Het ziekenhuis als ontwerpvraagstuk:Marktgerichte structuren als voorwaarde voor interdisciplinaire, 'team-based' zorg

Ziekenhuizen behoren tot wat Mintzberg professionele bureaucratieën noemt. Ze worden geconfronteerd met problemen op het gebied van kwaliteit, patiëntvriendelijkheid, veiligheid en efficiency. Een belangrijke vraag is of ze voor het bereiken van deze doelen de juiste structuur bezitten. In zijn boek Structure in fives (1983) presenteert Mintzberg een ontwerptheorie waarmee deze vraag beantwoord zou kunnen worden. Structuurparameters spelen hierin een belangrijke rol. In het samen met Glouberman geschreven artikel (2001) past hij zijn ontwerptheorie toe op het ziekenhuis. In dat artikel wordt de vraag beantwoord: aan welke knoppen moet de ontwerper draaien om de problemen van ziekenhuizen op te lossen? In deze bijdrage vergelijk ik de opvattingen van Mintzberg met recente opvattingen over een patiëntgerichte en procesgestuurde organisatie van het ziekenhuis. Ik kom tot de conclusie dat wat hierin voorgesteld wordt, overeenkomt met 1) wat volgens Minztberg niet kan (het kantelen van de ziekenhuisorganisatie van een functionele naar een marktgerichte structuur) en 2) wat volgens Minztberg niet mag (het standaardiseren van output en processen). Dat is praktisch belangrijk, immers, volgens Mintzberg krijgen we nooit de zo gewenste multidisciplinaire, patiëntgerichte en proces- gestuurde ziekenhuisorganisatie. In deze bijdrage beperk ik mij nadrukkelijk tot het ziekenhuis als vraagstuk van structuurontwerp. Ik kan, binnen dit bestek, niet ingaan veranderingskundige vraagstukken op meso- en met vraagstukken van institutioneel herontwerp op macroniveau. Deze bijdrage is onderdeel van een vergelijkend onderzoek van de sterke en zwakke punten van verschillende organisatieontwerpbenaderingen (zie Achterbergh, Vriens, 2009; Christis, 2005, 2009 en 2011)

Met de kunst van het lesgeven houdt een hele wetenschap zich bezig. De vorige keer verwees ik naar de Vlaming Pedro de Bruyckere. Dit keer verwijs ik naar de Amerikaanse hoogleraar Daniel Willingham, die in zijn bekende werk Why don’t students like school (2009) een reeks goed bewezen beweringen presenteert over het lesgeefambacht. Bij wijze van afsluiter maak ik een royale selectie van deze beweringen, die ik telkens zal toelichten

Crimmigratie en de morele economie van illegale vreemdelingen

In haar openbare les uit 2009 beschrijft Van der Leun het proces van ‘crimmigratie’, de samensmelting van strafrecht en migratiewetgeving, en gaat ze in op de vraag of dit proces zich ook in Nederland voordoet.1 Ze behandelt dit aan de hand van drie elementen: de aanpak van mensenhandel en mensensmokkel, de mogelijke strafbaarstelling illegaal verblijf, en de ongewenstverklaringen. Het aantal ongewenstverklaringen blijkt sinds 2000 fors te zijn toegenomen: van 750 personen naar 1750 personen in 2005 − en lijkt zich nu te stabiliseren rond 1500 personen. De strafrechtelijke aanpak van mensenhandel en mensensmokkel richt zich primair op de werkgevers, de mensensmokkelaars en de huisjesmelkers en niet zozeer op de vermeende slachtoffers. Ook was er in 2009 geen sprake van een kabinet dat illegaal verblijf op dat moment strafbaar wilde stellen. Integendeel, in het debat dat in die periode over strafbaarstelling van illegaal verblijf werd gevoerd, was de eindconclusie om hiervan verder af te zien vanuit de gedachte dat deze strafbaarstelling weinig zou toevoegen aan het palet van middelen dat de overheid al ter beschikking stond om illegaal verblijf te ontmoedigen en aan te pakken. Van der Leun concludeert dan ook dat als er al sprake is van samensmelting van het strafrecht en migratiewetgeving, het dan toch vooral om selectieve vormen gaat en stelt voorzichtig dat er ‘tekenen van crimmigratie zichtbaar zijn’. In deze bijdrage zal ik mij allereerst kort richten op het beantwoorden van de vraag of dit als gematigd getypeerde proces van crimmigratie inmiddels niet een stap verder is. In de tweede plaats zal ik de vraag beantwoorden wat de consequenties zijn van dit proces van crimmigratie voor het verblijf van illegale vreemdelingen in Nederland. Ik zal bij de beantwoording van deze laatste vraag vrijelijk putten uit Nederlands onderzoek dat sinds het midden van de jaren negentig van de vorige eeuw is verricht onder illegale vreemdelingen. ..

