OVERSPECIALISATION IN MEDICINE, OR WHAT ABOUT NOT SEEING THE FORREST BECAUSE OF THE TREES?

Since early historical medical approach: shamanic era, ancient Hyppocrates, Galen and Avicenna’s medical records, till today many progresses were made in the medical field. Yet, despite the technological achievements the efficiency of medical work didn’t always increase accordingly. A lot of diagnosis errors still occur. What could be the causes? One possibility among others could be overspecialization with seing just a part of the true. Some clinical cases are presented to point that situation out. In the meantime this paper emphasizes the importance of reassessing the way of medical thinking by highlighting the importance of integrative medical approach. The key for decreasing diagnosis mistakes is in fact a multiple step process. There are some sides related exclusivily to our doctor’s work as a routine and many others related to the providers ( medical system) by far more complex and with bigger impact. From my doctor’s point of view it’s mandatory to have the knowledge, that means a very good initially and if necessary further evaluation of the patient, integrative operational skills and experience leading to the relevant clinical data that will serve to a good orientation, completing the overall knowledge and decreasing the social costs, good and efficient communication system, active follow-up of the patients and if necessary rebuilding another way of thinking the first diagnosis, based on new evidence, reviewing and adjusting, trying not to be so “overconfident”, having the courrage and dignity to face our own mistakes, demanding a second opinion, relearning and becoming even better by using feedback. The lack of time and daily routine imposed by overspecialisation, overstandardization and many algorithms and guidelines ment to do our work easiear, somehow lead to an automatically judgement, putting us sometimes in the position of easyness and superficiallity

A miniaturized silicon based device for nucleic acids electrochemical detection

    Get PDF
In this paper we describe a novel portable system for nucleic acids electrochemical detection. The core of the system is a miniaturized silicon chip composed by planar microelectrodes. The chip is embedded on PCB board for the electrical driving and reading. The counter, reference and work microelectrodes are manufactured using the VLSI technology, the material is gold for reference and counter electrodes and platinum for working electrode. The device contains also a resistor to control and measuring the temperature for PCR thermal cycling. The reaction chamber has a total volume of 20 μL. It is made in hybrid silicon–plastic technology. Each device contains four independent electrochemical cells. Results show HBV Hepatitis-B virus detection using an unspecific DNA intercalating redox probe based on metal–organic compounds. The recognition event is sensitively detected by square wave voltammetry monitoring the redox signals of the intercalator that strongly binds to the double-stranded DNA. Two approaches were here evaluated: (a) intercalation of electrochemical unspecific probe on ds-DNA on homogeneous solution (homogeneous phase); (b) grafting of DNA probes on electrode surface (solid phase). The system and the method here reported offer better advantages in term of analytical performances compared to the standard commercial optical-based real-time PCR systems, with the additional incomes of being potentially cheaper and easier to integrate in a miniaturized device. Keywords: Electrochemical detection, Real time PCR, Unspecific DNA intercalato

The Library of the Istituto Superiore di SanitĂ  (ISS), the Italian National Institute of Health, is the main library for public health and biomedical research in Italy and holds a small but valuable special collection of ancient books. Known as the Rare Book Collection, this fund consists of over 1200 scientific printed volumes published between the XVI and the XIX century. The purpose of this paper is to illustrate the challenges and the process undertaken by the Library to share and digitalize this Collection

Operative Treatment of Patients with Pancreatic Pseudocysts

    Get PDF
The article presents the results of surgical treatment of patients with pancreatic pseudocysts spent in hospital surgical clinic Siberian State Medical University from 2004 to 2016. 7 (17.5%) patients underwent conservative therapy. In the early postoperative period and in the long term (18 months) the analysis of the effectiveness of different methods of surgical treatment. According to the results of the research, the operations of the internal drainage along with resection and resection-draining interventions in assessing the quality of life showed similar long-term results. In turn, these interventions have provided significantly better quality of life (p> 0.05) than external drainage operation

Counseling of the Importance of Family Medicine Plants (TOGA) as an Effort to Improve Healthy Lifestyle at the SDN Centong, Gondang District, Mojokerto Regency

Educational institutions are seen as a strategic place to promote health. Schools are also effective institutions for realizing health education, where students can be taught about the meaning of healthy behavior and back to nature. In addition, school age is a golden age to instill back to nature values and has the potential as an agent of change to promote back to nature both in the school, family, and community environment. School teachers as the target of promotion of back to nature, especially the basic education level (SD and SMP). This is because the teacher can transmit it to students, because this age group of students is easy to accept new innovations and has a strong desire to convey the knowledge and information they receive to others. School children are a group of children whose age is balanced. Some examples of the implementation of back to nature in schools can be conveyed, for example counseling on the importance of TOGA. SDN Centong, Gondang Subdistrict, Mojokerto Regency has a fairly large yard, which holds a lot of potential for the use of natural resources, one of which is the use of the yard to plant TOGA. TOGA is still something that is not yet known by students. Therefore, there is a need for counseling on the importance of planting TOGA in this school. This community service program aims to provide teachers with an understanding of the importance of TOGA so that it can be conveyed to students. The implementation method is (1) pretest, (2) counseling, (3) discussion and question and answer, (4) posttest. This activity involved the entire Board of Teachers and Employees of SDN Centong. The total value increased by 116 from 1000 to 1116, the average also increased by 9 from 71 to 80. Then the highest value increased by 8 from 84 to 92, and the lowest value also increased by 12 from 60 to 72 There was an increase in all scores from the pretest and posttest, this indicates that this outreach activity was running effectively because it could significantly increase the understanding of the teachers and employees of SDN Centong

EnFilter is a Proactive Password Checking System, designed to avoid password guessing attacks. It is made of a set of configurable filters, each one based on a specific pattern recognition measure that can be tuned by the system administrator depending on the adopted password policy. Filters use decision trees, lexical analysers, as well as Levenshtein distance based techniques. EnFilter is implemented for Windows 2000/2003/XP
