estimating in vivo soft tissue elasticity. It uses negative pressure to deform the tissue surface and captures several deformation stages to trace the behavioral curve. Using Finite Element inverse analysis and a Neo Hookean constitutive law, the tissue's Young modulus is evaluated. This paper compares LASTIC capabilities with standard tensile tests on four samples with elastic properties ranging from 10 kPa to 100 kPa. Although LASTIC overestimates Young modulus by an average of 24 %, it allows a first estimation of the elastic modulus of different materials

    Abstract-This paper introduces a new light device for the in vivo estimation of human soft tissues constitutive laws. It consists of an aspiration pipette able to meet the very severe sterilization and handling issues imposed during surgery. The simplicity of the device, free of any electronic circuitry, allows using it as an ancillary instrument. The deformation of the aspired tissue is imaged via a mirror using an external camera. The paper describes the experimental setup as well as the protocol that should be used during surgery. First feasibility measurements are shown for human tongue and forearm skin

    Development of experimental setups for the characterization of the mechanoelectrical coupling of cells in vitro

    The field of mechanobiology emerged from the many evidences that mechanical forces acting on cells have a central role in their development and physiology. Cells, in fact, convert such forces into biochemical activities and gene expression in a process referred as mechanotransduction. In vitro models that mimic cell environment also from the mechanical point of view represent therefore a key tool for modelling cell behaviour and would find many applications, e.g. in drug development and tissue engineering. In this work I introduce novel tools for the study of mechanotransduction. In particular, I present a system for the evaluation of the complex response of electrically active cells, such as neurons and cardiomyocytes. This system integrates atomic force microscopy, extracellular electrophysiological recording, and optical microscopy in order to investigate cell activity in response to mechanical stimuli. I also present cell scaffolds for the in vitro study of cancer. Obtained results, although preliminary, show the potential of the proposed systems and methods to develop accurate in vitro models for mechanobiology studies

    A FEEDBACK-BASED DYNAMIC INSTRUMENT FOR MEASURING THE MECHANICAL PROPERTIES OF SOFT TISSUES

    In this paper, a novel feedback-based dynamic instrument integrated into a Minimally- Invasive-Surgery (MIS) tool to evaluate the mechanical impedance of soft tissues is presented. This instrument is capable of measuring viscoelasticity of tissues if specific boundary conditions are known. Some important advantages of the proposed instrument are that it is robust and simple in comparison to other similar instruments as it does not require magnitude information of plant’s displacement output and no force sensor is used. The precision and accuracy of the measurements of the proposed instrument for soft tissues is noticeably higher than similar instruments, which are not optimized to work with soft tissues. The proposed dynamic instrument is designed to detect the frequency shifts caused by contacting a soft tissue using an improved phase-locked loop feedback system (closed loop). These frequency shifts can then be used to evaluate the mechanical properties of the tissue. The closed-loop method works fast (with an approximate resonance-frequency-shift rate of 15 Hz per second), and is capable of measuring dy­ namic mechanical properties of viscoelastic tissues, while previous focus was mostly on static/quasi-static elastic modulus. The instrument is used to evaluate the equivalent stiffness of several springs and cantilever beams, mass of reference samples, and also the frequency shifts of several phantoms with injected tumors, noting that these frequency shifts can be used to measure the viscoelasticity of the tissues. It is also shown that the instrument can be used for tumor localization in these phantoms

    Developing an Optomechanical Approach for Characterizing Mechanical Properties of Single Adherent Cells

    Mechanical properties of a cell reflect its biological and pathological conditions including cellular disorders and fundamental cellular processes such as cell division and differentiation. There have been active research efforts to develop high-throughput platforms to mechanically characterize single cells. Yet, many of these research efforts are focused on suspended cells and use a flow-through configuration. Therefore, adherent cells are detached prior to the characterization, which seriously perturbs the cellular conditions. Also, methods for adherent cells are limited in their throughput. My study is aimed to fill the technical gap in the field of single cell analysis, which is a high-throughput and non-invasive mechanical characterization of single adherent cells. I developed a multi-modal platform to mechanically characterize single adherent cells. The platform is based on optomechanical principle, which induces least perturbation on the cells and does not require cell detachment. Besides, multiple measurements can be performed on a single cell to track its mechanical behavior over time. Proposed platform can expand our understanding on the relationship between mechanical properties and cellular status of adherent cells. Single adherent cells are characterized optomechanically using the vibration-induced phase shift (VIPS). VIPS is a phase shift of apparent velocity of a vertically vibrating substrate measured with laser Doppler vibrometer (LDV), when the measurement laser passes through an adherent cell or any transparent objects on the substrate. The VIPS and height oscillation of a single cell on a vibrating substrate have negative correlation with the cell stiffness. An analytical model is established which demonstrates relationship between cell’s mechanical properties and its VIPS. With the VIPS measurements, at multiple frequencies on large population of cells, the statistical significant difference in the cell stiffness is confirmed after exposure to various drugs affecting cytoskeleton network. Also, a 3-dimensional finite element model is developed to extract the cell stiffness from VIPS. VIPS technique is used to reconstruct the detailed oscillation pattern of transparent objects such as water microdroplets and intracellular lipid droplets on a vibrating substrate, which can give us better understanding of mechanical behavior of biological transparent objects. In addition, using VIPS measurement mechanical interaction between extracellular matrixes (ECMs) and adherent cells is studied.     Realistic tool-tissue interaction models for surgical simulation and planning