[EN] In this paper, we obtain some results on the relationships between different ideal convergence modes namely, I K, I K∗ , I, K, I ∪ K and (I ∪K) ∗ . We introduce a topological space namely I K-sequential space and show that the class of I K-sequential spaces contain the sequential spaces. Further I K-notions of cluster points and limit points of a function are also introduced here. De omvorming van onszelf in filosofie, kunst en techniek.

In 2009 verschenen de laatste colleges van Foucault over “Diogenes en de Cynische filosofie”. Foucault kiest daarin uitdrukkelijk voor een benadering van filosofie gericht op de omvorming van onszelf. Dit geeft een nieuwe inkijk in Foucaults werk. In “Wat is Verlichting?” gaat het ook over de omvorming van onszelf, maar daar wordt het thema omzichtiger ingebracht via (gedeeltelijk verhulde) discussies met Kant, Heidegger, Sartre en Habermas. Foucault brengt zo de omvorming van onszelf terug in de hedendaagse filosofie. Deze inzichten kunnen vruchtbaar gebruikt worden bij actuele vraagstukken, bijvoorbeeld in de techniekfilosofie. Met verwijzing naar gebruikersonderzoek en een artistieke verkenningen rond nieuwe technologie geef ik een illustratie en toepassing van Foucaults benadering

EDHF: An update

The endothelium controls vascular tone not only by releasing NO and prostacyclin, but also by other pathways causing hyperpolarization of the underlying smooth muscle cells. This characteristic was at the origin of the term 'endothelium-derived hyperpolarizing factor' (EDHF). However, this acronym includes different mechanisms. Arachidonic acid metabolites derived from the cyclo-oxygenases, lipoxygenases and cytochrome P450 pathways, H 2O 2, CO, H 2S and various peptides can be released by endothelial cells. These factors activate different families of K + channels and hyperpolarization of the vascular smooth muscle cells contribute to the mechanisms leading to their relaxation. Additionally, another pathway associated with the hyperpolarization of both endothelial and vascular smooth muscle cells contributes also to endothelium-dependent relaxations (EDHF-mediated responses). These responses involve an increase in the intracellular Ca 2+ concentration of the endothelial cells, followed by the opening of SK Ca and IK Ca channels (small and intermediate conductance Ca 2+-activated K + channels respectively). These channels have a distinct subcellular distribution: SK Ca are widely distributed over the plasma membrane, whereas IK Ca are preferentially expressed in the endothelial projections toward the smooth muscle cells. Following SK Ca activation, smooth muscle hyperpolarization is preferentially evoked by electrical coupling through myoendothelial gap junctions, whereas, following IK Ca activation, K + efflux can activate smooth muscle Kir2.1 and/or Na +/ K +-ATPase. EDHF-mediated responses are altered by aging and various pathologies. Therapeutic interventions can restore these responses, suggesting that the improvement in the EDHF pathway contributes to their beneficial effect. A better characterization of EDHF-mediated responses should allow the determination of whether or not new drugable targets can be identified for the treatment of cardiovascular diseases. © The Authors Journal compilation © 2009 Biochemical Society.postprin
