Chronic fatigue syndrome; an approach combining self-management with graded exercise to avoid exacerbations.

Controversy regarding the aetiology and treatment of patients with chronic fatigue syndrome (CFS) continues to affect the medical professions. The Cochrane collaboration advises practitioners to implement graded exercise therapy for CFS sufferers using cognitive behavioural principles. In contrast there is evidence that exercise can induce symptom exacerbations in CFS where too vigorous exercise/activity promotes immune dysfunction, which in turn increases symptoms in patients with CFS. When designing and implementing an exercise programme it is important to be aware of both these seemingly opposing view points in order to deliver a programme without any detrimental effects on CFS pathophysiology. Using evidence from both the biological and clinical sciences, the present manuscript explains that graded exercise therapy for people with CFS can be safely undertaken without detrimental effects to the immune system. Exercise programs should be designed to cater for individual physical capabilities and should also account for the fluctuating nature of symptoms commonly reported by people with CFS. In line with cognitive behaviourally and graded exercise-based strategies, self-management for people with CFS involves encouraging the patients to pace their activities and respect their physical and mental limitations with the ultimate aim of improving their everyday function

Renewal through Participation in Global Food Security Governance: Implementing the International Food Security and Nutrition Civil Society Mechanism to the Committee on World Food Security

The food commodity price rises from 2006 to 2008 engendered a period of political renewal and reform in the governance of global food security. The Committee on World Food Security (CFS) was designated as the main international forum dealing with food security and nutrition in 2009 as part of this reform. Through the CFS reform process, civil society organizations secured the right to co-ordinate autonomously their engagement in the Committee as official participants and are doing so through the International Food Security and Nutrition Civil Society Mechanism (CSM). The CSM is an innovative institutional form designed to allow a broad range of civil society organizations from different regions of the world and from diverse constituencies, notably those who face food insecurity on a regular basis, to participate in global food security governance. The challenges and complexities of setting up and operationalizing the CSM are presented and illustrated. These findings are considered in the context of the longer-term move towards widening participation in global governance, with a particular focus on the trajectory of civil society participation in food security governance. The broad neo-liberal logic, or embedded neo-liberalism, that underpins contemporary world politics provides boundaries within which the innovative CSM is being given shape through the political agency of the participating civil society organizations. The study concludes by suggesting that while the Civil Society Mechanism faces some internal challenges, these are not insurmountable, and that the CSM represents an effective politicizing, engaging and connecting model for food-focused civil society organization entering into global governance

Implementing the international public sector accounting standards for consolidated financial statements: Facilitators, benefits and challenges

Portugal and Spain have reformed their national standards to adapt them to the International Public Sector Accounting Standards (IPSAS). This paper explains the process of implementing the IPSAS for consolidated reporting in the two countries and the advantages and disadvantages which have emerged. The results highlight the role of the IPSAS in improving the quality and use of consolidated financial statements (CFS) and will be of interest to other countries that are intending to implement the IPSAS. IMPACT This paper will be of value to academics and practitioners with an interest in IPSAS and in the use of consolidated financial statements (CFS). The authors describe the experiences of two European continental countries in the implementation of accrual basis providing important lessons for countries that are considering implementing accrual accounting and/or IPSAS. Findings from this study will help managers and policy-makers to decide about the use of CFS taking IPSAS as a reference
