    Application of critical controls for fatality prevention in mining operations

    In this study, a new risk management approach was applied to mitigate fatal incidents through the utilization of critical controls. The aim of this study was to create a scalable, minimally invasive proof-of-concept for AngloGold Ashanti that can successfully be implemented at any of the company’s mining operations. The system was designed by adhering to organizational requirements, and ensuring that it is suitable to any mining environment. The designed Critical Control Management System was subsequently implemented at Sunrise Dam, one of AngloGold Ashanti’s Australian mining operations. To ensure that critical controls were also assessed at the operational level, a workplace inspection process was modified to generate control data. All sources of data subsequently were fed into a Business Intelligence environment enabling insight into critical control performance to all company stakeholders. Doing so informs decision-making on safety priorities company-wide, based on real-time data generated on the operational level. Two case studies were performed to assess two of the most significant hazards at Sunrise Dam. The studies showed that the effectiveness of reactive controls changes irrespective of their compliance and performance. Furthermore, the influence of human factors within risk management remains difficult to quantify. Finally, it demonstrates the potential for integration of incident data into the Critical Control Management System, thus creating both leading and lagging indicators for safety performance. The conclusion of this study is that an effective and scalable Critical Control Management System can be successfully implemented in a mining operation if the right conditions are generated. The approach of integration in existing processes demonstrates that companies can achieve greater control over fatality prevention without the need for an additional safety management system. On this basis, it is recommended that other operations are supported in creating an environment suitable for adaptation before Critical Control Management is implemented

    The safety and sustainability of mining at diverse scales: Placing health and safety at the core of responsibility

    Mining plays a major role in meeting global resource demands with Europe hosting extensive mineral potential. However, few of these prospects are feasible for conventional exploitation due to their small size & ore grade, proximity to dense populations and tenement constraints. Hence, a significant paradigm shift towards switch-on, switch off small-scale mining (SOSO SSM) is needed in order to increase the viability of small, complex, high-grade deposits. The IMP@CT project developed mobile, modularised solutions to facilitate rapid deployment and in-situ extraction & processing, which necessitates the translation and extension of best practice safety and sustainability from established national regulations and industry standards. Despite decades of accumulated safety regulation, knowledge and experience, workplace errors and violations still lead to fatal accidents, particularly if immature safety attitudes and behaviours pervade an organisation. The presence of a mature safety culture is vital for mitigating the occurrence of injuries and fatalities, through a collective commitment to safety improvement. This study has aimed to consolidate safety and sustainability best practice that is tailored to SSM by identifying the critical safety considerations and applying safety culture maturity principles to the specific challenges associated with a semi-automated SOSO SSM system. Criteria-driven maturity modelling, informed by existing responsible mining initiatives and safety and socio-environmental culture perspectives from site personnel at all hierarchical levels, is developed to assess the environmental and social factors associated with small- to medium-scale regulated mining. The role of agile management for rapid adaptation and continuous improvement of safety and sustainability performance in SOSO SSM is discussed. This research has demonstrated that for SOSO SSM to effectively integrate a mature safety and socio-environmental culture within a flexible, containerised mining paradigm, managerial and technical agility, and human initiative must be encouraged to continuously drive progress in occupational health and safety and generate wider societal benefit

    Investigation of individual factors impacting the effectiveness of requirements inspections: a replicated experiment

Experimental replications are important for verifying results and investigating the generality of empirical studies. We utilized the lab package and procedures from the original study, with some changes and additions, to conduct the replication with 69 professional developers in three different companies in Turkey. In general the results of the replication were consistent with those of the original study. The main result from the original study, which is supported in the replication, was that inspectors whose degree is in a field related to software engineering are less effective during a requirements inspection than inspectors whose degrees are in other fields. In addition, we found that Company, Experience, and English Proficiency impacted inspection effectiveness

    Process Based Unification for Multi-Model Software Process Improvement

    A number of differences among quality approaches exist and there can be various situations in which the usage of multiple approaches is required, e.g. to strengthen a particular process with multiple quality approaches or to reach certification of the compliance to a number of standards. First of all it has to be decided which approaches have potential for the organization. In many cases one approach does not contain enough information for process implementation. Consequently, the organization may need to use several approaches and the decision has to be made how the chosen approaches can be used simultaneously. This area is called Multi-model Software Process Improvement (MSPI). The simultaneous usage of multiple quality approaches is called the multi-model problem. In this dissertation we propose a solution for the multi-model problem which we call the Process Based Unification (PBU) framework. The PBU framework consists of the PBU concept, a PBU process and the PBU result. We call PBU concept the mapping of quality approaches to a unified process. The PBU concept is operationalized by a PBU process. The PBU result includes the resulting unified process and the mapping of quality approaches to the unified process.Comment: PhD Thesi

Managing these LNG projects is challenging as they become increasingly complex and technologically demanding. The primary goal of this thesis is to develop a Total Constraint Management (TCM) method to improve construction work flow during LNG construction. Five controlled experiments were conducted and results show that successful implementation of TCM can significantly improve construction productivity and reduce schedule overruns

    An Investigation into quality assurance of the Open Source Software Development model

    A thesis submitted in partial fulfilment of the requirements of the University of Wolverhampton for the degree of Doctor of PhilosophyThe Open Source Software Development (OSSD) model has launched products in rapid succession and with high quality, without following traditional quality practices of accepted software development models (Raymond 1999). Some OSSD projects challenge established quality assurance approaches, claiming to be successful through partial contrary techniques of standard software development. However, empirical studies of quality assurance practices for Open Source Software (OSS) are rare (Glass 2001). Therefore, further research is required to evaluate the quality assurance processes and methods within the OSSD model. The aim of this research is to improve the understanding of quality assurance practices under the OSSD model. The OSSD model is characterised by a collaborative, distributed development approach with public communication, free participation, free entry to the project for newcomers and unlimited access to the source code. The research examines applied quality assurance practices from a process view rather than from a product view. The research follows ideographic and nomothetic methodologies and adopts an antipositivist epistemological approach. An empirical research of applied quality assurance practices in OSS projects is conducted through the literature research. The survey research method is used to gain empirical evidence about applied practices. The findings are used to validate the theoretical knowledge and to obtain further expertise about practical approaches. The findings contribute to the development of a quality assurance framework for standard OSSD approaches. The result is an appropriate quality model with metrics that the requirements of the OSSD support. An ideographic approach with case studies is used to extend the body of knowledge and to assess the feasibility and applicability of the quality assurance framework. In conclusion, the study provides further understanding of the applied quality assurance processes under the OSSD model and shows how a quality assurance framework can support the development processes with guidelines and measurements