    Surgical simulators present a safe and potentially effective method for surgical training, and can also be used in pre- and intra-operative surgical planning. Realistic modeling of medical interventions involving tool-tissue interactions has been considered to be a key requirement in the development of high-fidelity simulators and planners. The soft-tissue constitutive laws, organ geometry and boundary conditions imposed by the connective tissues surrounding the organ, and the shape of the surgical tool interacting with the organ are some of the factors that govern the accuracy of medical intervention planning.\ud \ud This thesis is divided into three parts. First, we compare the accuracy of linear and nonlinear constitutive laws for tissue. An important consequence of nonlinear models is the Poynting effect, in which shearing of tissue results in normal force; this effect is not seen in a linear elastic model. The magnitude of the normal force for myocardial tissue is shown to be larger than the human contact force discrimination threshold. Further, in order to investigate and quantify the role of the Poynting effect on material discrimination, we perform a multidimensional scaling study. Second, we consider the effects of organ geometry and boundary constraints in needle path planning. Using medical images and tissue mechanical properties, we develop a model of the prostate and surrounding organs. We show that, for needle procedures such as biopsy or brachytherapy, organ geometry and boundary constraints have more impact on target motion than tissue material parameters. Finally, we investigate the effects surgical tool shape on the accuracy of medical intervention planning. We consider the specific case of robotic needle steering, in which asymmetry of a bevel-tip needle results in the needle naturally bending when it is inserted into soft tissue. We present an analytical and finite element (FE) model for the loads developed at the bevel tip during needle-tissue interaction. The analytical model explains trends observed in the experiments. We incorporated physical parameters (rupture toughness and nonlinear material elasticity) into the FE model that included both contact and cohesive zone models to simulate tissue cleavage. The model shows that the tip forces are sensitive to the rupture toughness. In order to model the mechanics of deflection of the needle, we use an energy-based formulation that incorporates tissue-specific parameters such as rupture toughness, nonlinear material elasticity, and interaction stiffness, and needle geometric and material properties. Simulation results follow similar trends (deflection and radius of curvature) to those observed in macroscopic experimental studies of a robot-driven needle interacting with gels

    A novel experimental procedure based on pure shear testing of dermatome-cut samples applied to porcine skin

    This paper communicates a novel and robust method for the mechanical testing of thin layers of soft biological tissues with particular application to porcine skin. The key features include the use of a surgical dermatome and the highly defined deformation kinematics achieved by pure shear testing. Thin specimens of accurate thickness were prepared using a dermatome and were subjected to different quasi-static and dynamic loading protocols. Although simple in its experimental realisation, pure shear testing provides a number of advantages over other classic uni- and biaxial testing procedures. The preparation of thin specimens of porcine dermis, the mechanical tests as well as first representative results are described and discussed in detail. The results indicate a pronounced anisotropy between the directions along and across the cleavage lines and a strain rate-dependent respons

    A Novel Bio-Inspired Insertion Method for Application to Next Generation Percutaneous Surgical Tools

    The use of minimally invasive techniques can dramatically improve patient outcome from neurosurgery, with less risk, faster recovery, and better cost effectiveness when compared to conventional surgical intervention. To achieve this, innovative surgical techniques and new surgical instruments have been developed. Nevertheless, the simplest and most common interventional technique for brain surgery is needle insertion for either diagnostic or therapeutic purposes. The work presented in this thesis shows a new approach to needle insertion into soft tissue, focussing on soft tissue-needle interaction by exploiting microtextured topography and the unique mechanism of a reciprocating motion inspired by the ovipositor of certain parasitic wasps. This thesis starts by developing a brain-like phantom which I was shown to have mechanical properties similar to those of neurological tissue during needle insertion. Secondly, a proof-of-concept of the bio-inspired insertion method was undertaken. Based on this finding, the novel method of a multi-part probe able to penetrate a soft substrate by reciprocal motion of each segment is derived. The advantages of the new insertion method were investigated and compared with a conventional needle insertion in terms of needle-tissue interaction. The soft tissue deformation and damage were also measured by exploiting the method of particle image velocimetry. Finally, the thesis proposes the possible clinical application of a biologically-inspired surface topography for deep brain electrode implantation. As an adjunct to this work, the reciprocal insertion method described here fuelled the research into a novel flexible soft tissue probe for percutaneous intervention, which is able to steer along curvilinear trajectories within a compliant medium. Aspects of this multi-disciplinary research effort on steerable robotic surgery are presented, followed by a discussion of the implications of these findings within the context of future work
